Sometimes less is more: experience with endoscopic myringoplasty.

A retrospective review of the outcomes of patients who underwent endoscopic myringoplasties in our institution was conducted. The aim was to highlight our results with this procedure. The database of patient records was manually checked, and the patients who had undergone Endoscopic Myringoplasties were identified, and their demographics, admitting notes, operating notes, and discharge summaries were reviewed. Graft failure was considered if the patient had a perforation in the graft during the outpatient follow-up. The information was compiled, and basic statistics were derived. A total of 31

More than autophony: a case of Kennedy's disease presenting with autophony as an early clinical manifestation.

BACKGROUND: As autophony can be accompanied by several conditions, it is important to find co-morbidities. This paper reports a patient with Kennedy's disease (spinobulbar muscular atrophy, an X-linked, hereditary, lower motor neuron disease) having autophony as the first symptom. CASE REPORT: A 62-year-old male presented to the otorhinolaryngology department with autophony that began 2 years previously and worsened after losing weight 3 months prior to presentation. Otoscopic examination demonstrated inward and outward movement of the tympanic membrane, synchronised with respiration. Althoug

Vestibular reflexes in essential tremor: abnormalities of ocular and cervical vestibular-evoked myogenic potentials are associated with the cerebellum and brainstem involvement.

This study utilized cervical vestibular-evoked myogenic potentials tests (cVEMP) and ocular vestibular-evoked myogenic potentials tests (oVEMP) to investigate the vestibulocollic and vestibuloocular reflex arcs and to evaluate cerebellar and brainstem involvement) in essential tremor (ET). Eighteen cases with ET and 16 age- and gender-matched healthy control subjects (HCS) were included in the present study. Otoscopic and neurologic examinations were performed on all participants, and both cervical and ocular VEMP tests were performed. Pathological cVEMP results were increased in the ET group

Hearing loss due to urate deposition in the middle ear: A case report and literature review.

Gout is the most common cause of monoarthritis in men occurring classically in the great toe and the knee. Extra-articular gout manifestations are rare. Only a few cases of head and neck urate crystals deposits have been described in the literature. Precipitations in the middle ear cause conductive hearing loss with common otoscopic anomalies and difficult imaging diagnosis. We report a case of a healthy 58-years-old man with a middle ear urate deposit causing a progressive hearing loss as the very first symptom of gout. The nature of the deposit was unsure on computer tomography (CT) due to a

DFNA5 (GSDME) c.991-15_991-13delTTC: Founder Mutation or Mutational Hotspot?

Deafness due to mutations in the DFNA5 gene is caused by the aberrant splicing of exon 8, which results in a constitutively active truncated protein. In a large family of European descent (MORL-ADF1) segregating autosomal dominant nonsyndromic hearing loss, we used the OtoSCOPE platform to identify the genetic cause of deafness. After variant filtering and prioritization, the only remaining variant that segregated with the hearing loss in the family was the previously described c.991-15_991-13delTTC mutation in DFNA5. This 3-base pair deletion in the polypyrimidine of intron 7 is a founder mut

Transitory stapedial myoclonus in a patient with benign fasciculation syndrome.

OBJECTIVE: We report a previously undescribed association between transitory stapedial myoclonus, objective tinnitus and benign fasciculation syndrome. METHOD: Case report and review of the world literature regarding stapedial myoclonus. RESULTS: A 30-year-old man with a diagnosis of benign fasciculation syndrome abruptly developed severe, low-pitched tinnitus on the right side. Otoscopic examination revealed rhythmic movement of the tympanic membrane, which was synchronous with the tinnitus. No palatal spasm was noted on nasopharyngeal examination. Ipsilateral acoustic reflexes in infants.

Ipsilateral acoustic reflex thresholds were studied in otoscopically normal ears of infants 2 weeks to 12 months old. Probe tones of 220 and 660 Hz and pure tone activators were used. Reflex recordings were free of activator artifact and were measured using quantitative standards. Reflexes were detected in 85% to 95% of infant ears for 500, 1,000, and 2,000 Hz activators but were present in only 76% of ears at 4,000 Hz. Median thresholds of 80 to 85 dB hearing level were obtained with all activators and both probe tones. There were no differences in proportions of reflexes obtained with the tw

Otitis media in the neonatal intensive care unit.

Thirty-eight of 125 premature infants who were hospitalized in a neonatal intensive care unit (NICU) had abnormal tympanic membrane mobility compatible with otitis media. Twenty-five of these 38 had received antibiotics within one week of otoscopic examination and were considered to have either serous otitis or partially treated bacterial otitis media; tympanocentesis was not performed in them. Tympanocentesis was performed in the remaining 13 infants who had not received antibiotics. Bacterial otitis media was confirmed in ten of the 13. Either staphylococcal (six cases) or Gram-negative ente

Hidden Peril in the Ear: A Case of External Auditory Canal Cholesteatoma in a Teenager.

External auditory canal cholesteatoma (EACC) is a rare otologic condition characterized by the accumulation of keratinized material in the external auditory canal (EAC), leading to progressive bony erosion. While typically associated with middle ear disease, isolated EACC in young patients is uncommon and often misdiagnosed. We report the case of a 17-year-old male who presented with left-sided otalgia, purulent otorrhea, and progressive hearing loss over 3 months. Otoscopic examination revealed a firm, yellowish mass occluding the left EAC. High-resolution computed tomography demonstrated a s

[Inflammatory diseases of the petrous bone].

In most cases, inflammatory diseases of the petrous bone can be diagnosed clinically. However, for certain diseases, the extent of the findings must be assessed using cross-sectional imaging, or important complications such as cerebral complications must be excluded. The petrous bone is part of the temporal bone, consisting of the pars squamosa, pars tympanica, and pars mastoidea [1]. It encompasses the inner ear, the internal auditory canal, and the petrous bone itself. Anatomically, the middle ear and mastoid already belong to the pars tympanica and pars mastoidea [1]. Imaging should ideally

Bone cement repair of malleus handle fractures: Intraoperative video and case report of two patients.

Isolated malleus fractures are a rare occurrence with few reported cases in the literature. Symptoms include sudden otalgia, hearing loss, tinnitus and aural fullness. Work-up and diagnosis are based on a combination of thorough anamnesis and careful otoscopic evaluation or high-resolution computer tomography. We present two cases of isolated malleus handle fractures who were diagnosed based on a combination of pneumatic otoscopy and tympanometry. Both fractures were surgically repaired using hydroxyapatite bone cement as showcased in the supplemental video material. Post-operative audiometry
