A systematic review of complications and failures associated with medial patellofemoral ligament reconstruction for recurrent patellar dislocation.

BACKGROUND: Patellofemoral instability affects activities of daily living and hinders athletic participation. Over the past 2 decades, more attention has been paid to medial patellofemoral ligament (MPFL) reconstruction for the treatment of recurrent patellar dislocations/subluxations. Numerous techniques have been reported; however, there is no consensus regarding optimal reconstruction. PURPOSE: This study sought to report on the various techniques for MPFL reconstruction described in the literature and to assess the rate of complications associated with the procedure. STUDY DESIGN: Meta-a

Bilateral camptodactyly and recurrent patellar dislocation: a new sign of 22q11 deletions or an independent dominant disorder?

Three individuals from two families with bilateral camptodactyly and recurrent patellar subluxation are presented. All three have features of Di George syndrome owing to a 22q11 deletion. Hand abnormalities are infrequently seen in 22q11 deletions. Only one previous report exists of camptodactyly with patellar subluxation.

Surgery in acute patellar dislocation--evaluation of the effect of injury mechanism and family occurrence on the outcome of treatment.

A retrospective study was made of 270 patients and 284 knees with acute patellar dislocation treated operatively. The mean follow up time was 4.1 years. Medical history revealed 21.1% of cases with previous dislocations and 15.6% of cases with family occurrence of patellar dislocation. The dislocation resulted from an athletic performance in 41.5% of cases. The sport events most often associated with patellar dislocation were soccer, gymnastics, and ice hockey. All cases were treated with reefing of medial capsule. Release of lateral patellar retinacula was performed in 243 cases. Two cases we

Evaluation of Changes in Hip Morphology in a Rabbit Model of Patellar Dislocation via Computed Tomography Imaging.

BACKGROUND Although early patellar dislocation induces morphological changes in the patellofemoral joint, its potential influence on hip morphology during growth remains unclear. This study evaluated changes in hip morphology via computed tomography (CT) imaging in a rabbit model of patellar dislocation. MATERIAL AND METHODS Thirty female New Zealand White rabbits (2 months old) were included. The left hindlimbs of rabbits (n = 28 after 2 postoperative deaths) underwent patellar dislocation surgery and comprised the study group. Contralateral right hindlimbs (n = 28), which underwent no surgic

Cost-Effectiveness of Medial Patellofemoral Ligament Reconstruction for First-Time Patellar Dislocations: A Markov Analysis.

BACKGROUND: Nonoperative management versus medial patellofemoral ligament reconstruction (MPFLR) for first-time patellar dislocations remains controversial, and it is unclear whether the increased up-front costs from MPFLR are justified. PURPOSE: To compare the cost-effectiveness of an initial trial of physical therapy alone to that of early MPFLR to determine the preferred cost-effective treatment strategy for first-time patellar dislocations. STUDY DESIGN: Economic and decision analysis; Level of evidence, 3. METHODS: A Markov chain Monte Carlo probabilistic model was developed to evaluat

Clinical Results after Combined Distal Femoral Osteotomy in Patients with Patellar Maltracking and Recurrent Dislocations.

The purpose of this study is to analyze the clinical results after treatment of patellofemoral instability and maltracking caused by torsional or axial deformities of the lower extremity by combined distal femoral osteotomies (DFOs). We analyzed 31 DFOs (25 patients) with patellar maltracking and instability, treated in our clinic. Torsional angles and the leg axis in the frontal plane were measured preoperatively. Standardized scoring systems were determined pre- and postoperatively including a clinical examination. Nineteen cases of torsional and 12 cases of axis deformities were evaluated 2

Adolescent knee pain and patellar dislocations are associated with patellofemoral osteoarthritis in adulthood: A case control study.

BACKGROUND: There is a lack of information about the association between patellofemoral osteoarthritis (PFOA) and both adolescent anterior knee pain (AKP) and previous patellar dislocations. METHODS: This case-control study involved 222 participants from our knee arthroplasty database answering a questionnaire. One hundred and eleven patients suffering from PFOA were 1:1 matched by gender with a unicompartmental tibiofemoral arthritis control group. Multivariate correlation and binary logistic regression analysis were performed, with odds ratios (ORs) and 95% confidence intervals (CIs) calcul

Primary patellar dislocations without surgical stabilization or recurrence: how well are these patients really doing?

PURPOSE: While a significant research has gone into identifying patients at highest risk of recurrence following primary patellar dislocation, there has been little work exploring the outcomes of patients who do not have a recurrent patellar dislocation. We hypothesize that patients without recurrent dislocation episodes will exhibit significantly higher KOOSs than those who suffer recurrent dislocations, but lower scores than published age-matched normative data. METHODS: A retrospective review of patients with nonoperatively treated primary lateral patellar dislocations was carried out, and

What Components Comprise the Measurement of the Tibial Tuberosity-Trochlear Groove Distance in a Patellar Dislocation Population?

BACKGROUND: The tibial tuberosity-trochlear groove distance is used as an indicator for medial tibial tubercle transfer; however, to our knowledge, no studies have verified whether this distance is strongly affected by tubercle lateralization at the proximal part of the tibia. We hypothesized that the tibial tuberosity-trochlear groove distance is mainly affected by tibial tubercle lateralization at the proximal part of the tibia. METHODS: Forty-four patients with a history of patellar dislocation and forty-four age and sex-matched controls were analyzed with use of computed tomography. The t

Surgical versus non-surgical management for primary patellar dislocations: an up-to-date meta-analysis.

The aim of this up-to-date meta-analysis was to compare the effects of surgical versus non-surgical treatment of patients following primary patellar dislocation and to provide the best evidence currently available. A comprehensive literature search was conducted using multiple databases, including Medline, Embase, and Cochrane Registry of Clinical Trials. All databases were searched from the earliest records to May 2013. Eligible studies were selected, and data were extracted by two independent investigators. The primary outcome variable was the frequency of recurrent patellar dislocation. The

[Diagnostic and therapeutic management of primary and recurrent patellar dislocations - analysis of a nationwide survey and the current literature].

AIM: Patellar dislocations are a common injury of the knee joint. During patella dislocations injuries of soft-tissue structures can occur that can destabilise the patella and lead to recurrent dislocations. There are also congenital pathologies that predispose to patella dislocations. In the current literature, diagnostics and treatment of patellar dislocations are frequently discussed. Therefore the aim of our survey was to analyse and summarise actual diagnostic and therapeutic strategies regarding primary and recurrent patella dislocations. METHODS: An online questionnaire form was sent t

Magnetic resonance imaging of acute patellar dislocation in children: patterns of injury and risk factors for recurrence.

BACKGROUND: Acute patellar dislocation is a common traumatic condition of the knee seen in the active adolescent. The patterns of injury to the ligamentous and chondral surfaces following dislocation have not been well defined in the pediatric population. The purpose of this study was to characterize the patterns of medial patellofemoral ligament (MPFL), vastus medialis obliqus (VMO), and osteochondral injury on magnetic resonance imaging (MRI) following first-time acute lateral patellar dislocation in pediatric patients. METHODS: Following approval by the Institutional Review Board, a radiol

How reliable is MRI in diagnosing cartilaginous lesions in patients with first and recurrent lateral patellar dislocations?

BACKGROUND: Lateral dislocation of the patella (LPD) leads to cartilaginous injuries, which have been reported to be associated with retropatellar complaints and the development of patellofemoral osteoarthritis. Therefore, the purpose of this study was to determine the reliability of MRI for cartilage diagnostics after a first and recurrent LPD. METHODS: After an average of 4.7 days following an acute LPD, 40 patients (21 with first LPDs and 19 with recurrent LPDs) underwent standardized 1.5 Tesla MRI (sagittal T1-TSE, coronal STIR-TSE, transversal fat-suppressed PD-TSE, sagittal fat-suppress

[Current aspects of surgical management of patellar dislocation in the growth period with special reference to Goldthwait surgery].

RESEARCH QUESTION: The Goldthwait procedure is an operative treatment method for patellar dislocation in children which respects the growth plate. The aim of the study was to analyse the long term results of this procedure. METHODS: The results of 36 knees (26 children) operated on because of patellar dislocation according to the Goldthwait technique alone or in combination with other procedures (Krogius, patellar fixation) between 1941-1991 at Balgrist Clinic were analysed. Therefore, we used an own score. RESULTS: After a mean follow-up of 16 years (1.5 to 51 years) we found in 77.8% of th

Patella alta and patella infera. Their etiological role in patellar dislocation, chondromalacia, and apophysitis of the tibial tubercle.

The Insall-Salvati method of assessing the position of the patella was applied to four groups of patients:normal patients, and those with dislocation of the patella, chondromalacia of the patella, and apophysitis of the tibial tubercle. The ratio of patellar length to tendon length was 1.0 in the normal patients, 0.8 in the patients with dislocations, 0.86 in those with chondromalacia, and 1.2 in those with apophysitis of the tibial tubercle. Patella alta may be an important cause of dislocating patella and of chondromalacia patellae. The significance of the patella infera found in the Osgood-
