Implantable collamer lens for correction of ametropia in eyes with corneal scarring.

CASES: Two cases are reported in which implantation of a collagen copolymer phakic intraocular lens (implantable collamer lens) corrected refractive errors in eyes with corneal scarring. A previous accident, in both cases, resulted in a central linear scar on the surface of the cornea of one eye. In the first patient, a corneal scar was visible in the left eye. The distance corrected visual acuity was 0.3 (-7.75 -4×160°). An uncorrected distance visual acuity of 0.25 was obtained by implanting an implantable collamer lens. In the second patient an oblique corneal scar was visible in the right

Choroidal neovascularization as a late complication of ocular toxoplasmosis.

Macular detachment caused by choroidal neovascularization in eyes with previous toxoplasmic infections is one possible explanation of sudden loss of visual acuity. Three patients with quiescent chorioretinal scars, presumed to be toxoplasmic in origin, developed choroidal neovascular membranes that caused sudden loss of visual acuity. In none of the three cases was there any ophthalmoscopic evidence of intraocular inflammation. In two patients, the choroidal neovascularization was subfoveal and, therefore, not suitable for photocoagulation. However, visual acuity in each of the affected eyes w

Association Between Emergency Department Undertriage or Overtriage With Timeliness of Care and Patient Outcomes.

STUDY OBJECTIVE: Assess how emergency department (ED) mistriage is associated with timeliness of ED care and patient outcomes. METHODS: This was a retrospective cohort study of ED encounters from 2016 to 2020, across 21 EDs. Using operational measures of triage accuracy, we assessed how assigned Emergency Severity Index (ESI) matched downstream intensity of care and resource use. Patients with a mismatch between ESI assignment and downstream resource use were classified as undertriaged high-acuity or overtriaged low acuity, whereas those with no mismatch were classified as true low-, mid-, or

Cataract surgical outcome at the Vancouver Eye Care Centre: can it be predicted using current data?

BACKGROUND: Patients selected for cataract surgery have an increasingly wider range of coexisting ocular and systemic diseases. The aim of this study was to determine whether preoperative patient characteristics can assist surgeons in identifying patients who are at increased risk of having little or no improvement in visual outcome after cataract surgery. METHODS: We prospectively studied a randomly selected subset (20%) of patients with cataract undergoing cataract extraction at a tertiary centre in Vancouver. There were no patient exclusion criteria. Pre-, intra- and postoperative clinical

[Quality of life in patients suffering from choroideremia].

PURPOSE: To direct attention on every-day problems in choroideremia, which are not a barrier in patients occupation. MATERIAL AND METHODS: Two cases of patients with choroideremia which was diagnosed when they were 30 and 54 are described. In spite of widespread visual field changes patients with good visual acuity did not have to change their lifestyles or their jobs (miner and mechanic). Visual acuity, visual field, mesoptometry, adaptometry, electroretinography, electrooculography were done. The mother of the younger patient was also examined. Her fundus of the eye showed typical changes f

Exploring standard and low luminance visual acuity and the Moorfields Acuity Chart as outcome measures in inherited retinal disease.

INTRODUCTION: Standard visual acuity (VA) is often insensitive to subtle changes in vision that result from inherited retinal disease. Low luminance VA (LLVA) has grown in popularity as an alternative acuity measure. A new test, the Moorfields Acuity Chart (MAC) has been designed as a more sensitive and repeatable test for use in patients with age-related macular degeneration. The study explores the utility and repeatability of standard VA, LLVA and the MAC in a mixed cohort of patients with inherited retinal disease. METHODS: Participants were recruited as part of the visual function in reti

Artificial Intelligence in Triaging Patient Questions: An Evaluation of a Large Language Model for Distal Radius Fractures.

INTRODUCTION: Large language models (LLMs) are promising tools for clinical decision support but require thorough validation to ensure safety and reliability. This study assessed a knowledge and intelligence messaging interface (KIMI; RevelAi Health), an LLM enhanced with retrieval-augmented generation configured with American Academy of Orthopaedic Surgeons guidelines for distal radius fracture management and a persistent system-prompt layer. The goal was to evaluate KIMI's efficacy in acuity triaging and generating appropriate patient-facing responses for distal radius fracture management.

Dissatisfaction with cataract surgery in relation to visual results in a population-based study in Sweden.

PURPOSE: To evaluate dissatisfied patients and those with subjectively worse visual ability after cataract surgery and to analyze how these factors relate to poor visual acuity results (worse than 20/40). SETTING: Department of Ophthalmology, Norrlands University Hospital, Umeå, Sweden. METHODS: A prospective, population-based study of cataract surgery outcome was conducted. All patients (459 surgeries) who had cataract surgery during a 1 year period were studied using self-administered questionnaires and data from patient records. Outcome measures were (1) patient degree of satisfaction wit

The use of levodopa in the treatment of bilateral amblyopia secondary to uncorrected high hypermetropia.

The standard treatment of bilateral amblyopia secondary to uncorrected high bilateral hypermetropia has been spectacles and time. The patient described here failed to show adequate improvement in visual acuity after 18 months of standard treatment. Visual acuity improved dramatically when the patient began taking levodopa, dropped precipitously when the medication was discontinued, and returned quickly to post-treatment levels when levodopa was restarted. Visual acuity remained stable when the medication was slowly tapered and discontinued. This report suggests that dopamine may play a role in

Intolerance of a non-diffractive extended-depth-of-focus IOL with mini-monovision.

PURPOSE: The use of a non-diffractive extended-depth-of-focus (EDOF) intraocular lens (IOL) with slight myopia of -0.5 D on the non-dominant eye increases the spectacle independence and has good subjective tolerance with optical phenomena comparable to those of a monofocal IOL. This case report describes the course of a myopic patient who underwent refractive lens exchange, didn't tolerate mini-monovision and received IOL exchange therefore. OBSERVATIONS: A healthy, 62-year-old male with myopia of approximately -5 D underwent refractive lens exchange with a non-diffractive EDOF-IOL on both ey

Technological aided assessment of the acutely ill patient - The case of postoperative complications.

Surgical interventions come with complications and highly reported mortality after major surgery. The mortality may be a result of delayed detection of severe complications due to lower monitoring frequency in the general wards. Several studies have shown that continuous monitoring is superior to the manually intermittent recorded monitoring in terms of detecting abnormal physiological signs. Hopefully improved observations may result in earlier detection and clinical intervention. This narrative review will describe current monitoring possibilities for postoperative patients and how it may pr
