Sternocostal instability after Ravitch repair in adolescents; 3 case-reports and a review of surgical techniques in the literature.

UNLABELLED: Ravitch repair is a common surgical procedure to correct chest wall deformities. In this procedure, a subperichondreal cartilage resection of the deformed parasternal cartilage, and if necessary a repositioning of the sternum, is performed. Insufficient regeneration of the resected cartilage may result in sternocostal instability or even floating sternum. This rare complication presents with symptoms of pain and exercise intolerance. METHODS: We describe sternocostal instability in 3 adolescent patients after the Ravitch procedure for pectus carinatum and reviewed the literature o

Atomic force microscopy characterization of collagen 'nanostraws' in human costal cartilage.

Costal cartilage, a type of hyaline cartilage that bridges the bony ribs and sternum, is relatively understudied compared to the load bearing cartilages. Deformities of costal cartilage can result in deformation of the chest wall, where the sternum is largely pushed toward or away from the spine, pectus excavatum and pectus carinatum, respectively, with each condition having significant clinical impact. In the absence of extensive literature describing morphological features of costal cartilage, we characterized a sample from the costal margin immunohistologically and through atomic force micr

Idiopathic hypoplastic right breast in a 60-year-old female: A case report.

Breast hypoplasia may be acquired or congenital as well as unilateral or bilateral, it may also be isolated or associated with other pathologies such as anomalies and syndromes of the chest wall like Jeune and Poland syndromes, pectus excavatum and pectus carinatum. Usually, nonsyndromic breast hypoplasia is idiopathic though some have known causes like direct effect from radiation treatment of cutaneous hemangiomas in childhood. Other iatrogenic causes may be due to thoracotomy, previous tube thoracostomy, impaired breast development postsurgical removal of benign or malignant breast tumors d

A novel EFNB1 mutation (c.712delG) in a family with craniofrontonasal syndrome and diaphragmatic hernia.

We report on the occurrence of congenital diaphragmatic hernia in a family with craniofrontonasal syndrome found to have a previously unreported mutation in EFNB1. The female proband presented with hypertelorism, telecanthus, bifid nasal tip, widow's peak, frontal bossing, and a widened metopic suture. Her father was noted to have hypertelorism, telecanthus, widow's peak, and a history of pectus carinatum. He was found to have a previously unreported mutation in exon 5 of EFNB1 predicted to cause premature protein truncation. The parents of the proband previously had a female fetus with congen

Musculoskeletal phenotypes in 3q29 deletion syndrome.

3q29 deletion syndrome (3q29del) is a rare genomic disorder caused by a 1.6 Mb deletion (hg19, chr3:195725000-197350000). 3q29del is associated with neurodevelopmental and psychiatric phenotypes, including an astonishing >40-fold increased risk for schizophrenia, but medical phenotypes are less well-described. We used the online 3q29 registry of 206 individuals (3q29deletion.org) to recruit 57 individuals with 3q29del (56.14% male) and requested information about musculoskeletal phenotypes with a custom questionnaire. 85.96% of participants with 3q29del reported at least one musculoskeletal ph

[Satisfaction levels and radiological improvement in patients with chest deformity after reconstructive surgery].

Many methods for surgical correction of anterior chest deformities has been described; the modified Ravitch's technique is the most performed. We reviewed the clinical reports of 15 patients who had corrected chest deformity from 1991 to 1999. We compared the photographies, CT images and Haller's pre and postsurgery indexes. The modified Ravitch's technique was performed in 14 cases and the Nuss's technique in one. A postoperative questionnaire was done to know the grade of satisfaction that the patients reported after surgery. Of 15 patients, 14 were male. The mean age at the moment of surger

Comparison of cardiovascular and skeletal features of primary mitral valve prolapse and Marfan syndrome.

The association of primary mitral valve prolapse (MVP) with thoracic bony abnormalities has led to the suggestion that MVP may be a forme fruste of the Marfan syndrome. Echocardiographic, skeletal and anthropometric findings in 59 subjects with primary MVP and 59 age- and sex-matched patients with Marfan syndrome were compared with those in 59 control subjects. Subjects with mitral prolapse were similar to control subjects and differed (p less than 0.025 to p less than 0.001) from the patients with Marfan syndrome in aortic root dimensions, height, arm span, upper/lower segment ratio and preva

Chest Wall Deformities and Congenital Lung Lesions: What the General/Thoracic Surgeon Should Know.

Pectus excavatum, carinatum, and arcuatum are 3 developmental chest wall deformities that may evolve during childhood and cause cardiac and/or pulmonary compression. Evaluation may include nonsurgical subspecialty consultations and imaging studies. Treatment may be nonoperative or surgical. Long-term follow-up studies have identified rare complications of traditional open repair. Routine in utero ultrasonography has led to increasing identification of congenital lung anomalies, including congenital cystic adenomatoid malformations, pulmonary sequestrations, and bronchogenic cysts. Short-term f

Jeune's syndrome (asphyxiating thoracic dystrophy): congenital and acquired.

Familial asphyxiating thoracic dystrophy (ATD), also known as Jeune's syndrome, is a rare autosomal recessive disorder with variable severity and multiple musculo-skeletal manifestations. Respiratory distress may be severe, resulting in death during infancy. Surgical repair techniques have typically involved median sternotomy (with graft interposition), resulting in poor outcomes. Acquired ATD may rarely result from impairment of chest wall growth following "open" (Ravitch-type) repair of pectus excavatum or carinatum deformities. Symptomatic patients may have profound restriction of pulmonary

Optical imaging versus CT and plain radiography to quantify pectus severity: a systematic review and meta-analysis.

BACKGROUND: Computed tomography (CT) and two-view chest radiographies are the most commonly used imaging techniques to quantify the severity of pectus excavatum (PE) and pectus carinatum (PC). Both modalities expose patients to ionizing radiation that should ideally be avoided, especially in pediatric patients. In an effort to diminish this exposure, three-dimensional (3D) optical surface imaging has recently been proposed as an alternative method. To assess its clinical value as a tool to determine pectus severity we conducted a systematic review in which we assessed all studies that compared

Repair of pectus chest deformities in 320 adult patients: 21 year experience.

BACKGROUND: Severe pectus chest deformities are common, often causing physiologic impairment. Patients who do not undergo repair during childhood often experience progressive worsening of symptoms during adulthood. There are few published reports regarding pectus repair in adults. METHODS: A retrospective review from January 1986 through January 2007 was performed on patients age 19 years and older, who underwent surgical correction of pectus excavatum (PE) or carinatum (PC) at one hospital. RESULTS: Adult patients, including 268 PE (84%), 41 PC (13%), and 11 with combined deformities (3%) u

[Minimally Invasive Technologies in Surgery of the Chest Wall in Children].

Today, in our country, as well as throughout the world, there is no single approach to the tactics of managing patients with deformations of the anterior chest wall. These patients constitute a large and heterogeneous group. The team acquired great experience in treating children with pectus excavatum and pectus carinatum, as well as a unique experience in the surgical treatment of pediatric thoracic outlet syndrome and sternum clefts. Now in the world there are a lot of surgical methods used for the correction of congenital deformities of the chest wall, however, the majority of them are extr

Macrocephaly with multiple epiphyseal dysplasia: a second example of Al Gazali-Bakalinova syndrome?

Four individuals from one consanguineous family affected with macrocephaly, multiple epiphyseal dysplasia and distinctive facies were presented by Al Gazali and Bakalinova in 1998 (1) as a distinct clinical entity. To the best of our knowledge, no other similarly affected family has been presented in the literature. Here, we present an affected macrocephalic male, from a consanguineous family, with facial anomalies, cranial MRI findings and multiple epiphyseal dysplasia findings. We believe this is the second family with a similar clinical spectrum and the same inheritance pattern as those of

Neurolysis for analgesia following pectus repair in a national cohort.

INTRODUCTION: Pectus excavatum and pectus carinatum are the most common chest wall deformities of childhood. Surgical repair can be complicated by post-operative analgesic challenges. Thoracic epidural analgesia, patient-controlled analgesia, and multimodal pain control are among the most common strategies. We sought to define the current utilization of intraoperative thoracic neurolysis, hypothesizing that this would minimize length of stay (LOS) and post-operative narcotic use with relatively higher proportion of non-narcotic post-operative analgesia. METHODS: We performed a retrospective r

Modified nuss procedure in concurrent repair of pectus excavatum and open heart surgery.

BACKGROUND: Pectus excavatum (PE) can be associated with congenital and acquired cardiac disorders that also require surgical repair. The timing and specific surgical technique for repair of PE remains controversial. The present study reports the experience of combined repair of PE and open heart surgery at Johns Hopkins Hospital. METHODS: A retrospective case review was conducted of all patients who presented for repair of PE deformity while undergoing concurrent open heart surgery from 1998 through 2011. RESULTS: A total of 9 patients met inclusion criteria. All patients had a connective t

Neonatal diagnosis of Schwartz-Jampel syndrome with dramatic response to carbamazepine.

We report a female infant, born at 30 weeks of gestation, who exhibited generalized myotonia, facial dysmorphism, blepharophimosis, and short stature at birth. These clinical findings, along with abnormal electromyogram and muscle biopsy, are consistent with Schwartz-Jampel syndrome. Our patient, diagnosed at 4 weeks of life, lacks the major skeletal anomalies, such as pectus carinatum, congenital hip dysplasia, and bowing of the long bones usually associated with this syndrome. Treatment with carbamazepine, initiated at age 7 months (corrected age of 5 months) has produced marked and continue
