Evaluation of the walking pattern in clubfoot patients who received early intensive treatment.

The walking pattern in a group of nine adult male subjects who had received early intensive treatment for congenital clubfoot was evaluated and compared to the walking pattern in a control group of 15 adult healthy male subjects. All subjects were filmed with a five-camera video system as they walked across two force plates. A three-dimensional inverse dynamics approach was used to calculate average joint angles, moments, power, and work. The results showed that there were no differences in the joint angles between the two groups. The overall patterns of the joint moments were very similar in

[The metatarsophalangeal articulation and the transverse metatarsal ligament: anatomical observations].

The authors conducted a macroscopic anatomical study of the metatarsophalangeal joints of the middle toes in order to add to our knowledge of the anatomy and physiology of this structure. Anatomical observations reveal the foot's lack of transverse ligamentous formations (as the sagittal bundle in the hand) connecting the extensor tendon to the glenoid plate. Consequently, the containment of the extensor tendon above the metatarsophalangeal joint is considerably less stable in the foot than in the hand, since it is entrusted to the posterior portion of the interossei muscles and the deep and s

Biomechanical analysis of drop and countermovement jumps.

For 13 subjects the performance of drop jumps from a height of 40 cm (DJ) and of countermovement jumps (CMJ) was analysed and compared. From force plate and cine data biomechanical variables including forces, moments, power output and amount of work done were calculated for hip, knee and ankle joints. In addition, electromyograms were recorded from five muscles in the lower extremity. The results obtained for DJ appeared to depend on jumping style. In a subgroup of subjects making a movement of large amplitude (i.e. bending their hips and knees considerably before pushing off) the push-off pha

A rare combination of brachymetatarsia and congenital hallux varus: case report and review of the literature.

Hallux varus is defined as a medial deviation of the phalanx at the first metatarsophalangeal joint and can be congenital or acquired. Brachymetatarsia is defined as shortening of the metatarsal bones. A combination of hallux varus and brachymetatarsia is rare. A 15-year-old girl presented to our outpatient clinic complaining of problems with her feet. A distinctive hallux varus was present bilaterally combined with a brachymetatarsia of the first metatarsals. The patient reported discomfort. She was restricted in her activities and had severe psychological strain owing to the deformity. We de

Muscle power compensatory mechanisms in below-knee amputee gait.

OBJECTIVE: This three-dimensional and bilateral gait study on five below-knee amputees was undertaken to demonstrate the following: (1) how hip muscle powers can compensate for the lack of ankle function on the amputated side; and (2) how these compensatory mechanisms can influence muscle power activities in the sound limb. DESIGN: Gait data were assessed by an eight-camera high-speed video system synchronized to two force plates. The three-dimensional mechanical muscle powers were calculated at the joints of the lower limbs. Significant differences between each limb were determined using the

[Free transplantation of the 2d toe. Technical features, problems and solutions].

On the basis of seven transfers of the second toe and one "en bloc" transfer of the second and third toe, technical features are discussed. The use of a plate for osteosynthesis is proposed. It has been found very useful, particularly when a MP-arthrodesis is performed between the first metacarpal and the second toe. It gives stability and therefore secondary displacement and/or rotation of the toe are avoided. It enables more accurate tension to be placed on the repaired tendons. For the flexor tendons it is suggested to repair the communis longus only, whereas both extensors, longus and brev

Estimating tibial bone load during running using only inertial measurement units on the tibias and sternum: a Centre of Pressure approach.

Inverse dynamics is a method to estimate joint forces and external moments needed for movement by analysing kinematics and ground reaction forces (GRF). In a bottom-up inverse dynamics analysis using a full-body inertial measurement unit (IMU) setup, the Centre of Pressure (CoP) is the only missing variable to complete the calculation. This study aimed to estimate the anteroposterior CoP from the tibia IMU orientation to calculate the sagittal ankle moment and tibial bone load (TBL) in rearfoot strikers running at 2.5, 3.1, and 3.6 m/s, using both tibia and the sternum IMU. This achieved stron

Spinal pharmacology of thermal hyperesthesia induced by constriction injury of sciatic nerve. Excitatory amino acid antagonists.

This study evaluated the effects of spinally administered excitatory amino acid antagonists on the thermal hyperesthetic state induced by unilateral partial ligation of the sciatic nerve in the rat. The measured response was the latency to paw withdrawal of each hind paw after application of a focused heat lamp on the plantar surface of the paw through a glass plate upon which the animal stood. In this work, antagonists (MK801, DL-2-amino-5-phosphonovalerate, ketamine) of the N-methyl-D-aspartate receptor (NMDA), the glycine potentiation site at the NMDA receptor (5-chloro-indole-2-carboxylic

Individuals with unilateral chronic ankle instability exhibit deficits in bilateral postural control, muscle strength and proprioception.

To investigate whether people with unilateral chronic ankle instability (CAI) have deficits in bilateral static and dynamic postural control, ankle muscle strength and ankle proprioception. A total of 31 participants with CAI and 31 non-CAI controls were recruited. Static postural control was assessed using root mean square (RMS) of the plantar centre of pressure (CoP) displacements during single-leg stance. Dynamic postural control was evaluated by measuring the time to stabilisation (TTS) following a single-leg drop onto a force plate. Ankle proprioception and muscle strength were measured u

Differences in Plantar Pressure Distribution Between Adults with Asymptomatic and Symptomatic Flexible Flatfeet During Walking and Heel-Strike Running.

Flatfeet involve a collapse of the medial longitudinal arch, hindfoot valgus, and forefoot abduction. Flexible flatfoot is the most common type and can often be corrected with physiotherapy or orthotics. While some individuals remain asymptomatic, others develop symptoms for reasons that are not fully understood. This cross-sectional study compared plantar pressure distributions in 16 adults with asymptomatic and 16 with symptomatic flexible flatfeet (FPI-6 > 6; navicular drop > 5 mm), using a resistive-sensor-equipped pressure plate during walking and heel-strike running. During walking, symp

Neuromuscular fatigue and cognitive constraints independently modify lower extremity landing biomechanics in healthy and chronic ankle instability individuals.

The purpose was to determine the impact of both cognitive constraint and neuromuscular fatigue on landing biomechanics in healthy and chronic ankle instability (CAI) participants. Twenty-three male volunteers (13 Control and 10 CAI) performed a single-leg landing task before and immediately after a fatiguing exercise with and without cognitive constraints. Ground Reaction Force (GRF) and Time to Stabilization (TTS) were determined at landing in vertical, anteroposterior (ap) and mediolateral (ml) axes using a force plate. Three-dimensional movements of the hip, knee and ankle were recorded dur

Role of the NLRP3 inflammasome in a model of acute burn-induced pain.

The NLRP3 inflammasome is a multi-protein complex that assembles in response to tissue damage or infection, triggering activation of caspase-1, an enzyme that converts interleukin (IL)-1β into its active form. A role for the NLRP3 inflammasome is emerging in inflammatory pain, but its influence in other pain types is largely unexamined. Therefore the aim of this study was to assess the role of the NLRP3 inflammasome and its downstream product caspase-1 in a model of acute burn-induced pain in male mice. A superficial burn was induced on the plantar surface of the left hind paw using a hot plat

The load applied to the foot in a patellar ligament-bearing cast.

The purpose of this study was to determine whether a patellar ligament-bearing cast reduces the load applied to a foot in a cast. In a study of ten people who had no history of gait abnormalities, disease involving the motor system, or deformities of the lower extremities, we compared the load applied to the plantar aspect of a foot in a cast (as detected with F-Scan computer-monitored pedobarographic sensors) with the total load that an extremity in a cast receives relative to the ground (as detected with force-plates). Six trials were completed three times by each person. The trials consiste

Correlations between weight-bearing 3D bone architecture and dynamic plantar pressure measurements in the diabetic foot.

BACKGROUND: Measurements of plantar loading reveal foot-to-floor interaction during activity, but information on bone architecture cannot be derived. Recently, cone-beam computer tomography (CBCT) has given visual access to skeletal structures in weight-bearing. The combination of the two measures has the potential to improve clinical understanding and prevention of diabetic foot ulcers. This study explores the correlations between static 3D bone alignment and dynamic plantar loading. METHODS: Sixteen patients with diabetes were enrolled (group ALL): 15 type 1 with (N, 7) and without (D, 8) d

Deep transverse metatarsal ligament and static stability of lesser metatarsophalangeal joints: a cadaveric study.

BACKGROUND: The static support that guides motion around the lesser metatarsophalangeal joints (MTPJs) is complex. Biomechanical studies revealed important roles of both the plantar plane and collateral ligaments. Since part of the plantar plate is attached to the deep transverse metatarsal ligament (DTML), we hypothesized that the transection of the DTML in the intermetatarsal space may substantially reduce the MTPJ stability. METHODS: The second, third, and fourth MTPJ stabilities of 6 fresh-frozen human cadaveric foot specimens were measured under load control. Both dorsiflexion and dorsal

Comparative outcomes of conservative, steinmann pin, and plate fixation in calcaneal fractures: a subtype-based evaluation according to the essex-lopresti classification.

BACKGROUND: Calcaneal fractures are the most common tarsal fractures and often result in long-term disability. Although various treatment options exist, but the relationship between Essex-Lopresti subtypes, treatment methods, and dynamic functional recovery remains unclear. METHODS: This retrospective study included 66 patients with intra-articular calcaneal fractures, treated between 2011 and 2021. Fractures were categorized according to the Essex-Lopresti classification (1 A-1 C, 2 A-2 C) and managed by conservative treatment, Steinmann pin fixation, or plate fixation. Functional outcomes w

Lesser-toe deformity.

Lesser-toe deformity is frequent and varied, with severe functional impact. In elderly subjects, it leads to loss of autonomy and increases the risk of falls. The aim of the present study was to provide an update on management, addressing 5 questions. What are the normal anatomy and pathophysiology? These acquired deformities mainly result from imbalance between the intrinsic and extrinsic muscles of the foot or from capsule-ligament stabilizer failure. How to analyze the deformity? It is important to identify the cause, site, reducibility and metatarsophalangeal joint stability. What are the

Muscle Force Patterns in Lower Extremity Muscles for Elite Discus Throwers, Javelin Throwers and Shot-Putters - A Case Study.

Optimal release variables, as well as the kinematics and kinetics of athletes, are crucial for the maximization of throwing distance in athletics. Mathematical models and simulations allow throwing techniques to be studied. However, muscle force patterns and the contribution of specific muscle groups in athletics throwing events are not well understood and require detailed research. In this study, important variables of the muscle force generated during the javelin, discus and shot put events were determined using OpenSim software. Musculoskeletal simulations were carried out based on kinemati

[The Primary Metatarsalgia: Pathogenesis, Biomechanics and Surgical Treatment].

This paper presents a comprehensive review on the current concept of the diagnosis and treatment of central metatarsalgia on the basis of medical literature analyses. Metatarsalgia is the term for pain in the forefoot. This is a set of symptoms corresponding to a wide range of diseases. Central metatarsalgia is a kind of metatarsalgia which arises from structural-functional changes that lead to excessive pressure in the area of metatarsal heads. The data analysis demonstrated that presently various types of osteotomies of metatarsal bones are the main surgical treatment options with the chance
