Pregnancies and Neonatal Outcomes in Patients with Sickle Cell Disease (SCD): Still a (High-)Risk Constellation?

BACKGROUND: This monocentric study conducted at the University Hospital of Essen aims to describe maternal and fetal/neonatal outcomes in sickle cell disease (SCD) documented between 1996 to 2021 (N = 53), reflecting the largest monocentric analysis carried out in Germany. METHODS/RESULTS: 46 pregnancies in 22 patients were followed. None of the patients died. In total, 35% (11/31) of pregnancies were preterm. 15 pregnancies in eight patients were conceived on hydroxycarbamide (HC), of which nine had a successful outcome and three were terminated prematurely. There was no difference regarding

Identification of High-Risk Pregnancies in a Remote Setting Using Ambulatory Blood Pressure: The MINDI Cohort.

Background: Ambulatory blood pressure is a potential tool for early detection of complications during pregnancy, but its utility in impoverished settings has not been assessed. This cross-sectional study aimed to determine whether maternal infections, nutrient deficiencies and inflammation (MINDI) were associated with four measures of maternal blood pressure (BP) and to determine their association with symphysis-fundal-height (SFH). Methods: Environmental and dietary factors, intake of iron and a multiple-nutrient supplement (MNS), markers of inflammation, protein, anemia, folate, vitamins B12

Adverse pregnancy outcomes in deliveries prior to, at and beyond 39 weeks; low- and high-risk women.

BACKGROUND: Hypertensive disorders are associated with maternal and neonatal complications. Though they are more common in women with history of prior preeclampsia, they can occur in uncomplicated pregnancies. OBJECTIVE: To determine the proportion of adverse pregnancy outcomes in deliveries prior to or at ≥39 weeks, in uncomplicated singleton nulliparous women (low-risk), as well as women with history of preeclampsia in a prior gestation (high-risk). STUDY DESIGN: This was a secondary analysis from the multicenter trials of low dose aspirin for preeclampsia prevention in low and high-risk p

Noninvasive prenatal testing: more caution in counseling is needed in high risk pregnancies with ultrasound abnormalities.

OBJECTIVE: Non-invasive prenatal testing (NIPT) is increasingly being used in prenatal aneuploidy screening. The objective of this study was to assess the positive predictive value in our cohort of 68 cases with positive NIPT result. In addition, we wondered if the use of NIPT in cases with ultrasound abnormalities is appropriate, given the limited number of chromosomes investigated. DESIGN: We performed confirmative invasive testing using karyotyping, fluorescence in situ hybridization (FISH) and/or high-resolution chromosomal microarray analysis. RESULTS: In line with the published data, t

Case reports of the use of immunoadsorption or plasma exchange in high-risk pregnancies of women with antiphospholipid syndrome.

Conventional treatment of antiphospholipid syndrome (APS) pregnancies with aspirin and/or heparin is sometimes unable to counteract maternal and/or fetal complications. In this article we report the cases of two patients who were unresponsive to conventional treatment for APS during their first pregnancy, and who were treated in the following pregnancy with plasma exchange and immunoadsorption respectively, in addition to conventional therapy. Both patients had a history of thrombotic events, a previous pregnancy loss at the 11th week of gestation and the same antiphospholipid antibody profile

Adverse obstetric outcome in low- and high- risk pregnancies: predictive value of maternal serum screening.

OBJECTIVE: To determine whether the relationship between adverse pregnancy outcome and elevated maternal serum alpha-fetoprotein (MSAFP) and/or maternal serum hCG levels in women whose fetuses have no chromosomal abnormalities or neural tube defects is restricted to pregnancies with a priori elevated risk for pathology or also present in low-risk pregnancies. METHODS: The outcomes of pregnancy in two groups of patients with elevated MSAFP and/or maternal serum hCG values were compared with the outcomes of a reference group with normal serum values. The first study group consisted of 83 women

Mechanical Thrombectomy for High-Risk Pulmonary Embolism in Pregnancy: A Case Report and a Systematic Review of the Literature.

Pulmonary embolism (PE) in pregnancy is a major clinical challenge with potentially fatal maternal and fetal outcomes. Mechanical thrombectomy (MT) offers a promising alternative when thrombolysis is contraindicated, although evidence in pregnancy is limited. We describe a 26-year-old at 29 + 5 weeks' gestation with high-risk PE causing acute right heart failure, successfully treated, achieving rapid hemodynamic recovery, and minimal fetal radiation exposure. A systematic review identified 15 MT cases in pregnancy or peripartum (11 antepartum, 4 postpartum), reporting 100% maternal survival an

Improving care for women with high blood pressure in pregnancy to reduce future risk of heart disease.

BACKGROUND: Hypertensive disorders of pregnancy (HDP) are associated with increased risk of future cardiovascular disease and widening health inequalities, and are more prevalent in women of Black African and Caribbean heritage. Previous studies have highlighted the importance of patient education and lifestyle modification for cardiovascular risk reduction, yet awareness of increased risk remains low among healthcare professionals, and studies exploring patient and healthcare professional perspectives regarding risk reduction are lacking. AIM: This qualitative research aimed to explore lived

[Reducing the Invalid Alarm Rate for Fetal Heartbeat Monitoring in High-Risk Pregnancies in the Delivery Room].

BACKGROUND & PROBLEMS: Fetal heartbeat (FHB) monitoring is used to monitor the condition of a fetus in the uterus and estimate the expected time of delivery. It is crucial to accurately and comprehensively monitor fetal heart beats in high-risk pregnancies. However, invalid alarms frequently occur in clinical environments, easily promoting alarm fatigue among nurses and causing anxiety and distrust in the laboring women and their families. Health professionals may ignore valid alarms due to alarm fatigue and miss the opportunity to save a fetus' life. A survey in our hospital identified invali

First Balkan Brief Illness Perception Questionnaire (IPQ-B) among high-risk pregnancies.

BACKGROUND: Pregnancy is a particularly delicate period in which many health-related changes lead to changes in the perception of women's well-being, in both physiological and especially high-risk pregnancies. In high-risk pregnancies, the relationship between illness perception and general well-being is even more complicated as sometimes there might not be any apparent signs or symptoms, but the pregnant woman or foetus still might be at risk. AIMS: To assess the validity and reliability of the existing Serbian version of the Brief Illness Perception Questionnaire (B-IPQ) in a specific popul

Plasma cell-free DNA as a monitoring tool for high-risk pregnancies associated with antiphospholipid syndrome.

INTRODUCTION: Despite thromboprophylaxis, women with antiphospholipid syndrome (APS) face high-risk pregnancies due to proinflammatory and prothrombotic states. This highlights the need for new monitoring and prognostic tools. Recent insights into the pathophysiological role of neutrophil activation and extracellular trap (NET) formation in this syndrome led to the exploration of plasma cell-free DNA (cfDNA), a derivative of NETosis, as a promising biomarker. MATERIALS AND METHODS: cfDNA was isolated and quantified from plasma samples of healthy pregnant women (control group, HC) and women wi

Ductus venosus opens in high-risk pregnancies without signs of increased central venous pressure.

OBJECTIVES: It has been belived that changes in diastolic blood velocities in the fetal ductus venosus were due to increased central venous pressure secondary to increased fetal heart strain during hypoxia or heart failure. There have been recent reports of changes in ductus venosus blood velocity without signs of increased fetal heart strain. The aim of this evaluation was to compare blood velocity in the right hepatic vein as a marker of increased central venous pressure in relationship to changes in ductus venosus blood velocity. MATERIAL AND METHODS: Fifty pregnancies suspected of fetal g

Accounts of women identified as drinking at 'high risk' during pregnancy: A meta-ethnography of missing voices.

BACKGROUND: Drinking alcohol during pregnancy is associated with Foetal Alcohol Spectrum Disorders (FASD), and women who drink at higher levels are more likely to have a baby with FASD. Public health responses focus on population-level approaches to FASD prevention such as promoting abstinence and alcohol brief interventions. Efforts to better understand and respond to 'high risk' drinking during pregnancy have been largely ignored. This meta-ethnography of qualitative research aims to inform this policy and practice agenda. METHODS: Ten health, social care, and social sciences databases were

Role of Ultrasonography and Color Doppler in the Assessment of High-Risk Pregnancies and Their Accuracy in Predicting Fetal Outcome.

Introduction Various methods are employed to evaluate the well-being of the fetus in high-risk pregnancies which consists of a biophysical profile (BPP), a non-stress test (NST), and daily fetal movements. Detection of aberrant blood flow in fetoplacental beds has been revolutionized by recent developments in ultrasound technology, such as color Doppler flow velocimetry. The cornerstone of maternal and fetal care is lowering maternal and perinatal mortality and morbidity is antepartum fetal surveillance. Doppler ultrasound is a non-invasive way of obtaining a qualitative and quantitative evalu
