Perspectives on psychotherapy in Southeast Asia: Insights from psychiatrists and trainees (Part I: Attitudes, current practices and training preferences).

INTRODUCTION: Psychotherapy is an evidence-based treatment for mental disorders. Despite its global development, neuroscientific advances, and measurable efficacy, attitudes toward psychotherapy in Southeast Asia (SEA) remain underexplored, particularly among psychiatrists and psychiatric trainees. OBJECTIVES: This study aimed to assess attitudes toward psychotherapy, its current use in clinical practice, and stated preferences for future training among psychiatrists and trainees in SEA. METHODS: A cross-sectional online survey was conducted across Indonesia, Malaysia, Singapore, Thailand, a

Self-guided virtual reality therapy for social anxiety disorder: a study protocol for a randomized controlled trial.

BACKGROUND: Social anxiety disorder (SAD) is a highly prevalent mental disorder associated with enormous stress and suffering. Cognitive behavior therapy (CBT) is the first-line treatment for SAD, yet its accessibility is often constrained with long waiting times. Digital therapeutic applications, including psychoeducation and self-guided behavioral experiments in virtual reality (VR), could facilitate access and reduce waiting times. The study aims to investigate if ultra-short-time therapy involving self-guided digital therapeutic applications with VR components can reduce the severity of SA

Work-focused treatment of common mental disorders and return to work: a comparative outcome study.

The aim of this study was to compare the effectiveness of two individual-level psychotherapy interventions: (a) treatment as usual consisting of cognitive-behavioral therapy (CBT) and (b) work-focused CBT (W-CBT) that integrated work aspects early into the treatment. Both interventions were carried out by psychotherapists with employees on sick leave because of common mental disorders (depression, anxiety, or adjustment disorder). In a quasi-experimental design, 12-month follow-up data of 168 employees were collected. The CBT group consisted of 79 clients, the W-CBT group of 89. Outcome measur

Preferences for treatment and provider characteristics in outpatient psychotherapy: Results from a discrete choice experiment.

OBJECTIVE: Diagnosis rates of depression and demand for psychotherapy are increasing, posing significant challenges to the mental health care system. Availability of different treatments enables patients to express their preferences. Understanding these preferences can inform policy makers and clinicians and holds the potential to increase treatment adherence and improve clinical outcomes. METHOD: A web survey was carried out among German adults (18-74) with psychotherapy experience, using quota sampling to reflect the gender distribution of psychotherapy-seeking adults. We conducted a discre

How to Define Psychological Therapy and Psychotherapy? - An Interdisciplinary Proposal.

BACKGROUND: The definition of psychological treatments and psychotherapy has various implications for communication within research and clinical care, as well as for legal issues - particularly in countries with psychotherapy acts. It can define what should be covered by public healthcare systems and who should be permitted to provide these services. METHOD: We assembled a group of psychotherapists in Germany to develop an inclusive, scientific definition of psychotherapy. This definition should serve as an umbrella that is not limited to traditional treatment schools and is suitable for scie

Increasing the Therapeutic Dosage by Combining Cognitive Behavior Group Therapy With a Monitored Self-Help Group.

Objective: A problem in psychotherapy is the limited availability of psychotherapists. This can not only delay the start of therapy but also curtail the number of sessions and the therapeutic dosage. An option to extend the therapeutic dose without an increase in therapist time may be to combine group psychotherapy with a self-help group. The goal of the present study is to investigate how patients judge this treatment mode and to what degree the contents of a regular group treatment are pursued in a parallel self-help group.Method: Seventy-two psychosomatic inpatients participated in cognitiv

Posttraumatic stress disorder symptoms and mental health treatment parameters among Asian Indians residing in the United States.

OBJECTIVE: Asian Indians in the United States experience psychological impacts from traumatic experiences, such as posttraumatic stress disorder (PTSD) symptoms. However, Asian Indians underutilize mental health services that are attributed to culturally contextualized barriers and preferences for seeking mental health services. To advance this literature, we uniquely examined associations of PTSD symptom severity with institutional and internal help-seeking barriers for mental health services and with psychotherapy preferences. METHOD: Participants were a community sample of 77 trauma-expose

Pregorexia and its significance for physiological processes in the fetus - a review of current knowledge.

Anorexia occurring during pregnancy can have a devastating effect on the pregnant woman's physiological clinical picture, mental health, and fetal development. This is because eating behaviors, the fetal programming process, and behavioral-cognitive relationships all contribute to shaping intrauterine conditions. Dysfunction at the neural level predisposes to a disturbed relationship with food, reduced self-acceptance, perinatal depression, and a negative perception of body image. The health consequences of reduced energy intake in the course of anorexia during pregnancy contribute to the mani

Other Primary Headache Disorders.

PURPOSE: This article provides an overview of a diverse group of primary headache disorders that are categorized in the International Classification of Headache Disorders, 3rd Edition (ICHD-3), as "other primary headache disorders." This article provides clinicians with a distilled understanding of the diagnoses and their epidemiology, pathophysiology, and management. RECENT FINDINGS: Cough-induced headache requires neuroimaging to exclude posterior fossa pathology and recently has been reported as a common symptom in patients with CSF-venous fistula. Clinical overlap is observed between pati

Therapist effects on outcome and alliance in inpatient psychotherapy: The contribution of patients' symptom severity.

Therapists differ in their effectiveness regarding their patients' symptomatic improvement as well as their therapeutic alliances, but little is known about influencing factors. The current study aimed to investigate therapist effects on outcome and alliance in inpatient psychotherapy and to estimate the influence of patient severity on the magnitude of therapist effects in this specific setting. We examined differences between individual psychotherapists in their patients' early alliance and outcome at discharge in a psychotherapy setting of a psychiatric hospital. The sample included 33 ther

Examining therapist comfort in delivering family therapy in home and community settings: development and evaluation of the Therapist Comfort Scale.

This study reports on the development and psychometric properties of a new measure assessing therapist comfort in the home treatment context and the relationship between therapist comfort, related process variables, and therapist characteristics. Data were drawn from a longitudinal evaluation of 185 families treated by 51 therapists using Multisystemic Therapy (MST). Therapist comfort was measured at four time points. Psychometric evaluation indicated that the measure was internally and temporally consistent. Examination of the measure's validity indicated that therapists' feelings of safety a

Recovered-memory therapy and robust repression: influence and pseudomemories.

A subset of the psychotherapists practicing trauma-focused therapy predicate their treatment on the existence of a newly claimed, powerful form of repression that differs from repression as used in the psychoanalytic tradition and from amnesia in any of its recognized forms. Recovered-memory specialists assist patients to supposedly retrieve vast quantities of information (e.g., utterly new dramatic life histories) that were allegedly unavailable to consciousness for years or decades. We refer to the hypothesized mental mechanism as "robust repression" and call attention to the absence of evid

The single parent: power and the integrity of parenting.

I have explored some issues of power and the balance of power in the situation of the single parent and the family. The single parent may need the support of a holding environment to achieve psychological integration as exemplified by my treatment with Ms. B. Without a cooperative mate and with societal prejudice, the psychotherapeutic situation may be the only nurturing environment. The therapist empowers the single parent by providing a supportive, nurturing environment. Parenting is receiving bad press in this decade. More examples of child abuse and the extreme of homicide come to public a

Therapists' perspectives on optimal treatment for pathological narcissism.

This study used Q methodology to explore clinicians' perspectives regarding optimal psychotherapy process in the treatment of pathological narcissism, a syndrome of impaired self-regulation. Participants were 34 psychotherapists of various disciplines and theoretical orientations who reviewed 3 clinical vignettes portraying hypothetical cases of grandiose narcissism, vulnerable narcissism, and panic disorder without pathological narcissism. Participants then used the Psychotherapy Process Q set, a 100-item Q-sort instrument, to indicate their views regarding optimal therapy process for each hy

Self-compassion in depression: associations with depressive symptoms, rumination, and avoidance in depressed outpatients.

Self-compassion involves being kind to oneself when challenged with personal weaknesses or hardship and has been claimed to be associated with resilience in various areas. So far, there are only a handful of studies that investigate self-compassion and its relation to clinical depression. Therefore, the principal goals of the present study were (a) to compare self-compassion in clinically depressed patients and never-depressed subjects, (b) to investigate self-compassion and its relation to cognitive-behavioral avoidance and rumination in depressed outpatients, and (c) to investigate ruminatio

Disparities in Virtual Outpatient Mental Health Care Utilization Among Commercially Insured Children and Adults, 2023-2025.

INTRODUCTION: Early COVID-19 pandemic-era studies documented rapid adoption of telemental health, but current utilization patterns and disparities, particularly among children and young adults, remain poorly characterized since the COVID-19 Public Health Emergency ended in May 2023. The objective of this work was to characterize current utilization of virtual versus in-person outpatient mental health care in a large national cohort of commercially insured children and adults and to assess variation by patient, provider, clinical, and geographic characteristics. METHODS: In this retrospective

Digital Acceptance and Commitment Therapy for the Treatment of Chronic Pain: Findings of a Randomized Controlled Efficacy Trial.

BACKGROUND: People with chronic pain are confronted with limited access to established forms of treatment. Acceptance and commitment therapy (ACT) has been found to be an effective extension of cognitive behavioral therapy (CBT) for chronic pain. In this study, we evaluated the ACT-based online therapy program HelloBetter Chronic Pain in a clinical sample of patients with chronic pain in Germany. METHODS: In a randomized, controlled, and investigator-blinded trial, the program was tested against a treatment-as-usual control group (waiting list). A model training curriculum for psychedelic, psycholytic, and entactogen-assisted psychotherapy.

The authors offer a model for curriculum for education and training in substance-assisted psychotherapy (SAP), that is, psychedelic, psycholytic, and entactogen/MDMA (3,4-methylenedioxymethamphetamine)-assisted psychotherapy, addressing both the detailed contents of training and the question of experiential training. All authors of this model have an abiding interest and extensive experience in both the theory and practical aspects of SAP and questions relating to training. The model curriculum has been written through an international consensus building process and represents a consensus stat

Exploring the Role of Guilt in Eating Disorders: A Pilot Study.

BACKGROUND/OBJECTIVES: Eating disorders (EDs) are complex psychopathological conditions involving dysfunctional eating behaviors, excessive body image concerns, and impaired emotional regulation. Among moral emotions, guilt plays a significant role in ED dynamics, influencing both symptomatology and interpersonal relationships. This study examines specific guilt subtypes (normative and altruistic guilt) using a specific psychometric tool. METHODS: Forty-three adults with anorexia nervosa (AN), bulimia nervosa (BN), or binge eating disorder (BED) were recruited from the Eating Disorder Center
