A middle-aged woman with back and flank pain.

Xanthogranulomatous pyelonephritis is an uncommon variant of chronic pyelonephritis that predominantly affects middle-aged women. Patients usually present with fever, back or flank pain, flank mass, and the constitutional symptoms of fatigue, malaise, weight loss, and anorexia. Rarely, they may present with a draining sinus. There is usually a history of urinary tract infection, obstruction, or instrumentation. Other abnormalities include anemia, leukocytosis, abnormal liver enzymes, pyuria, and hematuria. Mild azotemia may be present, but frank renal failure is rare. Urine and renal tissue cu

Renal actinomycosis presenting as uro-cutaneous fistula.

Renal actinomycosis is a rare clinical entity. Diagnosis is usually made after resection. A 36-year-old male presented with uro-cutaneous fistula and left xanthogranulomatous pyelonephritis. He was offered left open radical nephrectomy. Intra-operatively, there was "woody" inflammation of the left kidney fistulizing to the splenic flexure of the colon. We successfully resected it and a segment of the colon that had fistulized. His tissue cultures grew Actinomyces odontolyticus. Post-operatively, he received 6 weeks of intravenous beta-lactam antibiotic. He recovered well without any complicati

Idiopathic renal replacement lipomatosis: a case report and review of literature.

Idiopathic renal replacement lipomatosis is a rare condition in which the renal parenchyma, especially the renal sinus is replaced with adipose tissue with increase in perirenal fat. We report a case of renal replacement lipomatosis presenting as a renal mass clinically. The computed tomography scan revealed low density and nodular masses of adipose tissue within the renal parenchyma. The differential diagnosis before surgery was angiomyolipoma, lipoma, and xanthogranulomatous pyelonephritis. After the histological examination and extensive literature search, the diagnosis of renal replacement

Lymphoepithelioma-like carcinoma of the renal pelvis.

We report the first documented case of undifferentiated carcinoma of the renal pelvis with a prominent lymphoid stroma (lymphoepithelioma-like carcinoma [LELC]). LELCs are morphologically identical to nasopharyngeal carcinoma and are rarely seen in the urinary tract, with only isolated cases involving the urinary bladder and ureter. The tumor was composed entirely of large pale staining malignant epithelial cells with ill-defined borders arranged in syncytial sheets separated by mainly reactive lymphocytes, occasional plasma cells and histiocytes. Tumor cells were immunoreactive to cytokeratin

Simple Nephrectomy for Non-Functioning Kidney: An Institutional Experience with Non-Neoplastic and Incidentally Detected Neoplastic Lesions.

Introduction. Chronic kidney diseases are attributed to a variety of etiologies, including diabetes mellitus, hypertension, vascular diseases, cystic kidney diseases, chronic tubulo-interstitial diseases, chronic pyelonephritis, and glomerular diseases. Simple nephrectomy is indicated when the loss of renal function in a unilateral non-functional kidney is permanent in a persistently symptomatic patient. Uncommonly, a non-functional kidney may develop neoplasm unsuspected pre-operatively and be diagnosed only during histopathological examination of nephrectomy specimen. Material and methods. W

Idiopathic renal replacement lipomatosis: a rare renal pseudo tumor.

Renal replacement lipomatosis (RRL) is a rare condition that occurs as an end result of renal atrophy or destruction of renal parenchyma by excessive lipomatous tissue in renal sinus and perinephric space. We report a case of RRL presenting as a right renal mass clinically. Intra-venous pyelography and renal scan revealed a left non-functioning kidney. A left nephrectomy was performed. After histopathological examination and extensive literature search, a diagnosis of RRL was given. In majority of cases, it is associated with renal calculi. Idiopathic variety is not common. It is a rare entity

Ureaplasma urealyticum infection presenting as pyelonephritis and perinephric abscess in an immunocompromised patient.

We report a case of an immunosuppressed 67-year-old woman who presented with fever of unknown origin. Further investigation revealed multiple left renal and perinephric abscesses. These were managed with percutaneous drainage and broad-spectrum antibiotics; however, no clinical improvement resulted. No organism was identifiable on standard microscopy and culture of all drain, blood and urine samples taken. Left nephrectomy with right urinary diversion was performed for sepsis management and to protect the patient's right kidney. Eventually, Ureaplasma species' RNA was detected in the patient's

Sorting the Alphabet Soup of Renal Pathology: A Review.

Diseases of the kidney often have their names shortened, creating an arcane set of acronyms which can be confusing to both radiologists and clinicians. This review of renal pathology aims to explain some of the most commonly used acronyms within the field. For each entity, a summary of the clinical features, pathophysiology, and radiological findings is included to aid in the understanding and differentiation of these entities. Discussed topics include acute cortical necrosis, autosomal dominant polycystic kidney disease, angiomyolipoma, autosomal recessive polycystic kidney disease, acute tub

Computed tomography of ileal loop urinary diversion in adults.

Surgically-placed urinary conduits remain the most common form of long-term, supravesical urinary drainage. We reviewed CT scans of 16 patients with ileal loops examined during a 3-year period to assess normal loop anatomy and associated pathological conditions. Computed tomography was indicated when conventional studies were considered inadequate. Indications for studies were suspected tumor recurrence in 12 (75%) and abscess localization in four (25%). The normal CT ileal loop lies medial to the cecum at the level of the sacral promontory. In 12 patients, CT identified abnormalities related

Isolated renal mucormycosis in a healthy immunocompetent patient: atypical presentation and course.

Isolated renal mucormycosis is rarely identified and has been described in only a handful of cases. We hereby report a case of isolated renal mucormycosis with an atypical presentation in an immunocompetent patient with no identifiable risk factors. A 30-year-old nondiabetic male presented with a poorly functioning right kidney with minimal constitutional symptoms. The patient underwent a right simple nephrectomy. Histopathology revealed necrotizing xanthogranulomatous pyelonephritis with mucormycosis. The postoperative period was uneventful and the patient was managed without any antifungal a

Colorenal fistula and urinary tract infection in a pediatric patient with acute lymphoblastic leukemia.

INTRODUCTION: Colorenal fistula is rare in the pediatric population. It may occur at any segment involved by ischemia, chronic inflammation, or necrosis. It is typically associated with a preliminary renal lesion that may arise as a result of interventional procedures, inflammatory conditions, colon tumor, and xanthogranulomatous pyelonephritis, among others. CASE REPORT: 15-year-old female patient diagnosed with acute lymphoblastic leukemia admitted at our institution for baseline condition management. During her stay, she experienced gastrointestinal and urinary infectious events. In the as

Inflammatory malignant fibrous histiocytoma of kidney: a case report.

Among the renal sarcomas, inflammatory malignant fibrous histiocytoma (MFH) is an extremely rare presentation. A 45-year-old woman presented with acute retention urine following an episode of gross hematuria. Computerized tomography showed a solid mass at the lower pole of the left kidney. The patient underwent left nephrectomy. Histologically and immunohistochemically, the tumor was diagnosed as an inflammatory subtype of MFH. Histological appearances of inflammatory MFH vary widely and frequently overlap with benign reactive conditions such as Xanthogranulomatous pyelonephritis (XGPN) and ma

[Epidermoid carcinoma of the kidney].

Epidermoid carcinoma of the kidney is a rare malignant tumor, characterized by high invasiveness, poor prognosis, and the association of renal stone and renal infection. Very little reports about its sonographic appearance were mentioned in the literature. We present a case of epidermoid carcinoma of the kidney. Its ultrasonography showed a heterogeneous renal mass with perirenal and pararenal invasion, destruction of the renal pelvis, and preservation of enlarged reniform outline. Although this tumor should be differentiated with renal cell carcinoma, transitional cell carcinoma, renal metast

Laparoscopic nephrectomy for benign and inflammatory conditions.

OBJECTIVES: To evaluate the outcomes of laparoscopic nephrectomy for benign renal conditions associated with dense perinephric inflammation, such as xanthogranulomatous pyelonephritis (XGPN), pyonephrosis, tuberculous pyelonephritis, and calculus pyelonephritis, and compare outcomes with a matched group of patients undergoing open nephrectomy for similar indications. An additional objective was to evaluate factors predictive of complications and open conversions. PATIENTS AND METHODS: We retrospectively analyzed data from 84 patients with benign inflammatory diseases who underwent laparoscopi

Renal Cell Carcinoma Masquerading as Pyonephrosis - A Case Report of A Rare Presentation.

We describe here an atypical case of pyonephrosis which on further evaluation turned out to be a renal cell carcinoma (RCC). The clinical presentation of the patient was that of an infective etiology. However, the renal mass associated with renal vein thrombus and lung metastasis was later diagnosed based on the clinical deterioration of the patient even after insertion of percutaneous nephrostomy. In this case, an underlying renal cancer was probably complicated secondarily leading to pyonephrosis which was the initial presenting manifestation which led to a delay in diagnosis. Pyonephrosis i
