Lemierre's syndrome is a severe complication of Fusobacterium necrophorum oropharyngeal infection associated with metastatic foci of infection, internal jugular vein thrombosis, and septicemia. Musculoskeletal manifestations include isolated or multifocal septic arthritis, soft tissue abscesses, pyomyositis, and osteomyelitis. This article describes a case of a variant of Lemierre's syndrome in a 17-year-old girl, demonstrating a relentless case of limb infection refractory to multiple surgical debridements and broad-spectrum and targeted antibiotics. The patient had F. necrophorum within a pe

OBJECTIVES: Identifying the causative bacterial pathogen for children with acute hematogenous musculoskeletal infections (MSKIs) allows for improved care. The purpose of our study was to determine if clinical markers could predict which patients will have a causative pathogen found on source culture alone, thus being highest yield to undergo operative diagnostic procedures.
METHODS: A single-center, retrospective cohort study was performed. Medical records for patients between 6 months and 18 years of age admitted between July 2014 and September 2018 with a discharge diagnosis of acute osteom

Patients with dermatomyositis (DM) are at an elevated risk of developing active tuberculosis (TB), particularly male patients and individuals on immunosuppressive therapy. We present three cases of dermatomyositis complicated by tuberculous pyomyositis during the initial presentation, each with a distinct clinical presentation, highlighting the challenges encountered in both diagnosis and treatment. A heightened level of suspicion, along with additional and repeated diagnostic testing, is crucial for diagnosing ETPB and also identifying and treating any possible secondary infections.

A 6-year-old girl with community-acquired disseminated infection caused by methicillin-resistant Staphylococcus aureus (MRSA) is described. She had sepsis, meningo-encephalitis, pyomyositis, osteomyelitis, pericarditis and pulmonary embolisation caused by a multi-resistant strain of MRSA. Vancomycin is not routinely recommended as the first-line antimicrobial agent for suspected Staphylococcus aureus infection; however, it should be considered pending susceptibility results in patients presenting with severe sepsis in areas where the prevalence of MRSA is high.

Musculoskeletal complaints are a frequent cause of emergency department visits in pediatric patients. While many of these visits are due to acute trauma, infection is also common and can cause substantial morbidity. This article provides a review of acute pediatric musculoskeletal infections with an additional emphasis on important differential diagnoses to consider. The scope includes cellulitis, abscess, necrotizing soft tissue infections, pyomyositis, infectious tenosynovitis, septic bursitis, septic arthritis, and osteomyelitis. Important differential considerations for these musculoskelet

Methicillin-resistant Staphylococcus aureus (MRSA) is an important causative agent in myositis and pyomyositis, but its involvement in quadriceps tendon tears has not been reported until now. In the case reported here, accurate diagnosis was delayed because of the unique presentation, and the infection was mismanaged with corticosteroids because of the presumptive diagnosis of an inflammatory pathology. Subsequently, aggressive surgical and antibiotic management produced a satisfactory outcome. Early detection and appropriate management of these infections are extremely important in preventing

Soft tissue infections and inflammatory conditions of the musculoskeletal system are a group of disorders commonly seen by emergency room physicians and radiologists. Many of these entities can either be limb- or life-threatening. Magnetic resonance imaging is currently the best imaging modality to evaluate these conditions. In this review, the characteristic imaging findings of cellulitis, abscess formation, necrotizing fasciitis, pyomyositis, diabetic ischemic infarction, acute and exertional compartment syndromes, and rhabdomyolysis will be emphasized as well as imaging factors that can hel

INTRODUCTION: Transient synovitis (TS) is a common and benign cause of hip pain in children, but must be distinguished from more serious entities such as septic arthritis, osteomyelitis, and pyomyositis. Our objectives were to determine the risk of missed bacterial musculoskeletal infection and rates of diagnostic testing in children diagnosed with TS.
METHODS: We performed a cohort study using the Pediatric Heath Information System of children 1-10 years diagnosed with TS in the ED. We determined rates of missed bacterial musculoskeletal infection (defined as a new diagnosis of septic arthri

Forty three patients with pyopericardium were managed over a six year period. Pyomyositis and septic arthritis were the most common associated conditions and Staphylococcus aureus the commonest microorganism isolated. Mycobacterium tuberculosis was isolated in only three of the cases. Pericardiocentesis and various forms of pericardial windows were found to be associated with high rates of recurrences, inadequate drainage and subsequent constrictive pericarditis. Pericardiectomy should be done as an initial form of surgical treatment where the general condition of the patient permits, since th

A clinical trial of bacampicillin was carried out in 20 patients. The antibiotic was found to be effective in acute lobar pneumonia, pyogenic meningitis, acute and chronic bronchitis, acute pharyngitis, acute tonsillitis, cellulitis, furunculosis, and pyomyositis caused by such ampicillin-sensitive organisms as Neisseria meningitidis, Diplococcus pneumoniae, Staphylococcus aureus, Haemophilus influenzae, Streptococcus pyogenes, Klebsiella pneumoniae, and Escherichia coli. Rash in three patients was the only side effect encountered. It is concluded that while bacampicillin may have overcome som

We describe a case of a previously healthy unvaccinated man in his 70s who developed penicillin-susceptible bacteraemic invasive pneumococcal disease due to non-vaccine serotype 23B with the unusual manifestations of multifocal myositis, intramuscular abscesses, polyarticular septic arthritis and synovitis. Blood cultures drawn prior to antibiotic therapy and culture of iliopsoas collection were helpful in making the diagnosis. At follow-up, he had persistent hip pain attributed to avascular necrosis of the head of femur, a possible late complication of his pyomyositis.

Optimal antimicrobial therapy for infections due to ertapenem-resistant Enterobacteriaceae remains undetermined. In this study, a diabetic patient with recurrent pyomyositis and osteomyelitis caused by extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumoniae developed ertapenem resistance after imipenem/cilastatin treatment, which was a currently recommended therapy. He was finally treated successfully using tigecycline. Ertapenem resistance was in part explained by the production of SHV-type ESBL and the absence of an outer membrane protein, OmpK36. Our observation suggests that t

The authors report a case of abscess in the obturator internus muscle, which is a rare occurrence. It was managed with antibiotics and surgical intervention was not necessary. Pyomyositis involving the muscles around the hip needs to be differentiated from septic arthritis and transient synovitis of the hip because these pathologies more commonly afflict this joint. However, when present, abscesses usually affect the psoas or the glutei. Only rarely is the obturator internus involved by the abscess. Magnetic resonance imaging is the imaging modality of choice to image the obturator internus an

New information provided on the pathogenesis and management of rheumatic fever is of current interest. Invasive disease by group A streptococci has been shown to be due to production of toxin A. The natural history and immunopathologic basis for chronic Lyme arthritis are reported. Attention is drawn to pyomyositis and clinical presentation of chronic fatigue syndrome in children. Patients with Sweet's syndrome often have antineutrophil cytoplasmic autoantibodies. Biopsy specimens of panniculitis should be taken to aid treatment. Long-term outcome in chronic osteomyelitis is favorable; recomme

Ten episodes of musculoskeletal sepsis have been seen in nine patients with HIV infection. Seven patients had AIDS, circulating CD4-positive lymphocyte counts being less than 0.1 x 10(9)/l in six. Septic arthritis recurred in seven patients, osteomyelitis in three and pyomyositis and bursitis each occurred in one patient. Staphylococcus aureus was isolated from four patients, atypical micro-organisms being found in three. Presentation of musculoskeletal infection in this patient group may be atypical but rapid diagnosis is important as early antimicrobial therapy is often successful.

Skin and soft tissue infection (SSTI) is common and important infectious disease. This work represents an update to 2012 Korean guideline for SSTI. The present guideline was developed by the adaptation method. This clinical guideline provides recommendations for the diagnosis and management of SSTI, including impetigo/ecthyma, purulent skin and soft tissue infection, erysipelas and cellulitis, necrotizing fasciitis, pyomyositis, clostridial myonecrosis, and human/animal bite. This guideline targets community-acquired skin and soft tissue infection occurring among adult patients aged 16 years a

Intrapericardial (Pericardial) abscess is quite rare as a complication of sepsis and as a cardiac complication of typhoid septicaemia. It is rapidly fatal if untreated. We report two cases of a schoolboy eight years old with septicaemia from pyomyositis of the right thigh and another of nine and a half years with typhoid perforation and typhoid septicaemia that developed pericardial abscesses. In well-developed centres, computerised tomography, ultrasonography and other radiological investigations are employed for both diagnosis and therapy. We emphasise that these can be accomplished by good

Spectrum of hip infections in children include septic arthritis, proximal femoral and pelvic osteomyelitis, and pericapsular pyomyositis. Infection from one site can extend to adjacent structures, resulting in overlapping and often indistinguishable clinical picture. Clinical and laboratory parameters, including fever, inability to bear weight and elevated inflammatory markers are helpful in distinguishing infectious from non-infectious causes of hip pain. However, they cannot reliably differentiate the site or extent of infection. Blood and local aspirate cultures are crucial for identifying

BACKGROUND: Acute hematogenous musculoskeletal infections are a common cause of hospitalization in children. A locally developed clinical care guideline (CCG) for acute musculoskeletal infections was implemented at our quaternary care pediatric hospital in July 2012. The purpose of this study was to evaluate the long-term sustainability of previously described improvements after CCG implementation.
METHODS: Clinical outcomes for children hospitalized with musculoskeletal infections at Children's Hospital Colorado from June 2009 through September 2018 were retrospectively reviewed. Patients we
