[Anti-voltage-gated potassium channel (VGKC) complex/leucine-rich glioma-inactivated protein 1 (LGI1) antibody-associated limbic encephalitis mimicking dementia with Lewy bodies in a patient with subacute REM sleep behavior disorder].

A 73-year-old woman was admitted to our hospital owing to abnormal diurnal behavior, sudden brief episodes of impaired awareness, and loud nocturnal sleep talking. Her symptoms had developed gradually over several months and had been treated as dementia with Lewy bodies (DLB) at another clinic. Video-polysomnography revealed brief sleep talking and gross movements associated with REM sleep without atonia. 18F-FDG PET revealed increased glucose metabolism in both medial temporal lobes. These findings led to a diagnosis of limbic encephalitis (LE) comorbid with REM sleep behavior disorder (RBD).

Cortical activation in REM behavior disorder mimics voluntary movement. An electroencephalography study.

OBJECTIVES: Motor symptoms of Parkinson's disease improve during REM sleep behavior disorder movement episodes. Our aim was to study cortical activity during these movement episodes, in patients with and without Parkinson's disease, in order to investigate the cortical involvement in the generation of its electromyographic activity and its potential relationship with Parkinson's disease. METHODS: We looked retrospectively in our polysomnography database for patients with REM sleep behavior disorder, analyzing fifteen patients in total, seven with idiopathic REM sleep behavior disorder and eig

Sleep in Adults: Parasomnias.

Parasomnias are sleep disorders involving undesirable physical events or experiences occurring during sleep onset, while sleeping, or on arousal. They include non-rapid eye movement (REM) parasomnias (eg, confusional arousals, sleep terrors, sleepwalking) and REM-related parasomnias (eg, nightmare disorder, REM sleep behavior disorder, sleep paralysis). Notable among these is REM sleep behavior disorder, which is closely linked to neurodegenerative conditions such as Parkinson disease. Early detection of this disorder is essential because it often precedes the motor symptoms of neurodegenerati

Neural circuitry underlying REM sleep: A review of the literature and current concepts.

As one of the fundamental sleep states, rapid eye movement (REM) sleep is believed to be associated with dreaming and is characterized by low-voltage, fast electroencephalographic activity and loss of muscle tone. However, the mechanisms of REM sleep generation have remained unclear despite decades of research. Several models of REM sleep have been established, including a reciprocal interaction model, limit-cycle model, flip-flop model, and a model involving γ-aminobutyric acid, glutamate, and aminergic/orexin/melanin-concentrating hormone neurons. In the present review, we discuss these mode

Parasomnias and Sleep-Related Movement Disorders in Older Adults.

Parasomnias and sleep-related movement disorders are important problems in older adults. Sleep paralysis is rare, but may occur in families. In a minority of patients with disorders of arousal, the episodes persist until the age of 70. Zolpidem and other medications may induce sleepwalking and sleep-related eating. Most patients with idiopathic rapid eye movement (REM) sleep behavior disorder eventually develop Parkinson's disease or dementia with Lewy bodies. Anti-IgLON5 disease includes abnormal behaviors in NREM and REM sleep. Restless legs syndrome prevalence increases with age. A severe f

Rapid eye movement sleep behavior disorder and rapid eye movement sleep without atonia in narcolepsy.

Narcolepsy is a rare disabling hypersomnia disorder that may include cataplexy, sleep paralysis, hypnagogic hallucinations, and sleep-onset rapid eye movement (REM) periods, but also disrupted nighttime sleep by nocturnal awakenings, and REM sleep behavior disorder (RBD). RBD is characterized by dream-enacting behavior and impaired motor inhibition during REM sleep (REM sleep without atonia, RSWA). RBD is commonly associated with neurodegenerative disorders including Parkinsonisms, but is also reported in narcolepsy in up to 60% of patients. RBD in patients with narcolepsy is, however, a disti

The effects of RBD medications on dream content: A critical review of evidence.

REM sleep behavior disorder, often an early marker of α-synucleinopathies, is characterized by the loss of REM-related muscle atonia and the occurrence of dream enactment behaviors. Dream reports in REM sleep behavior disorder are frequently described as more vivid, violent, and negative than typical dreams. However, evidence for this remains inconsistent, likely due to methodological variability and unaddressed confounds - most notably, the influence of pharmacological treatments. The extent to which medications affect REM sleep behavior disorder dream content remains poorly understood. This

[Parasomnias Excluded RBD].

Parasomnia is an undesirable physical phenomena that occurs during sleep and is classified into three groups: non-rapid eye movement (REM), REM, and other parasomnias. Non-REM parasomnias that occur during non-REM sleep include wakefulness disorders, including confusional arousal, sleepwalking, sleep terrors, and newly added sleep-related eating disorders. REM sleep parasomnias related to dream content during REM sleep include isolated sleep paralysis and nightmare disorder, in addition to REM sleep behavior disorder. Other types of parasomnia that are not specific to non-REM sleep and REM sle

Motor dysfunction during sleep in posttraumatic stress disorder.

A subjective disturbance of sleep, including the occurrence of repetitive, stereotypical anxiety dreams, is characteristic of posttraumatic stress disorder (PTSD). The phenomenology of the PTSD anxiety dream has seemed most consistent with an underlying rapid eye movement (REM) sleep dysfunction. However, motor behavior reportedly can accompany PTSD dreams, and normal REM sleep typically involves a nearly total paralysis of the body musculature. As a means of understanding this discrepancy, anterior tibialis muscle activity during sleep was studied in a group of Vietnam combat veterans with cu

Disorder-specific alterations of transient oscillatory dynamics during sleep across cortical and subcortical networks.

Transient sleep oscillations reflect the dynamic coordination of cortical and subcortical circuits, modulated by slow oscillatory activity. However, the disorder-specific signatures of these events across neurological, pain, and sleep disorders remain poorly characterized. In this exploratory study, we analyzed transient oscillatory dynamics in 99 individuals, including healthy controls and patients with narcolepsy type 1, non-REM parasomnia, idiopathic REM sleep behavior disorder, and fibromyalgia syndrome. Using slow oscillatory referenced time-frequency peak histograms, we applied principal

Protocol for the DREAMER study: Design and methodological framework of a multicenter trial-ready cohort of individuals with isolated REM sleep without atonia.

Isolated rapid eye movement sleep behavior disorder is a strong clinical marker of future alpha-synucleinopathy, but earlier stages of this risk pathway remain insufficiently characterized. Rapid eye movement sleep without atonia is the polysomnographic substrate of this disorder and may also be detected in individuals without clinical dream-enactment behavior. Whether isolated rapid eye movement sleep without atonia is a benign finding or an early risk state for future rapid eye movement sleep behavior disorder and neurodegeneration remains unknown. DREAMER is a multicenter, prospective, obse

[Sleep Disorders and Autonomic Nervous System Dysfunction].

Parasympathetic nervous system activity dominates during non-rapid eye movement (REM) sleep, whereas the sympathetic nervous system is dominant during REM sleep. The balance between the sympathetic and parasympathetic nervous systems reduces the heart rate and blood pressure during non-REM sleep and increases these parameters during REM sleep and wakefulness. Heart rate variability analysis, a non-invasive method of autonomic nervous system analysis, may be useful in screening for obstructive sleep apnea and also to predict orthostatic hypotension secondary to autonomic disturbances in patient

Rapid eye movement sleep disturbances in Huntington disease.

BACKGROUND: Sleep disorders including insomnia, movements during sleep, and daytime sleepiness are common but poorly studied in Huntington disease (HD). OBJECTIVE: To evaluate the HD sleep-wake phenotype (including abnormal motor activity during sleep) in patients with various HD stages and the length of CAG repeats. Because a mild hypocretin deficiency has been found in the brains of some patients with HD (hereinafter referred to as HD patients), we also tested the HD patients for narcolepsy. DESIGN AND PATIENTS: Twenty-five HD patients (including 2 premanifest carriers) underwent clinical

Research progress in rapid eye movement sleep behavior disorder.

Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by abnormal dream acting behavior such as vocalization and twitching related to dream content during REM sleep. The diagnosis requires polysomnography demonstrating a loss of normal skeletal muscle atonia during REM sleep. Both idiopathic RBD and secondary RBD are highly related to synucleinopathy including Parkinson's disease, dementia with Lewy bodies, and multiple system atrophy. Almost all idiopathic RBD patients will develop synucleinopathy after a few years. Therefore, RBD may be an early marker in the progression of

Associations Between Self-Reported Sleep Disturbances and Cognitive Impairment: A Population-Based Cross-Sectional Study.

OBJECTIVE: Cognitive impairment is a rapidly growing global public health problem in China and worldwide. In the recent decades, emerging studies have explored the associations between sleep disturbances and cognitive impairment. However, the variety of the results imply us that further studies should be conducted for the associations. METHODS: This is a cross-sectional study conducted between August and October 2018 in five cities in Hebei province, China. Subjects were 21,376 community residents. Cognitive impairment was screened by the Chinese version of the Mini-Mental State Examination (

Parasomnias are physical, behavioral and experiental phenomena ocurring during entry or along the sleep or on arousal/awakening. The behavior includes movements, emotional, perceptual or dreaming experience, frequently containing manifestations of autonomic nervous system. Parasomnias are devided into primary (ocurring etiher in NREM or REM sleep) and secondary (following organic system disease manifested during sleep). Primary parasomnias are further devided into those that appear during NREM, REM or states of consciousnes that do not respect boundaries between wake and sleep. Parasomnias rep

Circadian alterations in isolated Rapid-Eye-Movement sleep behavior disorder: associations with clinical, glymphatic, and dopaminergic imaging markers.

Alterations in sleep-wake patterns are well-recognized features of overt alpha-synucleinopathies. However, their presence in prodromal stages, characterized by isolated Rapid-Eye-Movement (REM) sleep behavior disorder, remains controversial, and their association with clinical symptoms and imaging risk factors has been underexplored. In this study, we used seven-day ambulatory circadian monitoring to characterize circadian and sleep-wake disturbances in a well-characterized sample of forty-two patients with video-polysomnography-confirmed isolated REM sleep behavior disorder and 23 healthy con
