Rotations of teeth-a systematic review.

BACKGROUND: Rotations are frequently evaluated through various assessment methods of crowding and arch dimension, with relatively few studies discussing the extent or direction of rotations and even fewer addressing the reliability of such assessments. This systematic review aims to comprehensively analyze existing classification systems for rotated teeth and assess rotation in anterior and posterior teeth, its clinical applicability, and its impact on retention and relapse. SEARCH METHODS: Two investigators conducted a comprehensive search in six databases, namely, PubMed, Scopus, Ovid, LILA

Bidirectional rotating biatrial tachycardia.

A 72-year-old man was treated for recurrent atrial tachycardia (AT) and underwent ablation. The AT was diagnosed as bi-AT based on the activation map and the postpacing interval. Another AT appeared and was diagnosed as bi-AT by the same method. Surprisingly, the circuits of both ATs were perfectly matched and rotated in opposite directions. The left atrial anteroseptal wall was ablated during the AT. The AT was immediately stopped and was no longer induced.

Rotator cuff assessment on imaging.

The rotator cuff is a group of four muscles and tendons surrounding the shoulder joint providing it strength and stability. The rotator cuff consists of the subscapularis, supraspinatus, infraspinatus and teres minor. Many shoulder complaints are caused by rotator cuff pathology such as impingement syndrome, tendon tears and other diseases e.g. calcific tendonitis. Diagnosis starts with clinical history and physical examination, after which imaging is often used to help confirm clinical findings depending on the differential diagnosis. The aim of the article is to review the frequently used im

The scapula in rotator cuff disease.

The scapula serves as the platform or base for the muscles of the rotator cuff. Rotator cuff function and dysfunction is therefore directly related to the functional state of the scapula. When the scapula is well stabilized, optimal positioning in both static and dynamic tasks allows the rotator cuff to function at an optimal level. However, scapular dysfunction produces an unstable base which in turn negatively affects the rotator cuff's biomechanical contributions to arm motion. Scapular alterations may be causative in rotator cuff dysfunction or may be the result of rotator cuff injury and

Instability and rotator cuff tear.

Prevalence of rotator cuff tears after traumatic dislocation increases with advancing age, as a consequence of the age-associated deterioration of structures and mechanical properties of the tendons of the rotator cuff. These act as effective stabilizers of the joint, compressing the humeral head in the three-dimensional concavity of the glenoid. It is impossible to establish whether a lesion of the capsulo-labrum complex or of the rotator cuff causes or follows a dislocation, regardless of whether it is anterior or posterior. A peripheral nerve or a brachial plexus injury can be associated wi

Human discrimination of rotational velocities.

Vestibular reflexes are critically important for stabilizing gaze and maintaining posture, but comparatively little is known about conscious perceptions of vestibular stimuli and how they may relate to balance function. We used psychophysical methods to determine the ability of normal subjects and a vestibular-deficient subject to discriminate among velocities of earth-vertical sinusoidal rotations. Discrimination thresholds in normal subjects rose from 2.26 deg/s at a peak velocity of 20 deg/s up to 5.16 deg/s at 150 deg/s. The relationship between threshold and peak angular velocity was well

Rotator cuff tears: an overview.

Rotator cuff tears are a common contributing factor of shoulder pain and occupational disability. Tears of the rotator cuff are becoming increasingly prevalent in today's musculoskeletal population. Accurate recognition and successful treatment of patients with rotator cuff tears require thorough comprehension of the normal and pathologic anatomy of the rotator cuff. This article will provide an overview of the rotator cuff that consists of normal anatomy, pathology, physical assessment, diagnostic imaging, and recommended treatment. This article also discusses the importance of providing adeq

Ipsilateral rotational autokeratoplasty: a review.

Corneal opacity is a major cause of monocular blindness and, after cataract, is also a leading cause of blindness worldwide. Keratoplasty techniques for the treatment of corneal opacities include deep anterior lamellar allokeratoplasty, penetrating allokeratoplasty, penetrating bilateral autokeratoplasty, and ipsilateral rotational autokeratoplasty (IRA). This review describes the indications, technique, and outcomes of IRA. IRA is only indicated for patients with a localised opacity leaving a minimum diameter of 4-5 mm of uninvolved clear cornea. For these few patients in whom the procedure i

Methylsalicylate: a rotational spectroscopy study.

We report the free-jet rotational spectra of methylsalicylate, a molecule with a possible tautomeric and conformational equilibrium. In the ground electronic state, the molecule adopts a form stabilized by an intramolecular hydrogen bond between the phenolic hydrogen and the carbonylic oxygen, and this structure is characterized as the lowest-energy form by quantum chemical calculations. All rotational transitions are split because of the internal rotation of the methyl group, and the value of the barrier for this motion was determined to be V(3) = 5.38 kJ mol(-1).

Three-dimensional rotational myelography.

Three-dimensional (3D) rotational radiography, initially developed to visualize intracranial aneurysms, is applied to the cervical spine after conventional myelography. We call this process 3D rotational myelography. 3D reconstruction and then postprocessing allows imaging in multiple planes. Spinal or nerve root sheath alterations caused by bony or soft tissue can be visualized and differentiated by using this technique.

Arthroscopic rotator cuff repair.

The arthroscopic operation for repair of full-thickness rotator cuff tears is successful and has the advantages of glenohumeral joint inspection, treatment of intraarticular lesions, smaller incisions, no deltoid detachment, less soft tissue dissection, less pain, and more rapid rehabilitation. However, these advantages must be balanced against the technical difficulty of the method, which limits its application to surgeons skilled in open and arthroscopic shoulder operations.

Rotator cuff tears of the hip.

Pain over the lateral aspect of the hip commonly is attributed to trochanteric bursitis. Typical findings include local tenderness and weakness of hip abduction. When conservative measures fail to relieve symptoms, surgical release of the iliotibial band over the greater trochanter has been recommended. In the management of seven such patients, an unusual finding was encountered: partial tear of the gluteus medius tendon at its attachment to the greater trochanter. Each patient presented with increasing hip pain of duration of months to years. There were no diagnostic findings on physical exam

Rotator cuff pathology in athletes.

The rotator cuff is the primary dynamic stabiliser of the glenohumeral joint and is placed under significant stress during overhead and contact sports. Mechanisms of injury include repetitive microtrauma, usually seen in the athlete involved in overhand sports, and macrotrauma associated with contact sports. Rotator cuff injury and dysfunction in the overhand athlete may be classified based on aetiology as primary impingement, primary tensile overload, and secondary impingement and tensile overload resulting from glenohumeral instability. A thorough history and physical examination are paramou

Morphology of the torn rotator cuff.

The morphological characteristics of shoulders with torn rotator cuffs were determined using 41 embalmed specimens. The following parameters were measured in the supraspinatus (SSP), infraspinatus (ISP) and subscapularis (SSC) muscles: the length, thickness and width of the extramuscular tendon; the length of the intramuscular tendon; the length and width of a tear, if present, muscle fibre length; and muscle volume. The cross-sectional area (CSA) of the tendon was measured on the photographic image of slices of the tendon using an image analysis system, and the CSA of the muscle was calculate

An analysis of rotator cuff repair.

Fifty-four patients who had surgery to repair a chronic rotator cuff tear (54 shoulders) were followed for an average of 25.9 months (range, 16 to 43). Forty-nine of 54 patients had at least 20 months of followup. The average age of the male patients (N = 37) was 52 (range, 24 to 80) and of the female patients (N = 17) was 56 (range, 38 to 72). All of these patients received a functional evaluation based on the Shoulder and Elbow Surgery Society classification system. Forty-eight patients had an individual examination and 33 patients had an isokinetic strength evaluation. There were 43 patient

Evaluation and treatment of the rotator cuff.

The use of the arthroscope for evaluation and treatment of rotator cuff problems is becoming more common. Technology has advanced to the point where we can now not only positively diagnose early rotator cuff problems, but successfully treat them; many times without open shoulder surgery. Arthroscopic cuff debridement and decompression are the mainstays of surgical prevention. Arthroscopic suture fixation of delaminated cuff tears or small rotator cuff avulsions is now possible. The miniopen rotator cuff approach is excellent for repairing most larger full-thickness tears. This article reviews

Treatment of rotator cuff impingement.

The author performed 497 shoulder decompression procedures between 1978 and 1989. Of these procedures, 252 (51%) had tears of the rotator cuff that were repaired concomitantly. The results were successful in over 90% of the cases. It is suggested that articular-surface tears or intratendinous tears are the possible cause of some of the failures of the open procedure. The findings also suggest that the use of arthroscopy or magnetic resonance imaging may be indicated in selected cases. However, for the appropriate patient, open shoulder decompression continues to be an excellent treatment.
