Transfer of latissimus dorsi for irreparable rotator-cuff tears.

We treated 12 shoulders in ten patients with irreparable rotator-cuff tears by transfer of the latissimus dorsi. There were nine men and one woman. Their average age was 64.0 years and the average follow-up was 35.6 months (26 to 42). The results were excellent in four shoulders, good in four, fair in one, and poor in three. Active forward flexion improved from a preoperative average of 99 degrees to a postoperative average of 135 degrees. Osteoarthritic changes appeared in five shoulders and proximal migration of the humeral head progressed in six. EMG revealed that nine of the 12 transferred

Clinical presentation of complete tears of the rotator cuff.

To determine whether the presence and extent of a tear of the rotator cuff could be predicted on the basis of a patient's history, physical examination, and radiographic findings, detailed data from the histories and physical examinations of 103 patients who were known to have a tear of the rotator cuff were correlated with the radiographic and operative findings on these patients. An age-matched control group of fifty-one patients who had similar symptoms, but whose arthrograms showed normal results, was used to establish a baseline incidence of ten specific radiographic findings in the shoul

[Radiographic signs of rotator cuff tears in the shoulder].

In 42 patients affected by disruption of the whole layer of the shoulder rotator collar the authors compared radiographic changes on a simple radiograph in anterior-posterior projection with operation findings. The proliferative changes on the large trochanter of humerus and the humeroacromial distance were decisive for prognosis of the operation therapy for the disrupted collar. The patients with a shorter humeroacromial distance were affected, after the operation, by a worse external rotation of the arm, worse extension and muscular strength of the arm in the extension position. Radiographic

Effect of PARP-1 Inhibition on Rotator Cuff Healing: A Feasibility Study Using Veliparib in a Rat Model of Acute Rotator Cuff Repair.

BACKGROUND: PARP-1 (poly[ADP-ribose]) was shown to influence the inflammatory response after rotator cuff tear, leading to fibrosis, muscular atrophy, and fatty infiltration in mouse rotator cuff degeneration. So far, it is not known how PARP-1 influences enthesis healing after rotator cuff tear repair. HYPOTHESIS/PURPOSE: This study aimed to examine the feasibility of oral PARP-1 inhibition and investigate its influence on rat supraspinatus enthesis and muscle healing after rotator cuff repair. The hypothesis was that oral PARP-1 inhibition would improve enthesis healing after acute rotator

Rice-Body Synovitis, Foreign Body Reaction, and Rotator Cuff Failure After Subacromial Balloon Spacer Augmentation of a Rotator Cuff Repair: A Case Report.

CASE: A 66-year-old woman presented with shoulder pain and weakness 4 months after augmentation of a rotator cuff repair with a Stryker InSpace subacromial balloon spacer. A magnetic resonance imaging (MRI) demonstrated a failed rotator cuff repair, large effusion with rice bodies, synovitis, axillary lymphadenopathy, loose anchors, and erosive changes to the greater tuberosity. Arthroscopy revealed balloon fragmentation surrounded by diffusely hyperemic synovium without repairable cuff tissue. Final cultures proved negative for infection. Histologic evaluation revealed ulcerated synovium with

Mini-open rotator cuff repair using a two-row fixation technique: outcomes analysis in patients with small, moderate, and large rotator cuff tears.

Mini-open rotator cuff repair has been used successfully to treat small- and moderate-sized rotator cuff tears but not large tears (>3 cm and <5 cm). We use a 2-row fixation technique of repairing the torn cuff to the humerus. We sought to determine if this technique would produce successful results in patients with larger rotator cuff tears. The results of 75 consecutive patients who underwent arthroscopic subacromial decompression with acromioplasty and mini-open rotator cuff repair were evaluated. All patients were evaluated at a minimum 24 months postoperatively (mean, 27 months). All pati

Preoperative pain associated with rotator cuff tears correlates with synovitis severity in the rotator interval.

BACKGROUND: The present study primarily aimed to investigate whether preoperative pain associated with rotator cuff tears correlates with the severity of synovitis in the rotator interval (RI) on arthroscopic findings retrospectively. METHODS: Between July 2017 and December 2022, 131 shoulders with arthroscopic rotator cuff repair were retrospectively investigated. The pain domain of the preoperative Constant Score and Shoulder36 were used to assess preoperative pain. Two examiners confirmed the degree of synovitis of RI in the arthroscopic findings. RESULTS: The mean patient age was 68.9 ye

The influence of medial reattachment of the torn cuff tendon for retracted rotator cuff tears.

We reattached the torn rotator cuff medial to the anatomic cuff insertion site if it was retracted. The purpose was to correlate the amount of medial reattachment of the cuff with shoulder function. We evaluated 63 shoulders in which repaired cuffs were followed with magnetic resonance imaging at a mean follow-up of 8 years. The amount of medial reattachment of the cuff tendon was determined by use of a T2-weighted oblique coronal view, which passed through the center of the humeral head, and was defined as the NCA angle (where N indicates the new cuff insertion point, C indicates the center o

Editorial Commentary: Rotator Cuff Repair Augmentation: Arguably the Only Meaningful Improvement in Rotator Cuff Repair Outcomes in Last 40 Years, While Being Cost-Effective.

Rotator cuff repair (RCR) remains one of the most commonly performed orthopaedic procedures and represents a dynamic scientific evolution. This progression toward optimizing patient outcomes has certainly been nonlinear, and it requires that we ask of ourselves: have advances in RCR delivered on this promise? Advances in surgical approach, repair configuration, and the introduction of marrow stimulation have produced largely equivalent outcomes, leaving retear rates persistently high, especially for large and massive tears. To that end, structural augmentation with allograft or xenograft has e

Suprascapular Nerve Release Improves the Histological Healing Morphology and Biomechanical Strength of the Rotator Cuff in a Rat Chronic Massive Rotator Cuff Tear Model.

BACKGROUND: While suprascapular nerve (SSN) entrapment is widely recognized as a complication of massive rotator cuff tears (MRCTs), its role as an independent factor affecting tendon healing remains underreported. Additionally, the therapeutic benefit of SSN release in MRCT repair remains controversial. PURPOSE: To investigate the incidence of SSN entrapment via histological stain and whether SSN release enhances rotator cuff healing in a rat model of delayed MRCT repair. STUDY DESIGN: Controlled laboratory study. METHODS: A chronic MRCT model was established in rats via bilateral transect
