Mortality in diabetic patients with cardiovascular autonomic neuropathy.

Cardiovascular autonomic diabetic neuropathy (CADN) may carry an increased risk of mortality. However, in previous studies the prognosis of patients with CADN seemed to be influenced by life-threatening macro- and microvascular complications which had already been present at the start of the study period. Between 1981 and 1983, 1015 diabetic patients have been examined for CADN (abnormal heart rate variation at rest and during deep respiration) at the Diabetes Research Institute, Düsseldorf. Thirty-five patients (28 with Type 1 diabetes, 7 with Type 2 diabetes) with CADN have been retrospectiv

Different Surgical Strategies in the Prevention of Frey Syndrome: A Systematic Review and Meta-analysis.

OBJECTIVE/HYPOTHESIS: To define the best surgical technique able to reduce Frey syndrome (FS) incidence after parotidectomy. STUDY DESIGN: Systematic review and network meta-analysis. METHODS: An arm-based network analysis was conducted using a Bayesian hierarchical model. The primary and secondary outcomes were the incidence of subjective (clinical) and objective (positive starch-iodine test result) FS, respectively. RESULTS: A total of 3830 patients with a median age of 50.35 years (n = 2323; IQR 44.25-54.18) were included for six interventions [temporoparietal fascia (TPFF), free fat gra

Prospective outcomes following drainless superficial parotidectomy with sternocleidomastoid flap reconstruction.

BACKGROUND: Patients undergoing superficial parotidectomy for benign parotid lesions are at risk of postoperative complications, most notably cosmetic complications such as facial paralysis and contour defects, and functional complications including Frey's syndrome. Traditionally, surgical drains have been placed at the end of surgery to prevent hematoma and sialocele formation. However, this can increase the risk of postoperative complications and contribute to a prolonged course in hospital. To try and prevent these risks and complications, we introduced a novel technique of a drainless paro

Free fat grafting in superficial parotid surgery to prevent Frey's syndrome and improve aesthetic outcome.

BACKGROUND: Frey's syndrome and cosmesis are important considerations in parotid surgery. Placement of an interpositional barrier can prevent these complications; however, surgical technique and efficacy remain controversial. METHODS: A prospective case series was collected comprising all patients undergoing primary superficial parotidectomy for benign pathology with abdominal free fat grafting between June 2007 and December 2010, performed by a single surgeon. A survey was also distributed to otorhinolaryngology consultants across Australia to assess current practice. RESULTS: Twenty-eight

Extracapsular dissection versus superficial parotidectomy in pleomorphic adenomas of the parotid gland.

OBJECTIVES: We aimed to compare the results of extracapsullary dissection (ED) with superficial parotidectomy (SP) in the surgical treatment of benign parotid tumors. PATIENTS AND METHODS: Forty-one patients who were diagnosed with pleomorphic adenoma of the parotid gland and underwent surgery between January 1992 and June 2000 were enrolled in this study. The patients were divided into two groups: the ED group (6 males, 15 females; mean age 47.2 years; range 32 to 57 years) and the SP group (7 males, 13 females; mean age 47.7 years; range 29 to 61 years). ED was performed on 21 patients and

Reconstruction after total parotidectomy using a de-epithelialized free flap.

INTRODUCTION: Total parotidectomy has aesthetic and functional sequelae, including Frey' syndrome and soft tissue depression of the region. The aim of repair is to reduce these problems, importing tissue into the surgical field to fill the region and to avoid Frey's syndrome. The soft tissues to use are those which are not resorbed in order to have a stable aesthetic result. OBJECTIVE: To propose a new method for avoiding negative functional and aesthetic sequelae of total parotidectomy. PATIENTS: Two patients underwent total parotidectomy because of recurrent sialoadenolithiasis in one case

Methylene blue staining in the parotid surgery: Randomized trial, 144 patients.

OBJECTIVE: To investigate usefulness of the methylene blue staining for parotid gland surgery. STUDY DESIGN: Randomized, prospective controlled trial. METHODS: The study analyzed 144 surgical operations (2000-2014) for parotidectomy. The cases were randomly divided into two groups: Group 1 (n=70) for surgeries with methylene blue staining, and Group 2 (n=74) for surgeries without staining. Surgical complications like temporary facial weakness, permanent facial nerve injury, and Frey's syndrome were taken for comparison between groups as well as rate of tumor recurrence. RESULTS: Temporary f

Extra-capsular dissection in benign parotid tumors.

UNLABELLED: The type of surgery for benign parotid tumors is still lacking of general consensus: this retrospective study, from 1964 to 2004 with a minimum follow-up of 7 years, is carried out with the aim to evaluate the results of parotidectomy and extracapsular dissection (ECD). PATIENTS AND METHODS: Three hundred forty-nine pts were operated for pleomorphic adenoma (219) or cystadenolymphoma (165). ECD was performed in 332 pts and parotidectomy in 52. Pts retrospectively provided health-related informations. The local recurrences and the complications were the main outcome measures. RESU

Prevention of Frey syndrome in parotid gland surgery.

OBJECTIVE: This study discusses the anatomy and surgical techniques for the prevention of Frey syndrome. By performing only a partial superficial parotidectomy, we probably preserve the connection between the auriculotemporal nerve and the facial nerve. By suturing the remainder of the parotid gland tissue and the superficial musculoaponeurotic system (SMAS) back to the sternocleidomastoid muscle, an interposition is made. Furthermore, by extending the incision not too far cranially, the auriculotemporal nerve cutaneous branch is spared. We also tried to preserve part of the great auricular ne

Radiotherapy for pleomorphic adenoma of the parotid gland.

A study was made of 187 patients with parotid pleomorphic adenoma treated by radiotherapy. This followed surgery but with incomplete removal or tumor spillage. In the early years of the study radiotherapy was given by radium needle implant done usually at the time of surgery, but from the late 1960s beam-directed external radiotherapy with a head shell was used most commonly. A 3-field technique or wedge pair was the standard technique. The median age was 46 with nearly half the patients (87/187) aged between 40 and 60, and the ratio of women to men was 1.4:1 (110:77). Median follow-up for all

Progression of diabetic autonomic neuropathy over a decade in insulin-dependent diabetics.

The prognosis for diabetics with autonomic neuropathy is little known. We therefore studied the progress of young insulin-dependent diabetics, first identified as having abnormal autonomic function 10-15 years ago. We have shown that the mortality of diabetics with symptomatic autonomic neuropathy is increased, but is less than previously reported. Mortality in asymptomatic diabetics with an isolated abnormality in autonomic function tests is not increased. The heart rate variability declines at 1.02 +/- 0.47 (SD) per annum in diabetics with an initially normal heart rate variability. While sy

Frey's syndrome after superficial parotidectomy: role of the sternocleidomastoid muscle flap: a prospective nonrandomized controlled trial.

BACKGROUND: The prevalence of Frey's syndrome (FS) after superficial parotidectomy in correlation to the sternocleidomastoid muscle flap (SCMMF) interposition is analyzed. METHODS: A prospective nonrandomized controlled multicenter trial included 130 patients. During superficial parotidectomy, SCMMF was dissected, if excised specimens' volume exceeded 25 mL (SCMMF group). Follow-up examinations took place after 6, 12, and 24 months and included a Minor's test. RESULTS: SCMMF was dissected in 30 (23.1%) patients. A total of 104, 80, and 68 patients completed the 1st, 2nd, and the 3rd follow-u

Morbidity of parotid gland surgery: results 1 year post-operative.

Patients operated on for parotid gland tumours were evaluated prospectively to study morbidity after this procedure. Nowadays, major complications such as recurrence of tumour and permanent facial nerve paresis are rare after primary surgery. Therefore, this study especially takes minor complications such as Frey's syndrome and sensory deficits into account. The study documents morbidity in 45 patients who completed a 1 year follow-up. Among those with primary benign tumours, we found no recurrences and no permanent paresis, nor did starch-iodine testing reveal any cases of Frey's syndrome. Ho

Prospective randomised trial of the benefits of a sternocleidomastoid flap after superficial parotidectomy.

Parotid surgery can cause postoperative facial nerve dysfunction, cosmetic impairment, and Frey's syndrome. Thirty-six patients listed for superficial parotidectomy were entered into a prospective randomised trial to find out if the use of a sternocleidomastoid flap could reduce the incidence of these complications. Partial facial nerve paresis was seen at 3 months in five patients in whom flaps were raised compared with six among those who did not have flaps (P=0.025). There was no difference between the two groups at 1 year. The flap was not associated with an improvement in either subjectiv

Superficial musculoaponeurotic system flap in partial parotidectomy and clinical and subclinical Frey's syndrome. Cosmesis and quality of life.

BACKGROUND: Frey's syndrome (FS), facial depression, and noticeable scar may have an impact on patient's quality of life after a parotidectomy. The superficial musculoaponeurotic system (SMAS) flap has been used to ameliorate those effects. METHODS: Forty-seven consecutive parotidectomies were analyzed; SMAS flap was performed in 26 of them. We investigated clinical FS, subclinical FS, and cosmetic satisfaction, and compared SMAS flap versus cases where this was not done. RESULTS: By performing the SMAS flap, the occurrence of clinical FS was reduced; however, there was no significant differ
