Open surgical excision of subglottic hemangioma in children.

Subglottic hemangioma is an unusual entity which may involute spontaneously without aggressive surgical intervention; although tracheotomy is sometimes necessary. The actual time course for involution is not clear by reviewing the literature [1,14]. Numerous treatment modalities are described for the treatment of this condition, including tracheotomy, CO2 laser ablation and intralesional steroid injection with or without endotracheal intubation. Surgical resection of subglottic hemangioma is an option which is described in the literature and may be utilized in certain selected cases. We presen

Upper airway volume segmentation analysis using cine MRI findings in children with tracheostomy tubes.

OBJECTIVE: The purpose of this study is to evaluate the airway dynamics of the upper airway as depicted on cine MRI in children with tracheotomy tubes during two states of airflow through the upper airway. MATERIALS AND METHODS: Sagittal fast gradient echo cine MR images of the supra-glottic airway were obtained with a 1.5T MRI scanner on seven children with tracheotomy tubes. Two sets of images were obtained with either the tubes capped or uncapped. The findings of the cine MRI were retrospectively reviewed. Volume segmentation of the cine images to compare the airway volume change over time

Method for dealing with severe aspiration using a new concept of intralaryngeal prosthesis: A case report.

BACKGROUND: Deglutition disorders are frequent in patients treated for cancer of the oropharynx, and are responsible for severe aspiration and, in some cases, may require the use of enteral nutrition by gastrostomy or tracheotomy. METHODS: We present the case of a patient with a medical history of infiltrating laryngeal carcinoma treated by partial laryngectomy and radiochemotherapy, presenting severe aspiration causing significant weight loss. The patient received a new intralaryngeal prosthesis to prevent aspiration with the objective of regaining safe oral nutrition and closure of the trac

Contrasting strategic approaches to the management of subglottic hemangiomas.

The standards of treatment of subglottic hemangiomas are steroids and laser vaporization. If these methods do not provide a sufficient airway, a tracheotomy may be required. Analysis of the data available in the literature shows that there is a place for other therapeutic solutions. Open surgical excision could be one of them but is controversial. So far, it represented a 'last' resort procedure before tracheotomy. Our approach to open surgical excision is different. A prospective decision is taken in the initial therapeutic approach. Infants are considered candidates for excision in cases of

Laryngotracheal stenosis in children.

The diagnosis of laryngotracheal stenosis should be suspected in children with stridor, feeding difficulties, or atypical croup. Only half of the children with congenital laryngotracheal stenosis require tracheotomy, and many of these children can be decannulated following uncomplicated surgical therapy. In contrast, tracheotomy-dependent patients with acquired laryngotracheal stenosis require more extensive surgical intervention, which should be carried out as early as possible to provide the best opportunity for developing normal oral communication.

Bilateral thoracotomy for coronary artery bypass grafting in a patient with unfavorable median sternotomy.

In this report, we describe a patient who underwent coronary artery bypass grafting (CABG) through bilateral thoracotomy and distal median sternotomy, because he had received terminal tracheotomy as a treatment to prevent aspiration pneumonia due to Wallenberg's syndrome. On the first day after the terminal tracheotomy, he suddenly complained of severe anterior chest pain. Emergency coronary angiogram revealed complete occlusion of the proximal right coronary artery (RCA), severe stenosis of the left anterior descending artery (LAD) and the large first diagonal branch. Catheter intervention fo

Laryngocele associated with laryngeal carcinoma.

Laryngocele is an abnormal dilatation of Morgagni ventricle (saccule) in direct communication with the laryngeal lumen. Symptoms are not characteristic: hoarseness, dyspnea, foreign body sensation and cough. Sometimes it is presented as cervical swelling causing airway obstruction and need an emergency tracheotomy. In this paper, we report a case of upper airway obstruction due to laryngocele associated with a laryngeal carcinoma who was treated by emergency tracheotomy and, finally by total laryngectomy. A review of the literature is also presented.

[Laryngotracheal fissure in infants. Indications, technique, results].

Since laryngotracheofissiure, or anterior cricotomy, has been introduced in 1980, the number of tracheotomies performed in infants has decreased as this procedure is an alternative to tracheotomy for difficult extubation. Since then, the indications of this procedure have become wider, but a number of preoperative criteria, such as the pulmonary capacity, must be strictly assessed. We report on our experience with 14 children who underwent laryngotracheofissiure during the past 4 years. The indications, the surgical technique and the results are evaluated.

Laryngeal papillomatosis with airway obstruction in an infant.

Laryngeal papillomatosis in infants and children is a benign condition, but the location and a marked tendency for recurrence makes the disease both dangerous and troublesome. This case report deals with a little girl who had suffered hoarseness and wheezing since she was born. The diagnosis of laryngeal papillomatosis was made when she was 17 months old. By that time, the tumour had reached a size that necessitated a tracheotomy to secure the airway. The symptoms and differential diagnoses are discussed, and it is stressed that chronic hoarseness and wheezing sounds in infants and children sh

[Lateral fixation of the vocal fold (Lichtenberger's technique): interest in the bilateral laryngeal immobilities].

UNLABELLED: In patients presenting a bilateral laryngeal immobility, the potential reversibility of certain cases, the refusal or the bad tolerance of long term tracheotomy bring up the question of the choice of the surgical technique if it is indicated. OBJECTIVES: To report our experience with the technique of lateralization of the paralyzed vocal fold (arytenoidopexy) suggested by Lichtenberger. CLINICAL CASES: After having described the technique, we report 5 cases (3 pos-thyroidectomy, I of central origine, 1 post-burn). From 1 to 12 months after surgery 2 patients were fully satisfied,

Advances and controversies in the management of supraglottitis and laryngotracheobronchitis.

Recent advances in pediatric airway intubation and the introduction of pharmacotherapeutic agents, active within the larynx and tracheobronchial tree, have created significant controversies in the treatment of acute obstructive inflammatory laryngeal disease. Although the etiology, pathophysiology, and clinical patterns of supraglottitis and laryngotracheobronchitis are no longer confused, decisions concerning their medical management and method of airway control remain controversial. Both inflammatory disorders are critically examined, with a reveiw of current concepts regarding the choice be

[Laryngo-Tracheal Resection as an Alternative to Permanent Tracheostomy].

INTRODUCTION: Cervical stenosis of the trachea caused by tracheotomy, tumor or without defined reason (idiopathic) can be treated by resection and anastomosis with good early and long-term results. Involvement of the ring cartilage makes the procedure technically demanding and increases the risk of morbidity. We describe our technique of laryngotracheal resection and reconstruction and compare the perioperative results with standard tracheal resection. PATIENTS AND METHODS: Between January 2005 and December 2015, we performed about 800 procedures on the trachea including 76 standard cervical

[Diagnosis and treatment of infantile congenital subglottic hemangioma].

OBJECTIVE: To discuss the diagnosis and treatment of infantile congenital subglottic hemangioma. METHOD: Nine patients with recurrent stridor, feeding difficulties and laryngeal obstruction were diagnosed as congenital subglottic hemangioma with bronchial endoscopy. The lesions were unilateral in 7 cases and bilateral in 2 cases. Eight cases were treated by intralesional Pingyangmycin injection after tracheotomy. Patients were discharged with metal trachea and were followed up endoscopically 2 weeks thereafter. A second treatment would be required if the tumor reduced in size but not disappea

Pyriform sinus mucosa stabilization for prevention of postoperative airway obstruction in arytenoid adduction.

OBJECTIVES: The Isshiki arytenoid adduction procedure directly closes the open posterior glottis. Postoperative airway obstruction that necessitates emergent tracheotomy is an important complication in arytenoid adduction patients when the standard posterior thyroplasty window is used to approach the posterolateral larynx. Immediate postoperative fiberoptic laryngoscopy shows ipsilateral edema and/or hematoma of the arytenoid and supraglottic mucosa, with occasional obstructing inspiratory collapse. In this study, I sought to modify the posterior window approach during arytenoid adduction surg

Modified stent for the treatment of tracheomegaly combined with a tracheoesophageal fistula: a case reportStent.

Tracheoesophageal fistulas (TEFs) are common in clinical practice and we address them in different ways according to their etiologies. Herein, we present a case of tracheomegaly combined with a TEF after long-term tracheotomy. We placed a modified silicone stent into the trachea to simultaneously cover the fistula and maintain an artificial airway for ventilation. After migration of the modified stent, we replaced it with a prolonged tracheotomy tube. This modified stent is a novel clinical attempt at addressing TEFs that should be more thoroughly explored.

Emergency cricothyrotomy: A procedure well worth knowing.

There are two possible approaches to the subglottic airway: tracheotomy, and cricothyroidotomy. Cricothyroidotomy is less well known by head and neck surgeons and has a bad reputation due to laryngeal morbidity, liable to progress to subglottic stenosis if not swiftly relayed by tracheotomy. Tracheotomy is taught early on in ENT training, notably to be performed under enhanced local anesthesia in patients with tumoral laryngeal dyspnea. It is, however, completely unsuited to other emergency situations, in patients that cannot be ventilated or in case of failure of orotracheal intubation, notab

[Submental intubation in maxillofacial traumatology].

INTRODUCTION: Free access to the upper respiratory tract may be a challenge for the management of severe facial trauma. Tracheotomy is routinely used in this case. Submental intubation is little used, but it is an alternative to tracheotomy. MATERIAL AND METHOD: A retrospective study was made over 1 year. Sixteen patients with facial smash were included. Patients who had undergone an immediate tracheotomy or a long-term intubation were excluded. Submental intubation was used in all patients with the same protocol. The studied data was: operative time, duration of ventilator disconnection, pos
