Cadaveric renal transplants in children under 6 years of age.

Thirty-nine cadaveric renal allografts were performed in 28 children under 6 years of age. Common primary renal diseases were glomerulonephritis, dysplasia/hypoplasia, and reflux/obstructive nephropathy. After a mean follow-up of 40 months of patients with surviving grafts, 19 patients had functioning grafts, 3 had returned to dialysis, and 6 had died. These children required an extraordinary amount of care postoperatively because of anorexia, diarrhea, and ileus. Their psychomotor and physical development was retarded prior to transplant; this reversed dramatically after transplant, but catch

Nosocomial Legionnaires' disease. Occurrence in recipients of bone marrow transplants.

Nosocomial pneumonia caused by Legionella pneumophila serogroup 1 occurred in five patients after bone marrow transplantation for hematologic malignancies. Two patients died as a result of the infection despite treatment with erythromycin. Serologic screening revealed no other cases of Legionnaires' disease in 40 consecutive recipients of bone marrow transplants, giving a frequency of infection of 13 percent. These five cases represent 23 percent of the pneumonia occurring in this group of patients. Patients undergoing bone marrow transplantation are highly susceptible to infectious complicati

Increased perinephric activity in 99mTc-DTPA studies of renal transplants.

All 99mTc-DTPA studies of renal transplant patients whose grafts infarcted during a three-and-one-half-year period were reviewed. Four patients demonstrated absent renal perfusion and clearance and absent bladder activity. Three of these exhibited increased perinephric 99mTc-DTPA activity and never regained renal function. The fourth patient did not demonstrate increased perinephric activity, and some return of graft function appeared in a subsequent 99mTc-DTPA study. The pathogenesis of increased perinephric activity is unknown, but may signify renal infarction, and its absence may indicate p

Factors related to success or failure of second renal transplants.

From January 1, 1968 to December 31, 1973, 50 patients received two or more kidney transplants. Patient and graft survival was highly dependent upon the source of the donor and to a lesser extent the functional duration of the first transplant and the elapsed time between first and second graft. Survival (patient and graft) was best in patients receiving two related grafts and worst in patients receiving two sequential cadaver grafts. Intermediate rates of success followed cadaver transplantation after rejection of a related graft. The highest failure rate was encountered when those patients w

[Long-term use of immunosuppressants in patients with kidney transplants].

The authors analyse the principal characteristics of immunosuppression needed after a kidney graft. They review various therapeutic methods and consider the main complication induced by long-term iummunosuppression. They associate published data with their own experience, based on more than 300 transplantations performed with corticosteroids and azathioprine at Necker Hospital.

Innate-immune crosstalk orchestrates T cell-mediated rejection in kidney transplants.

OBJECTIVE: T cell-mediated rejection (TCMR) remains a major barrier to long-term kidney allograft survival, driven by recipient immune responses against donor antigens. A comprehensive understanding of the cellular and molecular mechanisms underlying TCMR is essential for developing novel diagnostic and therapeutic strategies. METHODS: We integrated two public single-cell RNA sequencing datasets (GSE145927 and E-MTAB-12051) to construct a comprehensive single-cell atlas of kidney allograft biopsies. Unsupervised clustering, functional scoring, trajectory inference, gene regulatory network ana

Microbial and Proteomic Signature of Acute Cellular Rejection in Lung Transplants.

RATIONALE: Acute cellular rejection (ACR) is a common cause of early post lung transplant morbidity and mortality. Identification of ACR biomarkers may provide a less invasive mechanism for diagnosing ACR, improve understanding of the mechanisms behind ACR, and identify novel pathways for intervention. OBJECTIVES: To determine whether unique microbial or protein signatures exist in lung transplant patients with ACR. METHODS: We performed a nested case-control study utilizing bronchoalveolar lavage (BAL) fluid and plasma from 9 lung transplant cases with mild ACR (A2) paired with 9 ACR-free c

Graft rejection across solid organ transplants: mechanisms, monitoring, and immunosuppressive therapeutics.

BACKGROUND: Organ and tissue transplantation has transformed the management of end-stage organ failure, yet graft rejection remains a major barrier. Rejection arises from complex immune mechanisms involving MHC mismatch, T-cell allorecognition, and antibody-mediated injury. Advances in immunosuppressive therapy have improved graft survival, but significant challenges persist. AIM: This scoping review synthesizes current insights into the immunological basis of graft rejection and evaluates conventional, biologic, and emerging immunosuppressive strategies. Particular attention is given to orga

Beginning of ABO-Incompatible Transplants in Pakistan: A Single-Centre Experience.

Background The burden of end-stage renal disease (ESRD) is ever-increasing in Pakistan. Among the treatment options available, kidney transplantation remains the most effective treatment for ESRD, offering improved quality of life and survival benefits over dialysis. ABO blood group incompatibility has traditionally been viewed as a barrier to transplantation. However, we have started ABO-incompatible (ABOi) renal transplant in Pakistan since 2023 with good clinical outcomes. Objectives This study aims to document the initiation of ABOi renal transplants in Pakistan, while evaluating clinical

Out of sequence heart transplants: Why, how many, and to whom.

BACKGROUND: Heart transplantation (HT) is governed by the Organ Procurement and Transplantation Network, which commissions 56 organ procurement organizations (OPO) to perform allocation. Each allocation follows a match list of eligible donors ranked by urgency. We sought to describe the prevalence of OOS HT, characterize donor and recipient characteristics, and assess if the centers and recipients of OOS donors differ from non-OOS donors. METHODS: Using the Scientific Registry of Transplant Recipients all adult (≥18 years), single organ HT recipients between 1/18/2018-1/1/2024 were identified

Molecular signatures discriminating different types of rejection in human liver transplants.

BACKGROUND & AIMS: The role of antibody-mediated rejection (AMR) after liver transplantation (LT) remains controversial. Chronic AMR (cAMR) is often subclinical and may be missed without surveillance biopsies (svLbx). Transcriptome analyses have previously characterized molecular changes in T cell-mediated rejection (TCMR) after solid organ transplantation. We aimed to identify molecular signatures of cAMR after LT. METHODS: Indication and surveillance biopsies from two prospective institutional biorepositories were screened. We performed bulk RNA sequencing on liver biopsies (discovery cohor

Revolutionizing pancreatic islet organoid transplants: Improving engraftment and exploring future frontiers.

Type-1 Diabetes Mellitus (T1DM) manifests due to pancreatic beta cell destruction, causing insulin deficiency and hyperglycaemia. Current therapies are inadequate for brittle diabetics, necessitating pancreatic islet transplants, which however, introduces its own set of challenges such as paucity of donors, rigorous immunosuppression and autoimmune rejection. Organoid technology represents a significant stride in the field of regenerative medicine and bypasses donor-based approaches. Hence this article focuses on strategies enhancing the in vivo engraftment of islet organoids (IOs), namely vas

Canadian Highly Sensitized Patient Program Report: A 1000 Kidney Transplants Story.

PURPOSE: Highly sensitized patients (HSPs) with kidney failure have limited access to kidney transplantation and poorer post-transplant outcomes. Prioritizing HSPs in kidney allocation systems and expanding the pool of deceased donors available to them has helped to reduce their wait times for transplant and enhanced post-transplant outcomes. The Canadian HSP Program was established by Canadian Blood Services in collaboration with provincial organ donation and transplantation programs throughout the country to increase transplant opportunities for transplant candidates needing very specific ma

Outcomes in organ transplants from donors with melanoma: a systematic review.

Melanoma transmitted through organ transplantation is an increasingly reported event. Immunosuppression increases the risk of melanoma; however, transmission of malignancy from transplanted organs is a distinct etiology of melanoma occurrence. The risk of transmission of melanoma from an organ donor with melanoma has yet to be determined. The authors aimed to investigate this phenomenon by reviewing the outcomes of patients that received organs from donors with melanoma. A systematic literature review was conducted with emphasis on identifying organ donors with known histories of melanoma and

[From the first kidney transplants to the current pediatric kidney transplant].

Kidney transplantation is the preferred treatment for end-stage renal failure in children but remains a rare procedure with only 100 to 120 pediatric kidney transplants per year in France. Although the main principles of kidney transplantation are the same in children and adults, some specificities regarding underlying kidney diseases, surgical technique, immunosuppressive drugs metabolism and the risk of infectious complications require a specific expertise to care for these patients. Similarly, the major morbidity of dialysis in children and the need for repeated transplants during the patie

The Molecular Phenotype of Kidney Transplants: Insights From the MMDx Project.

This review outlines the molecular disease states in kidney transplant biopsies as documented in the development of the Molecular Microscope Diagnostic System (MMDx). These states include T cell-mediated rejection (TCMR), antibody-mediated rejection (AMR), recent parenchymal injury, and irreversible atrophy-fibrosis. The MMDx project, initiated through a Genome Canada grant, is a collaboration involving many centers. MMDx uses genome-wide microarrays to measure transcript expression, interprets the results using ensembles of machine learning algorithms, and generates a report. Experimental stu

Kidney function and transplants in prune belly syndrome: a scoping review.

BACKGROUND: Children with prune belly syndrome (PBS) are at higher risk of developing kidney dysfunction and requiring kidney replacement therapy (KRT). While studies have described surgical and survival outcomes in these populations, there has yet to be a focused synthesis of evidence regarding kidney outcomes in this population. Here, the focus of this scoping review was to highlight knowledge gaps and report standards on kidney outcomes in PBS of all ages. METHODS: Following scoping review methodology, EMBASE, MEDLINE, and Scopus were searched for peer-reviewed literature that describe kid

Molecular states associated with dysfunction and graft loss in heart transplants.

BACKGROUND: We explored the changes in gene expression correlating with dysfunction and graft failure in endomyocardial biopsies. METHODS: Genome-wide microarrays (19,462 genes) were used to define mRNA changes correlating with dysfunction (left ventricular ejection fraction [LVEF] ≤ 55) and risk of graft loss within 3 years postbiopsy. LVEF data was available for 1,013 biopsies and survival data for 779 patients (74 losses). Molecular classifiers were built for predicting dysfunction (LVEF ≤ 55) and postbiopsy 3-year survival. RESULTS: Dysfunction is correlated with dedifferentiation-decrea

Methylene blue and the NO/cGMP pathway in solid organs transplants.

The nitric oxide/cyclic guanosine monophosphate (NO/cGMP) pathway has a significative influence in hemodynamic changes that occur in transplants. Classically, the ischemia-reperfusion syndrome (IRS) is characterized by hypotension and low vascular resistance, when cGMP and nitric oxide (NO) are increased, contributing to oxidative stress, within an inflammatory context. These mechanisms occur in several types of transplants, such as liver, heart, lungs, kidney, which are a therapeutic choice in several clinical conditions when conventional treatments failed. It is well known the significant re

Tumor Formation of Adult Stem Cell Transplants in Rodent Arthritic Joints.

PURPOSE: While imaging matrix-associated stem cell transplants aimed for cartilage repair in a rodent arthritis model, we noticed that some transplants formed locally destructive tumors. The purpose of this study was to determine the cause for this tumor formation in order to avoid this complication for future transplants. PROCEDURES: Adipose-derived stem cells (ADSC) isolated from subcutaneous adipose tissue were implanted into 24 osteochondral defects of the distal femur in ten athymic rats and two immunocompetent control rats. All transplants underwent serial magnetic resonance imaging (MR
