Options for field diagnosis of human african trypanosomiasis.

Human African trypanosomiasis (HAT) due to Trypanosoma brucei gambiense or T. b. rhodesiense remains highly prevalent in several rural areas of sub-Saharan Africa and is lethal if left untreated. Therefore, accurate tools are absolutely required for field diagnosis. For T. b. gambiense HAT, highly sensitive tests are available for serological screening but the sensitivity of parasitological confirmatory tests remains insufficient and needs to be improved. Screening for T. b. rhodesiense infection still relies on clinical features in the absence of serological tests available for field use. Ong

Orchitis as an unusual manifestation of human African trypanosomiasis.

African trypanosomiasis is a re-emerging disease. We report the case of an African patient whose predominant symptom was infertility due to a granulomatous orchitis. The patient was afebrile and had not been in Africa for years. Lymphadenopathy and splenomegaly led us eventually to the diagnosis of sleeping sickness. After treatment with suramin his spermiogram returned to normal. Sleeping sickness evolves through clinically different stages and leads to death if left untreated. The disease may, however, present clinically extremely variable and may thus be difficult to diagnose.

[Contributions and limits of the diagnosis of human African trypanosomiasis].

Human African trypanosomiasis, or sleeping sickness, is still a worrying problem in Africa. Sleeping sickness is a disease for which a systematic monitoring is necessary, particularly for the trypanosomiasis caused by Trypanosoma brucei gambiense, which is characterized by a long asymptomatic stage. In the absence of specific clinical signs, mass screening of populations remains the only way to control the disease and to avoid its spreading. The lack of sensitivity and specificity of the diagnosis tests classically used led to the development of molecular tools. PCR amplification of parasite s

African trypanosomiasis in travelers returning to the United Kingdom.

Two returning safari tourists with African trypanosomiasis were admitted to the Hospital for Tropical Diseases, London, in a 3-day period, compared with six cases in the previous 14 years. We describe the clinical features, diagnosis, and problems encountered in accessing appropriate therapy, and discuss the potential for emergence of this disease in increasingly adventurous international travelers.

Perspectives for new drugs against trypanosomiasis and leishmaniasis.

Diseases caused by pathogenic trypanosomatids cause great suffering throughout the developing world. New drugs for these diseases are urgently needed. Recent technological advances have permitted the identification and validation of numerous drug targets in these organisms. However, efforts to develop inhibitors of these targets, that may then be taken forward for development into new drugs, have been comparatively scarce. In this review we discuss the design, synthesis and evaluation of inhibitors of two drug targets in trypanosomatids, 6-phosphogluconate dehydrogenase, the third enzyme of th

Options for vector control against trypanosomiasis in Africa.

Tsetse control has long been an important option for reducing the impact of African trypanosomiasis but, although many effective methods have been used, the results have seldom proved sustainable. Developments to reduce cost and environmental impact increasingly limit the choices available for control and the scale of operations has declined. Conversely, human trypanosomiasis has reached epidemic proportions in some countries. Here, Reg Allsopp argues that those tasked with managing trypanosomiasis or committed to poverty alleviation in Africa should consider large-scale, area-wide tsetse cont

Localization of genes controlling resistance to trypanosomiasis in mice.

Tsetse fly-transmitted trypanosomes (Trypanosoma spp.) cause "sleeping sickness' in man and have a serious impact on livestock-based agriculture in large areas of Africa. Multigene control of variation in susceptibility to trypanosomiasis is known to occur in mice, where the C57BI/6 (B6) strain is relatively resistant and the A/J (A) and Balb/c (B) strains are susceptible. Such resistance is also well described among several types of west African cattle. We report here the results of genome-wide scans for genes controlling this trait in the B6 mouse using crosses with two different susceptible

[Neuroimaging and efficacy of treatment in advanced African trypanosomiasis].

INTRODUCTION: West African trypanosomiasis (WAT) is rare in Spain. Delay in its diagnosis and treatment leads to irreversible diffuse meningoencephalitis and finally death of the patients. CLINICAL CASE: We described a patient in whom the diagnosis of advance WAT had been delayed. He had headache, alteration of the level of consciousness and sleep pattern, psychiatric disorders, crises, extrapyramidal, sensitive and endocrinological clinical alterations. Biochemical investigation showed slight anemia with marked thrombocytopenia, and raised ESR and IgM. Initially serology was negative for try

A new approach for the pathogenesis of human African trypanosomiasis.

From Trypanosoma brucei brucei, a molecule has been isolated which triggers the production of IFN-gamma by CD8+ T-cells. IFN-gamma modulates events in the hosts immune- and nervous system and provides a growth stimulus for the parasites. Furthermore, a molecule with IFN-gamma-like immunoreactivity has been detected in rat dorsal root ganglion cells and certain neurons in the brain. This neuron-derived IFN-gamma-like molecule differs in molecular weight from lymphocyte-derived IFN-gamma but shares important biological activities with the latter, which includes a growth stimulus for trypanosomes

[The distribution of sleep and wakefulness in human African trypanosomiasis].

Last century, patients with human African trypanosomiasis were described as sleepy by day and restless by night, and physicians referred to this condition as sleeping sickness. Such a description could have evoked a disturbance of circadian rhythms. However, it is only in 1989 that the first 24-hour recording was performed by our team in Niamey (Niger) in a patient with sleeping sickness. The patient was a Niger-born farm worker who had contracted the disease near Gagnoa (Côte d'Ivoire). Polysomnographic recordings (electroencephalogram, EEG, electrooculogram, electromyogram, electrocardiogram

[From sleep observation to the discovery of trypanosomiasis and glossina].

The african human trypanosomiasis constitutes one of the fascinating subjects of research about the expansion of the European biomedical science. Its history shows the scientific climate in which the investigations took place. Researching the cause which determined it, the researchers got lost in mistakes and comparisons as well as stimulating factors. It is in this atmosphere that trypanosome had been discovered in 1901 and glossina had been confirmed in 1903. But, the disease prevention needed again a lot of researches to correctly understand the complex links between pathogenesis and vector

Study of evoked potentials in human African trypanosomiasis.

Human African trypanosomiasis or sleeping sickness has a stage of neurological involvement characterized by the onset of diffuse meningoencephalitis with sleep disturbances and decreased wakefulness. The pathogenesis of this disease is not well understood. We studied auditory, visual, sensory, and motor evoked potentials in 16 patients with trypanosomiasis in the early stage of meningoencephalitis. In all patients, the brain-stem auditory evoked response (BAER) and the pattern-reversal visual evoked response (PVER) were normal. On the other hand, abnormalities of the somatosensory evoked respo

[New data on the control of African animal trypanosomiasis].

The glossina control turns towards cheaper, ecological, easier techniques: trapping, which the efficacy might be increased by odour attractants; synthetic pyrethrinoids (impregnated targets; dipping or application of "Pour-on" formulations on skin of cattle). Only one new trypanocide was set up during the last 25 years: Melarsamine, arsenical derivative, effective in camels, cattle, horses infected by Trypanozoon (3.5 mg/kg b. w. by IM or SC routes). The trypanotolerant cattle is characterized by a better control of parasitemia and anaemia, dominant gene A of hemoglobin and gene F of albumin,

Control of tsetse flies and trypanosomiasis: Myth or reality?

The African trypanosomiasis are among Africa's most devastating diseases. The human disease, sleeping sickness, and the animal disease, nagana, are caused by trypanosomes, protozoan parasites transmitted by tsetse flies, Glossina spp. Attempts have been made to control tsetse and trypanosomiasis for over 70 years, supported by ever-increasing amounts of foreign aid. Although progress has been made in the control of sleeping sickness, this disease still persists in many countries. Nogono excludes cattle from many of the potentially most productive areas of Africa and is a major constraint on ec

Prevalence of trypanosomiasis in cattle in south-west Zambia.

A trypanosomiasis survey was conducted in South-West Zambia. From a total of 3,346 cattle sampled 342 cattle showed a positive trypanosomiasis parasitaemia. During the survey trypanosome species and PCV values were also recorded. With simple statistical analysis populations with higher and lower prevalence rates were differentiated. The results indicated that the Kwando River Basin Tsetse Fly Belt and the Kafue River Basin Tsetse Fly Belt infested a larger area than originally assumed and that a link-up between both belts occurred or will occur in the near future.

The role of antibody in natural resistance to African trypanosomiasis.

The nature and usefulness of trypanoresistance in West African Baoule cattle was studied by exposing animals in areas of high Glossina density. Under such conditions, Zebu and some Baoule died soon with high parasitaemia while resistant Baoule showed little patent parasitemia, almost no anaemia and thrived. Subsequently the role that specific antibody may play in such trypanoresistance was analyzed. In the first instance we examined protective antibody titres during T. congolense infection of inbred mice strains, comparing results obtained in trypanosome-mouse combinations where the host contr
