OBJECTIVES: Determine whether laparoscopic surgery for painful syndromes of the right iliac fossa in young women can reduce the number of unnecessary appendectomies and shorten hospital stay.

Congestive heart failure (CHF) patients have typical radiological features that assist in diagnosis. This is a case that demonstrates an atypical radiologic presentation for CHF which showed diffuse pulmonary nodules on CT. Diffuse nodules can be present in several disease processes and clinical correlation is a necessity for diagnosis. It is important for radiologists and clinicians to be aware of uncommon radiological features of CHF to avoid unnecessary procedures or treatments.

The abdominal cocoon is a rare disease in which there is total or partial encapsulation of the small bowel by a fibrous membrane. A correct diagnosis is not often made pre-operatively. Awareness of this rare cause of surgical emergency may prevent delay in treatment and avoid unnecessary procedures for the patient, such as bowel resection. We report a patient diagnosed with abdominal cocoon who was subsequently treated.

Urinary retention in children is rarely caused by an extrinsic compression of an extragonadal pelvic mass. The sonographic appearance of these masses are divided into 3 categories: 1) solid, 2) fluid-filled, and 3) mixed. Familiarity with the sonographic spectrum of these lesions can limit the differential diagnosis, will suggest specific follow-up procedures needed for an accurate diagnosis and will eliminate the need for unnecessary invasive workups.

This review has emphasized the need to manage carcinoma of the lung with a systematic approach. Diagnostic and staging procedures should be performed to prevent unnecessary thoracotomy. In patients with limited pulmonary reserve, segmentectomy or wedge resection is an acceptable procedure. A staging mediastinal node dissection should be included in every resection for lung cancer. Those patients with positive mediastinal nodes found in this manner should receive postoperative mediastinal irradiation.

Cystic renal dysplasia, classified by Osathanondh and Potter as renal cystic disease types II and IV, is probably the result of urinary tract obstruction in fetal life. Since these kidneys function poorly, sonography plays a more significant role in their diagnosis than excretory urography. Familiarity with the sonographic spectrum of cystic renal dysplasia will help the physician to make an accurate diagnosis, and may obviate the need for unnecessary invasive procedures.

We conducted a preintervention-postintervention study to assess the effectiveness of a multimodal approach to reduce unnecessary urethral catheters in 5 Japanese intensive care units. After the intervention urethral catheter point prevalence decreased by 18%, from 79% preintervention to 61% postintervention, and catheter appropriateness increased by 28%, from 57% preintervention to 85% postintervention.
