INTRODUCTION: Delayed sternal closure after paediatric open heart procedure is often necessary. The risk of delayed sternal closure is infection: superficial wound or sternal and mediastinal infection. The incidence of sternal wound infection reported in the literature varies from 0.5 to 10%. The mortality for poststernotomy deep sternal infection continues to be high--from 14 to 47%. Established treatment includes surgical debridement, drainage and irrigation, antibiotics, frequent change of wound dressing and direct or secondary closure with omentum or pectoral muscle flap.

BACKGROUND: Numerous authors have reported promising results with the use of vacuum-assisted closure therapy in poststernotomy mediastinitis. The negative pressure applied to the anterior mediastinum substantially exceeds the normal negative pressure in the pleural cavities, and interaction with respiratory physiology cannot be excluded. The aim of the present study was to evaluate whether the application of six clinically relevant negative pressures between -50 mmHg and -175 mmHg to the sternotomy wound affects respiratory parameters in a porcine model.

INTRODUCTION: Vacuum therapy of hand defect problematic due to the interdigital folds and in particular due to the small surface. As a temporary cover in staged procedures, e. g. following excision of tumor-suspicious skin alterations at the hand, and/or at the fingers, the application of vacuum therapy results not only in a sterile, temporary cover of the wound, furthermore it promotes wound conditioning and reduction of the wound area as well as lymph and edema reduction.

SUMMARY OBJECTIVE: To review vacuum deliveries conducted at Komfo Anokye Teaching Hospital (KATH) Kumasi, over a one year period.
METHODS: The labour records of parturients who reported at KATH between 1(st) January and 31(st) December 2000 were retrieved. The records of those who had vacuum deliveries were reviewed for baseline practices and outcomes.

Subgaleal hemorrhage is a rare condition of the neonate often associated with instrumental delivery. It is a potentially fatal condition that is often underreported and underdiagnosed. The author was asked to provide an expert assessment of the case histories of 28 patients delivered by vacuum extraction. At the start of the extraction the foetal head level was at station 0 or above in 25 of the patients. In 12 cases the duration of extraction exceeded 14 min and in 7 of the cases it exceeded 19 minutes with a maximum of 45 minutes. OBJECTIVE: To compare a one-step (rapid) application of negative pressure (vacuum) with conventional stepwise application for ventouse extraction following a prolonged second stage of labour.

Altogether 4719 women who had had a legally induced abortion in Uppsala County during 1970-75 were followed by means of computerized record linkage, and deliveries during 1970-78 after abortion were noted. Out of 1 364 deliveries traced, 670 were to primiparas who had had a previous vacuum-aspiration (VA) abortion. The results of a preliminary clinical appraisal of a newly designed vacuum extractor - the McGoldrick rotational vacuum extracter is presented. This instrument is designed to combine the rotation advantages of the various rotating obstetric forceps with the multiple advantages of the conventional vacuum extractor in the management of persistent occipito posterior and occipito transverse positions. The design features include an elliptical shaped cup attached to a series of bars and universal joints which allow both rotational and traction forces to be applied to the foetal scalp.
