Childhood deafness in Cape Town.

An aetiological survey of a Cape Peninsula school for the deaf, in which 366 Cape Coloured and Asiatic children were examined, is reported. The relevant hereditary factors and the abnormal clinical findings are documented, and the more important of these are discussed. In 20% (74) of the children, the deafness was genetic; in 36% (132) it was acquired, and in 44% (160) it was cryptogenic.

Degree of Hearing Improvement and Reduction of Air-Bone Gap After Tympanoplasty in a Tertiary Hospital in Saudi Arabia.

BACKGROUND: A clinical condition known as chronic otitis media (COM) is characterized by tympanic membrane perforation, varying degrees of hearing loss, and otorrhea that lasts for two to six weeks. COM alone or with cholesteatoma may result in ossicular chain discontinuity and ossicular erosion. The hearing restoration procedure includes repairing the eardrum and building the ossicular chain in ears with damaged ossicles. Multiple studies suggest the predictive value of preoperative air-bone gap (ABG) to detect the ossicular chain status can help with proper preoperative planning for surgery.

Effect of freezing and embalming of human cadaveric whole head specimens on bone conduction.

BACKGROUND AND AIMS: Conserved specimens do not decay and therefore permit long-term experiments thereby overcoming limited access to fresh (frozen) temporal bones for studies on middle ear mechanics. We used a Thiel conservation method which is mainly based on a watery solution of salts. In contrast to pure Formalin, Thiel conservation aims to preserve the mechanical proprieties of human tissue. The aim of this study is to examine the effect of Thiel conservation on bone conduction in the same specimen before and after conservation. METHODS: Nine ears of five defrosted whole heads were stimu

A 1.6-Mb microdeletion in chromosome 17q22 leads to NOG-related symphalangism spectrum disorder without intellectual disability.

Microdeletions in chromosome 17q22, where the NOG gene resides, have been reported leading to the NOG-related symphalangism spectrum disorder (NOG-SSD), intellectual disability and other developmental abnormalities. In this study we reported a dominant Chinese Han family segregating with typical NOG-SSD symptoms including proximal symphalangism, conductive hearing loss, amblyopia and strabismus, but not intellectual disability. Sanger sequencing identified no pathogenic mutation in the coding regions of candidate genes NOG, GDF5 and FGF9. SNP genotyping in the genomic region surrounding NOG id

Clinical value of combined single photon emission computerized tomography and conventional computer tomography (SPECT/CT) in sports medicine.

Integrated hybrid single photon emission computerized tomography (SPECT)/computer tomography (CT) is a promising new diagnostic imaging modality for orthopedic patients. A high diagnostic yield is available from combining the detection of abnormal bone metabolism with SPECT, to the precise anatomical detail available in high resolution CT. With this review, we endeavor to illustrate the clinical value and future perspectives of SPECT/CT in sports medicine.

Revision stapedectomy for incus erosion: long-term hearing.

One of the most common ossicular problems in revision stapedectomy is the eroded incus. Revision surgery has been reported as successful in 70% to 80% of cases at 1 year. Little is written about long-term results or the association of erosion with various prostheses. We evaluated 83 cases from 1 to 20 years, including multiple revisions. In 23 cases the erosion was seen at initial stapedectomy. Surgery was performed with the patient under local anesthesia, with the use of the Lippy modified prosthesis. Initial success was seen in 72% (41/57), satisfactory results in 90%, no change in 5%, and n

Sudden deafness: a retrospective evaluation of dextran therapy.

A retrospective evaluation was performed in 112 patients treated during a 10-year period as inpatients with the diagnosis idiopathic sudden hearing loss. Excluding all patients in whom later other diagnosis were established, like Ménière's disease, collagenoses, mumps etc., 101 patients remained. 80 of them had been treated consistently according to a protocol as having idiopathic sudden hearing loss. These patients had all 5-day treatment with low molecular weight dextran and nicotinic acid and vitamin B during 1 month. 68% did completely recover or were markedly improved (> 30 dB), another 1

Multiple synostoses syndrome: Clinical report and retrospective analysis.

Multiple synostoses syndrome (SYNS1; OMIM# 186500) is a rare autosomal dominant disorder reported in a few cases worldwide. We report a Chinese pedigree characterized by proximal symphalangism, conductive hearing loss, and distinctive facies. We examined the genetic cause and reviewed the literature to discuss the pathogeny, treatment, and prevention of SYNS1. Audiological, ophthalmological, and radiological examinations were evaluated. Whole-exome sequencing (WES) was performed to identify mutations in the proband and her parents. Sanger sequencing was used to verify the results for the proba

A point mutation in Fgf9 impedes joint interzone formation leading to multiple synostoses syndrome.

Human multiple synostoses syndrome (SYNS) is an autosomal dominant disorder characterized by multiple joint fusions. We previously identified a point mutation (S99N) in FGF9 that causes human SYNS3. However, the physiological function of FGF9 during joint development and comprehensive molecular portraits of SYNS3 remain elusive. Here, we report that mice harboring the S99N mutation in Fgf9 develop the curly tail phenotype and partially or fully fused caudal vertebrae and limb joints, which mimic the major phenotypes of SYNS3 patients. Further study reveals that the S99N mutation in Fgf9 disrup

Wrist denervation in isolation: a prospective outcome study with patient selection by wrist blockade.

Surgical wrist denervation involves division of the anterior and posterior interosseous nerves and articular branches of the superficial radial nerve. In this outcome study, 37 patients were individually assessed and deemed suitable for denervation surgery due to appreciable symptom resolution following a local anesthetic wrist block. At a mean of 18 months following denervation surgery, median activity pain scores had decreased by 60% (p < 0.001) from initial assessment levels, and more than three quarters (30/37) of patients reported continued improvement in their activity pain (p < 0.001).

[Temporary sensory hearing deficits after ear surgery--a retrospective analysis].

BACKGROUND: Temporary and persistent threshold shifts can occur after ear surgery. So far, only few studies deal with temporary sensory hearing deficits after ear surgery. PATIENTS AND METHODS: In a retrospective study, thresholds at 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz were analysed in 393 patients (125 x mesotympanal chronic otitis media, 164 x cholesteatoma, 44 x tympanosclerosis, 60 x otosclerosis) before, the first 4 days and 3 weeks after ear surgery to evaluate possible temporary threshold shifts. RESULTS: Analysis of all patients in total proved statistically significant slight tempo

Short- and long-term results of stapedectomy in children.

Studies have indicated that stapedectomy can be an effective procedure in children for correcting conductive hearing losses due to juvenile otosclerosis. However, because childhood otosclerosis is rare and children commonly choose to use hearing aids in lieu of undergoing surgery, little outcome data are available. The purpose of this retrospective study was to provide additional outcome data in both the short and the long term. Stapedectomies were performed on 47 children. Preoperative hearing results were compared with 6-month postoperative hearing results. Hearing results for the children w

Stapedotomy with stapedius tendon preservation: technique and long-term results.

From 1975 to 1992, a total of 1459 primary stapedoplasty operations (i.e., stapedectomy, stapedotomy with tendon section, and stapedotomy with tendon preservation) were performed in 1323 subjects at the Ear, Nose, and Throat Department of the University of Verona. Pure-tone audiometry was carried out in all subjects who were operated on and who underwent 6-month and 10-year follow-ups. To evaluate the influence of stapedius tendon preservation on functional outcome, we randomly selected subgroups of patients using the three different stapedioplasty techniques and tested them by multiple-freque

Screening for Maternally Inherited Diabetes and Deafness in Large Cohorts of Hearing Impaired and Diabetic Patients.

OBJECTIVES: Mitochondrial DNA (mtDNA) mutations account for up to 5% of hereditary hearing loss (HL) cases. Most commonly, the m.3243A>G mtDNA variant contributes to rare monogenic Maternally Inherited Diabetes and Deafness or Mitochondrial Encephalopathy, Lactic Acidosis, and Stroke-like episodes syndromes. Different proportions of the mutated mtDNA (heteroplasmy) among the affected tissues result in variability in the clinical manifestation and severity of the phenotype. The aim of the presented study was to establish the prevalence of the m.3243A>G variant in large cohorts of patients with
