A Case of Celiac Disease, Epilepsy, and Cerebral Calcifications With Temporal Lobe Epilepsy.

This is a case report of a 55-year-old man with medication refractory right temporal lobe epilepsy since adolescence. He was found to have bilateral posterior cerebral calcifications on routine head computed tomography with confirmation on magnetic resonance imaging. He also had elevated antibody markers for celiac disease. He was diagnosed with the rare, but well-described syndrome of celiac disease, epilepsy, and cerebral calcifications (CEC). He failed a brief trial of gluten-free diet and went on to have a right temporal lobectomy with sustained freedom from disabling seizures. This case i

A Review of Clinical Signs Related to Ecchymosis.

Ecchymosis is a large area of discoloration caused by extravasation of blood into the subcutaneous tissue. It is an objective physical finding that may provide valuable clues as to its possible etiology. Ecchymosis is associated with eponyms based on the physician who first described the physical findings, which can be divided into 4 anatomical categories: base of the skull, abdominal wall and retroperitoneum, groin and scrotum, and lower extremity. Classic external signs and eponyms associated with ecchymosis are reviewed. Knowledge of these signs on physical examination may prove to be a use

Tolvaptan for SIADH in Myelodysplastic Syndrome with Blast Crisis.

Syndrome of inappropriate antidiuretic hormone secretion (SIADH) is a common cause of hyponatremia in cancer patients. It is most frequently reported in association with small-cell lung cancer, but has been reported in other cancers as well. Here we report the case of a patient with myelodysplastic syndrome and blast crisis who developed concurrent hyponatremia. The patient failed to respond to fluid restriction and administration of hypertonic saline. She was treated with tolvaptan, a vasopressin antagonist licensed for the treatment of adult patients with hyponatremia secondary to syndrome o

Hypocalcemia Secondary to Zoledronate Therapy in a Patient With Low Vitamin D Level.

Zoledronate (ZDA) is a bisphosphonate used to treat hypercalcemia that commonly occurs with malignancy, multiple myeloma, and bone metastases from solid tumors. It acts primarily by decreasing osteoclastic activity, thereby slowing the release of skeletal calcium. However, a potential adverse effect of ZDA is hypocalcemia that can be symptomatic, especially in patients with risk factors such as hypomagnesemia, hypoparathyroidism, renal failure, and vitamin D deficiency. We report the case of a patient with extensive stage small cell lung cancer with multiple osseous and visceral metastases who

Coronary dissection in a patient with essential thrombocytosis.

A 50-year-old man was admitted to the hospital with left shoulder and arm discomfort. He had no recent history of change in his energy level, limitations to activity, exertional chest pain, or shortness of breath. He had cardiac risk factors, including a strong family history of pre-mature coronary artery disease and dyslipidemia. He had a syncopal episode in 2003 with a positive troponin I, but had a negative cardiac workup, including cardiac catheterization that showed luminal irregularities and no significant coronary artery disease. An echocardiogram was performed to rule out potential car

Immediate total parenteral nutrition after radical cystectomy and urinary diversion.

INTRODUCTION: The purpose of this study is to determine if administration of total parenteral nutrition (TPN) immediately following radical cystectomy and urinary diversion provides significant recovery benefit when compared to patients who did not receive TPN. METHODS: Retrospective chart review was performed on patients who underwent open radical cystectomy and urinary diversion from February 2002 to June 2010. Patients were divided into 2 cohorts-those who received immediate postoperative TPN and those who did not. Preoperative demographics, length of hospital stay, time until tolerating a

Daptomycin-induced acute eosinophilic pneumonia.

INTRODUCTION: Daptomycin is a cyclic lipopeptide antibiotic with activity against gram-positive organisms. With increasing use, acute eosinophilic pneumonia is a rare, but potentially fatal adverse drug reaction that requires prompt recognition. The authors present a definite case of daptomycin-induced acute eosinophilic pneumonia. CASE SUMMARY: A 61-year-old woman with poorly controlled type 2 diabetes who presented with bilateral foot pain was found to have bilateral calcaneal osteomyelitis. She was started on an antibiotic regimen that included daptomycin. Within 1 week, she developed feve

Large epidermal inclusion cyst presenting as a pelvic mass.

INTRODUCTION: Epidermal inclusion cysts are common discrete nodules often formed in areas of previous trauma or surgery. A literature review indicated that large epidermal inclusion cysts of the pelvis are extremely rare. Accordingly, we present a case of a woman with a large pelvic epidermal inclusion cyst of the vaginal cuff, along with imaging studies and a review of the literature. CASE PRESENTATION: A 49-year-old woman, 13 years after vaginal hysterectomy, was found to have a 7-cm soft tissue pelvic mass, discovered on a computed tomography (CT) scan performed for abdominal pain. Streptococcus infection in a newborn.

Streptococcus salivarius is an uncommon cause of infection in neonates. Normally present in the oral flora of humans, S salivarius is the least pathogenic member of the viridans group streptococci and is often considered a contaminant when detected on blood culture. While rare, it has been shown in the literature to cause clinically relevant bacteremia and other invasive infections typically in the immunocompromised. We report the case of a well-appearing 1-day-old female with sequential positive blood cultures for S salivarius. This case has important implications as it demonstrates that S sa

Mastocytosis as an unusual cause of hip fracture in an elderly woman.

Mastocytosis is a type of myeloproliferative neoplasm characterized by accumulation and proliferation of morphologically and immunophenotypically abnormal mast cells in 1 or more organ systems. Clinical manifestations vary depending upon the organ involved and chemical mediators released by mast cells along with constitutional symptoms and musculoskeletal complaints. We report a case of isolated bone marrow mastocytosis in an 87-year-old woman who presented with a fall resulting in proximal femur fracture. Bone marrow biopsy revealed mastocytosis, and no evidence of systemic involvement or per

Idiopathic spontaneous rupture of an intercostal artery.

Spontaneous rupture of an intercostal artery is exceptionally rare without an inciting event such as trauma or nontraumatic arterial wall weakening. This report details the diagnosis and treatment of a 47-year-old man who presented with a spontaneous hemothorax from an intercostal artery. There are very few reports in the literature documenting spontaneous intercostal artery rupture without associated illness or injury.

Development of a pilot family medicine hand-carried ultrasound course.

BACKGROUND AND OBJECTIVES: A hand-carried ultrasound training session was organized as an initial step in developing a long-term ultrasound education program for family medicine residents and faculty. Comparative effectiveness studies examining the potential benefits, risks, and any possible cost savings associated with this technology will be predicated on having a sufficient number of primary care physicians trained and able to use hand-carried ultrasounds as part of routine care. The proposed training described here is a first step toward this broader conversation and empirical study of han

Bronchogenic cyst in the intradiaphragmatic location.

Bronchogenic cysts are congenital foregut malformations thought to develop due to abnormal budding of tracheal diverticulumand proximal bronchial structures during embryologic development. The cyst is lined by ciliated pseudostratified columnar epithelium and the wall contains cartilage and layers of smooth muscle. These lesions most commonly are seen in the mediastinum, lung, or pleural spaces. The intradiaphragmatic location of the bronchogenic cyst rarely has been reported in the literature. We report the clinical presentation and computed tomography and magnetic resonance imaging findings

Digital ischemia as a paraneoplastic consequence of squamous cell lung carcinoma.

We report the case of a 40-year-old man who presented with digital ischemia and squamous cell lung carcinoma. Based on review of the literature, to our knowledge this case represents the youngest patient with lung carcinoma associated with digital ischemia and the only one with this type of tumor. The patient's digital ischemia symptoms improved rapidly with systemic chemotherapy; however, he did eventually lose the distal portion of 1 finger to dry gangrene and mummification.

Bronchopulmonary sequestration presenting as recurrent pneumonia.

Bronchopulmonary sequestration is a rare congenital malformation of the lower respiratory tract. Its presentation is varied, and it rarely presents in adulthood. We report the case of a 31-year-old woman who was admitted with recurrent pneumonia. She had been treated for pneumonia multiple times within the last 3 years. On subsequent workup, she was found to have intra-lobar bronchopulmonary sequestration in the left lower lobe of the lung, which was diagnosed on computerized tomography (CT) of the chest. The chest CT revealed an anomalous blood vessel from the abdominal aorta ascending to the

Acute transient sensorineural hearing loss due to Anaplasma phagocytophilum.

We report the case of a patient who presented with a 3-month history of random epistaxis and recent onset of acute hearing loss associated with fever, chills, and myalgias. Pure tone audiometry revealed bilateral sensorineural hearing loss. Complete blood cell count showed an abnormal neutrophil count of 700/uL (normal >1900/microL) and platelet count of 25 x 10(3)/microL (normal >175 x 10(3)/microL). Giemsa-stained peripheral blood smear revealed neutrophilic intracytoplasmic inclusion consistent with anaplasma morulae. Polymerase chain reaction confirmed Anaplasma phagocytophilum. The patien

A report of 2 cases of myopericarditis after Vaccinia virus (smallpox) immunization.

BACKGROUND: To counter the possibility of smallpox being used as a biological weapon, in 2002 the US government restarted a smallpox vaccination campaign. Myopericarditis is a possible cardiac complication of smallpox vaccination. We report 2 cases of vaccine-associated myopericarditis in military recruits who were treated at our facility. Chest pain, shortness of breath, and electrocardiographic changes of pericarditis, with a recent history of smallpox vaccination, were useful in making the diagnosis of probable post-vaccinial myopericarditis. The management of Ixodes scapularis bites in the upper Midwest.

Ixodes scapularis, commonly referred to as the deer tick, is the vector of Lyme disease and anaplasmosis; both illnesses are endemic to the upper Midwest. Avoidance of I scapularis bites is the primary preventative strategy for both infections. Antibiotic prophylaxis has been demonstrated to prevent Lyme disease, but similar studies have not investigated antibiotic prophylaxis for the prevention of anaplasmosis. Thus, recommendations regarding the management of I scopularis bites are focused on the prevention of Lyme disease. This paper reviews the prevailing antibiotic prophylaxis recommendat
