Prevalence and genetic characterization of clinically relevant extended-spectrum β-lactamase-producing Enterobacterales in the Gulf Cooperation Council countries.

INTRODUCTION: Among Gram-negative bacteria (GNB), Enterobacterales (Enterobacterales), such as Escherichia coli (E. coli) and Klebsiella pneumoniae (K. pneumoniae), are the most clinically relevant pathogens in healthcare settings. Infections secondary to these pathogens are widely common but multidrug resistance (MDR) in Enterobacterales has become a significant challenge with increased morbidity, mortality, and cost of management. The escalating global prevalence of MDR in Enterobacterales has led to limited treatment options, raising an urgent need for novel antimicrobial therapy(s) and det

A Systematic Review on the Occurrence of Antimicrobial-Resistant Escherichia coli in Poultry and Poultry Environments in Bangladesh between 2010 and 2021.

Antimicrobial resistance (AMR) is a significant public health issue in Bangladesh like many other developing countries where data on resistance trends are scarce. Moreover, the existence of multidrug-resistant (MDR) Escherichia coli exerts an ominous effect on the poultry sector. Therefore, the current systematic review, following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, was conducted to find out the AMR scenarios in E. coli isolates sourced from poultry and poultry environments in Bangladesh between 2010 and 2021. Following the PRISMA guideli

Poor infection prevention and control standards are associated with environmental contamination with carbapenemase-producing Enterobacterales and other multidrug-resistant bacteria in Swiss companion animal clinics.

BACKGROUND: Intensive medical care in companion animal clinics could pose a risk for the selection and dissemination of multidrug-resistant organisms (MDROs). Infection prevention and control (IPC) concepts are key measures to reduce the spread of MDROs, but data on IPC standards in companion animal clinics is sparse. The study assessed IPC standards in seven companion animal clinics and practices in Switzerland by structured IPC audits and combined results with environmental MDRO contamination and MDRO carriage of the personnel. METHODS: IPC audits were held between August 2018 and January 2

Genomic and Resistance Epidemiology of Gram-Negative Bacteria in Africa: a Systematic Review and Phylogenomic Analyses from a One Health Perspective.

Antibiotic resistance (AR) remains a major threat to public and animal health globally. However, AR ramifications in developing countries are worsened by limited molecular diagnostics, expensive therapeutics, inadequate numbers of skilled clinicians and scientists, and unsanitary environments. The epidemiology of Gram-negative bacteria, their AR genes, and geographical distribution in Africa are described here. Data were extracted and analyzed from English-language articles published between 2015 and December 2019. The genomes and AR genes of the various species, obtained from the Pathosystems

Discovery and functional characterization of a novel PilV variant in a mcr-1-positive IncI2 plasmid.

IncI2-type plasmids are major vehicles for the dissemination of mcr-1. These plasmids encode a type IV secretion system (T4SS) that mediates conjugative transfer and a type IV pilus (T4P) that facilitates mating-pair formation. Shufflon-mediated diversity of tip adhesin PilV of T4P may contribute to the extensive dissemination of IncI2 plasmids. Herein, we identified a novel shufflon segment forming an active new PilV variant from an IncI2 plasmid, which was named PilV-E and was detected in 127 of 501 (25.35%) complete IncI2 plasmids in public databases. We further demonstrated that strains ha

Genomic phylogeny, taxonomic classification, genetic landscape, and antibiotic resistance of clinical Stenotrophomonas isolates.

BACKGROUND: Stenotrophomonas spp. are increasingly recognized opportunistic pathogens characterized by intrinsic multidrug resistance. Despite recent taxonomic reclassification and the delineation of new species within the S. maltophilia complex (Smc), routine diagnostics often misidentify these species, and their distinctive antibiotic resistance traits and genetic makeup remain underexplored. We aimed to characterize the phylogenomic diversity, genotypic and phenotypic antibiotic resistance traits, virulence markers and horizontally transferrable elements in a large national cohort of clinic

De-Escalation of Broad-Spectrum and Last-Resort Antibiotics in Critically Ill Adults with Gram-Negative Infections: A Scoping Review and Evidence-Informed Framework for Tertiary-Care ICUs.

BACKGROUND: Early broad-spectrum empirical therapy is life-saving in critical illness, but its unnecessary continuation drives resistance, toxicity, and cost; antibiotic de-escalation is the principal stewardship strategy for resolving this tension, yet its evidence base is fragmented and its practice inconsistent. This scoping review mapped the evidence on the definitions, timing, eligibility, implementation, safety, and clinical, microbiological, and resistance outcomes of de-escalating cephalosporins, carbapenems, colistin, and tigecycline in critically ill adults with suspected or confirme

Spread potential and architectural variant of a multiple-drug resistant Escherichia coli co-producing lipopolysaccharide-modifying determinants and extended-spectrum β-lactamases.

BACKGROUND: Abuse of β-lactam antibiotics and its spread of drug resistance challenged the applicability of antimicrobial agents, enabling polymyxins as a last-line treatment for anti-infections of multi-drug resistant (MDR) pathogen. Polymyxin is applicable to herb breeding/growth prompting and combatting Gram-negative bacteria by combining other antimicrobials by membrane dysfunction, permeability damage, vesicle-vesicle contacts and oxidative stress, with its increasingly ubiquitous resistance. However, the complex mechanisms and high transmission risk of polymyxin resistance pose a severe

High-risk lineages shape the resistome and virulome of multidrug-resistant Pseudomonas aeruginosa.

Multidrug-resistant (MDR) and extensively drug-resistant (XDR) Pseudomonas aeruginosa strains remain a major threat in clinical settings, yet no contemporary genomic data are available from the Olomouc region, Czech Republic. We performed whole-genome sequencing of 157 MDR isolates obtained from 133 patients hospitalized at two tertiary-care hospitals in Olomouc between April 2020 and April 2024. The aim of this study was to delineate the clonal structure, resistance and virulence repertoires shaping the local epidemiology. Of the 157 isolates, 81 (51.6%) met XDR criteria, most commonly exhibi

Patterns of antimicrobial resistance genes in pathogens across One Health sectors in Ireland: an in silico approach.

As part of a rapid risk assessment, an in silico approach was used to detect antimicrobial resistance (AMR) in pathogenic isolates from humans, animals, and the environment. A total of 11,670 genomic data sets were retrieved from the NCBI Pathogen Detection system for Ireland, which represented 47 pathogenic species, including Salmonella enterica, Escherichia coli/Shigella spp., Staphylococcus aureus, Klebsiella pneumoniae, and Enterococcus faecium. Identifying the most critical pathogenic strains over time is essential, as these organisms significantly contribute to mortality, morbidity, and

Evaluation of Oxford nanopore sequencing for antimicrobial resistance surveillance in Salmonella: comparison with phenotypic antimicrobial susceptibility in a large-scale study.

UNLABELLED: Salmonella is a major zoonotic foodborne pathogen, and antimicrobial resistance (AMR) in Salmonella presents a significant public health challenge. Compared with conventional antimicrobial susceptibility testing (AST), whole-genome sequencing (WGS) provides a more rapid and comprehensive approach to AMR characterization, thereby informing antimicrobial selection and supporting public health surveillance. In this study, Oxford Nanopore Technology (ONT)-based WGS was performed on 1,490 Salmonella isolates collected through nationwide surveillance in Taiwan in 2025. Genotypic resistan
