Bony destructive injuries of the calcaneus: long-term results of a minimally invasive procedure followed by early functional exercise: a retrospective study.

BACKGROUND: Bony destructive injury of the calcaneus (BDIC) represents one of the most severe comminuted fractures of the calcaneus in which soft tissue coverage remains intact. The features of this injury include a collapsed articular surface, significant widening, severe loss of height and an unrecognisable outline of the calcaneus. This study aims to present the long-term outcomes of BDIC treated in a minimally invasive fashion followed by supervised early exercise. METHODS: Twelve patients with unilateral BDICs were treated at our institution. The main surgical procedures included percuta

Increased-offset reverse shoulder arthroplasty for the treatment of failed post-traumatic humeral head replacement.

BACKGROUND: Late complications after humeral head replacement (HHR) for comminuted proximal humeral fractures are common and may necessitate revision surgery. This study evaluated the outcome of revision surgery of failed post-traumatic HHR with a less medialized reverse shoulder prosthesis. METHODS: Thirty-three patients with failed post-traumatic HHR due to rotator-cuff insufficiency and glenoid erosion, but with sufficient preservation of the glenoid bone stock to permit primary stability of an inverted glenoid implant, underwent revision using the Bayley-Walker reverse shoulder prosthesis

Primary adrenal insufficiency following traumatic brain injury: a case report and review of the literature.

Primary adrenal insufficiency (PAI) is a relatively rare but serious condition that can lead to signs and symptoms ranging from mild generalized weakness and fatigue to fulminant shock and death. We present the case of a previously healthy 31-year-old man who developed PAI while undergoing rehabilitation after a severe traumatic brain injury (TBI). The patient suffered a TBI with comminuted skull fractures, bifrontal confusions, and bilateral epidural hematomas in a jet-ski accident. Acute hospitalization was prolonged by several medical complications, and the patient was admitted for subacute

Regional anesthesia combined with esketamine analgesia and remimazolam sedation as a salvage strategy for intertrochanteric femur fracture surgery in a high-risk patient: A case report.

RATIONALE: The anesthetic management of intertrochanteric fractures in patients with dual contraindications to neuraxial and general anesthesia is highly challenging. We illustrate a unique salvage multimodal strategy combining a comprehensive ultrasound-guided regional blockade with esketamine-based sedation to manage such complex cases. PATIENT CONCERNS: A 55-year-old female (147 cm, 45 kg) was admitted following a slip-and-fall accident resulting in severe right hip pain and limited mobility. Due to her complex medical history, the patient and her family expressed significant anxiety regar

Complete recovery and resolution of neurological symptoms following a penetrating gunshot in the left frontal lobe: a case report.

BACKGROUND: Traumatic brain injury is a major global health concern, with penetrating brain trauma representing an uncommon but highly lethal subset, especially in conflict-affected regions. Outcomes in penetrating brain trauma are strongly influenced by admission Glasgow Coma Scale (GCS), extent of structural injury, projectile characteristics, and imaging findings. Early expert assessment, rapid neuroimaging, and timely surgical intervention are critical for improving survival in selected patients although long‑term recovery often requires multidisciplinary rehabilitation. We report a rare c

The "swing-ding": a golf-related head injury in children.

OBJECT: In recent years there has been an increased incidence of golf-associated head injuries in children and adolescents. At the authors' institution, they have identified a unique pattern of head injury associated with a swinging golf club. In this study, the authors highlight the mechanism of this injury and report their experience treating it. METHODS: The authors reviewed the database of Rainbow Babies and Children's Hospital Trauma Center and performed a retrospective analysis of golf injuries recorded over a 10-year period (January 2000-April 2010). They identified 13 children (9 boys

Transcranial motor evoked potential recording in a case of Kernohan's notch syndrome: case report.

OBJECTIVE AND IMPORTANCE: Compression of the cerebral peduncle against the tentorial incisura contralateral to a supratentorial mass lesion, the so-called Kernohan-Woltman notch phenomenon, can be an important cause of false localizing motor signs. Here, we demonstrate a case in which clinical, radiological, and electrophysiological findings were used together to define this syndrome. CLINICAL PRESENTATION: A 21-year-old man sustained a left temporal depressed cranial fracture from a motor vehicle accident. Serial computed tomographic examinations demonstrated no evolution of hematomas or con

Outcome of humeral shaft fractures treated by functional cast brace.

BACKGROUND: Functional brace application for isolated humeral shaft fracture persistently yields good results. Nonunion though uncommon involves usually the proximal third shaft fractures. Instead of polyethylene bivalve functional brace four plaster sleeves wrapped and molded with little more proximal extension expected to prevent nonunion of proximal third fractures. Periodic compressibility of the cast is likely to yield a better result. This can be applied on the 1(st) day of the presentation as an outpatient basis. Risk of radial head arthroplasty revision is correlated with radial head diameter: a multicenter analysis of 405 cases.

BACKGROUND: Radial head arthroplasty (RHA) is commonly used for the treatment of comminuted radial head fractures. Indications as well as implant types continue to evolve. RHA has had good outcomes with midterm longevity. The literature is limited to small case series with varying implant types, and larger studies are needed to determine the optimal implant type and radial head diameter. METHODS: A retrospective analysis of RHA cases performed by 75 surgeons at 14 medical centers in an integrated health care system between 2006 and 2017 was completed. Patient demographics, comorbidities, impl

Risk factors for the development of heterotopic ossification after acetabular fracture fixation.

BACKGROUND: Heterotopic ossification (HO) is a common complication of the operative treatment of acetabular fractures. Although the surgical approach has been shown to correlate with the development of ectopic bone, specific risk factors have not been elucidated. QUESTIONS/PURPOSES: The purposes of this study were to determine specific risk factors associated with the development of severe HO and the frequency with which patients develop severe HO after acetabular fracture fixation through an isolated Kocher-Langenbeck approach. METHODS: Using an institutional orthopaedic trauma database at

Assessing the Radiological and Functional Outcomes of Colles' Cast Versus Functional Position Cast Immobilization in the Conservative Treatment of Distal End of Radius Fractures.

INTRODUCTION: The distal end radius fracture represents a prevalent orthopedic condition that affects individuals across various age groups, commonly resulting from falls onto outstretched hands. Ongoing research endeavors have delved into diverse methodologies for addressing this condition, encompassing conservative and operative modalities, yielding variable outcomes. While the literature extensively delineates numerous mobilization approaches, such as the functional position cast and Colle's cast, scant comparative studies evaluate these immobilization forms. Consequently, our study sought

Traumatic Complex Hawkins Type IV Talar Fracture: A Case Report.

Fracture-dislocations of the talus are one of the most complex injuries involving the foot and ankle. These injuries are often accompanied by additional traumatic orthopedic injuries, avascular necrosis, and infection. When approaching limb reconstruction and salvage, the overall prognosis and functionality of the limb are key factors to consider. In the present report, we draw attention to the importance of a multidisciplinary team approach for formulating a treatment plan that incorporates the talar injury and associated injuries or pathologic features. We also reviewed the published data re

Lumbar burst fracture associated with bowel, bladder, and sexual dysfunction: case study.

Lumbar burst fractures occur when unusual force and flexion are placed on the spine, causing the vertebral body to rupture and possibly protrude into the spinal canal. A resulting conus medullaris injury is possible, but not common. In this case presentation, a young man suffered bowel, bladder, and sexual dysfunction after a 25-foot fall that caused a burst fracture of the first lumbar vertebra. The presentation's primary focus is the nursing education needed to care for patients who experience the unusual side effect of conus medullaris injury. The psychosocial aspects attributable to age, d

Controlling the angle between the distal locking screw and tibiotalar joint tangent helps to reduce the occurrence of misalignment of distal tibial fractures treated with intramedullary nail fixation.

BACKGROUND: Studies have shown that on the coronal plane, whether the direction of the distal locking screw is parallel to the tangent line of the tibiotalar joint can be used to determine whether there is varus or valgus deformity after the treatment of distal tibial fractures with intramedullary nail (IMN) fixation. However, there has been no statistical analysis of the included angle on the coronal plane, and there have been no reports on whether there is a relationship between the direction of the distal locking screw on the sagittal plane or the included angle of the tangent line of the t

Biomechanical and biological aspects of defect treatment in fractures using helical plates.

The clinical case of figure 1 through figure 11 shows a series of impressive failures of plate fixation. The plates were repeatedly applied bridging a comminuted bone segment in a heavy patient. The biomechanical analysis elaborates why this happened and proposes an unconventional procedure to prevent this failure with a minimally invasive procedure. A plate bridging an open gap or a defect in a long bone diaphysis is exposed to full functional load. According to clinical observations such plate application often fails even without external load such as weight bearing. The plate risks to break

Treatment of Hip Osteoarthritis with Severe Acetabular Bone Defects by Total Hip Arthroplasty Using the Restoration GAP II Reinforcement Ring: A Case Report.

INTRODUCTION: Low-energy traumatic irreducible anterior shoulder dislocations with concomitant greater tuberosity fractures in elderly females are rarely reported. The difficulty in reduction is attributed to the interplay between osseous impaction and soft tissue interposition. Three-dimensional computed tomography (CT) was utilized to delineate this combined obstruction mechanism. An open surgical approach incorporating joystick reduction techniques and preservation of the long head of the biceps tendon was implemented. To our knowledge, no systematic therapeutic protocols have been establis
