Hyperoxaluria, hypocitraturia, hypomagnesiuria, and lack of intestinal colonization by Oxalobacter formigenes in a cervical spinal cord injury patient with suprapubic cystostomy, short bowel, and nephrolithiasis.

Although urolithiasis is common in spinal cord injury patients, it is presumed that the predisposing factors for urinary stones in spinal cord injury patients are immobilization-induced hypercalciuria in the initial period after spinal injury and, in later stages, urine infection by urease-producing micro-organisms, e.g., Proteus sp., which cause struvite stones. We describe a patient who sustained C-7 complete tetraplegia in a road traffic accident in 1970, when he was 16 years old. Left ureterolithotomy was performed in 1971 followed by left nephrectomy in 1972. Probably due to adhesions, th

Carbapenem therapy is associated with improved survival compared with piperacillin-tazobactam for patients with extended-spectrum β-lactamase bacteremia.

BACKGROUND: The effectiveness of piperacillin-tazobactam (PTZ) for the treatment of extended-spectrum β-lactamase (ESBL) bacteremia is controversial. We compared 14-day mortality of PTZ vs carbapenems as empiric therapy in a cohort of patients with ESBL bacteremia who all received definitive therapy with a carbapenem. METHODS: Patients hospitalized between January 2007 and April 2014 with monomicrobial ESBL bacteremia were included. A decrease of >3 doubling dilutions in the minimum inhibitory concentration for third-generation cephalosporins tested in combination with 4 µg/mL of clavulanic a

Necrotizing fasciitis: improved survival with early recognition by tissue biopsy and aggressive surgical treatment.

BACKGROUND: Necrotizing fasciitis is a soft tissue gangrenous infection that is optimally treated by early diagnosis, radical surgical debridement of all involved necrotic tissue, broad spectrum antibiotics, and aggressive nutritional support. The early clinical diagnosis of an area of necrotizing fasciitis is difficult and frequently unreliable. We are reporting a series of cases in which an early, accurate diagnosis of necrotizing fasciitis was established by a frozen section tissue biopsy obtained at the bedside. METHODS: Over a 15-year period, a consecutive series of 43 patients had a bed

Infection control during parenteral nutrition therapy.

Parenteral nutrition therapy can achieve an anabolic state in patients who are unable to maintain normal nitrogen balance; however, it may be associated with infectious complications. Infections may be related to contamination of the cannula and the cannula wound, of the infusate, or of other parts of the parenteral nutrition system. A variety of microorganisms has been associated with these infections. The exact mechanisms that initiate cannula-related infection are poorly understood. Susceptibility of the host, the method and site of cannula insertion, colonization of parenteral nutrition ca

Schistosomiasis-Microbiota Interactions: A Systematic Review and Meta-Analysis.

INTRODUCTION: Schistosomiasis, a tropical disease affecting humans and animals, affected 251.4 million people in 2021. Schistosoma mansoni, S. haematobium, S. intercalatum, and S. japonicum are primary human schistosomes, causing tissue damage, granulomas, ulceration, hemorrhage, and opportunistic pathogen entry. The gut and urinary tract microbiota significantly impact a host's susceptibility to schistosomiasis, disrupting microbial balance; however, this relationship is not well understood. This systematic review and meta-analysis explores the intricate relationship between schistosomiasis a

Impact of Pre-Existing Urinary Antimicrobial Agents on Culture Yield, Diagnostic Accuracy, and the Detection of Significant Bacteriuria in Community-Acquired Urinary Tract Infections.

INTRODUCTION: Pre-existing antibacterial substances (ABS) in urine can significantly impact the diagnostic and therapeutic management of urinary tract infections (UTIs) by altering microbial culture outcomes and influencing the detection and resistance profiles of uropathogens. The presence of pre-existing ABS may lead to false-negative or insignificant culture results, affecting clinical decision-making. This study aims to assess the impact of pre-existing ABS on culture positivity, microbial distribution, and antibiotic susceptibility patterns in outpatients, particularly those with prior an

Antimicrobial resistance profile of extended-spectrum beta-lactamases, adenosine-monophosphate-cyclic, and carbapenemase-producing Gram-negative bacteria isolated from domestic animals.

BACKGROUND AND AIM: The production of beta-lactamase enzymes, such as extended-spectrum beta-lactamase (ESBL), adenosine-monophosphate-cyclic (AmpC), and Klebsiella pneumoniae carbapenemase (KPC), is one of the most important mechanisms of bacterial resistance to antimicrobials. Gram-negative bacteria show significant resistance due to various intrinsic and acquired factors. These intrinsic factors include low permeability of the outer membrane, various efflux systems, and the production of beta-lactamases, while acquired factors include chromosomal mutation and acquisition of resistance genes
