Causality Assessment Guidelines for Adverse Events Following Immunization with a Focus on Guillain-Barré Syndrome.

South Korea operates a National Vaccine Injury Compensation Program (VICP) for people who experience adverse events following immunization (AEFI). To run this program rationally, it is a prerequisite to confirm whether adverse events were caused by immunization. Guillain-Barré syndrome (GBS), a severe neurological disease with limb pain and muscle weakness as cardinal symptoms, is attracting attention as an AEFI. However, algorithm or guidelines for assessing the causality between vaccination and the incidence of GBS are lacking. We aimed to develop guidelines for causality assessment of GBS a

Stressful life events among homeless people in León (Nicaragua): Quantity, types, timing, and perceived causality.

Nicaragua is one of Latin America's least developed countries and has a lack of data on homeless people, a stigmatized group living in extreme poverty. We conducted structured interviews with homeless people living in the city of León (Nicaragua; n = 41) to obtain data on the quantity, types, timing, and perceived causality of stressful life events (SLEs) experienced during their childhood and adolescence as well as throughout their adult lives. The findings showed that our participants had experienced a high number of extremely severe SLEs at very early ages and most of these SLEs occurred be

Pharmacodynamics for antifungal drug development: an approach for acceleration, risk minimization and demonstration of causality.

The treatment of invasive fungal diseases constitutes a significant unmet medical need. There are relatively few antifungal agents in clinical development and a paucity of novel targets. Morbidity and mortality remain high and clinical outcomes are compromised by submaximal efficacy, emergence of drug resistance and drug-related toxicity. Thus, new antifungal agents are urgently required. A deep understanding of exposure-response relationships underpins the development of safe and effective clinical regimens of any therapeutic agent. Pharmacokinetics (PK) and pharmacodynamics (PD) is increasin

Causality Assessment in Premarketing Drug Clinical Trials: Regulatory Evolution in the USA and Ongoing Concerns.

Since 1993, how to assess the causality of serious adverse events in premarketing drug clinical trials has undergone sustained regulatory evolution in the USA. In that year, an investigational drug study for chronic hepatitis B virus infection was emergently stopped after a patient suddenly exhibited hepatic failure and lactic acidosis, which later developed, along with pancreatitis and peripheral neuropathy, in several others after drug discontinuation. Five patients eventually died, including three despite emergency liver transplantation. The drug's multisystem toxicity was not predicted by

Mendelian randomization supports causality between maternal hyperglycemia and epigenetic regulation of leptin gene in newborns.

Leptin is an adipokine that acts in the central nervous system and regulates energy balance. Animal models and human observational studies have suggested that leptin surge in the perinatal period has a critical role in programming long-term risk of obesity. In utero exposure to maternal hyperglycemia has been associated with increased risk of obesity later in life. Epigenetic mechanisms are suspected to be involved in fetal programming of long term metabolic diseases. We investigated whether DNA methylation levels near LEP locus mediate the relation between maternal glycemia and neonatal lepti

Suspected hepatotoxicity by Cimicifugae racemosae rhizoma (black cohosh, root): critical analysis and structured causality assessment.

Severe hepatotoxicity has been described as spontaneous or case reports in 42 patients in assumed causal relationship with the treatment by Cimicifugae racemosae rhizoma corresponding to the root of black cohosh (BC) for postmenopausal symptoms. However, an assessment by EMEA (European Medicines Agency) has shown a possible or probable causality in only 4 out of 42 patients. A diagnostic algorithm was now applied in the 4 patients with suspected BC hepatotoxicity, which included the qualitative and quantitative causality assessment of the updated system of the Council for International Organiz

Association of hypospadias and cardiac defect in a Parvovirus B19-infected stillborn: a causality relation?

Parvovirus B19 intrauterine infection is a known cause of hydrops fetalis and fetal death. It is also associated with congenital malformations, although the teratogenic potential seems to be low. Postmortem examination of a male stillborn of 29 gestational weeks revealed mild subcutaneous edema, malformed micropenis, perineoscrotal hypospadias and atrial septal defect, along with fetal erythroblastosis and villitis. Polymerase chain reaction detected Parvovirus B19 DNA genome in tissues from the fetus and the placenta, confirming the hypothesis of an intrauterine infection.

[Atopic dermatitis in children and food allergy: combination or causality? Should avoidance diets be initiated?].

Atopic dermatitis (AD) is one of the first manifestations of the atopic march. The natural history of food allergies (FA) is closely related to AD. Sensitivity to food is demonstrated with cutaneous tests (prick-tests and atopy patch-tests) or the presence of IgE specific to food. A true allergy to a foodstuff is revealed by oral provocation tests (OPT) or by improvement during an avoidance diet. Ingestion of the food allergen during OPT can provoke an onset of eczema, an immediate reaction (urticaria, oedema) or involve other target organs (digestive disorders, rhinitis, asthma or anaphylacti

A full-likelihood method for the evaluation of causality of sequence variants from family data.

In many disease genes, a substantial fraction of all rare variants detected cannot yet be used for genetic counselling because of uncertainty about their association with disease. One approach to the characterization of these unclassified variants is the analysis of patterns of cosegregation with disease in affected carrier families. Petersen et al. previously provided a simplistic Bayesian method for evaluation of causality of such sequence variants. In the present report, we propose a more general method based on the full pedigree likelihood, and we show that the use of this method can provi

Kolmogorov-Arnold causal generative models.

Causal generative models provide a principled framework for answering observational, interventional, and counterfactual queries from observational data. However, many deep causal models rely on expressive architectures with opaque mechanisms, limiting auditability in high-stakes domains. This paper introduces the Kolmogorov-Arnold causal generative model, a mixed-type tabular causal generative model that parameterizes each structural equation with a Kolmogorov-Arnold Network . The resulting mechanisms can be inspected through symbolic approximations and parent-child visualizations while preser

Causal Relationship Between Hypertension and Schizophrenia.

Hypertension is a significant risk factor for cardiovascular diseases, kidney disease, and psychiatric illnesses, and a major global issue. Schizophrenia is a chronic psychiatric disorder with inconsistent behavioral and cognitive abnormalities, profoundly impacting individuals and society. There is a close relationship between hypertension and schizophrenia, but the exact causal link remains unclear. We employed bidirectional Mendelian randomization (MR) analysis to evaluate the causal association of hypertension and schizophrenia. These instruments were derived from genome-wide association s

Causal Mechanisms of Monoamine Transporter Phosphorylation.

Monoamine transporters are essential proteins located at presynaptic terminals that play a crucial role in regulating neurotransmission of serotonin, dopamine, and norepinephrine by rapid reuptake of released amines from the synapse. Clinically used antidepressants and widely abused psychostimulants exhibit a high affinity for amine transporters. Function and expression of biogenic amine transporter are altered in subjects suffering from psychiatric diseases such as depression and in psychostimulant use disorder. Therefore, proper functional regulation of monoamine transporters is critical in

Hierarchical motion perception as causal inference.

Since motion can only be defined relative to a reference frame, which reference frame guides perception? A century of psychophysical studies has produced conflicting evidence: retinotopic, egocentric, world-centric, or even object-centric. We introduce a hierarchical Bayesian model mapping retinal velocities to perceived velocities. Our model mirrors the structure in the world, in which visual elements move within causally connected reference frames. Friction renders velocities in these reference frames mostly stationary, formalized by an additional delta component (at zero) in the prior. Inve

Using Causal Diagrams for Biomedical Research.

Causal diagrams are used in biomedical research to develop and portray conceptual models that accurately and concisely convey assumptions about putative causal relations. Specifically, causal diagrams can be used for both observational studies and clinical trials to provide a scientific basis for some aspects of multivariable model selection. This methodology also provides an explicit framework for classifying potential sources of bias and enabling the identification of confounder, collider, and mediator variables for statistical analyses. We illustrate the potential serious miscalculation of

Pneumoperitoneum and COVID-19. A causal association?

We read with great interest the letter from Pérez Naranjo et al. regarding the case we recently published in this journal. We believe that the case presented by the authors is undoubtedly useful as it contributes to the scarce existing casuistry on the relationship between COVID-19 and the development of pneumatosis/pneumoperitoneum. We would like to emphasize the difficulty of determining the causality of this association due to the frequent co-occurrence of other causes that could justify the presence of pneumatosis/pneumoperitoneum.

CAUSAL INTERPRETATIONS OF BLACK-BOX MODELS.

The fields of machine learning and causal inference have developed many concepts, tools, and theory that are potentially useful for each other. Through exploring the possibility of extracting causal interpretations from black-box machine-trained models, we briefly review the languages and concepts in causal inference that may be interesting to machine learning researchers. We start with the curious observation that Friedman's partial dependence plot has exactly the same formula as Pearl's back-door adjustment and discuss three requirements to make causal interpretations: a model with good pred

Dilated cardiomyopathy and aldosteronoma: a causal link?

The aim of this study is to reveal the causal relationship between dilated cardiomyopathy and aldosteronoma. A 44-year-old male dilated cardiomyopathy patient with aldosteronoma, who demonstrated a worse cardiac function after 1 year therapy with optimized dosage of sacubirtil/valsartan, furosemide, metoprolol, and spironolactone. The patient shows a promising prognosis after aldosteronoma removal procedure. Aldosteronoma may cause dilated cardiomyopathy. We assume that the optimal treatment for aldosteronoma-induced dilated cardiomyopathy is surgical removal combined with drugs.

Comorbidity Scoring with Causal Disease Networks.

In recent years, there has been numerous studies constructing a disease network with diverse sources of data. Many researchers attempted to extend the usage of the disease network by employing machine learning algorithms on various problems such as prediction of comorbidity. The relations between diseases can further be specified into causal relations. When causality is laid on the edges in the network, prediction for comorbid diseases can be more improved. However, not many machine learning algorithms have been developed to concern causality. In this study, we exploit a network based machine
