Restoration of vision II: residual functions and training-induced visual field enlargement in brain-damaged patients.

PURPOSE: Brain damage is often accompanied by visual field defects which have been considered to be non-treatable. In recent years, however, new diagnostic methods have revealed hitherto unknown residual vision, which was found, for instance, in transition zones near the blind visual field sectors and in spared islands of vision within the blind regions ("blindsight"). Furthermore, animal studies revealed a high degree of plasticity in the visual system suggesting the possibility that recovery of vision may be induced by systematic visual training. METHODS: Here we summarize a series of studi

Repeatability of a novel chromatic perimeter in a cohort of people with and without glucose dysfunction.

SIGNIFICANCE: Sensitive diagnostic techniques are crucial for the early detection of diabetes. Here, we tested a novel color perimeter and found it repeatable in control subjects. It also has the potential to detect functional changes in diabetic eye disease. PURPOSE: Type 2 diabetes (T2DM) causes retinal dysfunction before the appearance of retinopathy. Our group developed a color perimeter that measures chromatic sensitivity at discrete retinal locations. This study evaluates the repeatability of this perimeter in detecting chromatic sensitivity changes in normal and glucose-dysfunctional s

Visual dysfunction with basal skull tumours.

Early detection of visual loss caused by tumours of the base of the skull is accomplished by systematic tests of the pregeniculate optic pathway. Even when central acuity is relatively preserved, a relative afferent pupillary defect and reduced color perception yield evidence of impaired optic nerve conduction. Although confrontation tests of the visual fields are useful screening techniques, unexplained symptoms should be pursued with static and kinetic perimetry. Patterns of optic nerve and chiasmal field loss and atrophy of the dis or retinal nerve fibers are imprecise guides to the locatio

Component perimetry: a fast method to detect visual field defects caused by brain lesions.

PURPOSE: Noise field campimetry, performed according to Aulhorn and Köst, confronts patients with a large field of irregularly flickering dots, and many patients immediately perceive their visual field defects. The original method had a somewhat low specificity and sensitivity, especially for patients with visual field defects caused by cortical lesions. METHODS: The method was improved in two ways. First, the grain of the visual noise was increased toward the periphery of the visual field to accommodate the peripheral decrease in visual acuity. Second, the type of stimulus pattern was varied

Transient tritanopia in migraine: evidence for a large-field retinal abnormality in blue-yellow opponent pathways.

PURPOSE: To determine whether the magnitude of transient tritanopia (TT) differs between migraine and control groups. TT is a retinal phenomenon characterized by a paradoxical reduction in sensitivity to short-wavelength (purple) stimuli after extinction of long-wavelength (yellow) adapting displays. A group difference in the magnitude of TT would provide evidence for a retinal contribution to the S-cone-specific color-processing abnormalities that have been reported in migraine. METHODS: Thirty-two migraineurs and 32 age- and sex-matched control participants were tested with a four-alternati

[The use of optical coherence tomography in hereditary optic neuropathies: description of a family].

CASE REPORT: This report describes two siblings, a woman aged 44 years and her brother aged 29 years, who both complained of visual loss in both eyes. The woman had bilateral optic nerve (ON) temporal pallor and severe reduction of ON fibre layer thickness in this area. Both she and her brother had cecocentral defects in perimetry and color vision deficiency with a marked tritanopia deficit. DISCUSSION: ON pallor, limited to the temporal region, is the basic criterion for the diagnosis of dominant optic atrophy. Optical coherence tomography can be a useful technique in defining the diagnosis

[Early detection of color blindness from the viewpoint of occupational medicine with various references to internistic and human genetic symptom complexes].

Vision screening tests within the limits of industrial medicine examinations, together with physical examinations, were done on human individuals by means of pseudo-isochromatic charts in order to detect "red-green blindness". The tests were carried out on 1589 individuals (males and females) from 10 medium-scale plants of the Saarbrücken area (Federal Republic of Germany). The results obtained from male individuals by 919 Ishihara-tests were only considered, categorized and graphically represented according to their age groups. The data have been collected from the cases examined mostly betwe

Eight cases of congenital achromatopsia with amblyopia in two pedigrees from Northern Sweden.

Two families from northern Sweden with a total of 8 patients with typical symptoms of congenital achromatopsia with amblyopia were studied. In one of the families 4 affected children (3 brothers and 1 sister) also showed pallor of the optic discs and marked astigmatism. The transmission of the disease was consistent with an autosomal recessive inheritance in both families. The study confirmed that complete and incomplete achromatopsia might be different expressions of the same gene. Six out or 13 near relatives of the achromatic patients showed minor colour vision defects, suggesting a tendenc

Concurrent inheritance of achromatopsia and MMAT syndrome in a pedigree: Genetic and clinical insights.

BACKGROUND: Achromatopsia is a rare type of retinal dystrophy presenting with decreased visual acuity, pendular nystagmus, photophobia, impaired color discrimination, and central scotoma. In this study, we investigated achromatopsia in a proband and his family. METHODS: Whole-exome sequencing identified two novel variants in PDE6H and ADAMTS18. The presence of the variants was confirmed by Sanger sequencing and it was further utilized to determine the zygosity status of other family members. Lastly, the clinical presentations of the patients were thoroughly assessed. RESULTS: We identified t

Achromatopsia-Visual Cortex Stability and Plasticity in the Absence of Functional Cones.

PURPOSE: Achromatopsia is a rare inherited disorder rendering retinal cone photoreceptors nonfunctional. As a consequence, the sizable foveal representation in the visual cortex is congenitally deprived of visual input, which prompts a fundamental question: is the cortical representation of the central visual field in patients with achromatopsia remapped to take up processing of paracentral inputs? Such remapping might interfere with gene therapeutic treatments aimed at restoring cone function. METHODS: We conducted a multicenter study to explore the nature and plasticity of vision in the abs

Increased H3K27 trimethylation contributes to cone survival in a mouse model of cone dystrophy.

Inherited retinal diseases (IRDs) are a heterogeneous group of blinding disorders, which result in dysfunction or death of the light-sensing cone and rod photoreceptors. Despite individual IRDs (Inherited retinal disease) being rare, collectively, they affect up to 1:2000 people worldwide, causing a significant socioeconomic burden, especially when cone-mediated central vision is affected. This study uses the Pde6ccpfl1 mouse model of achromatopsia, a cone-specific vision loss IRD (Inherited retinal disease), to investigate the potential gene-independent therapeutic benefits of a histone demet

Blue Cone Monochromatism with Foveal Hypoplasia Caused by the Concomitant Effect of Variants in OPN1LW/OPN1MW and GPR143 Genes.

Blue cone monochromatism (BCM) is an X-linked recessive cone dysfunction disorder caused by mutations in the OPN1LW/OPN1MW gene cluster, encoding long (L)- and middle (M)-wavelength-sensitive cone opsins. Here, we report on the unusual clinical presentation of BCM caused by a novel mutation in the OPN1LW gene in a young man. We describe in detail the phenotype of the proband, and the subclinical morpho-functional anomalies shown by his carrier mother. At a clinical level, the extensive functional evaluation demonstrated in the proband the M/L cone affection and the sparing of S-cone function,

Cerebral trauma-induced dyschromatopsia in the left hemifield: case presentation.

BACKGROUND: Acquired color anomalies caused by cerebral trauma are classified as either achromatopsias or dyschromatopsias (Zeki, Brain 113:1721-1777, 1990). The three main brain regions stimulated by color are V1, the lingual gyrus, which was designated as human V4 (hV4), and the fusiform gyrus, designated as V4α. (Zeki, Brain 113:1721-1777, 1990). An acquired cerebral color anomaly is often accompanied by visual field loss (hemi- and quadrantanopia), facial agnosia, prosopagnosia, visual agnosia, and anosognosia depending on the underlying pathology (Bartels and Zeki, Eur J Neurosci 12:172-1
