Selection criteria for internal rectal prolapse repair by Delorme's transrectal excision.

PURPOSE: The aim of this study was to review our results of Delorme's transrectal excision for internal rectal prolapse, with a view to determining preoperative selection criteria associated with a satisfactory outcome. METHODS: Between 1992 and 1998, 20 patients with internal rectal prolapse underwent Delorme's transrectal excision. The last patient was excluded from the study because of a follow-up period shorter than six months. The remaining 19 patients were prospectively followed up and classified into two groups according to their preoperative selection criteria. Group I consisted of ei

Multicompartment pelvic floor prolapse: advances in diagnosis and surgical management.

PURPOSE OF REVIEW: Multicompartment pelvic organ prolapse, including concurrent rectal and vaginal prolapse, is increasingly recognized in aging populations. This review summarizes recent advances in diagnostic imaging, multidisciplinary evaluation, and surgical techniques for combined repair. RECENT FINDINGS: Multicompartment prolapse is increasingly prevalent and often underrecognized without coordinated evaluation. Dynamic defecography, particularly upright or MRI-based, improves detection of multicompartment descent. Minimally invasive combined sacrocolpopexy (SCP) and ventral mesh rectop

Descending perineum syndrome: are abdominal hysterectomy and bowel habits linked?

PURPOSE: This retrospective study evaluates the effect of abdominal hysterectomy on patients affected by descending perineum syndrome. METHODS: Eighty-nine female patients affected by descending perineum syndrome and one group of 10 healthy women with normal bowel habits were studied retrospectively. Thirty-two descending perineum syndrome patients (Group 1) had received an abdominal hysterectomy for benign diseases, while 57 descending perineum syndrome patients (Group 2) had not undergone this surgery. All 99 subjects underwent clinical evaluation, computerized anorectal manometry, and defe

[Internal rectal invagination treated with rectopexy and sigmoid resection].

BACKGROUND: Internal rectal intussusception, usually occurring in women, causes constipation and incomplete evacuation of stool. MATERIALS AND METHODS: Twenty-one women and one man (median age 48) were operated with suture rectopexy and sigmoid resection. The patients were examined with anoscopy and defecography, and symptomatic outcome, patients' satisfaction and morbidity were evaluated. Outcome was based mainly on the validated KESS score for constipation. RESULTS: There was a significant reduction in all ten symptoms. Faecal incontinence improved in the two afflicted patients after opera

Is rectal intussusception a cause of idiopathic incontinence?

PURPOSE: The cause of rectal intussusception in patients primarily dominated by symptoms of anal incontinence has not been fully elucidated, especially for patients with idiopathic incontinence. METHODS: Between 1991 and 1996, 51 patients referred with a diagnosis of idiopathic incontinence were prospectively evaluated by standard questionnaire, clinical examination, defecography, and anal manometry. Fourteen female patients were identified with rectal intussusception and were treated by transabdominal rectopexy. Postoperatively, clinical assessment and anal manometry were performed at regula

[Descending perineum in women].

The descending perineum syndrome, described in 1970 by Alan Parks, remains difficult to interpret clinically and pathophysiologically. A general review of descending perineum was conducted, based on review of the literature published between 1966 and 2004, and retrospective analysis of 1,023 colpocystograms. The symptoms observed are usually secondary to associated lesions. Radiological signs of descending perineum are not always associated with clinical symptoms. Colpocystogram shows perineal descent and associated disorders of anterior and middle pelvic tone, while defecography provides a be

[Diagnostic imaging in constipation].

Diagnostic imaging modalities play a key role in the definition of the possible causes of constipation. Barium Enema (BE), Defecography (DG), Intestinal Transit Time (ITT), Computed Axial Tomography (CT) and Magnetic Resonance (MR) are necessary diagnostic tools for the identification either of the possible organic causes of the disease or of the functional disorders. The ITT evaluation is the main investigation to look for functional colic constipation; this method is in fact able to distinguish between the hypertonic type (in which the fecal progression is slowed down to such an extent that

Descending perineum syndrome: a review of the presentation, diagnosis, and management.

INTRODUCTION AND HYPOTHESIS: Defecatory dysfunction is a relatively common and challenging problem among women and one that practicing pelvic reconstructive surgeons and gynecologists deal with frequently. A subset of defecatory dysfunction includes obstructed defecation, which can have multiple causes, one of which is descending perineum syndrome (DPS). METHODS: A literature search was performed to identify the pathophysiology, diagnosis, and management of DPS. RESULTS: Although DPS has been described in the literature for many decades, it is still uncommonly diagnosed and difficult to mana

Fecal incontinence: the role of the urologist.

Fecal incontinence is the involuntary loss of solid or liquid stool. While the true prevalence of fecal incontinence is difficult to discern, it is estimated that almost 9 % of non-institutionalized women in the US experience this condition. Disorders leading to fecal urgency alone are usually related to rectal storage abnormalities while incontinence is often a result of anatomic or neurologic disruption of the anal sphincter complex. Many risk factors exist for fecal incontinence and include female sex, increasing age, higher body mass index (BMI), limited physical activity, smoking, presenc

[The role of medical imaging in the exploration of pelvic floor disorders].

Colpocystodefecography (CCD) and dynamic MRI with defecography (MRId) permit an alternation between filling and emptying the hollow organs and the maximum abdominal strain offered by the defecation. The application in imaging of these two principles reveal the masked or underestimated prolapses at the time of the physical examination. Rigorous application of the technique guarantees almost equivalent results from the two examinations. The CCD provides voiding views and improved analysis of the anorectal pathology (intussusceptions, anismus) but involves radiation and a more invasive examinatio

[Rectal prolapse].

Rectal prolapse is the transposition of the entire rectal wall into the rectal lumen, the anal canal or through the anal canal out side. It differs from anal prolapse in thickness, circular plication of the mucosa and, if large, its extent. The cause is not clearly established, but disorders in bowel movement seem to be of importance. Symptoms reach from the feeling of incomplete evacuation to defecation block and irreducible prolapse. The diagnosis of outer prolapse is easy. The inner prolapse [intussusception] can be suspected by anamnesis and in the presence of solitary rectal ulcer. Defeco

Rome Constipation Symptoms Augmented by Painful Defecation Predicts Specific Subtypes of Refractory Constipation.

INTRODUCTION: Functional defecation disorders (FDD) are a prevalent etiology of refractory constipation. The diagnosis of FDD requires specific physiology testing, including anorectal manometry (ARM) and balloon expulsion test (BET). The aims of our study were to evaluate whether the complaint of painful defecation added to the Rome III symptoms questionnaire could help to differentiate subtypes of refractory chronic constipation. MATERIALS AND METHODS: One hundred and ninety-eight constipated patients (Rome III Criteria) who had failed a 30-day fiber/laxative trial were enrolled. Before ente

A radiomics nomogram utilizing T2-weighted MRI for accurate diagnosis of rectocele.

BACKGROUND: Rectocele (RC) is a common pelvic organ prolapse (POP) that can cause obstructed defecation and reduced quality of life. Magnetic resonance defecography (MRD) offers high-resolution, radiation-free visualization of pelvic floor anatomy but relies on time-consuming, observer-dependent manual measurements. Our research constructs a nomogram model incorporating intra-ROI and habitat radiomics features to improve MRD-based RC diagnosis. METHODS: We retrospectively analyzed 222 MRD patients (155 training, 67 testing). Clinical features were selected via univariate and multivariate logi

Influence of body posture on defecation: a prospective study of "The Thinker" position.

PURPOSE: We hypothesized that bending the upper body into what we have termed "The Thinker" position facilitates defecation. This study aimed to assess the influence of "The Thinker" position on defecation. METHODS: This is the prospective single-group study. Patients who could not evacuate the paste in normal sitting position on cinedefecography between January and June 2013 were enrolled in this study. Cinedefecography was first performed in the sitting position; if the patient was unable to evacuate the paste, images were obtained in "The Thinker" position. Patients who were able to evacua

Clinical relevance of transperineal ultrasound compared with evacuation proctography for the evaluation of patients with obstructed defaecation.

AIM: Dynamic evacuation proctography (DEP) is still considered the gold standard diagnostic procedure for posterior compartment pelvic disorders. The study aimed to assess the value of dynamic transperineal ultrasound (DTPU) compared with DEP in patients with obstructed defaecation syndrome (ODS). METHOD: In a prospective observational study, 54 consecutive female patients referred with symptoms of ODS between January and June 2009 were studied by clinical evaluation (including Wexner score), perineal ultrasound and defaecography. The tests were analysed by two experienced investigators unawa

Anterior-Only Sacrocolpopexy Complicated by Obstructive Enterocele: Should Both Compartments Always Be Repaired?

Sacrocolpopexy is widely regarded as the gold standard for apical prolapse repair, yet the extent of mesh placement remains debated. While some surgeons routinely place both anterior and posterior meshes, others favor a compartment-specific approach. We report the case of a 60-year-old woman with prior hysterectomy and anti-incontinence mesh surgery who underwent anterior-only robot-assisted sacrocolpopexy for apical and anterior prolapse. Six weeks later, she developed incomplete bowel emptying, pelvic pressure, and a posterior vaginal bulge. Examination revealed posterior compartment prolaps

Practical guide to dynamic pelvic floor MRI.

UNLABELLED: Pelvic floor dysfunction encompasses a spectrum of functional disorders that result from impairment of the ligaments, fasciae, and muscles supporting the pelvic organs. It is a prevalent disorder that carries a lifetime risk over 10% for undergoing a surgical repair. Pelvic floor weakness presents as a wide range of symptoms, including pain, pelvic pressure or bulging, urinary and fecal incontinence, constipation, and sexual dysfunction. A correct diagnosis by clinical examination alone can be challenging, particularly in cases involving multiple compartments. Magnetic resonance im
