Evaluating psychotherapies for veterans with PTSD at elevated acute risk of suicide: Protocol for a multi-site type 1 hybrid effectiveness-implementation trial.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a significant risk factor for suicide among veterans, but there is a critical knowledge gap about how to treat PTSD in the context of elevated acute suicide risk. Prior research suggests that a treatment that combines Dialectical Behavior Therapy (DBT) with the DBT Prolonged Exposure protocol (DBT PE) reduces PTSD and self-directed violence (SDV) among high-risk individuals while also being feasible, acceptable, and safe to deliver. However, a large-scale randomized controlled trial is needed. This study will compare the effectiveness of DBT

[On the Admission of Patients with Borderline Personality Disorder to Outpatient Psychotherapy].

BACKGROUND: Patients with borderline personality disorder (BPD) present difficulties in self-regulation and interaction, which is a challenge for psychotherapists that is also addressed in BPD-specific interventions. Against this background, outpatient psychotherapists were surveyed about the factors playing a decisive role in their treatment offer for patients with BPD. METHODS: Psychotherapists for adults were contacted via their email address published on the website of the Kassenärztliche Vereinigung (Association of Statutory Health Insurance Physicians), 231 of whom answered an online qu

Suicidal behavior in adolescents with post-traumatic stress disorder.

Recently, the prevalence of post-traumatic stress disorder (PTSD) in adolescence is higher than the prevalence of PTSD in adult populations. PTSD and suicidality are often found in populations of adolescents presenting with other emotional disorders (particularly mood disorders), traumatic grief, childhood abuse, and/or a family or peer history of suicide. The reasons and developments of the association between PTSD and suicidality in adolescence, however, remain unclear. Core psychobiological changes contributing to PTSD affect emotion, arousal, perception of the self and the world, irritabil

General Psychiatric Management for Adolescents With Borderline Personality Disorder and Eating Disorders.

Borderline personality disorder and eating disorders frequently co-occur among youths. These disorders emerge in adolescence, during the critical developmental period of building an independent sense of self and the capacity to relate to one's community. Because of core differences in the development and psychopathology of borderline personality disorder and eating disorders, adjustments are required when treating these disorders when they co-occur. Few established treatment approaches can address these disorders simultaneously. Evidence-based psychotherapies for borderline personality disorde

The Role of Affect Management for HIV Risk Reduction for Youth in Alternative Schools.

OBJECTIVE: Adolescents in alternative schools for behavioral and emotional problems have an earlier sexual onset and higher rates of sexual risk than their peers. They also often have difficulty managing strong emotions, which can impair sexual decision making. Human immunodeficiency virus (HIV) prevention programs for these adolescents may be most effective if skills for coping with strong emotions during sexual situations are included. METHOD: This article reports the 6-month outcomes of a three-arm randomized controlled trial comparing an HIV prevention intervention with affect management

Implementing evidence-based group psychotherapy in Veterans Health Administration (VHA): Implications for burnout and burnout reduction.

Among behavioral health care providers, use of evidence-based psychotherapies has been associated with lower burnout when there is perceived institutional support, specialty clinical training, and high-quality clinical supervision. Mental health care providers implementing dialectical behavior therapy (DBT) have reported reduced burnout, with some evidence that the DBT consultation team may prevent burnout. Within the Veterans Health Administration, we examined provider experiences of burnout before and during implementation of an adjunctive outpatient DBT skills group and provider consultatio

Treatment Outcomes for Co-Occurring Borderline Personality Disorder and Substance Use Disorders: A Systematic Review.

Objective: Almost half of patients diagnosed with borderline personality disorder (BPD) have a co-occurring substance use disorder (SUD). Patients with co-occurring BPD and SUD have poorer treatment outcomes, including lower rates of BPD remission and higher rates of substance-related harms. This systematic review aimed to evaluate existing treatments for co-occurring BPD and SUD. Methods: A systematic search was conducted using PsycINFO, Medline, PubMed, and Embase from inception to December 5, 2023. Selected studies assessed psychological or pharmacological interventions for patients with co

Assessing the Temporal Dynamics in Self-Regulated Learning Constructs as Predictors of Engagement in a Digital Health Intervention.

The promise of self-guided digital health interventions (DHIs) is consistently hampered by their poor engagement. While past research has identified various predictors of engagement, effect sizes have been small, and many have failed to replicate across studies. This poor predictive performance may, in part, be due to the field's overreliance on predictor assessments at pre-intervention (i.e., at baseline), which overlooks the possibility that those factors driving engagement can fluctuate across the user journey. Self-regulated learning (SRL) is a potentially relevant process underlying engag

Predicting Treatment Outcomes from Prefrontal Cortex Activation for Self-Harming Patients with Borderline Personality Disorder: A Preliminary Study.

Self-harm is a potentially lethal symptom of borderline personality disorder (BPD) that often improves with dialectical behavior therapy (DBT). While DBT is effective for reducing self-harm in many patients with BPD, a small but significant number of patients either does not improve in treatment or ends treatment prematurely. Accordingly, it is crucial to identify factors that may prospectively predict which patients are most likely to benefit from and remain in treatment. In the present preliminary study, 29 actively self-harming patients with BPD completed brain-imaging procedures probing ac

Feasibility of a parent-only DBT skills group in public mental health services: a pilot implementation study.

BACKGROUND AND OBJECTIVE: War-related mass trauma and large-scale terrorist attacks disproportionately affect clinically vulnerable adolescents, exacerbating self-harm and suicidality while burdening under-resourced public mental health systems. This study evaluated the feasibility and preliminary efficacy of a parent-only Dialectical Behavior Therapy (DBT) skills group within routine care. We hypothesized that the parent-only DBT skills group would be associated with improved parental emotion regulation, decrease helplessness and family conflict, and indirectly reduce adolescent risk behavior

Validation of the Spanish version of the Borderline Symptom List, short form (BSL-23).

BACKGROUND: The Borderline Symptom List-23 (BSL-23) is a reliable and valid self-report instrument for assessing Borderline Personality Disorder (BPD) severity. The psychometric properties of the original version have proven to be adequate. The aim of the present study was to validate the Spanish language version of the BSL-23. METHODS: The BSL-23 was administered to 240 subjects with BPD diagnosis. Factor structure, reliability, test-retest stability, convergent validity, and sensitivity to change were analyzed. RESULTS: The Spanish version of the BSL-23 replicates the one-factor structure

Mindfulness-based Interventions for Chronic Low Back Pain: A Systematic Review and Meta-analysis.

OBJECTIVE: This systematic review aimed to compile existing evidence examining the effects of mindfulness-based interventions (MBIs) for chronic low back pain (CLBP). CLBP leads to millions of disabled individuals in the United States each year. Current pharmacologic treatments are only modestly effective and may present long-term safety issues. MBIs, which have an excellent safety profile, have been shown in prior studies to be effective in treating CLBP yet remained underutilized. DESIGN: Ovid/Medline, PubMed, Embase, and the Cochrane Library were searched for randomized controlled trials (

The influence of politics on psychological interventions for pediatric gender dysphoria.

The care of children and youth presenting with gender dysphoria has recently changed from watchful waiting and psychotherapeutic exploration to primarily an affirmative medical pathway involving social transition, puberty suppression, cross-sex hormones and surgery. This shift has occurred alongside a rapid increase in referrals and has been reinforced by political and legal frameworks that strongly favor gender-affirming care (GAC) while restricting alternative therapeutic approaches. This article critically examines the influence of legislation on clinical practice for gender-distressed yout

Nonsuicidal Self-Injury in Adolescents.

BACKGROUND: 25-35% of adolescents in random samples drawn from German schools have been found to have manifested at least one episode of nonsuicidal self-injury (NSSI). The prevalence in samples from child and adolescent psychiatric clinics is approximately 50%. NSSI can arise as a symptom in the setting of various types of mental illness. METHODS: This review is based on a selective literature search carried out in the PubMed, PsycINFO, and Cochrane Library databases, with special consideration of regional study samples. RESULTS: NSSI is usually resorted to as a dysfunctional coping strateg

[Skills Training for Patients with Borderline Personality Disorder].

The emotionally instable personality disorder, mostly called borderline disorder, shows central abnormalities in impulse control as well as instability of mood and identity. It is composed of behaviour problems in creating relationships and in self-management, first of all by high psychophysiological tension. The prevalence of this disorder is 10 % in outpatients and 20 % in inpatients and has therefore high relevance for the medical-psychotherapeutic care system. The treatment is deemed to be complex and interminable. Regarding all evaluated techniques of treatment the best examined is the Di

Demographic and Clinical Predictors of Multiple Admissions in Inpatient Eating Disorder Treatment.

OBJECTIVE: Despite advances in treatment, up to 20% of cases of bulimia nervosa and anorexia nervosa (AN) persist chronically, with about 25% of individuals with AN being re-hospitalized. This study examined demographic and clinical characteristics associated with one versus multiple admissions to an inpatient eating disorder unit. METHOD: The sample included 1609 individuals, treated on an inpatient medical stabilization unit, diagnosed with an eating disorder. Of those, 25.4% (n = 326) required multiple admissions. Cox proportional hazard models evaluated adjusted associations of age, gende

Effectiveness of online family connections for family members of people with BPD: a naturalistic comparison with the face-to-face format.

INTRODUCTION: Borderline Personality Disorder (BPD) is a common condition with significant negative impacts on relatives due to its interpersonal instability and suicidal behaviors. Family Connections (FC) is currently the most empirically supported intervention for family members of individuals with BPD. However, access to FC remains limited. Online delivery could overcome geographic barriers, as digital formats have shown efficacy in psychotherapies for patients, though little is known about their effectiveness for family-oriented interventions. During the COVID-19 crisis, rapid adaptations

Feasibility and acceptability of an online group dialectical behavioural therapy skills training in a Transdiagnostic group with Anxiety and Depression.

Transdiagnostic approaches offers a paradigm shift in managing psychiatric disorders. Emotion regulation difficulties are a transdiagnostic prevalent across various mood and personality disorders. Dialectical Behavioural Therapy Skills Training (DBT-ST), initially designed as part of treatment for borderline personality disorder, targets emotion regulation and has shown promise in diverse psychiatric conditions. In lower middle-income countries with resource-constrained settings, online delivery of evidence-based interventions holds potential to bridge treatment gaps. This study assesses the f

The combination of dialectical behavioral skills training with EMDR in the treatment of borderline personality disorder and psychological trauma: a feasibility randomized trial.

BACKGROUND: Comorbid borderline personality disorder (BPD) and posttraumatic stress disorder (PTSD) predict poorer treatment outcomes. Available treatments for BPD and psychological trauma are often phase-based, first ensuring stabilization of dysregulated behaviors and emotions and later focusing on treating trauma. This study investigates the feasibility, acceptability, and preliminary efficacy of combining dialectical behavioral therapy group skills training (DBT-ST), consisting of mindfulness, emotion regulation, distress tolerance, and interpersonal-effectiveness skills, with eye movement
