Clinical characteristics and pathophysiological mechanisms of focal and diffuse traumatic brain injury.

Traumatic brain injury (TBI) is a frequent and clinically highly heterogeneous neurological disorder with large socioeconomic consequences. TBI severity classification, based on the hospital admission Glasgow Coma Scale (GCS) score, ranges from mild (GCS 13-15) and moderate (GCS 9-12) to severe (GCS ≤ 8). The GCS reflects the risk of dying from TBI, which is low after mild (∼1%), intermediate after moderate (up to 15%) and high (up to 40%) after severe TBI. Intracranial damage can be focal, such as epidural and subdural haematomas and parenchymal contusions, or diffuse, for example traumatic a

Association Between Proteomic Blood Biomarkers and DTI/NODDI Metrics in Adolescent Football Players: A Pilot Study.

While neuroimaging and blood biomarker have been two of the most active areas of research in the neurotrauma community, these fields rarely intersect to delineate subconcussive brain injury. The aim of the study was to examine the association between diffusion MRI techniques [diffusion tensor imaging (DTI) and neurite orientation/dispersion density imaging (NODDI)] and brain-injury blood biomarker levels [tau, neurofilament-light (NfL), glial-fibrillary-acidic-protein (GFAP)] in high-school football players at their baseline, aiming to detect cumulative neuronal damage from prior seasons. Integrating Human and Nonhuman Primate Data to Estimate Human Tolerances for Traumatic Brain Injury.

Traumatic brain injury (TBI) contributes to a significant portion of the injuries resulting from motor vehicle crashes, falls, and sports collisions. The development of advanced countermeasures to mitigate these injuries requires a complete understanding of the tolerance of the human brain to injury. In this study, we developed a new method to establish human injury tolerance levels using an integrated database of reconstructed football impacts, subinjurious human volunteer data, and nonhuman primate data. The human tolerance levels were analyzed using tissue-level metrics determined using har

Optic nerve and vitreal inflammation are both RGC neuroprotective but only the latter is RGC axogenic.

Intravitreal inflammation, induced by either lens injury, or intravitreal injection of zymosan (IVZ), protects RGC from apoptosis and stimulates axon regeneration after optic nerve transection. Here, we investigate the differential effects of intra-optic nerve zymosan (ONZ) and IVZ injections on RGC neuroprotection and axogenesis. After both IVZ and ONZ injection, zymosan-induced inflammation promoted a similar 4-/5-fold enhancement in RGC survival, compared to optic nerve transected controls, but only IVZ promoted RGC axon regeneration. IVZ was the most effective in activating retinal astrocy

Traumatic brain injury and disorders of consciousness.

Trauma is one of the most common causes of disorders of consciousness (DOC) worldwide. Traumatic brain injury (TBI) leads to heterogeneous, multifocal injury via focal brain damage and diffuse axonal injury, causing an acquired network disorder. Recovery occurs through reemergence of dynamic cortical and subcortical networks. Accurate diagnostic evaluation is essential toward promoting recovery and may be more challenging in traumatic than non-traumatic brain injuries. Standardized neurobehavioral assessment is the cornerstone for assessments in the acute, prolonged, and chronic phases of trau

Neuropathology of explosive blast traumatic brain injury.

During the conflicts of the Global War on Terror, which are Operation Enduring Freedom (OEF) in Afghanistan and Operation Iraqi Freedom (OIF), there have been over a quarter of a million diagnosed cases of traumatic brain injury (TBI). The vast majority are due to explosive blast. Although explosive blast TBI (bTBI) shares many clinical features with closed head TBI (cTBI) and penetrating TBI (pTBI), it has unique features, such as early cerebral edema and prolonged cerebral vasospasm. Evolving work suggests that diffuse axonal injury (DAI) seen following explosive blast exposure is different

Brain injury: the pathophysiology of the first hours.'Talk and Die revisited'.

In the 25 years since the 'Talk and Die' paper there have been substantial advances in the management of patients with severe closed head injury. This paper discusses developments in understanding of primary and secondary injury. Current management focuses on preventing secondary brain injury. That this has been successful is illustrated by a fall in mortality in recent decades. Evidence based guidelines have set standards of management but they do not take into account variations between individuals, between regions of the brain and variations with time from injury. Various monitoring techniq

Short-Term Cyclosporin A Treatment Reduced Serum Neurofilament-Light Levels in Diffuse but Not Focal Traumatic Brain Injury in a Piglet Model.

Background/Objectives: Traumatic brain injury (TBI) in the pediatric patient results in acute neurophysiological deficits and can have potential long-term sequelae, impacting neurodevelopment. Serum biomarkers are an active area of study for TBI prognosis and diagnosis. Cyclosporin A (CsA), an immunosuppressant drug with neuroprotective qualities, targets mitochondria to stabilize the neurometabolic energy crisis following TBI. The objective of this study was to determine the acute effect of CsA treatment following focal and diffuse TBI on piglet serum biomarkers associated with glial neurofil

Distinct patterns of neurodegeneration after TBI and in Alzheimer's disease.

INTRODUCTION: Traumatic brain injury (TBI) is a dementia risk factor, with Alzheimer's disease (AD) more common following injury. Patterns of neurodegeneration produced by TBI can be compared to AD and aging using volumetric MRI. METHODS: A total of 55 patients after moderate to severe TBI (median age 40), 45 with AD (median age 69), and 61 healthy volunteers underwent magnetic resonance imaging over 2 years. Atrophy patterns were compared. RESULTS: AD patients had markedly lower baseline volumes. TBI was associated with increased white matter (WM) atrophy, particularly involving corticospin

Trials and tribulations of using beta-amyloid precursor protein immunohistochemistry to evaluate traumatic brain injury in adults.

Axonal pathology is increasingly identified by beta-amyloid precursor protein (betaAPP) immunohistochemistry in the brains of patients who may or may not have a history of trauma. The presence of betaAPP-IR(+) has been variously interpreted as either that diffuse traumatic axonal injury (TAI) is indeed a universal finding in cases of fatal traumatic brain injury (TBI) or there are other causes of betaAPP-IR(+) axons which under certain circumstances may be sufficient to mimic TBI and therefore make the medico-legal interpretation of certain cases very difficult. To address some of the uncertai

Concussion pathophysiology: From biomechanical insult to clinical phenotype - or is injury truly the beginning?

Mild traumatic brain injury (mTBI) is associated with substantial morbidity worldwide. Emerging evidence demonstrates that both impact- and blast-related mTBI produce diffuse microstructural and functional alterations, e.g., diffuse axonal injury, astroglial and microglial activation, cerebrovascular dysfunction, and neurometabolic disturbance. Rotational acceleration in impact injury preferentially induces white matter shear. Blast overpressure exerts prominent effects at cerebrovascular interfaces, particularly within perivascular spaces (PVS). These biomechanical differences may yield disti

Inflammation and white matter degeneration persist for years after a single traumatic brain injury.

A single traumatic brain injury is associated with an increased risk of dementia and, in a proportion of patients surviving a year or more from injury, the development of hallmark Alzheimer's disease-like pathologies. However, the pathological processes linking traumatic brain injury and neurodegenerative disease remain poorly understood. Growing evidence supports a role for neuroinflammation in the development of Alzheimer's disease. In contrast, little is known about the neuroinflammatory response to brain injury and, in particular, its temporal dynamics and any potential role in neurodegene

Isolated bilateral abducens nerves palsy due to traumatic delicate arteriovenous fistula.

BACKGROUND: Ocular motor dysfunction is common in patients with head trauma. Also, traumatic lateral gaze palsy is usually associated with brain stem lesion, peripheral nerve injury with or without basilar skull fracture and lateral rectus muscle injury or entrapment. However, isolated bilateral abducens nerve palsy is extremely rare. CASE STUDY: This study describes a case of isolated bilateral abducens nerve palsy in a 35-year-old male with head trauma without cervical and skull fractures. He was diagnosed with mild diffuse axonal injury. During his assessment, he developed mild limb weakne

Detection of blast-related traumatic brain injury in U.S. military personnel.

BACKGROUND: Blast-related traumatic brain injuries have been common in the Iraq and Afghanistan wars, but fundamental questions about the nature of these injuries remain unanswered. METHODS: We tested the hypothesis that blast-related traumatic brain injury causes traumatic axonal injury, using diffusion tensor imaging (DTI), an advanced form of magnetic resonance imaging that is sensitive to axonal injury. The subjects were 63 U.S. military personnel who had a clinical diagnosis of mild, uncomplicated traumatic brain injury. They were evacuated from the field to the Landstuhl Regional Medica

Therapeutic drug approach to stimulate clinical recovery after brain injury.

The identification of strategies by which the central nervous system (CNS) can transform itself in response to injury has incited the systematic exploration of methods to enhance neurological recovery after CNS injury. Several pharmaceuticals have been shown to promote such recovery; however, more rigorous clinical trials are necessary to establish their clinical relevance. The major impediment for these strategies in the clinical arena is the astounding heterogeneity surrounding neuroplasticity and regeneration. Tolerance to injury and varied rates of recovery are likely governed by genetic a

Transcranial Photobiomodulation Promotes Neurological Resilience in Current Collegiate American Football Players Exposed to Repetitive Head Acceleration Events.

Repetitive head acceleration events (RHAE) are common in contact sports and associated with neuroinflammation, axonal injury, and long-term neurological impairments, including increased risk for chronic traumatic encephalopathy. Current strategies for addressing RHAE focus on post-injury care rather than proactive neuroprotection, leaving athletes vulnerable to cumulative neurotrauma. Transcranial photobiomodulation (PBM) has shown promise in reducing neuroinflammation and promoting neuroprotection in traumatic brain injury; however, its potential to mitigate the structural brain changes assoc

A role for protein arginine methyltransferase 7 in repetitive and mild traumatic brain injury.

Mild traumatic brain injury affects the largest proportion of individuals in the United States and world-wide. Pre-clinical studies of repetitive and mild traumatic brain injury (rmTBI) have been limited in their ability to recapitulate human pathology (i.e. diffuse rotational injury). We used the closed-head impact model of engineered rotation acceleration (CHIMERA) to simulate rotational injury observed in patients and to study the pathological outcomes post-rmTBI using C57BL/6J mice. Enhanced cytokine production was observed in both the cortex and hippocampus to suggest neuroinflammation. F

Impact of voluntary exercise on obesity-induced brain pathology: Insights from a multimodal approach in male Ldlr-/-.Leiden mice.

BACKGROUND: Impact of exercise on obesity-related brain pathology remains unclear. MRI is used to detect obesity-related brain changes, but its interpretation remains unclear. We aimed to evaluate effects of voluntary exercise on obesity-induced brain pathology and determine whether MRI signal intensity and diffusion metrics capture these alterations. METHODS: Male Ldlr-/-.Leiden mice were fed a low-fat chow diet from 2 to 4 months of age. At 4 months, mice (n = 10) were sacrificed as baseline group representing young non-obese adult mouse brains. Remaining mice were fed a high-fat diet (HFD)

Pathology of fatal diffuse brain injury in severe non-penetrating head trauma.

Traumatic brain injury (TBI) is recognised as a serious global public health problem that imposes a heavy socioeconomic burden on society. The vast majority of cases result from road traffic accidents and falls, and the injuries are mainly attributed to velocity-related mechanisms. Lethal cases are mostly found to suffer from severe diffuse brain injuries (DBI), comprising diffuse vascular injury, diffuse axonal injury (DAI), generalized cerebral edema and ischemic-hypoxic injury. Coup and contrecoup brain contusions may also occur. This study set out to describe the pathological findings of s
