Cancer remains a global health challenge with high morbidity and mortality, and the clinical application of conventional anticancer agents is severely limited by issues such as poor selectivity, systemic toxicity, and multidrug resistance. Chalcones, pyridines, and pyrimidines are privileged heterocyclic scaffolds in medicinal chemistry, with inherent anticancer potential, structural flexibility, and good biocompatibility. Molecular hybridization, a rational drug design strategy, has been widely applied to construct chalcone pyridine/pyrimidine hybrids by covalently conjugating these bioactive

Drug development in rare metabolic diseases is frequently limited by the lack of validated clinical trial endpoints, particularly for chronic disease modification. Propionic acidemia (PA) is an intoxication-type inherited metabolic disorder with significant morbidity and mortality that begins in early life, for which there are no approved disease-modifying therapies. While regulatory pathways such as accelerated approval permit the use of surrogate endpoints that are reasonably likely to predict clinical benefit, their acceptance requires strong biological rationale and evidence linking biomar

BACKGROUND: Polycystic kidney disease (PKD) is commonly inherited as a loss-of-function mutation in PC1 (polycystin-1) or PC2. The molecular functions of polycystins remain uncertain, and therapeutics remain limited for millions of patients. One major roadblock has been the scarcity of assays that re-create PKD-specific cystogenesis in vitro, to complement mouse models.
SUMMARY: Over the past decade, a human organoid model of PKD has emerged that promises to bridge this gap. Human kidney organoids derived from PKD1-/- or PKD2-/- pluripotent stem cells recapitulate the pathognomonic cystic phe

Hyperuricemia is a well-established direct trigger of gouty arthritis. Xanthine oxidase (XO), the rate-limiting enzyme in uric acid production, represents a core therapeutic target for gout. Although XO inhibitors like allopurinol and febuxostat are widely-used clinically, their limitations, including severe hypersensitivity and cardiovascular risks, necessitate the continuous pursuit of safer and more efficient agents. This review provides a systematic and critical assessment of recent advances in XO inhibitor research, proposing for the first time a unified classification framework based on

Incorporating patient preferences into drug development is crucial, particularly, for rare diseases with significant unmet needs. This study used Best-Worst Scaling type 2 (BWS-2) to explore benefit-risk trade-offs for patients and caregivers in two rare neuromuscular diseases (NMDs), myotonic dystrophy type 1 (DM1), and mitochondrial myopathy (MM). Patients with DM1 and MM, along with caregivers, completed a BWS-2 survey assessing four treatment benefits (muscle strength, energy and endurance, balance, cognition) and two risks (permanent liver damage, temporary blurring of vision). Participan

In recent years, declining success rates and rising development costs have posed major challenges in drug development, highlighting the need for more efficient and rational approaches to evaluating drug efficacy and safety. Although non-clinical studies have traditionally relied primarily on animal experiments, adverse events that are difficult to predict continue to emerge during clinical trials and post-marketing, underscoring limitations in human extrapolation. In this context, new approach methodologies (NAMs), including human cell-based models and in silico analyses, have gained increasin

Gynecological cancer poses a serious threat to women's health. Despite significant advances in immunotherapy and targeted therapeutic strategies for gynecological cancers, substantial challenges persist, including limited response rates, inevitable resistance, and adverse effects. In recent years, a milestone in gynecological cancer therapy has been the approval of antibody-drug conjugates (ADCs). In this review, we provide a comprehensive overview of the structural features, mechanisms of action, and molecular characteristics of ADCs that have been approved and are currently under development

Delirium is a common cause and complication of hospitalization in the elderly and is associated with higher risk of future dementia and progression of existing dementia, of which 70% is Alzheimer's disease (AD). AD and delirium, which are known to be aggravated by one another, represent significant societal challenges, especially in light of the absence of effective treatments. The intricate biological mechanisms have led to numerous clinical trial setbacks and likely contribute to the limited efficacy of existing therapeutics. Artificial intelligence (AI) presents a promising avenue for overc

Pharmaceutical analysis plays a pivotal role in every stage of drug development, from target identification and lead compound screening to clinical validation. Traditional affinity-based analytical approaches face significant limitations, especially when addressing complex intracellular or membrane-associated drug targets. These challenges include difficulty of optimal bioactive target molecule identification, complicated signal transduction steps and limited physiological relevance of target models. As a novel molecular recognition tool, nucleic acid aptamers offer high affinity, specificity,

A challenge that limits our understanding of the underlying pathobiology of pediatric acute respiratory distress syndrome (PARDS) is the lack of a preclinical airway model that can be leveraged for the study of mechanisms and specific molecules for drug testing. We developed a physiologic model system of the small airways for mechanistic application in PARDS using a co-culture of primary human-derived small airway epithelial cells (SAECs) cultured at the air-liquid interface and umbilical vein endothelial cells in a transwell system. The model was validated by exposing the SAECs to a rhinoviru

As with adult pulmonary hypertension (PH), high morbidity and mortality persist with diverse types of paediatric PH. Despite major advances in pharmacologic therapies based on extensive studies in adult PH, few drugs have been comprehensively studied in neonates, infants, and children, leaving current paediatric PH care largely dependent on small observational studies and extrapolation of evidence from adult clinical trials. Challenges in developing successful clinical trials in children include the need to define distinct disease phenotypes with well-characterised natural history and outcomes

Mycobacterium tuberculosis (Mtb) depends on the bifunctional enzyme catalase-peroxidase (KatG) for survival within the host. KatG exhibits both catalase and peroxidase activities, serving distinct yet critical roles. While its peroxidase activity is essential for activating the frontline tuberculosis drug isoniazid, its catalase activity protects Mtb from oxidative stress. This bifunctional enzyme is equipped with a unique, protein-derived cofactor, methionine-tyrosine-tryptophan (MYW), which enables catalase activity to efficiently disproportionate hydrogen peroxide in phagocytes. Recent stud

The encouraging therapeutic potential of sulfonamide-based derivatives has been unraveled by breakthrough discovery of Paul Ehrlich, who pointed out the possibility of fighting microbes with chemicals. Over the decades, the utility of sulfonamides has expanded beyond antimicrobial agents, revealing their usefulness in many areas of pharmacotherapy, including the treatment of central nervous system (CNS) diseases. Through a detailed analysis of preclinical and clinical data, we identify key sulfonamide-based compounds that have demonstrated significant CNS activity. We also discuss the challeng

Research for tumor treatment with significant therapy effects and minimal side-effects has been widely carried over the past few decades. Different drug forms have received a lot of attention. However, systemic biodistribution induces efficacy and safety issues. Intratumoral delivery of agents might overcome these problems because of its abundant tumor accumulation and retention, thereby reducing side effects. Delivering hydrogels, nanoparticles, microneedles, and microspheres drug carriers directly to tumors can realize not only targeted tumor therapy but also low side-effects. Furthermore, i

Maintaining mitochondrial homeostasis is a potential therapeutic strategy for various diseases, including neurodegenerative diseases, cardiovascular diseases, metabolic disorders, and cancer. Selective degradation of mitochondria by autophagy (mitophagy) is a fundamental mitochondrial quality control mechanism conserved from yeast to humans. Indeed, small-molecule modulators of mitophagy are valuable pharmaceutical tools that can be used to dissect complex biological processes and turn them into potential drugs. In the past few years, pharmacological regulation of mitophagy has shown promising

Osteoarthritis (OA) is the most common form of arthritis worldwide, affecting ~500 million people, yet there are no effective treatments to halt its progression. Without any structure-modifying agents, management of OA focuses on ameliorating pain and improving function. Treatment approaches typically have modest efficacy, and many patients have contraindications to recommended pharmacological treatments. Drug development for OA is hindered by the gradual and progressive nature of the disease and the targeting of established disease in clinical trials. Additionally, new medications for OA cann
