Intermediate nerve neuralgia can be diagnosed and cured by microvascular decompression.

Here, we present a case of a 55-year-old woman with a 10-year history of hemifacial spasm accompanied by 1-month ipsilateral paroxysmal otalgia. Magnetic resonance imaging revealed the presence of vessels around the facial nerve root. Surgical exploration via suboccipital retromastoid craniotomy showed converging compression of the facial nerve root and intermediate nerve from both sides by an anterior inferior cerebellar artery loop. The patient's hemifacial spasm and ipsilateral otalgia were completely relieved after microvascular decompression of the facial nerve root and intermediate nerve

Nasopharyngeal cylindrical cell papilloma.

INTRODUCTION: Cylindrical cell papillomas are rare tumours which usually arise in the sinonasal region. CASE REPORT: We report a case of a nasopharyngeal cylindrical cell papilloma in a 56-year-old man who presented with a four-month history of right-sided hearing loss, otalgia, vertigo and tinnitus. Investigation revealed a soft, nodular lesion obstructing the pharyngeal opening of the right eustachian tube; this was treated by wide endoscopic excision. CONCLUSION: Cylindrical cell papilloma is a possible cause of eustachian tube obstruction in adults. Effective treatment of these lesions u

Acute colonic diverticular perforation presenting as left ear pain and facial swelling due to cervical subcutaneous emphysema in a patient administered corticosteroids.

A 56-yr-old white female administered corticosteroids presented with left ear pain and facial swelling due to cervical subcutaneous emphysema from a diverticular perforation of the sigmoid colon. This case demonstrates that localized signs of a bowel perforation may be absent in patients administered corticosteroids, that these patients may present with unusual, remote findings, and that bowel perforation can produce cervical subcutaneous emphysema.

Acute otalgia in Nigerian children.

A study of 112 referred children with acute otalgia labeled 'acute otitis media' by the referring physicians was carried out at the E.N.T. clinic of Lagos University Teaching Hospital in 1981-1982. Only 11% of these were actually due to acute otitis media, reflecting poor technique at otoscopy. Of the acute otalgia cases 56% were due to ear pathology while 44% resulted from referred pain. Otological causes included foreign body in the ear (23%), acute otitis media (11%), otitis externa (10%), secretory otitis media (6%) and myringitis bullosa haemorrhagica (4%). Cases due to referred otalgia w

Otologic manifestations of temporomandibular disorders: effects of TMJ arthrocentesis.

INTRODUCTION: A bidirectional interaction may exist between temporomandibular disorders (TMD) and otologic symptoms. This study aimed to evaluate changes in patient-reported otologic symptoms following temporomandibular joint (TMJ) arthrocentesis. MATERIALS AND METHODS: This retrospective cohort study included 40 patients who underwent TMJ arthrocentesis. Demographic data, TMJ-related clinical findings, otologic symptoms, and visual analog scale (VAS) scores were assessed preoperatively and at a 30-day follow-up. RESULTS: .The majority of patients were female (82.5%), with a mean age of 35.4

Simultaneous binaural bithermal caloric testing: Clinical value.

The aim of this retrospective review was to determine the clinical value of simultaneous binaural bithermal caloric testing (SBT) versus alternate binaural bithermal caloric testing (ABB) in the setting of a tertiary care neurotology clinic. Charts of 131 adults who had presented with otologic complaints and had undergone both SBT and ABB examinations were included in the study. The main outcome measure was the identification of peripheral hypofunction. One hundred two patients had a normal ABB caloric examination; 86 of those 102 patients (84.3%) had normal ABB examinations but abnormal SBT r

Synchronous ipsilateral sebaceous lymphadenoma and membranous basal cell adenoma of the parotid.

A 46-year-old man presented with persistent right otalgia and hearing loss. Exam was significant for a mildly tender retromandibular mass with intact nonerythematous overlying skin. Computerized tomography with intravenous contrast of the neck revealed 2 relatively well circumscribed masses in the right parotid gland. Although 1 lesion was suspected to be a necrotic lymph node, histologic analysis after superficial parotidectomy demonstrated 2 unique salivary gland tumors. Diagnoses of both sebaceous lymphadenoma and membranous basal cell adenoma were rendered. The occurrence of unique, synchr

Facial nerve paralysis due to intra-aural Hyalomma tick infestation.

We present the case of a 33 year-old man from a village of the north-eastern part of central Anatolia admitted to the otolaryngology department of Yeditepe University Hospital with right facial asymmetry and pain on the right ear. A tick of the genus Hyalomma was observed in the external auditory canal of the right ear and it was removed with fine cup forceps under otomicroscopy. We are of the opinion that in patients presenting with sudden acute ear pain and facial palsy, the ear canal should be examined to exclude an infestation by ticks.

External auditory canal paraganglioma: an atypical presentation.

OBJECTIVE: We present the first published description of a painful paraganglioma of the external auditory canal. Atypical histopathology made the diagnosis difficult. We discuss the potential pitfalls of clinical diagnosis and treatment of such a case. CLINICAL PRESENTATION: A 49-year-old woman presented with left-sided otalgia, hearing loss and tinnitus. Physical examination revealed a firm swelling arising from the superior portion of the left external auditory canal. A clinical diagnosis of otitis externa was made. INTERVENTION: There was minimal response to medical treatment. The swellin

Aspergilloma of the middle ear mimicking necrotising otitis externa: case report.

OBJECTIVE: We report a case of a patient who presented with otalgia and facial nerve palsy secondary to an aspergilloma of the middle ear. CASE REPORT: A 72-year-old, diabetic man presented to the ENT department with a history of worsening right-sided otalgia, aural discharge and hearing loss, associated with a right-sided facial weakness. This was assumed to be secondary to malignant otitis externa, and treatment was commenced. Imaging showed soft tissue within the middle ear and no bony erosion. Surgical exploration and biopsy revealed an aspergilloma of the middle ear. The pre- and post-op

Aspergillus otomycosis in an immunocompromised patient.

Aspergillus niger, an opportunistic filamentous fungus, was identified as the cause of chronic unilateral otomycosis in a 55-year old, immunocompromised man who had been unresponsive to a variety of treatment regimens. The patient presented with intermittent otalgia and otorrhea and with a perforation of his left tympanic membrane. A niger was identified in a culture specimen obtained from the patient's left ear canal. In immunocompromised patients, it is important that the treatment of otomycosis be prompt and vigorous, to minimize the likelihood of hearing loss and invasive temporal bone inf

[Otalgia as a result of certain temporomandibular joint disorders].

INTRODUCTION: Different hypotheses of reasons for common incidence of temporomandibular disorders (TMD) and otalgia have been discussed. The hypothesis of this study was, that the high prevalence of otalgia in patients with TMD may result in part from pain in the M. masseter pars prof. or in the temporomandibular joint (TMJ) which, due to the close anatomic neighborhood, can feel like ear pain. MATERIAL AND METHODS: [corrected] We retrospectively analyzed the anamneses of 720 of our TMD-patients with regard to main treatment motivation: how many patients quoted "pain in the ear" as main treat

[The red tympanic membrane].

An otoscopic red tympanic membrane is for the examiner a challenge. A short directed history, as well as a few functional examinations associated to a systematic otoscopy allow in most cases to make the otological diagnosis. The differential diagnosis for a red tympanic membrane is very large, including malformations, traumas, infections, and even tumors and other degenerative pathologies. The first diagnosis to think about is an infectious disease. On the other side, there are rare pathologies which, even for experienced examiners are quite seldom.

Cranial polyneuropathy--Ramsay Hunt's syndrome: case report and discussion.

Ramsay Hunt's syndrome is an infectious cranial polyneuropathy caused by varicella zoster, the herpetic virus that also causes chickenpox and shingles. Its symptoms include facial paralysis, ear pain, and an auricular rash. Oral lesions are also present in most cases. This syndrome can affect any cranial nerve and usually affects multiple nerves, causing central, cervical, and peripheral effects. This article reports the case of a 35-year-old white female who was treated by the oral surgery service of a large urban hospital, after first reporting to the emergency clinic. Her reported symptoms

Prospective study of tympanosclerosis developing after grommet insertion.

Grommets have been used as a treatment for middle-ear effusions since their introduction by Armstrong in 1954. However, it is now accepted that the presence of a grommet in a tympanic membrane is associated with the development of tympanosclerosis in that membrane. Furthermore, it has been shown that this is a progressive phenomenon and that the incidence increases with time to as much as 65 per cent at 3 to 4 years after grommet insertion (Mawson and Fagan, 1972; Tos and Poulsen, 1976). Initially it was suggested that the development of tympanosclerosis might be due to the middle ear disease
