Emphysematous prostatic abscess: case series study and review.

OBJECTIVES: To report three cases of emphysematous prostatic abscess (EPA) and review the literature on this rare entity. METHODS: All relevant articles published in English over the last 50 years (1961-2011) were identified using a MEDLINE search for keywords 'emphysematous prostatitis' and 'emphysematous prostatic abscess'. A total of 12 patients were evaluated for their age, race, underlying disease, pathogens, diagnosis tool, treatment options, and clinical outcome. RESULTS: EPA is a high mortality disease (25%) and has an association with diabetes mellitus. The most common pathogen in t

Imaging of Renal Infections and Inflammatory Disease.

Acute pyelonephritis is a bacterial infection of the renal parenchyma and collecting system. Diagnosis is based on clinical findings of fever, flank pain, and urinary tract infection. Computed tomography findings include renal enlargement with wedge-shaped heterogeneous areas of decreased enhancement, known as a "striated nephrogram." Imaging is primarily used to diagnose complications such as emphysematous pyelonephritis, renal abscess, and pyonephrosis. Chronic pyelonephritis can have varying appearances on imaging ranging from xanthogranulomatous pyelonephritis or, in extreme cases, renal r

Severe combined emphysematous infections of the urinary tract in a diabetic patient: A case report.

Emphysematous infections of the urinary tract are rare but life-threatening conditions, most commonly affecting diabetic and immunocompromised patients. We report a case of a 55-year-old male with uncontrolled type 2 diabetes and chronic alcoholism who presented in septic shock. CT imaging revealed emphysematous pyelonephritis, emphysematous cystitis, a pelvic abscess in the recto-vesical pouch, and bilateral periurethral prostatic abscesses. The case illustrates the importance of early imaging, prompt diagnosis, and multidisciplinary management in preventing fatal outcomes.

The role of nuclear medicine in the diagnosis of common and specific diabetic infections.

Infections are usually detected in diabetes mellitus. They may be divided into: common infections such as fungal infections, pulmonary tuberculosis, pneumonia, bacteraemia, urinary tract infections, and diabetic foot infections and specific infections. The latter occur almost exclusively in diabetes and include rhinocerebral mucormycosis, malignant external otitis, emphysematous pyelonephritis, perirenal abscess, emphysematous cystitis and emphysematous cholecystitis. Radionuclide tests are decisive in the diagnosis and localisation of foot osteomyelitis, as well as the distinction of osteomye

[Case report : Emphysematous cystitis].

The emphysematous cystitis is a rare condition characterized by the presence of air in the wall and/or the bladder lumen. The clinical expression of this cystitis is variable. Some patients complain of abdominal pain or urinary symptoms. Other may present only pneumaturia or be totally asymptomatic. This condition is considered as potentially severe since it can lead to an emphysematous pyelonephritis with septicemia and septic shock. Peritonitis may also occur in case of necrosis and perforation of the bladder wall. However, this negative development can be avoided by a diagnosis and an early

A Patient with Generalized Weakness - A Case Report.

UNLABELLED: Emphysematous pyelonephritis (EPN) is a rare and life-threatening sequalae of a urinary tract infection marked by a gas-producing necrotizing infection of the renal parenchyma requiring prompt treatment. It has been found to be correlated with uncontrolled diabetes mellitus and is more common in females due to their increased susceptibility to urinary tract infections. Early recognition and treatment of EPN are essential to improve the patient's prognosis and prevent further complications. In this case we highlight a progressive case of pyelonephritis complicated by emphysematous a

We present a rare case of emphysematous cystitis associated with an emphysematous pyelonephritis in a diabetic Arab man who was admitted in a confusional state. A 60-year-old man was admitted to the emergency department with confusion and hypogastric mass. The Clinical examination found comatose patient with a mass in the tympanic hypogastric percussion. The pelvic computed tomography (CT) demonstrated intramural gas in the urinary tract, which suggested a diagnosis of emphysematous cystitis and emphysematous pyelitis. The treatment was based on an antibiotics associated with a bladder drainag

Emphysematous pyelitis presenting as an acute abdomen in an end-stage renal disease patient treated with peritoneal dialysis.

Emphysematous pyelitis is air in the renal collecting system in patients with urinary tract infections. This entity is uncommon and seen primarily in patients with diabetes mellitus. We report a case of a patient with end-stage renal disease treated with peritoneal dialysis who developed emphysematous pyelitis who presented with signs and symptoms that were more consistent with appendicitis. The spectrum of infections causing air in the urinary tract and the method by which end-stage renal disease patients are treated are discussed. Patients receiving dextrose peritoneal dialysis are at risk f

Primary emphysematous ureteritis diagnosed using CT and successfully treated with ureteral stenting: A case report.

Emphysematous urinary tract infections (EUTIs) are usually reported as emphysematous pyelonephritis or cystitis, whereas ureteral involvement as the main lesion is extremely rare. An 86-year-old woman presented with left lower abdominal pain and constipation. Computed tomography revealed gas formation predominantly in the left ureter, consistent with emphysematous ureteritis. During hospitalization, the patient had elevated levels of inflammatory markers and procalcitonin, and developed disseminated intravascular coagulation. Ureteral stenting and antibiotic therapy resulted in rapid improveme

Xanthogranulomatous pyelonephritis presentation in the ED: a case report.

Xanthogranulomatous pyelonephritis (XGP) is a slowly progressive, chronic kidney infection. It presents with nonspecific symptoms and is definitively diagnosed by surgical examination. Xanthogranulomatous pyelonephritis must be differentiated in the emergency department (ED) from acutely progressive conditions, such as emphysematous pyelonephritis, which require immediate intervention. Xanthogranulomatous pyelonephritis requires nephrectomy; however, emergent kidney removal is not crucial. The low prevalence and nonspecific presentation of XGP increase the importance of recognizing common risk

Radiology and Non-operative Management of Emphysematous Infections of the Pancreas, Gallbladder, and Urinary System.

Emphysematous abdominal infections are regarded as potentially life-threatening conditions and benefit from appropriate radiological imaging for timely diagnosis and treatment planning. A 70-year-old non-diabetic male presented with an acute abdomen and had computed tomography diagnosed emphysematous pancreatitis, cholecystitis, and pyelonephritis. Treatment included broad-spectrum antimicrobial therapy. Ultrasound-guided percutaneous drain placement into the peri-pancreatic collection permitted culture-directed antibiotic therapy and continued drainage of the purulent collection. He achieved

The Diagnosis and Management of Life-threatening Urologic Infections.

Genitourinary infections are commonly encountered and managed in inpatient, outpatient, and emergency settings. Fournier's gangrene, emphysematous pyelonephritis, and obstructive pyelonephritis represent the most serious urologic infections and have a high risk of mortality if not managed promptly. Due to the rarity of these infections, the evidence for specific treatment strategies is scattered. This review aims to provide comprehensive, evidence-based recommendations for the diagnosis and management of these life-threatening urologic infections.

[Pathologic features in diagnosis and differential diagnosis of pyelonephritis (author's transl)].

Morphological diagnosis and differential diagnosis of pyelonephritis are discussed particularily considering macroscopic features. A special form with radiological significance is ncrotizing "emphysematous" pyelonephritis. Chronic non-obstructive pyelonephritis is rarer than formerly thought if strict morphological criteria are applied. It should be differentiated from nephropathy caused by analgetics. In analgesic nephropathy bilateral papillary necrosis is the earliest and most characteristic alteration. Differentiation between chronic pyelonephritis and analgesic nephropathy is possible in

Emphysematous urinoma in a renal transplant patient.

Urinary infection is a common complication after kidney transplantation. In some instances, especially with Escherichia coli infections, there is formation and collection of gas in the parenchyma and collecting system of the kidney, giving rise to the condition of emphysematous pyelonephritis. Such a process could occur in collections of urine (urinoma) secondary to ureteric leak in the transplant kidney. This process has not been described so far. In this report, we describe the first case of an infected urinoma with an interesting radiologic finding, a so-called emphysematous urinoma.

[Emphysematous cystitis with air bubbles in the vena cava].

A 76-year-old diabetic woman was referred to our hospital with an episode of high fever and sub-abdominal pain. Computed tomography (CT) of the pelvis revealed gas accumulation within the lumen and wall of the bladder and CT of the abdomen demonstrated bubbles in the inferior vena cava. She recovered by urinary drainage and antibiotic therapy. Urinary culture revealed Escherichia coli. CT after the therapy didn't demonstrate gas accumulation of the bladder and bubbles in the inferior vena cava. Emphysematous cystitis: report of an atypical case.

We report the atypical case of a nondiabetic 66-year old male with severe abdominal pain and vomiting who was found to have emphysematous cystitis. Of all gas-forming infections of the urinary tract emphysematous cystitis is the most common and the least severe. The major risk factors are diabetes mellitus and urinary tract obstruction. Most frequent causative pathogens are Escherichia coli and Klebsiella pneumoniae. The clinical presentation is nonspecific and ranges from asymptomatic urinary tract infection to urosepsis and septic shock. The diagnosis is made by abdominal imaging. Treatment

Clinical Outcomes of Acute Pyelonephritis in Type 2 Diabetes Mellitus.

Background Diabetes mellitus (DM) is a prevalent predisposing factor for urinary tract infections (UTIs). Among hospitalized patients with acute pyelonephritis, UTIs are more common, severe, and associated with worse outcomes, particularly in those with type 2 DM. Pyelonephritis in DM patients is more frequently bilateral and linked to greater complications, with 90% of emphysematous pyelonephritis (EMPN) and cystitis cases occurring in diabetic individuals. Objective This study aims to evaluate the clinical and microbiological profiles, treatment outcomes, and complications of acute pyeloneph

Understanding Air in the Body: A Radiologic Perspective on Emphysematous and Traumatic Conditions.

BACKGROUND: Abnormal collections of air within body tissues and anatomical compartments, although relatively uncommon, represent critical radiologic findings. The causative factors vary from invasive infections by gas-forming organisms to traumatic and iatrogenic breaches of tissue planes. Early and accurate radiologic identification is essential, as delayed diagnosis may result in significant morbidity and mortality. The study objective is to describe the spectrum of computed tomography (CT) imaging features, etiopathogenesis, and clinical significance of some emphysematous infections, includ
