Symptoms Associated With Exercise Intolerance and Resting Heart Rate Following Mild Traumatic Brain Injury.

OBJECTIVES: People may experience a myriad of symptoms after mild traumatic brain injury (mTBI), but the relationship between symptoms and objective assessments is poorly characterized. This study sought to investigate the association between symptoms, resting heart rate (HR), and exercise tolerance in individuals following mTBI, with a secondary aim to examine the relationship between symptom-based clinical profiles and recovery. METHODS: Prospective observational study of adults aged 18 to 65 years who had sustained mTBI within the previous 7 days. Symptoms were assessed using the Post-Conc

[Cardiopulmonary exercise testing: Key practical aspects].

Functional exercise testing (FET) assesses an individual's capacity to adapt to effort and identifies limiting factors, particularly dyspnea. It orients therapeutic choices, predicts the progression of chronic pathologies, and estimates preoperative risks, at times contraindicating surgery. The aim of this article is to provide a summary of the specific indications for functional exercise testing, test protocol selection, test equipment, appropriate personnel, and patient and test safety. This article is intended for healthcare professionals conducting or considering functional exercise testin

Influence of the mode of heating on cerebral blood flow, non-invasive intracranial pressure and thermal tolerance in humans.

KEY POINTS: The human brain is particularly vulnerable to heat stress; this manifests as impaired cognition, orthostatic tolerance, work capacity and eventually, brain death. The brain's limitation in the heat is often ascribed to inadequate cerebral blood flow (CBF), but elevated intracranial pressure is commonly observed in mammalian models of heat stroke and can on its own cause functional impairment. The CBF response to incremental heat strain was dependent on the mode of heating, decreasing by 30% when exposed passively to hot, humid air (sauna), while remaining unchanged or increasing wi

[Factors limiting exertion in respiratory diseases].

Factors limiting exercise capacity in patients with respiratory disease are determined in part by their response to an exercise test. Exercise tests evaluate the overall oxygen transport chain, i.e. exercise tolerance. The basic parameter is maximal oxygen consumption, VO2max which is very low in respiratory diseases. Decreased exercise tolerance results first from limited ventilatory capacity at maximal exercise: ventilatory reserve in these patients is low. At submaximal exercise, ventilation is on the contrary increased for a given exercise load. This relative hyperventilation is the conseq

New physiological insights into dyspnea and exercise intolerance in chronic obstructive pulmonary disease patients.

Dyspnea and reduced exercise tolerance are common consequences of chronic obstructive pulmonary disease (COPD) and contribute importantly to poor perceived health status. While the origins of dyspnea and reduced exercise tolerance are complex and multifactorial, there is increasing evidence that lung hyperinflation is an important contributory factor that can be targeted for treatment. In this review, the authors summarize current concepts of the origin and clinical and physiological consequences of both static and dynamic lung hyperinflation in COPD. In particular, they review recent studies

Blood flow restriction exercise during microgravity exposure in parabolic flight.

This case report evaluates whether it is possible to perform blood flow restriction (BFR) exercise during exposure to microgravity. The objectives were three-fold: (1) to determine if a personalised tourniquet system (PTS) hardware technology performs nominally and enables BFR exercise in microgravity; (2) to determine if BFR augments the exercise stimulus in microgravity in a similar manner to its application on Earth; and (3) to evaluate tolerance and acceptability of performing BFR exercise and operating the PTS hardware in microgravity. Two participants performed resistance squat and deadl

Impaired cardiac reserve and exercise capacity in patients receiving long-term thyrotropin suppressive therapy with levothyroxine.

To assess cardiac function and exercise tolerance in patients receiving long term TSH-suppressive therapy with levothyroxine (L-T4), we studied maximal exercise capacity with a bicycle ergometer and left ventricular function at rest and during physical exercise by radionuclide angiography. The evaluation was performed in 10 patients receiving L-T4 therapy (2.31 +/- 0.13 microgram/kg) for 5-9 yr, presenting with effort dyspnea and symptoms of adrenergic overactivity, and 10 matched control subjects. The patients were reassessed after 4 months of administration of the selective beta-adrenergic b

Diltiazem as adjunctive oral therapy in patients with chronic atrial fibrillation.

Digoxin frequently fails to control the heart rate in patients of chronic atrial fibrillation particularly during exertion. We have studied in 20 such patients the effect of adding diltiazem (180 mg/day) on resting and peak exercise heart rates. An attempt was also made to determine its effects on exercise tolerance by using treadmill and 6 minute walk test. Addition of diltiazem resulted in significant attenuation of heart rate both at rest and at peak exercise. The resting and exercise mean heart rates on digoxin alone were 98.9 +/- 21.5 b.p.m. and 160.2 +/- 35.68 b.p.m. respectively. After

Hypohydration Increases Heart Rate Despite Preserved Hemodynamic Compensation During Blood Flow Restricted Exercise.

During blood flow restriction (BFR) exercise, blood becomes sequestered in the active limbs due to impeded venous return, contributing to central hypovolemia. Dehydration also reduces blood volume and can exacerbate the ability of the cardiovascular (CV) system to defend against CV stressors, such as exercise and orthostatic stress. As BFR becomes more widely used, its application in individuals who may be suboptimally hydrated underscores the need to understand how hydration status influences cardiovascular strain. We tested whether hypohydration affects the CV response to BFR exercise and it

Exercise limitation and pulmonary rehabilitation in chronic obstructive pulmonary disease.

Impairment of exercise tolerance is a common problem in patients with severe chronic obstructive pulmonary disease. The cause of exercise intolerance in patients with severe chronic obstructive pulmonary disease is multifactorial and includes impaired lung mechanics, fatigue of inspiratory muscles, impaired gas exchange, right ventricular dysfunction, malnutrition, occult cardiac disease, deconditioning, and psychologic problems; however, impaired lung mechanics and gas exchange abnormalities seem to be the major limiting factors. Recently, the approach to management of pulmonary rehabilitatio

Hypertensive women with dyspnea exhibit an unfavorable central blood pressure response to exercise.

Limited exercise tolerance and dyspnea in patients with uncomplicated hypertension may pose a diagnostic challenge, particularly when blood pressure is normal and assessment results do not support a diagnosis of heart failure. The purpose of this study was to assess the differences in central blood pressure (cBP) response to exercise between females with hypertension and good exercise tolerance (non-dyspneic females, nDFs; n = 27) and those with dyspnea on exertion (dyspneic females, DFs; n = 25). We also investigated the relations of cBP and its dynamics with peak oxygen consumption (peak VO2

Should We Recommend Exercise after the Fontan Procedure?

BACKGROUND: The Fontan procedure, the last of a series of operations performed in patients with congenital heart defects, offers improved exercise capacity compared to baseline function but is still reduced compared to healthy peers. Exercise training may improve exercise tolerance but there is no consensus on the safety of this practice or the optimal training regimen. We performed a systematic literature review on the effects of exercise training in patients with a Fontan circulation. METHODS: Medline and Embase databases were systematically searched for articles regarding Fontan Procedure
