Superficial surgical landmarks for the frontal sinus.

OBJECT: There is a lack of reports in the literature that contain descriptions of superficial anatomical landmarks for the identification of the internally located frontal sinus. Neurosurgeons must often enter the cranium through the frontal bone and knowledge of the frontal sinus is essential to minimize complications. METHODS: Seventy adult cadaveric frontal sinuses were evaluated. Measurements included both the lateral and superior extent of the frontal sinus in reference to a midpupillary line, and the superior extent of the frontal sinus from the nasion. Frontal sinuses were found bilate

Trigonocephaly: Case Report, Review of Literature and a Technical Note.

BACKGROUND: Premature fusion of the metopic suture results in a type of craniosynostosis known as trigonocephaly. The treatment of trigonocephaly is surgical and is likely to remain so. Surgical methods and techniques for correction of craniosynostosis-related skull deformities have evolved, and a single best procedure is yet to be presented. CASE REPORT: Here we present a technical remark in a case of open cranial vault reconstruction. CONCLUSION: Although the literature, in general, prefers barrel stave (radial) frontal bone osteotomies, a technique with longitudinal frontal bone osteotomi

Rare case of Pott's puffy tumor due to nasal foreign body.

BACKGROUND: Pott's Puffy tumor (PPT) is a complicated frontal sinusitis that is also described as frontal bone osteomyelitis and a localized sub periosteal abscess. Early diagnosis and immediate active treatment are necessary to prevent severe neurologic sequelae. CASE PRESENTATION: Here, we report on a case of Pott's puffy tumor in a previously healthy 13-year-old girl with a worsening headache and swelling of the frontal bone accompanied by vomiting and fever. Magnetic resonance imaging revealed osteomyelitis of the frontal bone. A foreign body was discovered and removed via endoscopy which

An autopsy case of misdiagnosis based on postmortem computed tomography findings.

A middle-aged man was found lying beside his bicycle on an early winter morning. The cause of death was diagnosed by clinicians as traumatic intracerebral hemorrhage and cerebral contusion with frontal bone fracture based on the findings of Computed Tomography (CT) of the head. However, forensic autopsy revealed that there were no evidences of intracerebral hemorrhage and left frontal bone fracture but the defect of golf ball size on the frontal lobe which was considered to be a complication from the old cerebral contusion and old bone fracture. The bleeding and pooling blood from subarachnoid

[Optimization of reconstructive surgery on the frontal sinus].

Results of the treatment of three patients with frontal sinus injuries of different severity demonstrate that it is desirable to start surgical intervention for their management in the early period after injury, i.e. as soon as the acute process is completed. The extent of surgery should be estimated based on the results of computed tomography of the accessory sinuses of the nose in the preoperative period. The frontal sinus was approached through the modified Zutter skin incision which allowed to avoid a cosmetic facial defect, an injury to temporal muscles and frontal branch of the facial ne

Frontofacial Reconstruction Technique Modification With Preservation of Blood Supply to the Monobloc Segment.

Craniosynostosis syndromes, including Apert Syndrome, Pfeiffer Syndrome, and Crouzon Syndrome, share similar phenotypes, including bicoronal craniosynostosis, midface hypoplasia, hypertelorism, and exorbitism. The standard surgical treatment for these craniofacial abnormalities is monobloc osteotomy with distraction osteogenesis. Complications of this technique include the failure of osteogenesis or resorption of the frontal bone. The authors propose an alternative surgical technique with a frontal arch in continuity with the midface segment to ensure vascularization to anterior and posterior

[Fibrous dysplasia of the facial bones (author's transl)].

On the occasion of three cases of fibrous dysplasia (two of frontal sinus and the ethmoids and one of the upper jaw) the problem of fibrous dysplasia of the jaws is analysed. The clinical picture is characterized by a slow and gradual emergence of facial assymetric or exophathalmos. Histologically there is a variety of form. The treatment is largely a remodeling osteotomy. The writers effectively applied on all three cases a radial removal of the lesion.

Management of frontal sinus fractures.

Frontal sinus fractures are relatively uncommon maxillofacial injuries, making up only 5 to 12 percent of all facial fractures. Associated intracranial, ophthalmologic, and other maxillofacial injuries are very common because of the force of injury required to fracture the frontal bone. High-resolution computed tomography of the frontal region in multiple planes is essential for predicting the degree of frontal injury, associated injuries, and the type of procedure indicated. Exploration of the frontal sinus with reduction alone is reserved for a small minority of very simple fractures. Most f

Long-term outcome of fibrous dysplasia: reconstruction with dysplastic bone--case report.

A 10-year-old girl presented with facial asymmetry associated with bulging of the right fronto-orbital region with no symptoms. Computed tomography demonstrated enlargement of the right zygomatic, frontal, ethmoid, and sphenoid bones. Abnormal proliferation of the bone had obliterated the sphenoid, right frontal sinus, and right ethmoid sinuses. These radiological findings suggested right optic nerve compression due to fibrous dysplasia. Right optic canal decompression was performed. In preparation for recurrence, the resulting bone defect in the right orbital roof was reconstructed using the

[Endoscopic frontal sinus revision following transcutaneous endoscopic puncture and diaphanoscopy].

BACKGROUND: Endoscopic revision sinus surgery in case of frontal sinus pyocele may poses a great surgical challenge for various reasons. Due to the often troublesome anatomical changes caused by prior resection and findings of scaring with new bone formation, the identification of the frontal sinus in revision surgery is frequently a challenge for the surgeon. An easy endoscopic technique for a safe endonasal identification of the frontal sinus in revision surgery is therefore of major importance. MATERIAL AND METHODS: 4 patients (3 men, 1 female) were enrolled with an acute frontal pyocele f

Schematic of mean thickness distribution on the lateral aspect of the canine frontal sinus as an experimental model of sinus surgery.

The dog frontal sinus may represent an alternative model dental implant research; its topographical resemblance to the maxillary sinus renders it a potentially favorable experimental environment. The aim of this study was thus to elucidate the anatomical configuration of the canine frontal sinus and histological characteristics, and to determine whether it could be a new canine experimental model for dental implant research. Twenty-four sides of canine frontal bones were harvested. The distance from the nasion to the emerging point of the lateral aspect of the canine frontal sinus was measured

An allometric study of the frontal sinus in Gorilla, Pan and Pongo.

There is considerable speculation about the role and significance of the paranasal sinuses in the Hominoidea, and this study aims to present new data about an old problem from cephalograms of dried crania. Measurements of frontal sinus volumes were determined for Gorilla gorilla gorilla; G. gorilla beringei and Pan troglodytes. By adopting an allometric approach it was determined that the frontal sinus volume of Gorilla is relatively smaller than that of Pan, and that the frontal sinus of G. g. gorilla is relatively smaller than that of G. g. beringei. Frontal sinus volume scales in a positive

[A case of multiple myeloma associated with a large subcutaneous mass in the frontal region].

A case of multiple myeloma having plasmacytoma in the frontal bone and clivus is reported. A 68-year-old female was referred to our hospital because of a subcutaneous mass in the frontal region. She was diagnosed as having multiple myeloma 7 years ago and had been treated with chemotherapy. On admission, severe anemia, hyperproteinemia and elevation of serum lambda type immunoglobulin G (IgG) were pointed out. Plain skull X-rays showed numerous punched out lesions with a large bone defect of the frontal bone. CT scan and MRI revealed a mass lesion in the clivus in addition to a large epidural

A Pott's Puffy Tumor Associated with Epidural - Cutaneous Fistula and Epidural Abscess: Case Report.

BACKGROUND: Pott's puffy tumour is characterized by a fluctuate swelling of the frontal region as a result of osteomyelitis of the frontal bone. This inflammatory lesion may propagate endocranially, resulting in acute meningitis, epidural abscess, subdural empyema, cavernous sinus trombophlebitis, cerebritis, and frontal lobe abscess of the brain. CASE REPORT: We present an unusual case of a 33-year-old man suffering from Pott's puffy tumour whose condition was further complicated by a draining epidural-cutaneous fistula and an epidural abscess. We confirmed the diagnosis by contrast-enhanced

Extent of the frontal sinuses in the aspect of neurosurgical craniotomy approaches.

BACKGROUND: Frontal sinuses within the frontal bone can be a potential obstacle in neurosurgical approach in this region. Their unintended opening during craniotomy is not beneficial due to the risk of brain infections by bacteria inhabiting the sinus mucosa. Therefore, such opening should be avoided whenever the surgical procedure does not involve the sinus itself. The aim of the study was the morphometric analysis of the frontal sinuses based on computed tomography imaging. MATERIALS AND METHODS: The width, height, projection surface area and location of the highest and most lateral points

Frontal Base Endodermal Cyst: A Case Report and Review of Literature.

BACKGROUND: Endodermal cyst (EC) is a rare congenital cyst of endodermal origin, but the pathogenesis of this entity remains uncertain. Supratentorial EC is particularly uncommon, but some cases have been reported. Here, we report a case of supratentorial EC that developed at the frontal base which indicates posttraumatic development rather than a congenital origin. CASE DESCRIPTION: A 65-year-old man who had a history of orbital bone fracture without rhinorrhea sustained in a traffic accident presented with gradually enlarging frontal-base cystic lesions. Multiple cystic lesions were removed

[The expanding skull fracture (author's transl)].

An expanding skull fracture with exceptional extension into the frontal sinus, the orbit and the ethmoidal cells is discussed. A 12 year old boy fell down the stairs at the age of 8 years and fractured his left frontal bone with extension to the base of the skull. He was seen because of a swelling of the scalp, an intermittent orbital oedema and convulsions. At neurosurgery a large gaping fracture of the frontal bone was found and this was interposed with fibrous tissue and relatively denatured cerebral tissue. The management both neurosurgical and rhinological is discussed.

Orbital "blow-in" fractures: clinical and CT features.

During a 16 month period, seven patients were admitted to our trauma center with "blow-in" fractures of the orbital roof. This injury results from a significant direct blunt force applied to the supraorbital region of the frontal bone with transmission of energy to the thin orbital plate of this bone and displacement of bone fragments downward into the superior orbit. High resolution CT with multiplanar reformation and three-dimensional display proved very useful in identifying and characterizing the bone and soft tissue abnormalities found in our patients. Our experience suggests that CT demo

Fractures of the frontal sinus.

Fractures of the frontal sinus are frequently seen in patients with cranio-facial injuries. Trauma to the posterior wall and more deeply located tissues: anterior fossa, dura and brain, give an indication of the seriousness of such injuries. We point out some particular aspects of our experience; in the neurosurgical approach to such lesions; we use a classification based on treatment: -when the posterior wall of the sinus is not, or only slightly damaged, we drain it using a thin suction catheter pulled through the fronto-nasal duct, kept in place for six to ten days. -when a comminuted fract
