Will 68Ga PSMA-radioligands be the only choice for nuclear medicine in prostate cancer in the near future? A clinical update.

Prostate Cancer (PCa) represents the most common malignant tumor in men but according to the European Association of Urology (EAU) guidelines, a mass screening for PCa diagnosis should not be performed due to over-diagnosis and over-treatment related problems. An early clinical diagnosis is possible, mainly based on digital rectal examination and Prostatic Specific Agent (PSA) testing. However, the only mandatory test to define the presence of PCa is ultrasound guided-biopsy, obtained on multiple samples, which has also a high prognostic value. In this context, diagnostic imaging plays an impo

Potential Impact of 68Ga-PSMA-11 PET/CT on the Planning of Definitive Radiation Therapy for Prostate Cancer.

Standard-of-care imaging for initial staging of prostate cancer (PCa) underestimates disease burden. Prostate-specific membrane antigen (PSMA) PET/CT detects PCa metastasis with superior accuracy, having a potential impact on the planning of definitive radiation therapy (RT) for nonmetastatic PCa. Our objectives were to determine how often definitive RT planning based on standard target volumes covers 68Ga-PSMA-11 PET/CT-defined disease and to assess the potential impact of 68Ga-PSMA-11 PET/CT on definitive RT planning. Methods: This was a post hoc analysis of an intention-to-treat population

The Theranostic PSMA Ligand PSMA-617 in the Diagnosis of Prostate Cancer by PET/CT: Biodistribution in Humans, Radiation Dosimetry, and First Evaluation of Tumor Lesions.

UNLABELLED: PET imaging with the prostate-specific membrane antigen (PSMA)-targeted radioligand (68)Ga-PSMA-11 is regarded as a significant step forward in the diagnosis of prostate cancer (PCa). More recently, a PSMA ligand was developed that can be labeled with (68)Ga, (111)In, (177)Lu, and (90)Y. This ligand, named PSMA-617, therefore enables both diagnosis and therapy of PCa. The aims of this evaluation were to clinically investigate the distribution of (68)Ga-PSMA-617 in normal tissues and in PCa lesions as well as to evaluate the radiation exposure by the radioligand in PET imaging. 68Ga-siderophores for PET imaging of invasive pulmonary aspergillosis: proof of principle.

UNLABELLED: The diagnosis of invasive pulmonary aspergillosis (IPA) is difficult and lacks specificity and sensitivity. In the pathophysiology of Aspergillus fumigatus, iron plays an essential role as a nutrient during infection. A. fumigatus uses a specific and highly efficient iron uptake mechanism based on iron-complexing ferric ion Fe(III) siderophores, which are a requirement for A. fumigatus virulence. We aimed to evaluate the potential of siderophores radiolabeled with (68)Ga, a positron emitter with complexing properties comparable to those of Fe(III), as a radiopharmaceutical for imag

Outcome Prediction After [177Lu]Lu-PSMA I&T in Metastatic Castration-Resistant Prostate Cancer: External Validation of a [68Ga]Ga-PSMA-11 PET-Based Model with [18F]F-Flotufolastat.

177Lu-prostate-specific membrane antigen (PSMA)-targeted radiopharmaceutical therapy is an established treatment for metastatic castration-resistant prostate cancer, though responses vary. A prognostic model using [68Ga]Ga-PSMA-11 PET-derived parameters was developed to predict therapy outcomes. This retrospective analysis validates the model in an independent cohort imaged with [18F]F-flotufolastat ([18F]rhPSMA-7.3 PET) before [177Lu]Lu-PSMA I&T. Methods: In total, 174 consecutive patients treated with [177Lu]Lu-PSMA I&T at a single center were included in this analysis. The Cox proportional

Similarity of Biological Information Captured by [68]Ga-PSMA-11 PET and ASL Perfusion MRI in Glioblastoma.

BACKGROUND AND PURPOSE: The overexpression of prostate-specific membrane antigen (PSMA) in the microvasculature of glioblastoma (GBM), in contrast to its negligible expression in normal brain parenchyma, provides a compelling rationale for using PSMA-targeted radioligands in the diagnosis and radiation therapy of GBM. Arterial spin-labeling (ASL) perfusion imaging, an advanced MRI technique, allows noninvasive assessment of tumor microvascular density and perfusion. This study aimed to investigate the relationship between gallium 68 [68]Ga-PSMA-11 PET and ASL perfusion imaging, as well as the

Fast (< 30 min) "All-in-One" whole-body MRI for TNM staging in high-risk prostate cancer (PCa): Feasibility and comparison to 68Ga-Prostate Specific Membrane Antigen (PSMA)-PET/CT.

PURPOSE: Next-generation imaging techniques, including PSMA-PET/CT and whole-body MRI (WB-MRI), are disrupting the management of prostate cancer (PCa). This study aimed to build a fast "All-In-One" WB-MRI protocol and to compare it to 68Ga-PSMA-PET/CT for local (T), nodal (N), and distant staging (M1a, M1b, M1c). METHODS: Fifty-two PCa patients at high-risk for metastases underwent a fast "All-in-One" WB-MRI (combining biparametric prostate assessment based on rapid T2-weighted and diffusion-weighted imaging (DWI) following the PI-RADS v2.1 guidelines and the MET-RADS-P guidelines). [68Ga]Ga-PSMA-11 PET/CT in medullary thyroid carcinoma: comparison with [18F]FDG PET/CT and immunohistochemical analysis.

INTRODUCTION: Medullary thyroid carcinoma (MTC) is a rare neuroendocrine malignancy. Despite the use of various imaging modalities, including positron emission tomography combined with computed tomography (PET/CT), a significant proportion of patients with biochemical evidence of disease have no detectable lesions. Prostate-specific membrane antigen (PSMA) is expressed by the neovasculature of several solid tumours, including thyroid cancer. While case reports suggest that PSMA-targeted PET/CT may detect MTC lesions, its diagnostic value remains unverified. This study aimed to compare the clin

68Ga-PSMA-11 PET/MR Detects Local Recurrence Occult on mpMRI in Prostate Cancer Patients After HIFU.

High-intensity focused ultrasound (HIFU) is a promising new modality for the treatment of localized prostate cancer (PCa). Follow-up of patients is recommended with biopsies and multiparametric MRI (mpMRI). However, mpMRI in the postinterventional setting is often false-negative. It was our aim to investigate if the new tracer targeting the prostate-specific membrane antigen (68Ga-PSMA-11) could be used to localize recurrent disease with PET/MR in patients with discrepant findings between mpMRI and template biopsies. Methods: Interim analysis was performed of the first 10 patients scanned betw

Comparison of 68Ga-labelled PSMA-11 and 11C-choline in the detection of prostate cancer metastases by PET/CT.

PURPOSE: Prostate-specific membrane antigen (PSMA) is expressed ubiquitously on the membrane of most prostate tumors and its metastasis. While PET/CT using 11C-choline was considered as the gold standard in the staging of prostate cancer, PET with radiolabelled PSMA ligands was introduced into the clinic in recent years. Our aim was to compare the PSMA ligand 68Ga-PSMA-11 with 11C-choline in patients with primary and recurrent prostate cancer. METHODS: 123 patients underwent a whole-body PET/CT examination using 68Ga-PSMA-11 and 11C-choline. Suspicious lesions were evaluated visually and semi

Brain Nocardiosis Mimicking Intracerebral Invasion on 68 Ga-Pentixafor PET/CT in a Patient With Multiple Myeloma.

Brain nocardiosis is an uncommon but severe disease associated with high mortality. We present a case of brain nocardiosis with elevated tracer uptake on both 68 Ga-pentixafor and 18 F-FDG PET/CT, mimicking intracerebral invasion of multiple myeloma. This case demonstrates that nocardiosis should be considered in the differential diagnosis of brain lesions found on PET/CT with increased tracer accumulation in immunocompromised patients.

In vitro and in vivo evaluation of the bifunctional chelator NODIA-Me in combination with a prostate-specific membrane antigen targeting vector.

INTRODUCTION: We recently developed a chelating platform based on the macrocycle 1,4,7-triazacyclononane with up to three five-membered azaheterocyclic arms for complexation of the PET nuclides gallium-68 and copper-64. The main objective of this study was to evaluate the stability and pharmacokinetics of 68Ga- and 64Cu-complexes of the bifunctional chelator NODIA-Me 1 covalently bound to a PSMA targeting vector in vivo. METHODS: NODIA-Me 1 was conjugated to the PSMA targeting Glu-NH-CO-NH-Lys moiety to give the bioconjugate NODIA-Me-NaI-Ahx-PSMA 4. The stability of [68Ga]4 and [64Cu]4 was as

Evaluation of human transferrin radiolabeled with N-succinimidyl 4-[fluorine-18](fluoromethyl) benzoate.

UNLABELLED: Iron metabolism plays a key role in cell proliferation and survival in rapidly growing cancer cells. Uptake is mediated by the carrier protein transferrin. The increased need for iron has been used as a method to target tumors and there is well-documented evidence that certain tumors can be imaged with tracers such as 67Ga, that mimic transferrin-mediated iron uptake. To obtain a tracer that would be better able to quantitate transferrin kinetics and indirectly evaluate iron metabolism, we have labeled human transferrin with the positron emitter, 18F, with a one-step high-specific

FAPI-74 PET/CT Using Either 18F-AlF or Cold-Kit 68Ga Labeling: Biodistribution, Radiation Dosimetry, and Tumor Delineation in Lung Cancer Patients.

68Ga-fibroblast activation protein inhibitors (FAPIs) 2, 4, and 46 have already been proposed as promising PET tracers. However, the short half-life of 68Ga (68 min) creates problems with manufacture and delivery. 18F (half-life, 110 min) labeling would result in a more practical large-scale production, and a cold-kit formulation would improve the spontaneous availability. The NOTA chelator ligand FAPI-74 can be labeled with both 18F-AlF and 68Ga. Here, we describe the in vivo evaluation of 18F-FAPI-74 and a proof of mechanism for 68Ga-FAPI-74 labeled at ambient temperature. Methods: In 10 pat

A novel bifunctional metabolizable linker for the conjugation of antibodies with radionuclides.

A novel heterogeneous bifunctional reagent containing an ester bond, N-[[4-(2-maleimidoethoxy)-succinyl]oxy]succinimide (MESS), was designed and synthesized for the conjugation of antibodies with the gallium-67 (67Ga) chelate of succinyldeferoxamine (SDF) via the ester bond. MESS was synthesized by the acylation of N-(2-hydroxyethyl)maleimide with succinic anhydride, followed by the activation of the resulting carboxylic acid to a succinimido ester. MESS possesses a maleimide group for protein conjugation and an active ester group for deferoxamine (DFO) coupling, and the two functional groups

Localization of 11C-radiopharmaceuticals in the Greene melanoma of hamsters.

Seventy Syrian golden hamsters bearing SC transplants of Greene melanoma were used to evaluate the degree of tumour uptake of several 11C-radiopharmaceuticals selected for their potential specificity for melanoma. Tissue distribution studies were performed at 30 and 60 min after IV injection of 11C-compounds and compared with the 24-h uptake of 67Ga-citrate. Gamma camera images were also compared. The highest tumour uptake at 1 h was observed with 11C-methionine (2.42% +/- 0.72%) and although activity in liver, spleen and kidney exceeded that in melanoma the tumour was demonstrated on gamma ca

The use of 68Ga-PET/CT PSMA in the staging of primary and suspected recurrent renal cell carcinoma.

PURPOSE: The role of 68Ga-PSMA PET/CT in the staging of prostate cancer is well known. PSMA is also overexpressed in the neovasculature of other tumours including renal cell carcinoma (RCC), suggesting there may be a role for the use of 68Ga-PSMA PET/CT. Thus far, there has been limited literature documenting the use of 68Ga-PSMA PET/CT in the investigation and management decisions of RCC. METHODS: This was a retrospective case series of patients who received a 68Ga-PSMA PET/CT scan for staging or restaging of RCC between July 2016 and December 2018. Primary outcome measure was to identify wh

PSMA PET/CT in biochemical recurrence of prostate cancer with PSA levels ≤ 0.2 ng/mL: a German multicenter analysis of conventional PSMA tracers, including [68Ga]Ga-PSMA-11, [68Ga]Ga-PSMA I&T, and [18F]PSMA-1007.

BACKGROUND: Prostate-specific membrane antigen (PSMA)-directed positron emission tomography/computed tomography (PET/CT) has emerged as a highly accurate imaging modality for detecting tumor lesions in patients with biochemical recurrence (BCR) of prostate cancer (PC). While detection rates of lesions suspicious for PC relapse are known to increase with rising prostate-specific antigen (PSA) levels, data on the efficacy of PSMA PET/CT at very low PSA values (≤ 0.2 ng/mL) remain limited. METHODS: In this multicenter study, we analyzed 321 patients with BCR and a PSA value ≤ 0.2 ng/mL across fi
