Is the xylose test still a worth-while investigation?

In a retrospective survey the results of a D-xylose absorption test have been assessed in relation to jejunal morphology and final diagnosis in 152 adult patients with various types of gastrointestinal disease. Neither urine excretion rates nor serum concentrations alone provide an adequate separation between patients with definite mucosal lesions and those without evidence of gut disease or with other types of gastrointestinal pathology. It is suggested that when a jejunal biopsy can readily be performed the xylose test serves little useful purpose in routine practice and can be positively mis

Subacute combined degeneration of the spinal cord is associated with tripterygium glycoside tablet usage.

BACKGROUND: Subacute combined degeneration (SCD) is a neurodegenerative disease caused by vitamin B12 deficiency. The lesions mainly involve the posterior cord, lateral cord, and peripheral nerves. Occasionally, the lesions also involve brain white matter and optic nerves in severe cases. Reports of drug-induced impaired absorption and metabolism of vitamin B12 resulting in SCD are scarce. INTRODUCTION: A patient developed SCD after long-term use of tripterygium glycoside tablets in the treatment of glomerulonephritis. However, after discontinuation and vitamin B12 treatment with tripterygium

Post-Pyloric Administration of Rivaroxaban With Therapeutic Monitoring in Pediatric Cardiac Patients: Experience in 3 Patients.

Thromboembolism in pediatric patients with congenital heart disease (CHD) is a commonly encountered morbidity. Feeding intolerance is also commonly encountered in patients with CHD and has a direct effect on thromboembolism treatment options, as the site of gastrointestinal absorption for each agent varies. Historically, standard anticoagulation options included unfractionated heparin, enoxaparin, or vitamin K antagonists such as warfarin. Non-vitamin K oral anticoagulants, such as rivaroxaban, have become more widely used in pediatrics since obtaining Food and Drug Administration approval, bu

Sucrosomial® Iron: An Updated Review of Its Clinical Efficacy for the Treatment of Iron Deficiency.

Iron deficiency (ID) and iron deficiency anemia (IDA) are highly prevalent worldwide. Oral iron salts, especially ferrous sulfate, are commonly used for the treatment of iron deficiency (ID). However, its use is associated with gastrointestinal side effects, thus compromising treatment compliance. Intravenous iron administration is a more costly and logistically complex alternative and is not risk-free, as infusion and hypersensitivity reactions may occur. Sucrosomial® iron is an oral formulation consisting of ferric pyrophosphate conveyed by a phospholipid and sucrester matrix (sucrosome®). I

A new gastro-intestinal mathematical model to study drug bioavailability.

This work focuses on a new mathematical model which describes the gastro-intestinal absorption of drugs and the effect of food interactions on drugs bioavailability. The model structure consists of five compartments (stomach, duodenum, jejunum feeding, intestine and blood) simulated though different in-series reactors. All the enzymatic reactions taking place in the gastro-intestinal system are described through the Michaelis-Menten kinetic equations. The model has been tested for drug administration (paracetamol and ketoprofen) with and without the meal digestion. The model has been validated

Effect of lactic acid bacteria on diarrheal diseases.

Microbial balance is an important factor in the maintenance of intestinal homeostasis, and yogurt or fermented milk supplementation has been proposed to control diarrheal diseases. A number of studies using animal models and clinical studies in humans have confirmed the beneficial effect of such fermented products in case of lactose intolerance, viral diarrhea or antibiotics-associated diarrhea. The mechanisms by which lactic acid bacteria exert their effects are multiple. Bacterial lactase improves the absorption of lactose, but fermented products slow down the intestinal transit facilitating

Evaluation of the effect of ethanol's toxic metabolite acetaldehyde on the gastrointestinal oligopeptide transporter, PEPT1: in vitro and in vivo studies.

BACKGROUND: The effects of alcohol consumption and its subsequent metabolism on drug transport, absorption and pharmacokinetics are poorly understood. This study examines the effects of the ethanol metabolite, acetaldehyde, on the clinically relevant drug transporter, PEPT1. The metabolism of ethanol and the following acetaldehyde formation is thought to modulate the uptake capacity of PEPT1 within the gastrointestinal tract for a variety of clinically important peptidomimetic drug compounds. METHODS: Glycylsarcosine ([(3)H]-GlySar), a nonhydrolysable PEPT1 specific substrate was used in our

Safety considerations with gastrointestinally active lipid-lowering drugs.

Gastrointestinally active agents such as cholesterol absorption inhibitors (CAIs) (eg, ezetimibe) and bile acid sequestrants (BAS) (the resins cholestyramine and colestipol, or colesevelam, a nonabsorbable polymer) offer important options for lipid-lowering therapy. Ezetimibe is a novel CAI that inhibits the absorption of dietary and biliary cholesterol without affecting the absorption of triglycerides or fat-soluble vitamins. In clinical trials, there has been no evidence of increased rates of myopathy or rhabdomyolysis associated with ezetimibe, whether in use as monotherapy or in a combinat

The Pharmacokinetics of Sildenafil May Be Affected by Intestinal Absorption Rate in Children Admitted to the Intensive Care Unit.

This study was performed for a better understanding of the pharmacokinetics of sildenafil (SIL) and N-desmethyl sildenafil (DMS) in 13 children treated in the intensive care unit (ICU). Blood samples were taken periodically after the first oral administration of SIL (0.5 mg/kg). Plasma concentrations were analyzed by tandem LC/MS. Of the 13 patients, apparent peaks in the plasma concentration of SIL were observed in four patients, with the other nine patients showing reduced or delayed drug absorption of SIL. The maximum plasma concentrations of SIL after administration varied in range from 7.

Plant origin prebiotics affect duodenal brush border membrane functionality and morphology, in vivo (Gallus Gallus).

The intra-amniotic administration approach has been used to evaluate the effects of plant origin prebiotics on intestinal health and on brush border membrane functionality and morphology. Prebiotics are fermentable dietary fibers, which can positively affect the host by selectively stimulating the growth and activity of colon bacteria, thus improving intestinal health. The consumption of prebiotics increases digestive tract motility, which leads to hyperplasia and/or hypertrophy of intestinal cells, increasing nutrient digestive and absorptive surface area. This review collates information abo

Gastrointestinal fate of bioactive peptides: A mechanistic review of stability, transport, and bioavailability.

Bioactive peptides (BAPs), encrypted within food proteins, offer significant potential for health promotion and disease prevention. However, their practical application as nutraceuticals or therapeutics is hampered by low and variable oral bioavailability. The discrepancy that often exists between potent in vitro activity and diminished in vivo efficacy is largely attributable to the formidable challenges encountered during gastrointestinal (GI) transit. This review provides a critical and mechanistic examination of the GI fate of BAPs, focusing on the interplay between their stability, transp

From intestinal permeability to dysmotility: the biobreeding rat as a model for functional gastrointestinal disorders.

BACKGROUND: Impaired intestinal barrier function, low-grade inflammation and altered neuronal control are reported in functional gastrointestinal disorders. However, the sequence of and causal relation between these events is unclear, necessitating a spontaneous animal model. The aim of this study was to describe the natural history of intestinal permeability, mucosal and neuromuscular inflammation and nitrergic motor neuron function during the lifetime of the BioBreeding (BB) rat. METHODS: Normoglycemic BB-diabetes prone (DP) and control rats were sacrificed at different ages and jejunum was

[Neuro-psychiatric aspects of gastrointestinal disease (author's transl)].

On principle one has to distinguish between inflammatory (infectious) and non-inflammatory intestinal diseases. These may affect merely certain parts of the gastrointestinal tract or influence the entire digestive system. Leading symptoms of all these diseases are disturbances of intestinal motility and of secretion, both causing diarrhoea or, rarely, constipation. The clinical picture can be acute or chronic. Acute manifestations, especially with high temperatures, include various florid psychopathological phenomena of an exogenous type of reaction. Chronic diseases of the gastrointestinal sy

Gastric retention for cardiovascular therapy: matching drug properties, dosage-form design, and translational evidence.

Gastroretentive delivery offers a selective strategy for cardiovascular drugs the performance of which depends on upper-gastrointestinal (GI) residence, sustained or modified exposure, or timed release. In this review, I synthesize the rationale, candidate drugs, platform-selection logic, and translational evidence for gastroretentive cardiovascular systems. Particularly strong cases involve narrow absorption windows, poor lower-GI absorption, pH-dependent solubility or instability, low or variable bioavailability, distal-GI efflux, and circadian cardiovascular risk. I proposes a drug-first fr

Inter-subject variability in intestinal drug solubility.

Variability in oral drug absorption is a well-known phenomenon, but it is often overlooked for its potential effects in oral drug delivery. Understanding the mechanisms behind absorption variability is crucial to understanding and predicting drug pharmacokinetics. In this study, the solubility of furosemide and dipyridamole - drugs known to have highly variable oral bioavailabilities - was investigated in individual ileostomy fluids from 10 subjects with ulcerative colitis. For comparison, drug solubility was also determined in pooled upper gastrointestinal fluids from healthy human subjects a
