[Dynamia correction of hallux valgus deformity with musculus extensor hallucis longus shifting].

OBJECTIVE: To discuss the mechanisms and clinical effect of musculus extensor hallucis longus shifting in correcting hallux valgus (HV) deformity. METHODS: From April 2004 to December 2006, 25 cases of HV (38 feet) were treated by musculus extensor hallucis longus shifting. There were 2 men and 23 women, aging from 22-60 years (mean 46.3 years). HV angle was 21-45 degrees (mean 31.30 degrees), intermetatarsal (IM) angle was 7-21 degrees (mean 12.52 degrees). The HV were corrected by cutting osteophyma of the first metatarsal bone, cutting transverse head of adductor pollicis, transferring mus

[Endoprosthetic replacement of hallux rigidus].

Replacement of the first metatarsophalangeal joint (MTP1) is still controversial when compared with MTP1 fusion in treating hallux rigidus and other disabling conditions of the MTP1 joint. Prospective studies concerning endoprosthetic replacement of the MTP1 joint are not available yet. Nevertheless, better understanding of biomechanics and tribology show favour towards total endoprosthesis. Some currently available models are described (Toefit-Plus, Roto-glide, ReFlexion, Bio-Action, Moje). Furthermore the authors report their results with 30 Toefit-Plus total endoprostheses. Five complicatio

[Compartment syndromes of the lower extremities (author's transl)].

A reduction of the arterial blood pressure of the muscles of the 4 tight fascial compartments of the shank may be initiated by: Trauma and operation of the lower extremities, spontaneous bleeding, occlusion of the arteries, general and local hypotonia due to elevation of the leg, as well as by drug abuse and increased stress of the muscles of the lower extremities. This is followed by an increased swelling and tension of the tissue with ischemia and consecutive paralyses of the nerves and muscles in the presence of almost simultaneous muscle contractures. We may encounter a double insult, a "r

Transfer of abductor hallucis tendon combined with scarf osteotomy versus single scarf osteotomy in moderate to severe hallux valgus deformity: a comparative retrospective cohort study.

BACKGROUND: Scarf osteotomy (SO) was broadly applied in moderate to severe hallux valgus (MSHV), and the results were satisfactory. However, due to the complicated pathologic changes in hallux valgus, the ideal surgical treatment is still controversial. Transfer of the abductor hallucis tendon combined with Scarf osteotomy (TAHTCSO) was an innovative alternative technique. This retrospective cohort study aimed to define if TAHTCSO mode resulted in improved outcomes as compared with the single SO in MSHV. METHODS: Of 73 patients (92 ft) with MSHV, 36 (45 ft) were treated through TAHTCSO and 37

The influence of two different types of foot orthoses on first metatarsophalangeal joint kinematics during gait in a single subject.

OBJECTIVE: To quantify the effect of two distinct foot orthotic designs on in vivo multisegment foot and leg motion; in particular, the first metatarsal and first metatarsophalangeal (MTP) joint during gait. METHODS: A 23-year-old man had an excessively pronated foot structure as measured during a clinical orthopedic examination. The Optotrak Motion Analysis System was used to collect three-dimensional position and orientation data from four modeled rigid body segments (hallux, first metatarsal, calcaneus, and tibia) during the stance phase of walking. The subject walked at a self-selected co

Free fat graft for congenital hand differences.

BACKGROUND: Free fat graft has been used for the treatment of congenital hand differences. However, there have been a few reports about the outcome of that treatment. In this study, the outcome of free fat grafts for congenital hand and foot differences was investigated. METHODS: Fourteen bones with longitudinal epiphyseal bracket, 3 wrists with Madelung deformity, and 5 cases of osseous syndactyly were treated with free fat graft with osteotomy, physiolysis, or separation of osseous syndactyly. Of the fourteen bones with longitudinal epiphyseal bracket, 9 were treated with open wedge osteoto

First tarsometatarsal arthrodesis for severe hallux valgus using the tension band principle - Technical tip and case series.

BACKGROUND: Hallux valgus (HV) is a common foot pathology. Severe HV in the presence of Tarsometatarsal joint (TMTJ) instability is often managed with arthrodesis of the 1st TMTJ. There are concerns regarding non-union and malunion (particularly the early loss of inter-metatarsal angle correction before complete arthrodesis). We report our medium-term results of a small series of patients that underwent an evolved surgical technique utilising orthogonal staples and a transverse suture button fixation to address biomechanical concerns with traditional Lapidus arthrodesis. METHODS: A retrospect

Introduction the revolving scarf osteotomy for treating severe hallux valgus with an increased distal metatarsal articular angle: a retrospective cohort study.

BACKGROUND: Hallux valgus(HV) with an increased distal metatarsal articular angle (DMAA) is one of the most common foot deformities among adults. Double metatarsal osteotomy (DMO) is effective in treating severe HV deformity with an increased DMAA. However, this technique presents the risk of avascular necrosis (AVN) of the metatarsal head and transfer metatarsalgia due to shortening of the first metatarsal. The aim of this study was to introduce a surgical procedure defined as revolving scarf osteotomy (RSO) and compare the clinical and radiological results of RSO and DMO performed for treati

Dropped hallux after the intramedullary nailing of tibial fractures.

We made a prospective study of 208 patients with tibial fractures treated by reamed intramedullary nailing. Of these, 11 (5.3%) developed dysfunction of the peroneal nerve with no evidence of a compartment syndrome. The patients with this complication were significantly younger (mean age 25.6 years) and most had closed fractures of the forced-varus type with relatively minor soft-tissue damage. The fibula was intact in three, fractured in the distal or middle third in seven, with only one fracture in the proximal third. Eight of the 11 patients showed a 'dropped hallux' syndrome, with weakness

Surgical Reconstruction of the Forefoot with Hallux Valgus Associated with Metatarsus Adductus.

Metatarsus adductus is a structural deformity that may be associated with hallux valgus and that may lead to metatarsal pain and functional abnormality of the foot. Correcting hallux valgus is complicated in these cases because of the deviation in adduction of the lesser metatarsals, especially the second metatarsal that occupies the first intermetatarsal space. We report the case of a 49-year-old man who underwent a scarf osteotomy in the first metatarsal, shortening and abductor oblique distal osteotomies of the lesser metatarsals, and arthrodesis of the central toes with Z-lengthening of th

The Ripple Effect: How Hallux Valgus Deformity Influences Ankle and Knee Joint Kinematics During Gait.

Hallux valgus (HV) is described as a lateral deviation of the great toe at the first metatarsophalangeal joint (MTP), which is a very common foot deformity in the clinic. This deformity extends beyond localized foot mechanics to affect the entire lower extremity kinetic chain, potentially increasing dynamic instability during locomotion. This study aimed to characterize the kinematics of ankle and knee joints during walking in HV patients compared to controls. In total, 23 patients with bilateral HV and matched healthy controls were recruited. The 6-DOF kinematics data of ankles and knees were

Hypermobility of the first metatarsal bone in patients with rheumatoid arthritis treated by Lapidus procedure.

BACKGROUND: Foot deformities and related problems of the forefoot are very common in patients with rheumatoid arthritis. The laxity of the medial cuneometatarsal joint and its synovitis are important factors in the development of forefoot deformity. The impaired joint causes the first metatarsal bone to become unstable in the frontal and sagittal planes. In this retrospective study we evaluated data of patients with rheumatoid arthritis who underwent Lapidus procedure. We evaluated the role of the instability in a group of patients, focusing mainly on the clinical symptoms and X-ray signs of t

Intermetatarsal angle after first metatarsophalangeal joint arthrodesis for hallux valgus.

BACKGROUND: First metatarsophalangeal (MTP) joint arthrodesis is commonly done for hallux valgus with an arthritic joint. In patients with a wide preoperative first intermetatarsal (IM) angle an important question is whether the metatarsus varus will be corrected by the first MTP joint fusion alone or whether an additional basal osteotomy is necessary. METHODS: The charts and radiographs of 20 patients who had arthrodesis of the first MTP joint were retrospectively reviewed. All 20 patients were female with a mean age of 54.2 (range 42 to 78) years. Either a Hallu-S plate (Integra Life Scienc

Shape of the lateral edge of the first metatarsal head changes depending on the rotation and inclination of the first metatarsal: a study using digitally reconstructed radiographs.

BACKGROUND: Correction of pronation deformity and metatarsal primus varus is an important component of hallux valgus surgery, necessary to achieve a satisfactory correction and to prevent post-operative recurrence. Roundness of the lateral edge of the first metatarsal head (round sign) on the dorsoplantar radiograph of the foot has been empirically advocated as an indicator of first metatarsal pronation. The purpose of this study was to clarify the effect of rotation and inclination of the first metatarsal on the shape of the lateral edge of the first metatarsal head. METHODS: Computed tomogr

Relationships Between First Metatarsal and Sesamoid Positions and Other Clinically Relevant Parameters for Hallux Valgus Surgery.

Relationships between hallux valgus (HV) and other measurements within the first ray have been extensively studied. It is becoming more popular to correct HV deformity with tarsometatarsal joint arthrodesis while internally (varus) rotating the first metatarsal. This, in turn, reduces the sesamoid position when viewed in the dorsoplantar projection on radiographs. However, it has been shown that not all HV deformities have pathological external (valgus) rotation of the first metatarsal. In this study, we explored the relationships between frontal-plane rotations of the first metatarsal as well

Rotational Osteotomy for Hallux Valgus. A New Technique for Primary and Revision Cases.

UNLABELLED: More than 200 different surgical techniques exist for hallux valgus (HV). Some of them are designed for mild, moderate, or severe deformities depending on their correction power. Nevertheless, they all correct only the coronal and/or sagittal plane deformity. Just a handful of them correct the known axial malrotation that exists in most HV cases. This malrotation is one possible factor that could be the source of recurrence of an operated HV as it has been described. We describe a new technique which simultaneously corrects the metatarsal internal rotation and varus deformity by ro

The Ludloff metatarsal osteotomy: guidelines for optimal correction based on a geometric analysis conducted on a sawbone model.

Ideally, osteotomy for hallux valgus deformities corrects varus angle and pronation, while minimizing elevation, depression, and shortening. We used a serial linkage tracking device to evaluate five variations of the Ludloff osteotomy, a stable proximal metatarsal osteotomy that incorporates an oblique dorsal to plantar cut and a lateral swing or pivoting correction of the dorsal fragment. A neutral osteotomy (perpendicular to the sagittal plane) yielded the greatest correction (14 degrees) but with shortening (average, 2.85 mm), elevation (average, 1.36 mm), and additional pronation (average,
