Indications of primary osteosynthesis in patients with fractures of long bones of extremities with special regard to multiple and associated fractures of femur diaphysis.

Fractures of the long shaft-shaped bones were evaluated upto the sixties as an isolated therapeutic problem. During the past fifteen years, however, views as regards treatment of fractures of long bones began to change along with the increasing number of multiple fractures and polytraumas. It was revealed that fractures of the long bones and pelvic fractures have a bearing on the development of the syndrome of fat embolism, the syndrome of acute respiratory distress in adults, on the development of shock damage of the lungs, heart, kidneys, liver, pancreas etc. Evidence was accumulating that u

[Results of surgical treatment of fractures of the metacarpus and fingers].

Ninety-one patients with 108 fractures of the metacarpal and/or finger bones were treated by surgery between January 1974 and June 1982. 70 patients (76.9%) with 81 surgically treated fractures could be checked up after 3 3/4 years on an average. 24.3% out of these patients suffered from associated soft tissue leasions, 25.7% suffered from multiple fractures. 41 out of the 81 surgical interventions were performed for fractures of the metacarpus and 40 for fractures of the finger bones. The results were classified according to an evaluation scheme established for this purpose and containing the

Treating post-traumatic osteomyelitis of a metacarpal fracture with the 'mini-masquelet' procedure.

BACKGROUND: Post-traumatic osteomyelitis is a challenging complication after a fracture, requiring long-term treatment to prevent loss of function. One treatment strategy is the biphasic masquelet technique, focussing on both control of the infection and bone reconstruction. This technique is mainly used to treat defects of the long bones. Very little literature exists about the masquelet procedure for treatment of defects of smaller bones. We describe a case of post-traumatic osteomyelitis after a metacarpal fracture, treated with the 'mini-masquelet' technique. PATIENT CASE: A 23-year old w

[A rare inferior dislocation of the distal radio-ulnar joint caused by spontaneous post-fracture epiphysiodesis of the radius].

The authors report a case of radial deviated hand following fracture of the distal forearm bones in a seven year old child who was not followed-up after minimal initial treatment. They discuss the mechanism of this deformity due to growth disturbances affecting the extremities of the two bones of the forearm, especially the distal radius which stopped growing at the time of the accident, resulting in relative-shortening of the radius of 25 mm in relation to the ulna and 90 mm in relation to the healthy side. The injured ulna consequently slowed its rate of growth without stopping altogether. T

Pathologic Fractures in Benign Neoplasms of the Fingers.

Background: To describe the rate and predictors of pathologic fractures in benign neoplasms of the finger, as variables from prediction models for pathologic fractures of the long bones of the extremities are not necessarily applicable to the bones of the hand. Methods: In this retrospective chart review, 69 histologically confirmed neoplasms affecting the bones of the fingers, 49 phalanges and 20 metacarpals, were identified in patients presenting at 2 urban hospitals over a 24-year period. Different variables were studied as possible predictors of pathologic fractures. Results: Forty-nine pe

Melorheostosis in the upper extremity.

Melorheostosis is a rare mesodermal disease affecting the skeleton and adjacent soft tissues. Often it is incidentally detected on radiographs. In the standard radiology and orthopedics literature, melorheostosis is described as a "flowing hyperostosis, resembling dripping candle wax as an incidental radiographic finding." A 22-year-old man presented with a 2-year history of right-hand pain. Radiologic evaluation of the hand showed massive sclerotic changes in the first and second metacarpal and phalangeal bones on the right side. Further radiographic evaluation of the right upper extremity re

The Emergent Evaluation and Treatment of Hand and Wrist Injuries: An Update.

Injury patterns of the hand and wrist can be complex and challenging for the emergency physician to diagnose and treat. The ability of the hand to perform delicate maneuvers requires a very intricate interplay of bones, ligaments, and tendons. Unfortunately, due to the omnipresence of the hand, the hand and wrist are commonly injured. These injuries can be debilitating if not treated correctly and can be both time-consuming and fraught with medicolegal risk. This article provides the necessary knowledge to diagnose and treat common hand and wrist injuries encountered in the emergency departmen

digiBONE: an automated tool for segmental Greulich-Pyle bone age assessment of Indian children and adolescents.

INTRODUCTION: Accurate bone age assessment (BAA) is essential for diagnosing and managing pediatric endocrine and growth disorders, as it reflects biological maturity beyond chronological age. The widely used Greulich-Pyle (GP) method estimates bone age by visually comparing full-hand radiographs with standardised reference images. Although widely used, this technique-and most automated systems based on it-assumes uniform skeletal maturation across the hand. In practice, however, skeletal maturation progresses at different rates in the anatomical segments of the hand under varied hormonal infl

The use of corundum ceramic implants after excision of tumours of the extremities.

Implants of corundum ceramic have been used in the treatment of 38 patients with bony tumours of the limbs. Data are presented concerning the replacement with ceramic implants of the joint ends and diaphyseal defects of long bones, as well as in various bones of the hand and foot. Experiments carried out over a long period have shown that corundum ceramic is a strong material and is biologically inert.

Aneurysmal bone cyst of the index sesamoid.

Sesamoid bone disorders are rare and usually are a result of trauma or degenerative causes. Tumors of the sesamoid bones of the hand are encountered less frequently. We report a case of an aneurysmal bone cyst of the radial index sesamoid of the hand. The tumor was treated successfully by sesamoid bone resection.

Multiple drill-hole osteotomy in hand surgery - description of a novel application and proof of feasibility.

BACKGROUND: Malunion of phalangeal and metacarpal bones are often associated with impairment of hand function and pose a challenging task for treating surgeons in most cases. When applicable, corrective osteotomy is the treatment of choice, where the affected bone is cut to correct malalignment using chisels or saws. The use of these instruments is associated with several drawbacks especially in hand surgery. We aimed to determine whether a multiple drill-hole (MDH) osteotomy technique was suitable for performing corrective osteotomies of metacarpal and phalangeal bones. METHODS: This case se

Hand development in trisomy 21.

The purpose of the present study was to evaluate hand size and maturity in fetuses with trisomy 21 (Down syndrome). Twenty-five fetuses, crown-rump length (CRL) 55-222 mm, foot length (FL) 8-42 mm, were included in the study. After whole-body radiography (Hewlett Packard Faxitron), special radiographs of the hand and foot were taken. Hand length was measured as the length of the third finger from the distal tip of the distal phalanx to the proximal tip of the metacarpal bone, the digital-metacarpal length (DML). The lengths of the proximal phalangeal bone (PPL) and the metacarpal bone (MCL) of

[Osseous vascular anatomy in the hand and wrist].

The design and the realization of vascularized osseous grafts at the hand and the wrist require a precise knowledge of the general and regional anatomy. This article gives first a progress report on current knowledge about the general organization of arterial and venous vascularization, of the long bones (number and localization of the nutrient foramina, communication between the epiphyseo-metaphyseal and diaphyseal networks) and of the short bones, in the adult and the child, before the closing of the growth plate. The general organization of arterial vascularization of the hand and the wrist

[Radiographic aspects of the hand in diffuse idiopathic skeletal hyperostosis].

Radiographs of the hand of 53 patients with diffuse idiopathic skeletal hyperostosis (DISH) were compared with 20 acromegalic subjects and 100 matched controls. Bone qualitative changes, characteristic of DISH, include: (1) arrow-heading of the distal phalangeal tufts; (2) enthesopathy of the proximal phalanges; (3) enlarged sesamoid bones; (4) hook-like exostoses at the metacarpophalangeal heads; (5) cortical thickness of diaphyseal tubular bones. Therefore, the hand in this disease appears as a "robust" form, particularly in males, resembling acromegalic pattern, but in this disease, the han

Congenital and Traumatic Conditions of the Pediatric Hand: Overview for the Radiologist.

Alignment, longitudinal growth, and function of the musculoskeletal unit of the pediatric hand is complex due to the combination of open growth plates, ossification variations, and their relationship with finger tendinous and ligamentous attachments. This review presents the basics of normal development, acquired and congenital variations, and traumatic conditions of the pediatric hand from the perspective of the pediatric musculoskeletal radiologist and hand surgeon. This multimodality imaging article focuses on non-oncologic and non-rheumatologic conditions of the pediatric hand, inclusive o

Tumours of the hand presenting as pathological fractures.

Tumours represent one of the important differential diagnoses that need to be considered while investigating fractures of the small bones of the hand, as this can sometimes be the sole or the first presenting complaint. We conducted a retrospective study of the Scottish Bone Tumour Registry, analysing the records of patients with hand tumours which primarily presented as pathological fractures. The registry held records of 233 patients with tumours involving the bones of the hand, of which 53 (22.7%) had pathological fractures as the first presenting complaint. The average age at presentation

Mycetoma caused by Nocardia caviae in the first Brazilian patient.

BACKGROUND: Mycetoma is a chronic subcutaneous mycosis caused by exogenous fungi or actinomycetes. This infection has a progressive course and shows a typical clinical characteristic of tumefaction, draining sinuses, and grains. Infection initiation is related to local trauma and can spread to muscle, underlying bone, and adjacent organs. Nocardia brasiliensis is the most frequent actinomycete isolated, while N. caviae is a rare agent. METHODS: We present a case of mycetoma in a 37-year-old African-American man on the right hand. The infection had been apparent for four years prior to the con

[Computerized tomography of the hand with high resolution technique].

The results of 175 high-resolution computed tomography (CT) scans of the hand and wrist are analysed and compared to the results of conventional radiographs. Advantages of the procedure can be demonstrated when assessing pathological changes within the complex three-dimensional structure of the hand and wrist, and in the analysis of articular surfaces. In some cases, fractures and dislocations of the distal radioulnar joint and the carpal bones were missed in conventional radiographs. In cases of scaphoid fracture and nonunion, CT scans give valuable additional information surpassing the possi

Trapezial metastasis as the first indication of primary non-small cell carcinoma of the lung.

Metastasis to the bones of the hand and wrist is not common, and its discovery may reveal an advanced primary tumor located centrally. Clinically, hand metastasis is hard to differentiate from other more common hand pathologies. Its rarity, coupled with a lack of unique clinical manifestations, makes hand and wrist metastasis difficult to diagnose. However, its diagnosis is critical to initiate an appropriate course of treatment. We present a patient in whom lung carcinoma metastasis to the trapezium was definitively diagnosed upon surgical management of symptoms that were consistent with thum
