Targeting the adaptive immune continuum in atherosclerosis and post-MI injury.

Atherosclerotic disease is a cholesterol-rich lipoprotein particle-driven disease resulting in the formation of atherosclerotic plaques in large and medium size arteries. Rupture or erosion of atherosclerotic plaques can trigger the formation of a thrombus causing the obstruction of the blood flow in the coronary artery and thereby leading to myocardial infarction (MI). Inflammation is a crucial pillar of the mechanisms leading to atherosclerosis and governing the cardiac repair post-MI. Dissecting the complex and sophisticated networks of the immune responses underlying the formation of ather

Effect of reperfusion therapy on cardiac rupture after myocardial infarction in Japanese.

BACKGROUND: Cardiac rupture after acute myocardial infarction (AMI) is unpredictable and almost always fatal, so the present study reviewed all the clinical characteristics of patients with cardiac rupture to determine if its occurrence can be predicted. METHODS AND RESULTS: The clinical characteristics of 1,296 consecutive AMI patients from January 1992 to February 2002 were retrospectively evaluated using multi-logistic analysis. Cardiac rupture occurred in 45 patients (3.5%), comprising left ventricular free wall rupture (n=23), ventricular septal perforation (n=20) and papillary muscle ru

Two-patch technique for postinfarction inferoposterior ventricular septal defect.

We describe 4 patients with postinfarction inferoposterior ventricular septal defect treated by the two-patch technique for infarct exclusion operation. The ventricular septal defects were closed using two bovine pericardial patches as follows. The septal patch was sutured to the noninfarcted septum covering the defect, and the free wall patch was sutured to the endocardium adjacent to the posterior papillary muscle. The two patches were sutured together and all infarcted areas were excluded from the left ventricular pressure. This technique seems to be useful in specific circumstances, such a

Real time three-dimensional transesophageal echocardiography in partial posteromedial papillary muscle rupture.

A 60-year-old hypertensive male with non-ST segment elevation myocardial infarction had a hyperdynamic LV with mild posterior leaflet mitral valve prolapse without significant regurgitation on transthoracic echocardiogram. Two-dimensional (2D) TEE showed severe mitral regurgitation with prolapse of P2 and P3 scallops. Posteromedial papillary muscle rupture was suspected. Three-dimensional (3D) TEE demonstrated severely prolapsing P2 and P3 valve scallops with a clear view of the partially ruptured posteromedial papillary muscle. Patient underwent emergency coronary bypass and mitral valve repl

Mechanical complications after myocardial infarction reliably predicted using C-reactive protein levels and lymphocytopenia.

We assessed the accuracy of C-reactive protein (CRP) levels and lymphocyte counts to predict a mechanical complication (MC) after myocardial infarction (MI). Within 10 years, we identified 36 patients with 39 echocardiographically confirmed MC within 30 days of MI: ventricular septal defect (17 cases), papillary muscle rupture (10 cases), and left ventricular free wall rupture (12 cases). They were compared to 41 controls with an uncomplicated hospital course after MI. Peak CRP levels and minimum relative lymphocyte counts obtained within 96 h of the acute MI (AMI) and before diagnosis of the

Extracorporeal membrane oxygenation system as a bridge to reparative surgery in ventricular septal defect complicating acute inferoposterior myocardial infarction.

Post-infarction ventricular septal defect (VSD) is a rare but potentially lethal complication of acute myocardial infarction. Medical management is usually futile, so definitive surgery remains the treatment of choice but the risk surgery is very high and the optimal timing for surgery is still under debate. A 55-year-old man with no previous medical history attended the emergency-room for 12 h evolution of oppressive chest pain and strong anginal pain 7 days ago. On physical examination, blood pressure was 96/70 mmHg, pansystolic murmur over left sternal border without pulmonary crackles. An

Toll-like receptor 7 deficiency promotes survival and reduces adverse left ventricular remodelling after myocardial infarction.

AIMS: The Toll-like receptor 7 (TLR7) is an intracellular innate immune receptor activated by nucleic acids shed from dying cells leading to activation of the innate immune system. Since innate immune system activation is involved in the response to myocardial infarction (MI), this study aims to identify if TLR7 is involved in post-MI ischaemic injury and adverse remodelling after MI. METHODS AND RESULTS: TLR7 involvement in MI was investigated in human tissue from patients with ischaemic heart failure, as well as in a mouse model of permanent left anterior descending artery occlusion in C57B

Left atrial infarction: a case report and review of the literature.

The majority of cardiac related deaths are due to ischemic heart disease, with the most common clinical scenario being severe coronary artery atherosclerosis resulting in left ventricular myocardial infarction. However, infarction of other cardiac chambers does occur, and often has specific clinical associations. We report a case of a 70-year-old man who suffered from left atrial infarction that resulted in a transmural rupture of his left atrium. The patient had a history of rheumatic heart disease, mitral valve stenosis, and severe atherosclerotic coronary artery disease. Four days before de

Massive mitral regurgitation secondary to acute ischemic papillary muscle rupture: the role of echocardiography.

Papillary muscle rupture is an infrequent but often fatal mechanical complication of acute myocardial infarction (AMI). We report the case of an AMI complicated by the development of an abrupt cardiogenic shock due to the rupture of the head of the postero-medial papillary muscle with echocardiographic demonstration of severe mitral regurgitation due to flail posterior mitral valve leaflet. After initial stabilization with medical therapy and diagnostic coronary angiography, the patient was referred for urgent cardiac surgery and successfully underwent mitral valve replacement with implantatio

Dynamic left ventricular outflow tract obstruction after anterior myocardial infarction. A potential mechanism of myocardial rupture.

This study describes a dynamic left ventricular outflow tract gradient complicating an acute anterior myocardial infarction in two patients. In both cases, hyperdynamic contraction of the non-infarcted segments were noted in addition to large antero-apical a- or dyskinesia. Both patients had evidence of mitral annular calcifications, and the occurrence of dynamic outflow tract obstruction preceded sudden death. Early recognition of the underlying pathophysiological mechanism might have major therapeutic implications and could decrease the risk of myocardial rupture.

[Successful surgical management of papillary muscle rupture following acute myocardial infarction in an elderly patient].

Papillary muscle rupture is rare but catastrophic complication of acute myocardial infarction. We report a 91-year-old woman who underwent successful management of papillary muscle rupture following acute myocardial infarction. She was transferred to our hospital because of severe pulmonary edema and cardiogenic shock. Echocardiography revealed severe mitral valve regurgitation due to total rupture of anterolateral papillary muscle. After intubation, intraaortic balloon pumping (IABP) and percutaneous cardiopulmonary support (PCPS) insertion, mitral valve replacement was successfully performed

Free wall rupture following rescue angioplasty and intravenous abciximab for failed thrombolysis.

Early thrombolytic therapy reduces the risk of cardiac rupture but delayed thrombolysis may increase this risk, despite improving overall survival. The mechanism appears to be related to both unsuccessful early reperfusion and haemorrhagic transformation following delayed reperfusion. The effect of antiplatelet therapy with glycoprotein IIb-IIIa receptor blockers (abciximab) on cardiac rupture is unknown. It is possible that they may contribute to cardiac rupture by promoting haemorrhagic transformation of the infarcted area. In this report we describe a 57 year old man who underwent emergency

Transient constrictive pericarditis following cardiac surgery.

Transient constrictive pericarditis (CP) is quite rare, with only a few reports presented. Herein, we report 2 cases of transient CP following cardiac surgery. Case 1 was an 80-year-old man who underwent repair of a left ventricular free wall rupture, while Case 2 was a 69-year-old woman who underwent a conventional aortic valve implantation procedure. In both cases, constrictive features developed about 2 weeks after surgery, which were treated successfully by administrations of non-steroidal anti-inflammatory drugs (NSAIDs). Prior to performing a pericardiectomy, NSAID administration should

Late-Onset Left Ventricular Pseudoaneurysm After Dor Operation in an Elderly Male Patient.

Left ventricular (LV) pseudoaneurysm is a rare complication that occurs shortly after myocardial infarction or previous cardiac surgery. We present here a case of an elderly patient with an LV pseudoaneurysm that developed over a long period of time after Dor operation. An 83-year-old man with rapid LV enlargement 18 years after Dor operation presented with dyspnea. We diagnosed his mass as an LV pseudoaneurysm using multiple non-invasive imaging modalities. The suture between the patch and the LV myocardium became fragile and detached over time. However, due to adhesion in the thorax and slow

Right Ventricular Perforation and Subsequent Cardiac Tamponade Caused by IVC Filter Strut Fracture Migration.

Cardiac tamponade, if not recognized and treated immediately, is a life threatening condition with various etiologies. Most common causes of cardiac tamponade encountered in emergency rooms are due to trauma, post myocardial infarction wall rupture, cancer and all other causes of pericardial effusion. Iatrogenic causes of cardiac tamponade include anticoagulation and procedures related. Currently there is a general comfort level amongst physicians that inferior vena cava (IVC) filters are not associated with significant complications. However, one of the feared life-threatening immediate compl

Early repair of postinfarction ventricular septal defect with gelatin-resorcin-formol biological glue.

BACKGROUND: Early surgical repair of postinfarction ventricular septal defect has improved early mortality rate. Mortality remains high in patients presenting within 1 week of infarction, or when rupture has occurred in the inferior part of the septum. METHODS: We describe a surgical technique for repair of postinfarction ventricular septal defect that involves no infarctectomy: continuous suturing of a bovine pericardial patch to healthy myocardium around the infarcted area and use of gelatin-resorcin-formol biological glue as a sealant between the patch and the interventricular septum. Ventricular Septal Rupture Despite Early Pharmaco-Invasive Reperfusion in Anterior STEMI With Favorable Surgical Outcome.

Ventricular septal rupture (VSR) is a rare but catastrophic mechanical complication of acute myocardial infarction, even in the contemporary reperfusion era. Although early reperfusion strategies have significantly reduced its incidence, they do not completely eliminate the risk. We report the case of a 76-year-old man with a history of hypertension and no prior structural heart disease who presented with an anterior ST-elevation myocardial infarction (STEMI). He received tenecteplase 2 hours and 37 minutes after symptom onset and was subsequently referred for routine percutaneous coronary int
