Risk-Based Newborn Drug Testing in a Setting With a Low Prevalence of Maternal Drug Use.

OBJECTIVES: Our objective in this study was to determine the predictive value of an institutional risk-based newborn drug-testing tool for detecting maternal drug use during pregnancy. METHODS: For 5.5 months, the umbilical cords of all newborns born at the study institution were collected and analyzed at a national reference laboratory. In the context of usual clinical care, the decision to perform newborn drug testing is based on an institutional risk assessment tool. For the cohort without clinical indication for testing, cords were deidentified during the study period. Chart review was no

Trends in Medical Use of Opioids in the U.S., 2006-2016.

INTRODUCTION: The U.S. is experiencing an opioid epidemic which is at least partially iatrogenic and fueled by both prescription and illicit misuse. This study provides a nationwide examination of opioid distribution patterns during the last decade. METHODS: Data were obtained from the U.S. Drug Enforcement Administration's Automation of Reports and Consolidated Orders System for 2006-2016. Analyses include quantities of ten opioids legally dispensed nationwide by weight and converted to Morphine Milligram Equivalents. Geospatial and state-level analyses were also completed in 2017. RESULTS:

Opioid drug poisonings in Ohio adolescents and young adults, 2002-2014.

CONTEXT: Opioids represent a drug class that adolescents and young adults intentionally misuse and abuse. When taken on their own or with other substances in this manner, opioids pose an increased risk of overdose and potential death. OBJECTIVE: To determine trends of opioid drug poisonings among adolescents and young adults in Ohio from 2002 to 2014 using Poison Control Center (PCC) data. METHODS: Data were obtained from Ohio PCCs from 2002 to 2014 for opioid drug poisonings amongst 10-29 year olds. Trends were evaluated with Poisson regression. Ohio counties with higher opioid drug poisoni

Opportunities to Prevent Overdose Deaths Involving Prescription and Illicit Opioids, 11 States, July 2016-June 2017.

In 2016, 63,632 drug overdose deaths occurred in the United States, 42,249 (66.4%) of which involved opioids (1). The development of prevention programs are hampered by a lack of timely data on specific substances contributing to and circumstances associated with fatal overdoses. This report describes opioid overdose deaths (referred to as opioid deaths) for decedents testing positive for prescription opioids (e.g., oxycodone and hydrocodone), illicit opioids (e.g., heroin, illicitly manufactured fentanyl, and fentanyl analogs), or both prescription and illicit opioids, and describes circumsta

Correlates of Long-Term Opioid Abstinence After Randomization to Methadone Versus Buprenorphine/Naloxone in a Multi-Site Trial.

Opioid use disorder (OUD) is a chronic, relapsing condition with severe negative health consequences. Previous studies have reported that 5-year opioid abstinence is a good predictor of reduced likelihoods of relapse, but factors that shape long-term opioid abstinence are poorly understood. The present study is based on data from a prospective study of 699 adults with OUD who had been randomized to either methadone or buprenorphine/naloxone and who were followed for at least 5 years. During the 5 years prior to the participants' last follow-up interview, 232 (33.2%) had achieved 5-year abstine

Effects of μ-opioid receptor agonists in assays of acute pain-stimulated and pain-depressed behavior in male rats: role of μ-agonist efficacy and noxious stimulus intensity.

Pain is associated with stimulation of some behaviors and depression of others, and μ-opioid receptor agonists are among the most widely used analgesics. This study used parallel assays of pain-stimulated and pain-depressed behavior in male Sprague-Dawley rats to compare antinociception profiles for six μ-agonists that varied in efficacy at μ-opioid receptors (from highest to lowest: methadone, fentanyl, morphine, hydrocodone, buprenorphine, and nalbuphine). Intraperitoneal injection of diluted lactic acid served as an acute noxious stimulus to either stimulate stretching or depress operant re

Intranasal drug-induced fungal rhinopharyngitis.

BACKGROUND: Intranasal drug abuse has long been recognized as an etiology of sinonasal pathology. However, the intranasal use of prescription and nonprescription drugs has surpassed the use of illicit drugs, and the pattern of presentation and required therapeutic intervention appears to be different. We report on our experience with these patients, along with a successful treatment algorithm for this disease process. METHODS: Retrospective chart review of 9 consecutive patients who presented with rhinopharyngitis and a history of intranasal opioid and/or acetaminophen abuse, from 2007 to 201

Self-intoxication with morphine obtained from an infusion pump.

A 36-year-old Caucasian male was found unresponsive by his wife. He had white foam around his mouth and was pronounced dead shortly thereafter. He had a history of back pain and was treated with intrathecal morphine because of his previous addiction to oral opiate medications. Because of crimping of the pump catheter, it was replaced 4 days before his death. Toxicological findings included urine screen positive for amitriptyline, nortriptyline, opiates, hydrocodone metabolites, ibuprofen, acetaminophen, caffeine, nicotine, and metabolite. Drug concentrations were as follows: blood, 0.260 mg/L

Opioids and Disability Among Women of Reproductive Age.

Background: Despite nearly one in five U.S. women of reproductive age reporting a disability, limited research exists on opioid behaviors in this vulnerable population. This study examined associations between disability and past-year prescription opioid use and misuse, and described types of opioids, sources, and motives for opioid misuse among nonpregnant women of reproductive age. In addition, the effects of social, medical, and behavioral determinants of health on opioid use and misuse were assessed in this population of women with disabilities. Lumbar plexus blockade reduces pain after hip arthroscopy: a prospective randomized controlled trial.

BACKGROUND: Hip arthroscopy causes moderate to severe postoperative pain. We hypothesized that performance of a lumbar plexus block (LPB) would reduce postoperative pain in the postanesthesia care unit (PACU) for patients discharged home on the day of surgery. METHODS: Patients received a combined spinal epidural with IV sedation, ondansetron, and ketorolac. Half of the patients (n = 42) also underwent a single-injection bupivacaine LPB. Postoperative analgesia (PACU and after discharge) was provided with oral hydrocodone/acetaminophen (5/500 mg) and an oral nonsteroidal antiinflammatory drug

Renal infarction during the use of rizatriptan and zolmitriptan: two case reports.

Rizatriptan and zolmitriptan are both used to relieve acute migraine and cluster headaches. The mechanism of action is similar to the other triptans, in that they reverse abnormal cerebral vasodilation through their activity as 5-HT1B receptor agonists. Triptan-induced vasoconstriction is attributed to its activity on peripheral 5-HT1B receptors and has rarely been reported to result in stroke, myocardial infarction and ischemic colitis. We present two cases of renal infarction associated with therapeutic triptan use. The first patient is a 57-year-old man with a history of hypertension that w

NaV1.8 Inhibition and the Future of Nonopioid Analgesia in Otolaryngology: A Narrative Review.

IMPORTANCE: Postoperative pain management in otolaryngology requires balancing effective analgesia while minimizing adverse effects, such as opioid-related complications. Suzetrigine is a first-in-class selective voltage-gated sodium channel (NaV1.8) inhibitor that has been recently approved by the US Food and Drug Administration. Suzetrigine selectively inhibits NaV1.8 channels expressed in peripheral nociceptive neurons, thereby reducing pain signaling without the respiratory depression, sedation, or misuse potential associated with opioids. This review summarizes the pharmacology, efficacy,

Exploring risk signals of association between drugs and suicide: a retrospective investigation from 2004 to 2024.

BACKGROUND: Suicide is a serious public health issue associated with the interaction of biological, psychological and social factors. Although relevant studies are relatively mature, research on the association between drugs and suicide remains limited and lacks systematic analysis. OBJECTIVE: This study aimed to explore the potential association signals between drugs and suicide-related adverse events (SAEs) using the U.S. Food and Drug Administration Adverse Event Reporting System (FAERS) database. METHODS: This study collected data from the FAERS database spanning the first quarter of 200

Characteristics of United States Food and Drug Administration Drug Recalls Involving Opioid Medications, 2002-2025.

PURPOSE: To characterize the frequency, causes, and severity of opioid medication recalls using United States Food and Drug Administration (FDA) Enforcement Reports. METHODS: We conducted a 2002-2025 descriptive analysis of recalls involving seven opioids. FDA free-text recall reasons were categorized into five standardized domains (wrong dose/potency, contamination, mispackaging/mislabeling, defective delivery system, and quality assurance deviations) by two independent reviewers, with agreement assessed using Cohen's κ (95% CI). Recall severity (Class I-III) was compared across drugs using

U.S. Poison Center exposure cases involving marijuana and selected comparator substances.

BACKGROUND: With increasing number of U.S. states legalizing marijuana use, nearly half now allow adult use. Consequently, harms from marijuana exposures are increasing. We describe recent levels, characteristics, and trajectory of harms associated with intentional and unintentional marijuana use compared to other drugs. METHODS: We examined exposure characteristics, medical outcomes, and utilization-adjusted rates (per million people with past-year nonmedical use from National Survey on Drug Use and Health) for U.S. Poison Center cases (2013-2021) involving marijuana recreational use (RU) an

Opioid biomarkers in urine as reliable and valid correlates of opium use characteristics: A 10-year longitudinal assessment.

BACKGROUND: Biomarkers can clarify the mechanistic bases of health effects associated with opiate use and improve evaluating dose-response relationships by quantifying the absorbed dose through different routes and patterns of use, supporting the generalizability of opium research findings to broader opioid use. METHODS: We recruited 449 individuals who used opium and 66 individuals who did not, 10 years after baseline evaluation in a cohort study. At both time points, we collected self-reported characteristics of opium use (route, frequency, type, and dose) and measured urinary concentration

A Novel UHPLC-MS/MS Method for the Quantification of Seven Opioids in Different Human Tissues.

BACKGROUND: Opioids are considered the cornerstone of pain management: they show good efficacy as a first-line therapy for moderate to severe cancer pain. Since pharmacokinetic/pharmacodynamic information about the tissue-specific effect and toxicity of opioids is still scarce, their quantification in post-mortem autoptic specimens could give interesting insights. METHODS: We describe an ultra-high-performance liquid chromatography coupled with tandem mass spectrometry method for the simultaneous quantification of methadone, morphine, oxycodone, hydrocodone, oxymorphone, hydromorphone and fen
