Emotional distress in chronic medical illness: treatment with time-limited group psychotherapy.

Time-limited (12 sessions) group psychotherapy was provided for 14 patients with a variety of serious medical illnesses. All patients were referred for psychiatric assistance and most had minimal or no psychiatric disorder. Patients not terminally ill, in continuous pain, or cognitively impaired, but otherwise unselected, were offered treatment. Emphasis was placed on validation of feelings, interpersonal understanding, and problem solving. Increased sense of well being was noted in all patients by self-report; this was contrasted by findings on the Millon Behavioral Health Inventory indicatin

Effectiveness, relapse prevention and mechanisms of change of cognitive therapy vs. interpersonal therapy for depression: Study protocol for a randomised controlled trial.

BACKGROUND: Major depression is a common mental disorder that substantially impairs quality of life and has high societal costs. Although psychotherapies have proven to be effective antidepressant treatments, initial response rates are insufficient and the risk of relapse and recurrence is high. Improvement of treatments is badly needed. Studying the mechanisms of change in treatment might be a good investment for improving everyday mental health care. However, the mechanisms underlying therapeutic change remain largely unknown. The objective of the current study is to assess both the effectiv

Understanding "patient deterioration" in psychotherapy from depressed patients' perspectives: A mixed methods multiple case study.

OBJECTIVE: This study scrutinizes the meaning of deterioration in psychotherapy beyond the widely used statistical definition of reliable symptom increase pre-to-post treatment. METHOD: An explanatory sequential mixed-methods multiple case study was conducted, combining quantitative pre-post outcome evaluation of self-reported depression symptoms and qualitative analysis of patients' interviews. In a Randomized Controlled Study on the treatment of Major Depression, three patients showing reliable increase in symptom severity on the BDI-II pre-to-post therapy were selected. An interpretative p

Disinhibition domain and facets uniquely predict changes in depressive symptoms and psychosocial functioning.

This study aimed to understand the role of disinhibition (low conscientiousness)-in conjunction with the other major personality traits of negative affectivity, detachment, antagonism, and psychoticism-in predicting changes in depressive symptoms and psychosocial functioning. Both the disinhibition trait domain and its primary facets (i.e., irresponsibility, impulsivity, and distractibility) were examined. In a large sample (Time 1 N = 605, Time 2 N = 497) of psychiatric outpatients and high-risk community residents, personality traits, depressive symptoms (both self-reported and interviewer-r

Exploring illness and recovery in eating disorders: a qualitative study of patients' group therapies experiences.

BACKGROUND: Recovery in eating disorders (EDs) is often defined in terms of symptom remission, potentially overlooking the broader meanings individuals attribute to illness and recovery. Qualitative research has highlighted the multidimensional nature of recovery, yet less is known about how therapeutic contexts may shape how individuals make sense of illness, change, and recovery. This study aimed to explore how individuals with EDs conceptualize illness and recovery, and how these meanings may be shaped through participation in group psychotherapy. METHODS: A qualitative study was conducted

The Swara-Informed model of autonomic synchrony in psychotherapy: theoretical foundations and hypotheses.

BACKGROUND: Autonomic dysregulation is increasingly recognized as a central mechanism complicating psychotherapeutic work across anxiety, trauma-related, and mood disorders. Persistent hyperarousal (sympathetic activation) or hypoarousal (persistent parasympathetic activation or shutdown/ dissociation) may put clients out of their "window of tolerance," thus limiting access to cognitive, emotional, and interpersonal processes that are essential to the effectiveness of psychotherapy. Swara yoga, a traditional Indian system, assigns functional significance to nasal dominance and unilateral nostr

A neurocognitive account of complex PTSD: self-modelling, affective dysregulation, and implications for MDMA-assisted and targeted psychotherapies.

Background: Complex post-traumatic stress disorder (C-PTSD) is a trauma-related condition characterized by pervasive disturbances in affect regulation, self-concept, and interpersonal functioning that extend beyond the symptom profile of post-traumatic stress disorder (PTSD). While neurobiological studies have implicated limbic, salience, and prefrontal systems in C-PTSD, mechanistic accounts linking these findings to disturbances in self-organization and treatment-relevant processes remain underdeveloped. Objective: This narrative review develops a hypothesis-generating neurocognitive account

Posttraumatic stress disorder from early childhood to late adolescence: a resilience-oriented approach to prevention, diagnosis, and clinical management.

Posttraumatic stress disorder (PTSD) in children and adolescents remains underrecognized despite its significant developmental, relational, and public health impact. This narrative review synthesizes evidence on the prevention, diagnosis, and management of PTSD from early childhood through late adolescence, emphasizing a resilience-oriented and multisystemic framework. Developmentally informed assessment is essential to capture age-specific symptom patterns and to distinguish PTSD from complex PTSD, increasingly observed after chronic interpersonal trauma. Trauma-focused psychotherapies-partic

The impact of childhood trauma on psychological interventions for depression during pregnancy and postpartum: a systematic review.

Women who have experienced childhood trauma (CT) are at increased risk for depression during pregnancy and postpartum, pregnancy complications, and adverse child outcomes. There are effective psychotherapeutic interventions to treat depression during pregnancy and postpartum, yet there is a paucity of literature on the impact of CT on treatment outcomes. This review aims to determine whether and how maternal CT history affects the outcomes of psychological interventions for depression during pregnancy and postpartum. PubMed, PsycINFO, and Cochrane Library searches were conducted to identify pa

Therapist effects on outcome and alliance in inpatient psychotherapy: The contribution of patients' symptom severity.

Therapists differ in their effectiveness regarding their patients' symptomatic improvement as well as their therapeutic alliances, but little is known about influencing factors. The current study aimed to investigate therapist effects on outcome and alliance in inpatient psychotherapy and to estimate the influence of patient severity on the magnitude of therapist effects in this specific setting. We examined differences between individual psychotherapists in their patients' early alliance and outcome at discharge in a psychotherapy setting of a psychiatric hospital. The sample included 33 ther

Binge eating disorder and obesity.

Binge eating disorder (BED) was included in the DSM IV as a proposed diagnostic category for further study and as an example for an eating disorder not otherwise specified (EDNOS). BED is characterized by recurrent episodes of binge eating in the absence of regular compensatory behavior such as vomiting or laxative abuse. Related features include eating until uncomfortably full, eating when not physically hungry, eating alone and feelings of depression or guilt. BED is associated with increased psychopathology including depression and personality disorders. Although BED is not limited to obese

Metacognitive Interpersonal Therapy in Group for Avoidant Personality Disorder-A Comparison With Best Available Practice.

INTRODUCTION: Avoidant personality disorder (AvPD) is a prevalent condition characterized by low self-esteem and social withdrawal. Nonetheless, targeted psychotherapy remains limited. This study aimed to examine short-term metacognitive interpersonal therapy in group (MIT-G) tailored for AvPD and to compare the outcomes of patients receiving this treatment with patients participating in outpatient group therapies within specialized mental health services for personality disorders (representing best available practice). METHODS: The study had a quasi-experimental design and included 56 patien

Exploring the context, mechanisms, and outcomes of school-based group interpersonal therapy for depressed adolescents in Nepal: Analysis of qualitative process data from a pilot realist cluster-randomized controlled trial.

OBJECTIVE: Evidence-based psychotherapies constitute first-line treatments for youth depression, yet their effects remain modest and largely unchanged over the past three decades. A critical gap persists in understanding how and for whom these therapies work. Realist trials address this gap by iteratively developing and testing context-mechanism-outcome configurations (CMOC) within randomized controlled trials (RCTs). As an initial step in that process, this study reports on qualitative research embedded in a pilot realist cluster RCT of group interpersonal therapy (IPT) for depressed adolesce

Patients With Personality Disorders in Everyday Clinical Practice–Implications of the ICD-11.

BACKGROUND: Patients with difficult personalities or personality disorders are a special challenge for primary care physicians. Their style of interpersonal interaction is often difficult. As the ICD-11 classification comes into use, a new systematic approach to diagnosis is being introduced that focuses on the patient's functional impairments in everyday life. We describe the implications for the diagnosis and treatment of patients of this type. METHODS: This review is based on pertinent publications retrieved by a selective search, with particular attention to primary care and to somatic mo

Early interpersonal relations: experience of four treatment methods in phobic women.

One hundred and three phobic women not suitable for insight-oriented psychotherapy were randomized to either therapist-directed prolonged exposure in vivo (PE), supportive therapy of dynamic type (ST), relaxation therapy (R), or basal therapy (B). History taking was lengthy and included ratings of early interpersonal relations. These were combined into a scale with a range from 0, least favourable, to 13 most favourable. The patients filled in an inventory with regard to their experience of the therapist and the therapy at the beginning of the treatment, at midtreatment, and at the end of trea
