BACKGROUND: For some patients, standard optical correction for presbyopia is not satisfactory. Using a specially designed mask, we developed a procedure for correcting presbyopia with excimer laser photorefractive keratectomy (PRK).
METHODS: A mask consisting of a mobile diaphragm formed by two blunt blades was used to ablate a 10 to 17 microm deep semilunar-shaped zone immediately below the pupillary center, steepening the corneal curvature in that area. Three eyes of three presbyopic patients were treated, aiming at a near addition of +3.00 D. Follow-up time was 24 months.
RESULTS: After a

This paper represents an update on a study that has been reported elsewhere (Nagy ZZ et al: Ophthalmology 104:375-380, 1997). The aim of the study was to evaluate the clinical and light- and electron-microscopic effects of ultraviolet-B (UV-B) exposure on the outcome of photorefractive keratectomy (PRK). A total of 42 pigmented rabbits were used in the study. One eye from each of 12 rabbits received a 193 nm 45-microm deep (-5.0 diopters [D]) excimer laser PRK, one eye from each of 12 rabbits received a 135-microm deep (-15.0 D) excimer laser PRK, and one eye from each of 12 rabbits received a

In corneal tissue from a diabetic patient 15 months after delayed epithelial healing following vitrectomy surgery, electron microscopy revealed a discontinuous basement membrane; immunohistochemically, fibronectin was present in the basement membrane zone. To determine whether alterations in repair of the basement membrane, anchoring fibrils and hemidesmosomes occur in wounds in diabetics, we studied the pattern of histochemical localization of bullous pemphigoid antigen (the intracellular hemidesmosome plaque component), laminin and type VII collagen (the anchoring fibril collagen) in alloxan

Corneal dystrophies are a group of non-inflammatory inherited disorders of the cornea. This review considers treatment options for epithelial-stromal and stromal corneal dystrophies: namely Reis-Bücklers, Thiel-Behnke, lattice, Avellino, granular, macular and Schnyder corneal dystrophies. Where there is visual reduction, treatment options may include either phototherapeutic keratectomy (PTK) or corneal transplantation. Due to the anterior location of the deposits in Reis-Bücklers and Thiel-Behnke dystrophies, PTK is considered the treatment of choice. For lattice, Avellino, granular and macula

Customized photorefractive keratectomy with minimal tissue ablation combined with corneal cross-linking seems to be a long-term safe and effective strategy for anatomical and visual management of keratoconus, postsurgical ectasia, and other ectasia management. Multiple published studies, many with long-term follow-up, have supported the Athens Protocol and its various forms as a means to manage corneal ectatic disorders, which not only stabilize corneal shapes but also improve functional vision.

TOPIC: The use of LASIK surgery to correct vision in a patient with postzoster corneal scarring.
CLINICAL RELEVANCE: The cornea is commonly involved in cases of herpes zoster ophthalmicus, and as a result, corneal scarring after a varicella-zoster virus corneal infection is common. Corneal scars can be treated by lamellar keratoplasty or keratectomy, which may be performed using a microkeratome or excimer laser phototherapeutic keratectomy (PTK). However, articles on studies concerning the treatment of postherpetic scars by PTK have been published, offering conflicting results. LASIK surgery

Nontuberculous Mycobacterium keratitis is characterized by its indolent course and poor response to antibiotic drugs. Between November 1989 and September 1993, 10 eyes diagnosed to have nontuberculous Mycobacterium keratitis underwent therapeutic lamellar keratectomy in conjunction with fortified topical medications. Nine operated eyes (90%) recovered with a shortened course and healed with an inactive scar. Six patients (60%) achieved a visual acuity of 20/40 or better postoperatively. For recalcitrant nontuberculous Mycobacterium keratitis, therapeutic lamellar keratectomy is highly recommen
