Assessment of clinical outcome of percutaneous needle quadriceps tenotomy in the treatment of congenital knee dislocation.

PURPOSE: Treatment of congenital knee dislocation (CDK) depends on the severity and flexibility of the deformity. Various modalities of treatment ranging from serial cast, open quadricepsplasty and minimally invasive quadricepsplasty have been described. We describe percutaneous needle quadriceps tenotomy for treatment of flexible CDK and present our result of retrospective case series. METHODS: This was a retrospective study of 12 patients (20 knees) with flexible CDK. Eight patients with bilateral and four with unilateral deformities. The mean age of intervention was 14.5 days (range, 4-55 …

Long-term followup of surgically treated knee dislocations: stability restored, but arthritis is common.

BACKGROUND: Surgical treatment of knee dislocations is intended to correct the anatomic injury and restore knee stability and patient function. Several studies have shown successful results with surgical treatment of knee dislocations with up to 10 years of followup, but longer-term studies are uncommon. QUESTIONS/PURPOSES: We evaluated patients treated surgically for knee dislocations at 10-year followup to assess (1) knee stability; (2) return to preinjury level of function; (3) development of arthrosis; and (4) range of motion (ROM) loss. METHODS: This study was a retrospective review of …

Femoral shortening in the congenital dislocation of the knee joint: results of mid-term follow-up.

Femoral shortening has an advantage of gaining reduction in severe grades of congenital dislocation of the knee joint without release or lengthening of the quadriceps tendon, thereby reducing the incidence of extensor lag. We report our mid-term functional results in six patients (10 knees) with grade III congenital dislocation of the knee joint who were treated with femoral shortening. At an average follow-up of 4.1 years, the mean active range of motion was -0.5 to 107° and none of the patients had extensor lag. The results in arthrogryposis multiplex congenita were noteworthy for the absenc

Acute Concomitant Anterior Cruciate Ligament and Patellar Tendon Tears in a Non-dislocated Knee.

Anterior cruciate ligament (ACL) tears are common and may occur in isolation or with other internal derangements of the joint. Tears of the patellar tendon (PT) occur less frequently and are rarely associated with intra-articular pathology. Acute combined tears of both the ACL and PT are known complications of high-energy traumatic knee dislocations. We present a case of an acute concomitant ACL and PT tears in a low-energy non-dislocated knee. To our knowledge, this injury has only been described in a limited number of case reports in the orthopedic literature. We present the imaging findings…

Bilateral congenital dislocation of the knee with ipsilateral developmental dysplasia of the hip--report of three patients.

Congenital dislocation of the knee (CDK) is a very rare condition. Here we report our strategy and results in treatment of three children with CDK. All three patients were treated with conservative method, and only one had underwent a surgical procedure on one knee. Of the remaining, we recorded a good outcome with conservative treatment in three knees, while two had poorer outcome as a result of musculoskeletal anomalies. We also present here a unique case of a child born without cruciate ligaments and patellas on both sides. We performed the operative procedure by Z-plasty of the extensor ap

A new hinged dynamic distractor, for immediate mobilization after knee dislocations: Technical note.

Knee dislocation is always a difficult topic mainly with regard to the structures that have been damaged and the best treatment option. The purpose of this study is to describe a new dynamic external fixator (EF) to be applied after knee dislocation, in combination with posterior cruciate ligament (PCL) reconstruction. This approach permits an immediate postoperative mobilization of the joint, with the possibility to perform a delayed ACL reconstruction and eventually reducing the complication rate related to multiple ligament surgery.

Knee dislocations-a retrospective study comparing operative versus closed immobilization treatment outcomes.

Controversies over operative versus closed immobilization of traumatic complex multiple ligamentous knee injury are still debated. The aim of our study is to evaluate the outcome of reconstructive vs non-reconstructive (closed immobilization) treatment outcomes. This is a retrospective review of cases seen at our institution. All cases admitted with a diagnosis of knee dislocation, defined as patients with multidirectional knee instability in the setting of trauma, were reviewed. Twenty-nine consecutive patients from January of 1996 to June of 2002 were reviewed. Twenty-six patients (89.7%) we…

Two cases of irreducible knee dislocation occurring simultaneously in two patients and a review of the literature.

Knee dislocations normally respond to closed reduction; however, a small percentage must be reduced by open operations. A 24-year-old man exhibited typical medial joint-line puckering during repeated reduction attempts and a mild lateral displacement on roentgenograms. A 31-year-old woman had a complete dislocation of her knee, with the femur "buttonholed" through medial soft tissue and entrapped by the joint capsule. Longitudinal incision of the medial joint capsule was the method of reduction. The lateral collateral ligament and both lateral and medial menisci were intact in both cases. Howe…

Popliteal arterial injuries associated with fractures or dislocations about the knee as a result of blunt trauma.

A total of 73 patients with popliteal arterial injuries due to blunt trauma were treated at the Los Angeles County/University of Southern California Medical Center between January 1975 and January 1986. The injuries resulted from high-energy trauma (70% were motor-vehicle-related), and were associated with a variety of fractures and dislocations about the knee. Forty-eight of the patients (66%) had open injuries, 27% of which became infected; the infection rate for all injuries was 22%. The average time from injury to anastomosis was 14.6 hours. The overall amputation rate in the series was 15

Congenital dislocation of the knees--report on two cases treated without surgery and a review of the literature.

The author reports on two cases of congenital dislocation of the knees submitted to orthopaedic treatment. The early treatment and the relatively benign nature of the main deformity and of the associated phenomena appear to have contributed to the excellent results obtained after a six year follow-up. A review of the literature on the subject is given.

Can magnetic resonance imaging predict posterior drawer laxity at the time of surgery in patients with knee dislocation or multiple-ligament knee injury?

BACKGROUND: Previous studies indicate that isolated posterior cruciate ligament injuries demonstrate magnetic resonance imaging (MRI) and clinical evidence of healing when treated nonoperatively; however, the authors are unaware of any other study that has looked at whether initial MRI can predict posterior cruciate ligament stability at the time of surgery in patients with knee dislocation. HYPOTHESIS: An MRI grading system will predict laxity on posterior drawer testing at the time of surgery in patients with knee dislocations. STUDY DESIGN: Cohort study (prognosis); Level of evidence, 2. …

Clinical and Functional Outcomes of Documented Knee Dislocation Versus Multiligamentous Knee Injury: A Comparison of KD3 Injuries at Mean 6.5 Years Follow-up.

BACKGROUND: Previous research has found that the incidence of neurovascular injury is greatest among multiligamentous knee injuries (MLKIs) with documented knee dislocation (KD). However, it is unknown whether there is a comparative difference in functional recovery based on evidence of a true dislocation. PURPOSE: To determine whether the knee dislocation-3 (KD3) injury pattern of MLKI with documented tibiofemoral dislocation represents a more severe injury than KD3 MLKI without documented dislocation, as manifested by poorer clinical outcomes at long-term follow-up. STUDY DESIGN: Cohort st…

Challenges in a neglected case of traumatic anterior dislocation after total knee replacement: A case report.

Anterior tibio-femoral dislocation following total knee replacement (TKR) is a dreaded complication. We present a case of a 43-year-old lady who presented to us 3 months after a domestic ground level fall with pain, deformity and, inability to bear weight on her right knee. She had undergone a successful TKR 3 years ago with no complaints up until the fall. On examination, the limb was well perfused with no neurologic damage. A vascular surgery opinion was taken and arterial doppler showed good flow at and below the level of the knee. Radiologic assessment revealed a complete anterior dislocat

Recurring intraprosthetic dislocation of rotating-hinge total knee prosthesis. Effect of implant design on intrinsic stability.

During total knee replacement, hinged total knee implants are used in cases where ligament balancing cannot be achieved with less-constrained implants. The case of a patient who experienced two episodes of intraprosthetic dislocation of his rotating-hinge total knee prosthesis is described. There are very few reports of this type of dislocation with these implants. The implant's design, particularly of the hinge, plays an important role in stability. The balance between the flexion and extension spaces is very important even when using a hinged total knee implant. The role of the extensor mech

Posterior dislocation of meniscal bearing insert in mobile bearing unicompartmental knee arthroplasty mimicking a Baker cyst.

Unicompartmental knee arthroplasty (UKA) is a well-established procedure for isolated medial compartment osteoarthritis. Meniscal dislocation is a well-recognized complication in mobile bearing UKA especially when performed for the lateral compartment. Dislocation in medial UKA is relatively uncommon and is usually anterior when encountered. We would like to report a case of posterior dislocation of the bearing insert in a mobile bearing medial UKA presenting as a Baker cyst, which was subsequently revised to total knee arthroplasty.

Concomitant ipsilateral traumatic dislocation of the hip and knee following high-energy trauma: a case report.

Traumatic dislocation of the hip or knee can occur after high-energy trauma and is often associated with concomitant injuries and secondary complications. Concomitant traumatic dislocation of both hip and knee is rare. We describe a case of combined ipsilateral posterior hip dislocation with a posterior acetabular fracture and a complete open knee dislocation with disruption of the popliteal artery that resulted in amputation.

Neglected congenital bilateral knee dislocation treated by quadricepsplasty with semitendinosus and sartorius transfer: A case report.

BACKGROUND: Congenital knee dislocation (CKD) is a rare condition, which accounts for 1% of congenital hip dislocations. It can present as an isolated condition or coexist with other genetic disorders. Treatment options include serial casting, percutaneous quadriceps recession, and V-Y quadricepsplasty (VYQ). The pathogenesis and hereditary patterns of CKD are not fully understood, with most cases being familial. CKD is usually managed immediately after birth. However, in this report, the patient was neglected for 2 years. CASE SUMMARY: A 2-year-old girl with bilateral CKD after birth present…

Long-term Follow-up for Successful Salvage of Knee Dislocation Complicated by Infection: A Case Report.

CASE: A 44-year-old morbidly obese man suffered an ultra-low velocity knee dislocation with anterior and posterior cruciate and medial collateral ligament tears after falling from a stationary bike. He underwent open reduction, external fixator application, and multiligamentous reconstruction. Postoperatively, he developed septic arthritis requiring debridement, graft removal, and antibiotic therapy, with eventual conversion to total knee arthroplasty (TKA). We present nearly 20-year follow-up on this complex case. CONCLUSION: Morbid obesity amplifies the risks of knee dislocation management,…
