Neurobiology of panic and pH chemosensation in the brain.

Panic disorder is a common and disabling illness for which treatments are too frequently ineffective. Greater knowledge of the underlying biology could aid the discovery of better therapies. Although panic attacks occur unpredictably, the ability to provoke them in the laboratory with challenge protocols provides an opportunity for crucial insight into the neurobiology of panic. Two of the most well-studied panic provocation challenges are CO(2) inhalation and lactate infusion. Although it remains unclear how these challenges provoke panic animal models of CO(2) and lactate action are beginnin

Diagnosing mitral valve prolapse by improving the predictive power of classifiers.

Mitral valve prolapse (MVP) has been described as one of the most common cardiac valvular abnormalities in industrialized countries, and can result in sudden death. This study focused on various feature selection mechanisms that might improve the predictive power of a classifier to diagnose MVP. The experiment included selection mechanisms using classical greedy feature selection approaches (forward selection and backward elimination), a genetic algorithm (GA) approach and a cellular automaton (CA) approach. The main aim of this latest approach is to use CA with GA for the data transformation

Particularly interesting cysteine- and histidine-rich protein in cardiac development and remodeling.

Integrin-mediated cell-extracellular matrix interaction plays key roles in tissue morphogenesis and integrity. The Lin11-Isl-1-Mec-3 (LIM) domain-only particularly interesting cysteine- and histidine-rich (PINCH) protein functions as an adaptor essential for the assembly and function of the focal adhesion complex that links integrin signaling to the cytoskeleton and other intracellular signaling pathways and regulates diverse cellular processes such as cell adhesion, migration, growth, differentiation, and survival. Recent biochemical and genetic studies have greatly advanced our knowledge sur

The epidemiology of cancer.

The epidemiology of cancer has a long history. It led to the discovery of several causes of cancer before techniques for the production of the disease in laboratory animals became available. In recent years, epidemiological studies have contributed to knowledge of cancer in five ways: by demonstrating geographical and temporal variations in incidence, by correlating incidence in different communities with the prevalence of social habits and environmental agents, by comparing the experience of individuals with and without cancer, by intervening to remove suspected agents and observing the resul

Scaling Biomedical Text-Mining: Transformers, GenAI, and Drug Discovery.

The exponential growth of biomedical literature has made manual curation and systematic knowledge extraction increasingly impractical, driving the need for robust and automated text mining. This chapter traces the evolution of biomedical text mining (BioTM) from rule-based and classical machine learning to deep learning, with a particular focus on transformer architectures and generative AI (GenAI). We examine how domain-adaptive pretraining, ontology-aware modeling, and long-context transformers improve foundational tasks (named entity recognition, normalization, and relation extraction) whil

Explainable AI-based clinical decision support system for hearing disorders.

In clinical system design, human-computer interaction and explainability are important topics of research. Clinical systems need to provide users with not only results but also an account of their behaviors. In this research, we propose a knowledge-based clinical decision support system (CDSS) for the diagnosis and therapy of hearing disorders, such as tinnitus, hyperacusis, and misophonia. Our prototype eTRT system offers an explainable output that we expect to increase its trustworthiness and acceptance in the clinical setting. Within this paper, we: (1) present the problem area of tinnitus

The chequered history of the antiphospholipid syndrome.

Consideration of the chronology of advances in medical knowledge can provide useful insights into the pathogenesis, diagnosis and treatment of diseases. The antiphospholipid syndrome is an enigmatic disorder and this is reinforced by the misleading associated terminology, the adoption of which results directly from early discoveries relating to the condition. Thus the target antigen of the causative autoantibodies in antiphospholipid syndrome does not reside on phospholipid, and the frequently associated lupus anticoagulant is not restricted to subjects with systemic lupus erythematosus and, p

Hereditary leiomyomatosis and renal cell cancer (HLRCC).

Hereditary leiomyomatosis and renal cell cancer (HLRCC) (MIM 605839) is a recently identified autosomal dominant tumor susceptibility syndrome characterized by predisposition to benign leiomyomas of the skin and the uterus (fibroids, myomas). Susceptibility to early-onset renal cell carcinoma and uterine leiomyosarcoma is present in a subset of families. Renal cell carcinomas are typically solitary and aggressive tumors displaying papillary type 2 or collecting duct histology. The disease predisposing gene was identified as fumarate hydratase (fumarase, FH) (MIM 136850). FH encodes an enzyme t

Human carbonic anhydrase modulators: the past, present, and future.

Human carbonic anhydrases (hCAs) are a superfamily of metalloenzymes with considerable, though yet poorly exploited therapeutic potential. They were primarily implicated in ocular and renal diseases and are targeted by antiglaucoma and diuretic drugs. However, emerging evidence has expanded our understanding of the roles of hCA modulators in other pathologies, including obesity, inflammation, hypoxic tumors, neurodegeneration, and neuropathic pain. In this review, we trace the evolution of our knowledge of hCA modulators, from their discovery to recent advances and future prospects. We also hi

Pro- and anti-inflammatory cytokines: the hidden keys to autoimmune gastritis therapy.

Autoimmune gastritis (AIG) is an autoimmune disorder characterized by the destruction of gastric parietal cells and atrophy of the oxyntic mucosa which induces intrinsic factor deficiency and hypo-achlorhydria. AIG predominantly affects the antral mucosa with AIG patients experiencing increased inflammation and a predisposition toward the development of gastric adenocarcinoma and type I neuroendocrine tumors. The exact pathogenesis of this autoimmune disorder is incompletely understood although dysregulated immunological mechanisms appear to major contributors. This review of autoimmune gastri

Genomics of Obsessive-Compulsive Disorder and Related Disorders: What the Clinician Needs to Know.

A wealth of evidence has shown that genetics plays a major role in susceptibility to obsessive-compulsive disorder (OCD) and all of its related disorders. Several large-scale, collaborative efforts using modern genomic methods are beginning to reveal the genetic architecture of these traits and identify long-sought risk genes. In this article, we summarize current OCD and related disorder genomic knowledge and explain how to communicate this information to patients and their families. The article concludes with a discussion of how genomic discovery in OCD and related disorders can inform our u

The Biological Basis of Histologic Transformation.

Histologic transformation of follicular lymphoma remains the leading cause of follicular lymphoma-related mortality in the rituximab era. Both the diverse timing of transformation and heterogeneity in associated genomic events suggest that histologic transformation may itself comprise distinct disease entities. Successive indolent and transformation episodes occur by divergent clonal evolution from an inferred common progenitor cell, representing a potential therapeutic target. Existing biological knowledge largely pre-dates anti-CD20 therapy, and further prospective validation is essential. I

A possible role of stem cells in nasal polyposis.

Since its discovery, the understanding of stem/progenitor cells raised dramatically in the last decade. Their regenerative potential is important to develop new therapeutic applications, but the identification advanced much faster than our understanding of stem/progenitor cells. In nasal polyposis, little is known about stem cells/progenitor cells and their ability. However, the further characterization of stem cells/progenitor cells may provide new treatment options for combating nasal polyposis. This review highlights the knowledge of the current literature about stem cells/progenitor cells

Classifications of neurogenetic diseases: An increasingly complex problem.

Neurodegenerative disorders represent a wide group of diseases affecting the central and/or peripheral nervous system. Many of these disorders were described in the 19th century, but our genetic knowledge of them is recent (over the past 25 years). However, the continual discovery of disease-causing gene mutations has led to difficulties in the classification of these diseases. For this reason, our present proposals for updating and simplifying the classification of some of these conditions (Charcot-Marie-Tooth diseases, distal hereditary motor neuropathies, hereditary sensory and autonomic ne

Ophthalmic artery arising from the anterior cerebral artery diagnosed by MR angiography.

Extremely rarely, the ophthalmic artery (OphA) arises from the A1 segment of the anterior cerebral artery (ACA). Discovery of this anomalous OphA during angiography or surgery has been reported in several patients. We report a case in which an OphA of ACA origin was diagnosed by magnetic resonance (MR) angiography and confirmed by selective cerebral angiography. To our knowledge, this is the first report of this variation with MR angiography. Careful observation of MR angiographic images is important for detecting rare arterial variations. Partial maximum-intensity-projection images are useful

Discovery of two clinical histamine H(3) receptor antagonists: trans-N-ethyl-3-fluoro-3-[3-fluoro-4-(pyrrolidinylmethyl)phenyl]cyclobutanecarboxamide (PF-03654746) and trans-3-fluoro-3-[3-fluoro-4-(pyrrolidin-1-ylmethyl)phenyl]-N-(2-methylpropyl)cyclobutanecarboxamide (PF-03654764).

The discovery of two histamine H(3) antagonist clinical candidates is disclosed. The pathway to identification of the two clinical candidates, 6 (PF-03654746) and 7 (PF-03654764) required five hypothesis driven design cycles. The key to success in identifying these clinical candidates was the development of a compound design strategy that leveraged medicinal chemistry knowledge and traditional assays in conjunction with computational and in vitro safety tools. Overall, clinical compounds 6 and 7 exceeded conservative safety margins and possessed optimal pharmacological and pharmacokinetic prof

The importance of localizing pulmonary veins in atrial septal defect closure!

An 8-year-old girl was admitted for a simple closure of echocardiographically diagnosed Atrial Septal Defect (ASD). During the operation the right pulmonary veins orifices were not detected in the left atrium and attempt to localize them led to the discovery of three additional anomalies, namely Interrupted Inferior Vena Cava (IIVC), Scimitar syndrome, and systemic arterial supply of the lung. Postoperatively these finding were confirmed by CT angiography. This case report emphasizes the need for adequate preoperative diagnosis and presents a very rare constellation of four congenital anomalie

Molecular mechanisms of glucocorticoids action: implications for treatment of rhinosinusitis and nasal polyposis.

Intra-nasal glucocorticoids are the most effective drugs available for rhinosinusitis and nasal polyposis treatment. Their effectiveness depends on many factors and not all of them have been well recognized so far. The authors present the basic information on molecular mechanisms of glucocorticoid action, direct and indirect effects of glucocorticoids on transcription of genes encoding inflammatory mediators. They focus on recently proved nongenomic mechanisms which appear quickly, from several seconds to minutes after glucocorticoid administration and discuss clinical implications resulting f

Short stature and dysmorphology associated with defects in the SHOX gene.

Since its discovery in 1997, knowledge about the SHOX gene ( Short stature HOmeoboX-containing gene) has rapidly advanced. Although originally described as causing idiopathic short stature, SHOX mutations are also responsible for growth retardation in Léri-Weill dyschondrosteosis, Langer mesomelic dysplasia and Turner syndrome. Furthermore, SHOX has a broad functional scope and leads to a variety of different morphological-skeletal stigmata associated with these syndromes. This article reviews clinical and molecular data associated SHOX gene defects. Functional ongoing studies are expected to

Pharmacological management of narcolepsy.

Narcolepsy is a disabling disorder characterised by excessive daytime sleepiness, cataplexy, hypnagogic hallucinations, sleep paralysis and disturbed nocturnal sleep. Traditionally, a wide variety of substances are used in the symptomatic pharmacological treatment of narcolepsy. Several generally more-tolerated substances have been added to the therapeutic repertoire after extensive testing in large patient populations during recent years. This review addresses the state-of-the-art knowledge about the pharmacological treatment of narcolepsy along with the personal view of the authors. The rece
