Anatomy and Embryology of the Middle Ear, Labyrinth, and Intracranial Vestibular Pathways.

The intricate anatomy and embryology of the middle ear, labyrinth, and vertigo-related intracranial pathways involve complex developmental processes and contributions from multiple germ layers. The middle ear, comprised of the tympanic cavity, ossicles, and Eustachian tube, develops from the first and second branchial arches and clefts. In contrast, the inner ear originates from the otic vesicle, forming the bony and membranous labyrinths. The embryological timeline spans from the 40th day of gestation to the 24th week. The vestibulocochlear nerve (VIII cranial nerve) emerges with the inner ea

The specialized inner ear labyrinth of worm-lizards (Amphisbaenia: Squamata).

High-resolution computed tomography (HRCT) has become a widely used tool for studying the inner ear morphology of vertebrates. Amphisbaenians are one of the most specialized groups of fossorial reptiles but are poorly understood relative to other squamate reptile. In this paper we survey the anatomy of the inner and middle ear of these fossorial reptiles using HRCT models and we describe qualitatively and quantitatively (using 3D morphometrics) the anatomy of the inner ear. Amphisbaenians are diverse in skull anatomy, especially in the configuration of the snout, which correlates with digging

Imaging Analysis for Cholesteatoma Extension to the Perilymphatic Space in Labyrinth Fistulae.

OBJECTIVES/HYPOTHESIS: Disturbed perilymph behind a labyrinth fistula can lead to hearing deterioration; thus, delicate manipulation is required during surgery for cholesteatomatous fistulae with matrix extension to the perilymphatic space (EPS). However, it remains challenging to identify the EPS preoperatively. This study aimed to evaluate the diagnostic value of computed tomography (CT) for preoperative prediction of the EPS of cholesteatomatous fistulae. STUDY DESIGN: Retrospective study. METHODS: We included serial high-resolution CT images showing a cholesteatomatous bone defect in the

Physical and geometric constraints shape the labyrinth-like nasal cavity.

The nasal cavity is a vital component of the respiratory system that heats and humidifies inhaled air in all vertebrates. Despite this common function, the shapes of nasal cavities vary widely across animals. To understand this variability, we here connect nasal geometry to its function by theoretically studying the airflow and the associated scalar exchange that describes heating and humidification. We find that optimal geometries, which have minimal resistance for a given exchange efficiency, have a constant gap width between their side walls, while their overall shape can adhere to the geom

A New Kind of Nursing: Walking the labyrinth of Cook County Jail.

This paper examines the application of public health nursing education with an emphasis on the social determinants of health within a correctional facility. The innate tension between public health practice and the punitive nature of correctional confinement is discussed through the perspective of student involvement. Exemplars of unexpected contingencies endemic to this practice setting are provided as are insights gleaned from this pilot collaboration between a college of nursing and a county correctional system.

Membrane Stress in the Human Labyrinth and Meniere Disease: A Model Analysis.

Introduction The nature and extent of membrane damage encountered in Meniere disease remains unexplained. Pressure-induced membrane stress may underlie the characteristic hydropic distention. Analysis of stress in the several vestibular chambers may offer insight into the nature and progression of Meniere disease. Objective Membrane stress levels will be assessed by constructing a specific model of the human membranous labyrinth through the application of human dimensions to an existing generic model of the mammalian labyrinth. Methods Nominal dimensions for a model of the human membranous lab

The genetics of ataxia: through the labyrinth of the Minotaur, looking for Ariadne's thread.

Among the hereditary cerebellar ataxias (CAs), there are at least 36 different forms of autosomal dominant cerebellar ataxia (ADCAs), 20 autosomal recessive cerebellar ataxias (ARCAs), two X-linked ataxias, and several forms of ataxia associated with mitochondrial defects. Despite the steady increase in the number of newly discovered CA genes, patients, especially those with putative ARCAs, cannot yet be genotyped. Moreover, in daily clinical practice, ataxia may present as an isolated cerebellar syndrome or, more often, it is associated with a broad spectrum of neurological manifestations inc

Immunotherapy for malignant melanoma--tracing Ariadne's thread through the labyrinth.

A working group (FEBIM) within the European Organisation for Research and Treatment of Cancer undertook extensive studies on the possible association of infectious diseases and the risk of malignant melanoma. These studies provided evidence that several infectious diseases and also some vaccines including the anti-tuberculosis vaccine, BCG, derived from Mycobacterium bovis, confer a significant level of protection against this form of cancer. In recent years, the importance of immunoregulatory networks in the establishment of tolerance to tumour antigens and the key role of the innate immune s

HOXA13 Is essential for placental vascular patterning and labyrinth endothelial specification.

In eutherian mammals, embryonic growth and survival is dependent on the formation of the placenta, an organ that facilitates the efficient exchange of oxygen, nutrients, and metabolic waste between the maternal and fetal blood supplies. Key to the placenta's function is the formation of its vascular labyrinth, a series of finely branched vessels whose molecular ontogeny remains largely undefined. In this report, we demonstrate that HOXA13 plays an essential role in labyrinth vessel formation. In the absence of HOXA13 function, placental endothelial cell morphology is altered, causing a loss in

[Influence of the cerebellum on the postural labyrinth reflex--experimental and clinical data].

The subject of the study was to estimate the influence of the cerebellum and the postural labyrinth reflex. The pigeons after hemicerebellectomy or vermis ablation this reflex was analysed during body rotation around sagittal and transversal axis. In the clinical part of observations, stabilometric measure of head motion during balance on the movable platform was performed in patients with supratentorial and cerebellar tumors. The results showed the greatest reflex disturbances in vermis ablation and vermis tumors. In hemispheric tumors of the cerebellum these disorders were slighter, but grea

Effect of noise exposure on blood-labyrinth barrier in guinea pigs.

The influence of noise exposure on the endothelial transport system in the cochlea was investigated using cationic polyethyleneimine (PEI), since systemically administered PEI passes through the capillary endothelial cell and attaches to basal lamina (BL) anionic sites in the cochlea. Under general anesthesia, all guinea pigs were administered an intravenous injection of 0.5% PEI. Thirty minutes later, five animals were exposed to noise (10 kHz, broad band noise, 105 dB SPL) for 30 min, via speakers inserted into the external auditory canal. The remaining five animals (controls) were left with

Dexamethasone perfusion of the labyrinth plus intravenous dexamethasone for Ménière's disease.

Recent clinical and laboratory evidence indicates that Meniere's disease is an immune-mediated disease. Dexamethasone perfusion of the inner ear through the round window plus intravenous dexamethasone often will stop the dizzy spells, reduce the fullness and low-frequency tinnitus, and sometimes improve the hearing in patients with Meniere's disease. The dexamethasone must act mostly on the endolymphatic sac and, to a lesser extent, on the stria vascularis and spiral ligament, the known targets of immune response in the inner ear, to reduce the endolymphatic hydrops and restore the fluid dynam

[Possibilities for preserving hearing in labyrinth fistulas of different degrees of severity].

The labyrinthine fistula continues to be one of the most common complications in ears with cholesteatoma. Fifty-one patients with labyrinthine fistula were identified in a series of 1243 cases with cholesteatoma operated upon between 1989 and 1993 at the University ENT Clinic Wuerzburg. The surgical management concept comprised of removing the cholesteatoma matrix, categorizing the fistula type, and immediate covering of the labyrinthine capsule defect with bone dust, perichondrium and fibrin glue. The classification system of the fistulae used in the study used the depth of labyrinthine invol

Do the organs of the labyrinth differentially influence the sympathetic and parasympathetic systems?

It has long been recognized that the vestibular system plays a major role in autonomic control. The nature of this control remains in dispute, however, as some evidence points to a vestibularly mediated parasympathetic activation, whereas other evidence points to a sympatho-excitatory role for labyrinthine outputs. A theoretical explanation is offered that attempts to resolve this issue by postulating that the utricles exert a predominantly sympatho-excitatory influence via their interactions with brain noradrenergic pathways, while the semicircular canals (and possibly saccules) increase para

The significance for postoperative hearing of preserving the labyrinth in acoustic neurinoma surgery.

Among 186 patients with preoperative hearing, a total of 189 acoustic neurinomas were removed through a lateral suboccipital approach with anatomical preservation of the cochlear nerve. Functional hearing was preserved in 92 (49%) of these patients; despite anatomical preservation of the cochlear nerve, deafness was the result in 51% of the series. Many factors have been considered to cause hearing loss in patients whose cochlear nerve was intact after surgery; these include nerve retraction, nerve or cochlear ischemia, overheating and vibration damage to the nerve, and opening of the labyrint
