Advocacy for health rights with a group of patients with chronic diseases in a low-income neighbourhood in Ecuador.

OBJECTIVE: We describe an advocacy process developed with a diabetes patient club from a low-income area, using participatory research methods aimed at securing governmental support and recognition of peer networks as a fundamental principle of health promotion for chronic patients. RESULTS: This participatory process, led by local facilitators from the Chimbacalle Diabetes Patients' Club, followed three key steps: social diagnosis (using participatory tools such as a sociogram and SWOT analysis), development of a community narrative through digital storytelling, and process evaluation. Automated Chest X-Ray Screening: Can Lung Region Symmetry Help Detect Pulmonary Abnormalities?

Our primary motivator is the need for screening HIV+ populations in resource-constrained regions for exposure to Tuberculosis, using posteroanterior chest radiographs (CXRs). The proposed method is motivated by the observation that radiological examinations routinely conduct bilateral comparisons of the lung field. In addition, the abnormal CXRs tend to exhibit changes in the lung shape, size, and content (textures), and in overall, reflection symmetry between them. We analyze the lung region symmetry using multi-scale shape features, and edge plus texture features. Shape features exploit loca

Sensorimotor cortical activity in patients with complete spinal cord injury: a functional magnetic resonance imaging study.

Residual activation of the cortex was investigated in nine patients with complete spinal cord injury between T6 and L1 by functional magnetic resonance imaging (fMRI). Brain activations were recorded under four conditions: (1) a patient attempting to move his toes with flexion-extension, (2) a patient imagining the same movement, (3) passive proprio-somesthesic stimulation of the big toes without visual control, and (4) passive proprio-somesthesic stimulation of the big toes with visual control by the patient. Passive proprio-somesthesic stimulation of the toes generated activation posterior t

Development of functionally patent lymphatic meshes in postsurgical long-term resolution of peripheral secondary lymphedema.

OBJECTIVE: In oncologic surgery, secondary lymphedema of male external genital organs and upper or lower limbs frequently develops as a result of excision or mechanical obstruction of collecting lymphatic trunks. We evaluated whether the short-term and long-term outcomes of microsurgical treatment of limb and genital organs improves tissue drainage in patients with secondary lymphedema by restoring the pre-existing lymphatic networks or through new lymphangiogenesis. METHODS: Of 110 secondary lymphedema patients, microsurgery was performed in 45 hospitalized patients. Patients were aged 25 to

Recognition, Localization and 3D Geometric Morphology Calculation of Microblind Holes in Complex Backgrounds Based on the Improved YOLOv11 Network and AVC Algorithm.

Microblind hole processing quality inspection, especially accurately identifying microblind hole contour features and precisely detecting 3D and morphological parameters, has always been challenging, especially for accurately identifying those of different sizes, depths, and contour features simultaneously. This poses a great challenge for identifying and localizing microblind hole contours based on machine vision and accurately calculating three-dimensional parameters. This study takes cigarette microblind holes (diameter of 0.1-0.2 mm, depth of approximately 35 µm) as the research object. It

Heightened degree connectivity of the striatum in obsessive-compulsive disorder induced by symptom provocation.

BACKGROUND: Neurosurgical intervention studies have provided direct evidence that the ventral striatum/nucleus accumbens (NAc) mediates symptoms of obsessive-compulsive disorder (OCD), yet meta-analysis of functional neuroimaging studies using symptom provocation revealed no striatal activation differences, and the existing studies reporting hyperactivity found abnormalities in dorsal but not ventral striatal subdivisions. Resting-state neuroimaging evidence holds that corticostriatal areas are more connected both locally and to distant regions, but the functional inferences to be drawn from t

Alterations in dopaminergic innervation and receptors in focal cortical dysplasia.

Focal cortical dysplasia (FCD) type 2 is the most common malformation of cortical development associated with pharmaco-resistant focal epilepsy and frequently located in the frontal cortex. Neuropathological hallmarks comprise abnormal cortical layering and enlarged, dysmorphic neuronal elements. Fundamentally altered local neuronal activity has been reported in human FCD type 2 epilepsy surgical biopsies. Of note, FCD type 2 emerges during brain development and forms complex connectivity architectures with surrounding neuronal networks. Local cortical microcircuits, particularly in frontal lo

Sociospatial distribution of access to facilities for moderate and vigorous intensity physical activity in Scotland by different modes of transport.

BACKGROUND: People living in neighbourhoods of lower socioeconomic status have been shown to have higher rates of obesity and a lower likelihood of meeting physical activity recommendations than their more affluent counterparts. This study examines the sociospatial distribution of access to facilities for moderate or vigorous intensity physical activity in Scotland and whether such access differs by the mode of transport available and by Urban Rural Classification. METHODS: A database of all fixed physical activity facilities was obtained from the national agency for sport in Scotland. Facili

Implementing a Locally Made Low-Cost Intervention for Wound and Lymphedema Care in Western Kenya.

In Western Kenya, the burden of chronic wounds and lymphedema has a significant impact on functionality and quality of life. Major barriers to provision of care include availability, affordability, and accessibility of bandages. At the Academic Model Providing Access to Healthcare, dermatologists and pharmacists collaborated to develop a 2-component compression bandage modeled after the Unna boot, using locally available materials, that is distributed through a revolving fund pharmacy network. In partnership with nursing, use of these bandages at a national referral hospital and a few county f

An archaeobotanical and stable isotope approach to changing agricultural practices in the NW Mediterranean region around 4000 BC.

It has recently been observed, that a change in the crop spectrum happened during the so-called Middle Neolithic in France at ca. 4000 BC. An agricultural system based on free-threshing cereals (naked wheat and naked barley) seems to shift to one based on glume wheats. This is a major change for traditional farmers and this paper aims to shed light on its possible causes. Here we describe the results of new investigations in a key area for the understanding of this process: the NW Mediterranean arch, where free-threshing cereals are the main cultivars since ca. 5100 BC. New data confirm that t

A Novel Fuzzy Relative-Position-Coding Transformer for Breast Cancer Diagnosis Using Ultrasonography.

Breast cancer is a leading cause of death in women worldwide, and early detection is crucial for successful treatment. Computer-aided diagnosis (CAD) systems have been developed to assist doctors in identifying breast cancer on ultrasound images. In this paper, we propose a novel fuzzy relative-position-coding (FRPC) Transformer to classify breast ultrasound (BUS) images for breast cancer diagnosis. The proposed FRPC Transformer utilizes the self-attention mechanism of Transformer networks combined with fuzzy relative-position-coding to capture global and local features of the BUS images. The

Deep Neural Network-Aided Histopathological Analysis of Myocardial Injury.

Deep neural networks have become the mainstream approach for analyzing and interpreting histology images. In this study, we established and validated an interpretable DNN model to assess endomyocardial biopsy (EMB) data of patients with myocardial injury. Deep learning models were used to extract features and classify EMB histopathological images of heart failure cases diagnosed with either ischemic cardiomyopathy or idiopathic dilated cardiomyopathy and non-failing cases (organ donors without a history of heart failure). We utilized the gradient-weighted class activation mapping (Grad-CAM) te

Analysis of illicit drugs in wastewater - Is there an added value for law enforcement?

Assessing illicit drug use through the analysis of wastewater is progressively being integrated into existing methods used to monitor the epidemiology of drug use. However, the approach's potential to deliver pertinent information for law enforcement has been discussed only limitedly. Thus, this work focuses on evaluating the added value of the approach from the perspective of law enforcement. Results from wastewater analysis carried out in two cities in Switzerland were scrutinised, taking into account intelligence derived from the work of drug enforcement in the area. Focus was set on three

Impaired Topographic Organization in Patients With Idiopathic Blepharospasm.

Background: Idiopathic blepharospasm (BSP) is a common adult-onset focal dystonia. Neuroimaging technology can be used to visualize functional and microstructural changes of the whole brain. Method: We used resting-state functional MRI (rs-fMRI) and graph theoretical analysis to explore the functional connectome in patients with BSP. Altogether 20 patients with BSP and 20 age- and gender-matched healthy controls (HCs) were included in the study. Measures of network topology were calculated, such as small-world parameters (clustering coefficient [C p], the shortest path length [L p]), network e

[The cerebral representation of the Christmas spirit. A transcultural functional magnetic resonance study].

INTRODUCTION: Functional magnetic resonance imaging has been used to detect specific cognitive functions of the human brain. To our knowledge, no attempt has yet been made to localize the Christmas spirit using this technique. MATERIAL AND METHODS: We used a 3 tesla MR scanner and a visual stimulation paradigm consisting of Christmas-associated images and matched neutral images. We studied two healthy subjects with and two healthy subjects without a Christmas celebrating cultural background. RESULTS: In the Christmas celebrating group we observed an extensive cerebral response to Christmas a

The organization of individually mapped structural and functional semantic networks in aging adults.

Language function in the brain, once thought to be highly localized, is now appreciated as relying on a connected but distributed network. The semantic system is of particular interest in the language domain because of its hypothesized integration of information across multiple cortical regions. Previous work in healthy individuals has focused on group-level functional connectivity (FC) analyses of the semantic system, which may obscure interindividual differences driving variance in performance. These studies also overlook the contributions of white matter networks to semantic function. Here,

Altered cerebrocerebellar functional connectivity in patients with obstructive sleep apnea and its association with cognitive function.

STUDY OBJECTIVES: Previous functional MRI studies have reported altered brain networks in patients with obstructive sleep apnea (OSA). However, the extent and pattern of abnormal connectivity were inconsistent across studies, and cerebrocerebellar connections have been rarely assessed. We investigated functional network changes in cerebral and cerebellar cortices of OSA patients. METHODS: Resting-state functional MRI, polysomnography, and neuropsychological (NP) test data were acquired from 74 OSA patients (age: 45.8 ± 10.7 years) and 33 healthy subjects (39.6 ± 9.3 years). Posterior parietal influences on visual network specialization during development: An fMRI study of functional connectivity in children ages 9 to 12.

Visual processing in the primate brain is highly organized along the ventral visual pathway, although it is still unclear how categorical selectivity emerges in this system. While many theories have attempted to explain the pattern of visual specialization within the ventral occipital and temporal areas, the biased connectivity hypothesis provides a framework which postulates extrinsic connectivity as a potential mechanism in shaping the development of category selectivity. As the posterior parietal cortex plays a central role in visual attention, we examined whether the pattern of parietal co

Altered topological properties of the cortical motor-related network in patients with subcortical stroke revealed by graph theoretical analysis.

Cerebral neuroplasticity after stroke has been elucidated by functional neuroimaging. However, little is known concerning how topological properties of the cortical motor-related network evolved following subcortical stroke. In the present study, we investigated 24 subcortical stroke patients with only left motor pathway damaged and 24 matched healthy controls. A cortical motor-related network consisting of 20 brain regions remote from the primary lesion was constructed using resting-state functional MRI datasets. We subsequently used graph theoretical approaches to analyze the topological pro
