Varied presentation of sinonasal phosphaturic mesenchymal tumour: report of a case series with follow-up.

PURPOSE: Phosphaturic mesenchymal tumour (PMT) of the paranasal sinuses is a rare tumour that is associated with oncogenous osteomalacia causing predominant musculoskeletal symptoms. We present a series of eight patients diagnosed to have PMT of the paranasal sinuses with varied presentation and highlight the appropriate evaluation required to arrive at the diagnosis. METHODS: Retrospective review of eight patients diagnosed to have PMT-causing tumour-induced osteomalacia with follow-up data. RESULTS: Eight patients, 4 males and 4 females, aged 36-67 years (mean = 44 years) presented with va

Histopathological aspects of benign mesenchymal tumors located in "high risk areas" of the tongue.

Although a series of premalignant or potentially malignant lesions are described in the tongue, there are many other lesions whose potential of degeneration can be determined by a prolonged action of irritant or carcinogenic factors. Even if they are practically considered to be harmless, benign formations of mesenchymal origin with lingual location represent in the same time a neoplastic lesion under the influence of local and general factors, for a variable period of time. When these structures are located in major risk areas from the oral cavity and in particular, those directly related to

Phosphaturic Mesenchymal Tumor: From Invisibility to Bedridden Morbidity.

Tumor-induced osteomalacia (TIO) is a rare paraneoplastic syndrome caused by fibroblast growth factor 23 (FGF23) secreting phosphaturic mesenchymal tumors. We report a 53-year-old man with progressive osteomalacia resulting in bedridden status. Renal phosphate wasting despite normal vitamin D, intact parathormone, and renal function raised suspicion of TIO, confirmed by elevated FGF23. However, the occult tumor was localized only by Ga-68 DOTANOC wholebody PET-CT, highlighting the diagnostic challenges in TIO.

CD56 may be a more useful immunohistochemical marker than somatostatin receptor 2A for the diagnosis of phosphaturic mesenchymal tumors.

Phosphaturic mesenchymal tumors (PMTs) are the most typical cause of tumor-induced osteomalacia (TIO) associated with mesenchymal neoplasms. Specifically, TIO is attributed to the production of phosphatonins, such as fibroblast growth factor 23 (FGF23), participating in the homeostasis of phosphate. Although immunohistochemistry (IHC) for FGF23 showed characteristic positive staining in PMTs, FGF23 antibodies that can be used for the reliable diagnosis of PMTs are hard to obtain in common pathology laboratories. Somatostatin receptor 2A (SSTR2A) has been previously proposed as an alternatively

Tumor-induced osteomalacia with elevated fibroblast growth factor 23: a case of phosphaturic mesenchymal tumor mixed with connective tissue variants and review of the literature.

Tumor-induced osteomalacia (TIO), or oncogenic osteomalacia (OOM), is a rare acquired paraneoplastic disease characterized by renal phosphate wasting and hypophosphatemia. Recent evidence shows that tumor-overexpressed fibroblast growth factor 23 (FGF23) is responsible for the hypophosphatemia and osteomalacia. The tumors associated with TIO are usually phosphaturic mesenchymal tumor mixed connective tissue variants (PMTMCT). Surgical removal of the responsible tumors is clinically essential for the treatment of TIO. However, identifying the responsible tumors is often difficult. Here, we repo

Mithramycin treatment of hypercalcaemia and renal failure in a patient with paratesticular embryonic sarcoma.

A 17-year-old patient with a small paratesticular embryonic sarcoma presented with symptoms of renal failure, polyuria and widespread bone metastases. Investigation revealed hypercalcaemia and uraemia without any evidence of hyperparathyroidism. The hypercalcaemia responded over a period of weeks to administration of mithramycin with initial improvement in the symptoms and metabolic derangements. Control was lost with the necrosis of intra-abdominal tumour deposits and haemorrhagic polypoid deposits in the alimentary tract. The value and hazards of mithramycin are well demonstrated by these ra

Phosphaturic mesenchymal tumor of the tibia with oncogenic osteomalacia in a teenager.

Phosphaturic mesenchymal tumor is an uncommon cause of a paraneoplastic syndrome that can be associated with osteogenic osteomalacia. This tumor most commonly occurs in middle-aged men and women. We report a rare case of a phosphaturic mesenchymal tumor in a 16-year-old girl with multiple fractures as a result of severe osteoporosis. CT and MRI showed a mass arising from the tibia.

Phosphaturic mesenchymal tumor: a report of 6 patients treated at a single institution and comparison with reported series.

Osteogenic osteomalacia (OO)-associated phosphaturic mesenchymal tumors (PMTs) might represent a single histopathologic paraneoplastic entity. These tumors are largely misunderstood, ignored, or unknown by pathologists and clinicians. To elucidate the characteristics of OO-associated PMTs, we retrospectively analyzed the clinicopathologic features of PMTs from 6 patients, with either known OO or features suggestive of PMT-mixed connective tissue variant, who were studied and managed at a single center during the period from 1993 to 2011. Histologically, the tumor showed proliferation of spindl

Persistent tumor-induced osteomalacia confirmed by elevated postoperative levels of serum fibroblast growth factor-23 and 5-year follow-up of bone density changes.

OBJECTIVE: To describe a case of persistent tumor-induced osteomalacia, determine whether serum fibroblast growth factor-23 (FGF-23) levels postoperatively indicate incomplete tumor resection, and report lumbar spine and forearm bone mineral density (BMD) changes during 5 years of follow-up. METHODS: We present clinical, radiologic, histologic, and bone densitometry data as well as serum FGF-23 levels (determined with use of a novel C-terminal enzyme-linked immunosorbent assay) from the study patient and discuss these findings in the context of previous literature. RESULTS: A 52-year-old man

[Mesenchymal tumors of the mediastinum].

Mesenchymal neoplasms of the thymus and mediastinum account for only 2 % of neoplasms of the mediastinum and are therefore very rare. With very few exceptions the histology, immunohistochemistry and (based on current knowledge) molecular biology of mediastinal soft tissue tumors are not different from their counterparts in other organs. Characteristic features are more concerned with clinical epidemiological and therapeutic aspects as well as the multitude of possible differential diagnoses. With the exception of organ-specific tumors, such as gastrointestinal stromal tumors (GIST), virtually

Oesophageal mesenchymal tumours: clinicopathological features and absence of Epstein-Barr virus.

BACKGROUND: Recent studies have suggested that the Epstein-Barr virus (EBV) is associated with smooth muscle tumours (leiomyoma and leiomyosarcoma) in patients with human immunodeficiency virus and in organ transplant recipients. Leiomyoma is the most common mensenchymal tumour found in the oesophagus. AIM: To report a single institution experience on oesophageal mesenchymal tumours and to determine whether EBV is associated with these tumours. METHODS: 40 sporadic oesophageal mesenchymal tumours were studied and their diagnosis confirmed on pathological review and immunohistochemical studie

Electron-microscopic analysis of nephroblastomas induced transplacentally in the IIIVO/J rabbit by a single dose of N-ethylnitrosourea.

Eight examples of nephroblastoma induced transplacentally in the partially inbred IIIVO/J strain of rabbit by a single intraperitoneal (60 mg/kg) dose of N-ethylnitrosourea (ENU) were studied by transmission electron-microscopy. At the light-microscopic level the ENU-induced tumors displayed the complex array of histotypic components described in a previous report (Hard GC, Fox RR: Histologic characterization of renal tumors (nephroblastomas) induced transplacentally in IIIVO/J and WH/J rabbits by N-ethylnitrosourea. Undifferentiated (embryonal) sarcoma of the liver: report of 31 cases.

Thirty-one cases of undifferentiated (embryonal) sarcoma of the liver are presented. The tumor is found predominantly in the pediatric age group, the majority of patients (51.6%) being between 6 and 10 years of age. An abdominal mass and pain are the usual presenting symptoms. Radiographic examination is nonspecific except to demonstrate a space-occupying lesion of the liver. The tumors are large, single, usually globular and well demarcated, and have multiple cystic areas of hemorrhage, necrosis, and gelatinous degeneration. Histologic examination shows a pseudocapsule partially separating th
