Establishment of recurrent laryngeal nerve injury animal models using intraoperative neural monitoring: SD rat models.

OBJECTIVE: To reproduce the Sprague Dawley (SD) rat model of recurrent laryngeal nerve (RLN) injury using intraoperative neural monitoring (IONM), this study investigates its key focuses, challenges and solutions, verifies the reproducibility of the established porcine model in rats, and evaluates the feasibility of the SD rat model as an alternative to the porcine model. METHODS: Twenty-four SD rats weighing 200-250 g were selected. Anesthesia was induced with 3%-5% isoflurane in oxygen at a flow rate of 1 L/min, and then maintained with 1.5%-2.5% isoflurane in oxygen at a flow rate of 0.2-0

Sequential intraoperative protocol to improve bulbocavernosus reflex monitorability during pediatric spinal surgery.

PURPOSE: Bulbocavernosus reflex (BCR) monitoring assesses sacral S2 to S4 pathway integrity during pediatric spinal surgery; however, its intraoperative utility is limited when responses are absent or unilateral. We prospectively evaluated a sequential intraoperative protocol designed to improve the interpretation of these patterns and reduce uncertainty during sacral monitoring. METHODS: We prospectively studied 242 consecutive pediatric patients aged 0.4-12 years who underwent spinal surgery between 2023 and 2025. Bilateral external anal sphincter (EAS) recordings were obtained using subder

Unlocking the potential of high-resolution multimodality neuromonitoring for traumatic brain injury management: lessons and insights from cases, events, and patterns.

Multimodality neuromonitoring represents a crucial cornerstone for patient management after acute brain injury. Despite the potential of multimodality neuromonitoring (particularly high-resolution neuromonitoring data) to transform care, its full benefits are not yet universally realized. There remains a critical need to integrate the interpretation of complex patterns and indices into the real-time clinical decision-making processes. This requires a multidisciplinary approach, to evaluate and discuss the implications of observed patterns in a timely manner, ideally in close temporal proximity

In vivo Testing of the Pressio Intracranial Pressure Monitor.

BACKGROUND: Measuring reliable intracranial pressure (ICP) is critical for patients with acute brain injuries. The aim of this study was to evaluate zero drift of the intraparenchymal strain gauge Pressio transducer (Sophysa, Orsay, France) in clinical conditions. METHODS: A prospective observational multicenter study was conducted in four French intensive care units of university hospitals. Patients with acute brain injuries were included if they needed ICP measurement using the Pressio transducer. The zero drift was measured at the explantation of the sensor. ICP-related adverse events were

[MONITORING OF CEREBRAL OXYGENATION AND COGNITIVE FUNCTION IN CAROTID ENDARTERECTOMY: THE ROLE OF TRANSIENT BYPASS OF CAROTID ARTERY].

UNLABELLED: BACKGROUND AND OBJECTVE: Carotid endarterectomy (CEE) is an effective surgical technique to prevent cerebral ischemia and stroke, but can be associated with intervention-related complications. The surgical shunting of the intervention area may reduce the risk of the intraoperative ipsilateral cerebral ischemia following the carotid artery clamping but is controversial. The goal of this study was to compare the cerebral tissue oxygen saturation (SctO2) and early changes of cognitive functions in CEE in the settings of transient vascular bypass and without this method. METHODS: 45 a

Revisiting the Concept of Nerve-Sparing Radical Hysterectomy: A Proof-of-Concept Prospective Study From a Cervical Cancer Cohort.

BACKGROUND: Nerve-sparing radical hysterectomy aims to preserve the autonomic innervation of pelvic organs. In most studies, the preservation of the parasympathetic component of pelvic innervation is achieved by sparing the lymphovascular-neural tissue located at the base of the lateral paracervical tissue. This study explores an alternative surgical technique based on a different anatomical pathway of parasympathetic fibers, running more cranially and deeply within the pararectal space. METHODS: This was a prospective proof-of-concept cohort study in six patients with stage IB2-IIA cervical

Combination of neuroendoscopic hematoma evacuation and endovascular coil embolization for a ruptured anterior choroidal artery aneurysm in patients with moyamoya disease: illustrative cases.

BACKGROUND: The treatment strategy for hemorrhagic moyamoya disease (MMD) due to a ruptured aneurysm at the distal portion of the anterior choroidal artery remains controversial. The authors successfully treated the ruptured aneurysm with neuroendoscopic hematoma evacuation, followed by endovascular coil embolization. OBSERVATIONS: The authors encountered two patients with massive hemorrhagic MMD whose MMD had already been diagnosed and who had a periventricular anastomosis due to a ruptured aneurysm of the distal portion of the anterior choroidal artery involving the periventricular anastomo

Neurophysiological criteria for intraoperative prediction of pure motor hemiplegia during aneurysm surgery. Case report.

The value of motor evoked potentials (MEPs) as an intraoperative neurophysiological monitoring tool for detecting selective subcortical ischemia of the motor pathways during intracerebral aneurysm repair is described and the use of such measures to predict postoperative motor status is discussed. The authors present the case of a 64-year-old woman in whom there was an incidental finding of two right middle cerebral artery (MCA) aneurysms. During the aneurysm clipping procedure, an intraoperative MEP loss in the left abductor pollicis brevis and tibial anterior muscles occurred during an attemp

Auricular Transcutaneous Vagus Nerve Stimulation Diminishes Alpha-Band-Related Inhibitory Gating Processes During Conflict Monitoring in Frontal Cortices.

BACKGROUND: Pursuing goals is compromised when being confronted with interfering information. In such situations, conflict monitoring is important. Theoretical considerations on the neurobiology of response selection and control suggest that auricular transcutaneous vagus nerve stimulation (atVNS) should modulate conflict monitoring. However, the neurophysiological-functional neuroanatomical underpinnings are still not understood. METHODS: AtVNS was applied in a randomized crossover study design (n = 45). During atVNS or sham stimulation, conflict monitoring was assessed using a Flanker task.

Utility and Value of Movement Recording with Combined EEG-EMG Monitoring in the Intensive Care Unit.

Continuous electroencephalographic (EEG) monitoring has become a standard of care in several contexts in the intensive care unit (ICU), especially for the management of refractory status epilepticus. ICU patients often present movement disorders that may be of epileptic or nonepileptic origin, and their correct identification is crucial for the diagnostic and therapeutic process. Video analysis is often insufficient to precisely detect or characterize movement disorders and the ICU environment is prone to many artifacts. Combined EEG electromyogram (EMG) monitoring can enhance the detection of

Effects of Adjunct Analgesics and Novel Anesthetic Agents on Intraoperative Neuromonitoring: A Scoping Review.

Intraoperative neuromonitoring (IONM) plays a critical role in assessing neural integrity and guiding surgical decision-making. The effects of traditional anesthetic agents on IONM are well-established, though the impact of novel anesthetics and adjunct analgesics remains unclear. This scoping review aims to evaluate the effects of novel anesthetic and adjunct analgesic agents on somatosensory evoked potentials (SSEPs) and motor evoked potentials (MEPs) in the intraoperative setting. A comprehensive literature search was conducted using Medline, Cochrane Central, CINAHL, Scopus, LILACS, and Em

Advanced neuromonitoring techniques for medical and neurological ICU patients.

Newly developed neurological complications are not confined to patients admitted to the neurology ICU but also occur in the medical ICU. In addition to patients with traumatic brain injuries, critically ill patients with sepsis, encephalopathy, and post-cardiopulmonary resuscitation are at risk of developing neurological complications. The situation can worsen when patients are sedated and intubated, which prevents the clinical manifestations to be presented. In this regard, non-convulsive seizures and status epilepticus, new onset strokes, and neuromuscular weakness syndromes could be missed

Pediatric craniopharyngioma with significantly increased intraoperative visual evoked potential amplitude and postoperative visual acuity improvement: a case report.

Visual evoked potential (VEP) is an established modality that allows safe brain tumor resection and preservation of optical function. We herein present a case of a pediatric craniopharyngioma with significant improvement in the VEP amplitude detected during endoscopic transsphenoidal surgery (ETS) and obvious postoperative improvement in visual acuity. A 13-year-old boy presented with visual acuity disturbance in his right eye and was followed up for 5 months by an ophthalmologist. His visual acuity rapidly worsened, and a suprasellar lesion with calcification was found on brain computed tomog

Intraoperative Monitoring of Scoliosis Surgery in Young Patients.

Intraoperative neurophysiologic monitoring has added substantially to the safety of spinal deformity surgery correction since its introduction over four decades ago. Monitoring routinely includes both somatosensory evoked potentials and motor evoked potentials. Either modality alone will detect almost all instances of spinal cord injury during deformity correction. The combined use of the two modalities provides complementary information, can permit more rapidly identification of problems, and enhances safety though parallel redundancy should one modality fail. Both techniques are well establi

Temporary vessel occlusion in cerebral aneurysm surgery guided by direct cortical motor evoked potentials.

BACKGROUND: Temporary clipping is an important tool in the vascular neurosurgeon's armamentarium. We routinely utilize intraoperative neurophysiological monitoring (IONM) for complex brain aneurysm surgery cases, relying on direct cortical motor evoked potential (DCMEP) alerts to guide the duration of temporary clipping. Previous studies have argued for relatively short and intermittent temporary clipping strategies. In this study, we sought to assess the maximal permissive temporary clipping time during complex aneurysm surgery. To do this, we assessed patient outcome in relation to temporary

Foundations of Magnetic Resonance-Guided Focused Ultrasonography.

The ability of ultrasonography to safely penetrate deeply into the brain has made it an attractive technology for neurological applications for almost 1 century. Having recognized that converging ultrasound waves could deliver high levels of energy to a target and spare the overlying and surrounding brain, early applications used craniotomies to allow transducers to contact the brain or dural surface. The development of transducer arrays that could permit the transit of sufficient numbers of ultrasound waves to deliver high energies to a target, even with the loss of energy from the skull, has
