Orbital exenteration: a 10-year experience of a general oncology hospital.

BACKGROUND: The aim of this study is to review the 10-year experience with orbital exenteration, determining the indications, clinical features of patients and the factors that cause the need for exenteration. METHODS: The records of patients that underwent exenteration between 1997 and 2007 were reviewed retrospectively. The age, sex, the location of tumour, the duration of symptoms, histopathological diagnosis, and previous recurrences were noted. RESULTS: Orbital exenteration was performed on 68 patients. Bone resection in 6 and sinusectomy in 13 patients was combined with exenteration. T

[Prosthetic supply of tissue defects in head and neck surgery].

BACKGROUND: Cancer surgery, malformation or trauma may cause broad facial defects that cannot be covered by patients because of their exposed site. Such defects lead to functional deficits and enormous psychological strain and require rehabilitation at all ages. Prosthetic supply has been developed into a functional and aesthetic alternative to plastic and reconstructive surgery. METHODS AND PATIENTS: The condensed experience with prosthetic supply after persistent surgical defects of more than twenty patients during the recent years is described. RESULTS: Efficient prosthetic supply in the

Reconstruction of complex orbital exenteration defects: a single center's experience with a five-year follow-up.

BACKGROUND: Orbital exenteration presents a challenge to the reconstructive surgeon. Defects may be treated with split skin grafts, local advancement flaps, or free flap coverage. There are few published series showing the long-term outcomes of reconstruction. Our results, with at least a 5-year follow-up, are presented. METHODS: A retrospective review of 15 immediate reconstructions after orbital exenterations for malignancies in 12 patients at a tertiary referral center over a 5-year period, was done. All flaps were followed up for at least 5 years (mean, 75 months; range, 3-118 months). R

Structural fat grafting in complex reconstructive surgery.

The use of adipous tissue transfer for the correction of maxillo-facial defects was reported for the first time at the end of the 19 century and has been the subject of numerous studies. Grafted fat tissue has been used for years as an excellent filler during facial enhancement and recontouring. Several techniques have been proposed for harvesting and grafting the fat. However, due to the damage caused by excessive adipose tissue during these maneuvers, the results were not satisfying. The situation required a larger amount of fat to be harvested even for small corrections. In 1988, the Americ

Superficial malignant fibrous histiocytoma presenting as recurrent atypical fibroxanthoma.

Atypical fibroxanthoma (AFX) and malignant fibrous histiocytoma (MFH) are neoplasms of probable fibrohistiocytic lineage. The precise relationship between AFX and MFH remains controversial. We present two cases in which a lesion was excised from the face of an elderly person and reported as AFX. Both cases recurred within 12 months and were reclassified as MFH. They were excised with slow Mohs' micrographic surgery, with adjuvant radiotherapy in one case. In the other, a further recurrence was treated with orbital exenteration and radiotherapy. We review the difficulties in differentiating bet

Primary extraosseous Ewing sarcoma of the orbit.

A 40-year-old man presented with painless, progressive vision loss and mild proptosis of the OD. CT revealed a right intraconal mass with slight penetration of the optic canal not contiguous with any bony structure. Incisional biopsy through a transfrontal orbitotomy revealed a diffuse growth of homogeneous, small, round cells. Immunohistochemical stains were positive for vimentin and MIC2 (CD99), and the translocation at EWS gene (22q12) was detected. Metastatic workup and a full-body bone scan were negative, confirming primary orbital extraosseous Ewing sarcoma. The patient received neoadjuv

Management and long-term outcome of adenoid cystic carcinoma with intracranial extension: a neurosurgical perspective.

Adenoid cystic carcinoma is a tumor of minor and major salivary glands that often invades the cranial base and intracranial cavity via local and perineural spread. In the past, the role of neurosurgeons in managing these tumors has been limited. The growth of interdisciplinary cranial base surgical approaches has now increased the involvement of neurosurgeons in the management of these tumors. We present a series of 16 patients with adenoid cystic carcinomas with cranial base and intracranial extension, the largest series reported in the neurosurgical literature. We have focused our approach o

[Is orbital exenteration indicated for extensive primary mucosal melanomas of the sinunasal system?].

Primary mucosal melanomas are most frequently localized in the sinunasal system. Orbital involvement must be excluded in the mostly advanced tumor stage. The aim of our study was to find possible indications for orbital exenteration in the T3-4 sinunasal tumor stage. We evaluated 14 treatment courses at our department from 1988 to 2004. The analysis disclosed orbital involvement in 43 % of the patients. The one patient submitted to orbital exenteration had a survival of only 28 months, which was not relevantly longer than in the other 9 patients with a T3-4 tumor stage (median survival of 27 m

Posterior auricular muscle patch graft for exposed orbital implant.

The purpose of this article is to describe a surgical technique to repair an exposed orbital implant by posterior auricular muscle autograft. A retrospective review was conducted of four patients with an exposed orbital implant that were treated with a posterior auricular muscle graft. Four patients received posterior auricular muscle patch graft to the exposed orbital implant. The donor site healed with minimal scarring and remained well hidden. The graft incorporated fully into surrounding orbital tissue with no recurrent exposure at average of 13 month follow-up. The posterior auricular mus

Clinicopathological features of melanotic neuroectodermal tumor of infancy: Report of two cases.

Melanotic neuroectodermal tumor of infancy (MNTI) is an extremely rare, pigmented neoplastic entity of neural crest origin. Histological and immunohistochemical profiles indicate the presence of two components, small rounded neuroblast-like cellular areas and areas with large melanin-containing cells which consist of combination of neural, melanocytic, and epithelial cell types. Here we present two interesting cases of infants with MNTI which have different clinicopathological features. The first case is a 3-month-old female with rapidly growing MNTI involving the lacrimal sac and inferior wal

The Value of Orbital Exenteration for Eyelid Sebaceous Carcinoma in Stages II to IV: A Cohort Study of 78 Patients.

PURPOSE: To evaluate the prognosis of eyelid sebaceous carcinoma (SeC) in patients with disease stage worse than IIA. METHODS: This retrospective, single-center study included 78 SeC patients. For stage II patients, 1:3 propensity score matching (PSM) was applied between those undergoing orbital exenteration and those receiving eye-sparing treatments. Risk factors in the eye-sparing group were analyzed using Cox regression, and Kaplan-Meier survival analysis assessed metastasis-free survival (MFS), recurrence-free survival (RFS), disease-specific survival (DSS), and progression-free survival

Challenges in the management of massive intraorbital and hemifacial arteriovenous malformation as causing life-threatening epistaxis.

Arteriovenous malformations are congenital lesions that may evolve with time and manifest in a plethora of presentations. They can occur as torrential epistaxis when it extensively involves the facial region. Multi-imaging modalities are available to assist in characterizing the structure of the lesion as well as its location and extent. This complex disease requires a multidisciplinary team approach with preoperative embolization and surgery. We present a rare cause of life-threatening epistaxis in a gentleman with a longstanding orbital and hemifacial arteriovenous malformation and discuss t

Pancreatic metastases from ocular malignant melanoma: the use of endoscopic ultrasound-guided fine-needle aspiration to establish a definitive cytologic diagnosis: a case report.

BACKGROUND: When encountering solid pancreatic lesions, nonpancreatic primary metastases are rare and differentiating a metastasis from a primary neoplastic lesion is challenging. The clinical presentation and radiologic features can be similar and the possibility of a pancreatic metastasis should be considered when the patient refers to a history of a different primary cancer. Endoscopic ultrasound offers a key anatomical advantage in accessing the pancreas and endoscopic ultrasound-guided fine-needle aspiration has become the gold standard method for diagnosing pancreatic lesions. Risk factors for endophthalmitis requiring evisceration or enucleation.

Endophthalmitis has devastating sequelae resulting in blindness and even loss of eyeball. Although the prognosis of endophthalmitis has much improved with the advances of antibiotics and vitreoretinal surgery, of the number of patients that required evisceration or enucleation is still significant. We retrospectively reviewed the charts of 210 eyes of 210 patients with endophthalmitis andcompared the group that required evisceration or enucleation with those that received salvaging therapies. Regression analysis was used to identify the risk factors for evisceration or enucleation. Thirty eyes
