Maxillary defects due to COVID-19 associated mucormycosis: Impact on quality of life after rehabilitation with an obturator.

STATEMENT OF PROBLEM: Aggressive surgical debridement including maxillectomy in patients with coronavirus disease 2019 (COVID-19) associated mucormycosis of the maxilla alters facial appearance, function, and an individual's overall quality of life (QOL). These patients require rehabilitation of defects with obturators to help them recover from esthetic and functional disabilities and regain near-normal quality of life. Because COVID-19 associated with mucormycosis is a newer entity, less is known about the quality of life of these patients after rehabilitation with obturators. Hence, the effe

Oral health-related quality of life after rehabilitation with maxillary obturators: A comparison of patients with maxillectomy defects associated with cancer and post-COVID-19 mucormycosis.

STATEMENT OF PROBLEM: Maxillectomy alters function and esthetics, negatively affecting oral health-related quality of life (OHRQoL). OHRQoL has been assessed after prosthetic rehabilitation in patients with maxillectomy associated with cancer. However, OHRQoL studies after prosthetic rehabilitation in patients with coronavirus disease 2019 (COVID-19) associated mucormycosis are lacking. PURPOSE: The purpose of this clinical study was to compare OHRQoL before and after rehabilitation with maxillary obturators in patients with maxillectomy defects associated with cancer and post COVID-19 mucorm

Assessment in the treatment of hypernasal speech with motor neuron disease: a clinical report.

The application of nasal anemometry in this patient's treatment proved to be of value in obtaining an objective assessment of this intervention. Nasal anemometry may be of use in other situations, for example, (1) in preassessment and postassessment of cleft palate surgery and surgical interventions to improve the velopharyngeal seal (such as pharyngoplasty), (2) in the evaluation of nasalized speech with conventional speech therapy, (3) in the evaluation of nasalized speech by use of a palatal training appliance, and (4) in the investigation of snoring.

Mandibular pathological fracture during treatment with a dynamic mouth opening device: A clinical report.

Trismus is a well-known complication of head and neck cancer treatment. It is defined as a progressive tonic contraction of the muscles of mastication that results in decreased mouth opening. This condition can lead to impairment of speech and eating, malnutrition, poor oral hygiene, and difficulty with dental treatment. Its prevalence in patients with head and neck cancer ranges from 5% to 38%. Different treatments are available to improve muscle length and function. Mouth opening devices along with exercising of the mandible immediately after surgery and/or radiation therapy have been found

Failure of Guided Endodontic Access in a Severely Obliterated Root Canal: A Case Report of Drill Deviation.

Pulp canal obliteration (PCO) increases the risk of procedural errors during canal localization. Guided endodontics improves access accuracy, but its success depends on precise guide design, proper drill-sleeve alignment, and material stability. When these factors are compromised, deviations and unsuccessful canal location may occur. The present clinical case reports the management of a severely obliterated root canal in a maxillary lateral incisor in which static-guided endodontic failed to achieve canal localization, emphasizing the technical limitations and the successful conventional appro

Nonsurgical endodontic management of maxillary sinusitis of endodontic origin: a CBCT-guided case report.

BACKGROUND: Maxillary sinusitis of endodontic origin (MSEO) is a recognized complication of odontogenic infection, arising from the close anatomical relationship between the posterior maxillary dentition and the maxillary sinus floor. Failure to identify the odontogenic etiology of sinus pathology risks misdiagnosis, delayed treatment, and unnecessary surgical intervention. This case report presents the diagnostic approach and clinical management of a maxillary second molar associated with sinus mucosal thickening, emphasizing the role of advanced imaging and conservative endodontic therapy in

Occlusal relationship in patients with bilateral cleft lip and palate during the mixed dentition stage: does neonatal maxillary arch configuration predetermine outcome?

OBJECTIVE: To determine whether there is any correlation between initial maxillary arch dysmorphology patterns in complete bilateral cleft lip/palate (BCLP) and the subsequent occlusal relationship in early mixed dentition. DESIGN: This retrospective study evaluated 41 consecutive patients with complete BCLP who were managed by a single tertiary cleft team between 1983 and 1992. All patients had bilateral synchronous lip adhesions with placement of a passive alveolar molding appliance at 4 to 8 weeks of age; definitive modified Manchester lip repair at 6 to 8 months of age; and a one-stage "t

[Treatment of infants with Pierre Robin sequence].

The Pierre Robin sequence (PRS) characterized by mandibular micro- or retrognathia and glossoptosis with or without cleft palate, presents clinically with intermittent upper airway obstruction (UAO). It is associated with other malformations in about half the cases. The incidence is about 1:8,500. Isolated PRS without other malformations does not, by itself, appear to affect neurodevelopment. Active intervention may therefore be required to reduce the risk for neurocognitive impairment resulting from UAO. Current treatment options for UAO range from prone positioning, use of a nasopharyngeal t

Orofacial functions in patients with congenital and acquired maxillary defects: a fluoroscopic study.

By means of videofluoroscopic recordings, chewing, swallowing and speech were monitored in nine patients with congenital and acquired maxillary defects. All of them were rehabilitated with a maxillary obturator prosthesis. The defective region was seldom used for chewing. No leakage between the obturator and surrounding tissue was observed, either for solids or for liquids. All of the prosthetic reconstructions were surprisingly stable during function. In the phonetic analysis, speech production was judged to be restored almost to normal. Videofluoroscopy may be valuable as a complement to oth

Single Suture Synostosis and Isolated Cleft Palate in Non-Apert Syndrome Patients.

INTRODUCTION: Single suture craniosynostosis (SSC) and isolated cleft palate (ICP) in non-Apert syndrome patients rarely occur together. Management includes airway optimization, timing surgery appropriately, and assessing both cranial vault aesthetics and speech outcomes. The aim of this study was to compare treatment pathways and outcomes in patients with both conditions to standard treatment for these conditions in isolation. METHODS: Patient hospital medical records were retrospectively reviewed for demographic data, timing of surgery, aesthetic outcome (using the Whitaker grading system f

Presurgical nasoalveolar molding assisted primary reconstruction in complete unilateral cleft lip palate infants.

BACKGROUND: Various methods have been described for the primary surgical reconstruction of the unilateral cleft lip and palate deformity (UCLP) in infants. There have been several attempts at restoring the normal anatomy of the nose at the time of lip repair in the affected individuals with varying degrees of success. Presurgical nasoalveolar molding (PNAM) is a presurgical infant orthopedic procedure that attempts to target the nasal deformity leading to a more esthetic surgical repair. OBJECTIVE: At our center we aimed to use PNAM to help in providing the surgical team with a better foundat

Two-stage palatoplasty, is it still a valuable treatment protocol for patients with a cleft of lip, alveolus, and palate?

INTRODUCTION: Speech development is of utmost importance and requires early closure of a palatal cleft. On the other hand, it is well known that all types and timings of surgical repair of facial clefts are detrimental to maxillary growth. Nevertheless, these days one is more and more confronted with a world-wide tendency in favour of the one-in-all operation to close clefts of the lip, alveolus, and palate. Therefore, a three-centre study was performed for testing - once more - the value of two-stage palatoplasty as a means to reduce the detrimental effects of surgery on palatal growth and at

Application of supercritical fluid carbon dioxide in improving food shelf-life and safety by inactivating spores: a review.

INTRODUCTION: Sarcomas of the head and neck region account for less than 10% of soft tissue sarcomas, and comprise less than 1% of head and neck malignancies. Approximately 80% of sarcomas arise from soft tissue, with the remaining originating from bone or cartilage. Head and neck sarcomas typically occur more frequently in men. CASE REPORT: Our patient was a 30 year old male who presented with a ulceroproliferative mass over the soft palate increasing in size over past 6 months. Punch biopsy of the lesion was suggestive of carcinosarcoma. MRI PNS was suggestive of 38 × 39 × 33 mm sized lobul

Outcome of Temporalis Myofascial Flap in Maxillary Reconstruction.

The maxilla is an important component of the midfacial skeleton from both an aesthetic and functional view point. In addition to cosmetic impairment, surgical procedures in this region have substantial ramifications for speaking, swallowing and mastication, all of which may lead to psychological anguish for the patient. This necessitates rehabilitation with an obturator prosthesis or surgical reconstruction. The temporalis muscle flap has considerable potential for reconstructing various maxillofacial area abnormalities. It appears to be one of the finest solutions among all other regional fla

Clinical strategies for successful palato-mesiobuccal canal management-Report of 2 cases.

AIM: To describe the management of the palato-mesiobuccal (PMB) canal in maxillary second molars with fused roots using conventional techniques. SUMMARY: Root canal treatment success hinges upon effectively addressing the intricate and variable anatomy of molar teeth. Failure to do so can lead to persistent infections and treatment failure. Recent advancements in imaging technologies have provided unparalleled insights into dental anatomy, especially in molars. Among these discoveries is the PMB canal, a unique anatomical variant recently reported for the first time in Endodontic literature.

A two-centre study on facial morphology in patients with complete bilateral cleft lip and palate at nine years of age.

The aim of this study was to compare craniofacial morphology and soft tissue profiles in patients with complete bilateral cleft lip and palate at 9 years of age, treated in two European cleft centres with delayed hard palate closure but different treatment protocols. The cephalometric data of 83 consecutively treated patients were compared (Gothenburg, N=44; Nijmegen, N=39). In total, 18 hard tissue and 10 soft tissue landmarks were digitized by one operator. To determine the intra-observer reliability 20 cephalograms were digitized twice with a monthly interval. Paired t-test, Pearson correla

Incidence of mutans streptococci and lactobacilli in oral cleft children wearing acrylic plates from shortly after birth.

Children with a palatal cleft can be treated with preoperative infant orthopedics including an acrylic plate that is applied shortly after birth to obturate the cleft. It is advised to wear these plates until the 18th month of age. Such a plate, being a hard non-shedding surface, may be expected to facilitate early colonization of mutans streptococci. The first aim of the present investigation was to assess the incidence of mutans streptococci and lactobacilli in children with cleft lip and/or palate during the first 2 years of life. The second aim was to study whether preoperative orthopedics
