Amiodarone-associated pulmonary fibrosis. Evidence of an immunologically mediated mechanism.

A case of pulmonary fibrosis attributed to amiodarone was studied immunologically and morphologically. A specific antibody of the IgG class was identified in the serum of this patient which reacted with the patient's own lung tissue. The immunoglobulin did not react with normal lung tissue nor was there evidence of reaction with lung tissue from patients with pulmonary fibrosis but without a history of amiodarone therapy. The patient probably developed a humoral antibody response to a lung-amiodarone complex with the amiodarone acting as a hapten which bound in vivo to the lung tissue. The sub

A real-time screening assay for GIRK1/4 channel blockers.

The cardiac acetylcholine-activated K(+) channel (I(K,Ach)) represents a novel target for drug therapy in the treatment of atrial fibrillation (AF). This channel is a member of the G-protein-coupled inward rectifier K(+) (GIRK) channel superfamily and is composed of the GIRK1/4 (Kir3.1 and Kir3.4) subunits. The goal of this study was to develop a cell-based screening assay for identifying new blockers of the GIRK1/4 channel. The mouse atrial HL-1 cell line, expressing the GIRK1/4 channel, was plated in 96-well plate format, loaded with the fluorescent membrane potential-sensitive dye bis-(1,3-

Comparative evaluation of HERG currents and QT intervals following challenge with suspected torsadogenic and nontorsadogenic drugs.

The purpose of the present study was to comparatively evaluate human HERG currents and QT intervals following challenge with suspected torsadogenic and nontorsadogenic drugs. Various concentrations of 14 different drugs were initially evaluated in terms of their relative potency to block I(HERG) in stably transfected human embryonic kidney cells. Four general categories of drugs were identified: high-potency blockers (IC50 < 0.1 microM) included lidoflazine, terfenadine, and haloperidol; moderate-potency blockers (0.1 microM < IC50 < 1 microM) included sertindole, thioridazine, and prenylamine

Variation in Antiarrhythmic Management of Infants Hospitalized with Supraventricular Tachycardia: A Multi-Institutional Analysis.

Supraventricular tachycardia (SVT) is the most frequent form of symptomatic tachyarrhythmia in infants. The purposes of this study were to describe practice patterns of the management of infants hospitalized with SVT and factors associated with 30-day hospital readmission. This was a multi-institutional, retrospective review of the pediatric health information system database of SVT hospitalizations from 2003 to 2013. High-volume centers (HVC) were defined as those at the upper quartile of admissions. Infants with an ICD-9 code of paroxysmal SVT were included. Antiarrhythmics investigated incl

[Atrial fibrillation after open-heart surgery--incidence, prevention and treatment].

Atrial fibrillation is a common problem in the postoperative period following open-heart surgery. The pathogenesis of postoperative atrial fibrillation is likely to be multifactorial, however increased sympathetic activation may play a significant role. The aim of the study was to detect the incidence and possible reasons of atrial fibrillation in the first three postoperative days after open-heart surgery. Atrial fibrillation was detected in a total of 48 patients (mean age 64.8 +/- 8.8 years) of the 302 consecutive patients included in the study. The incidence of atrial fibrillation was 15.9

[Therapy and prevention of atrial fibrillation].

In the last decade, broader insights into the mechanisms and clinico-prognostic significance of atrial fibrillation have prompted a dramatic shift in the clinician's decision making process in terms of treatment and prevention of this common rhythm disturbance. In fact, evidence of short- or long-term untoward effects on cardiac function and the risk of cerebrovascular accidents, together with a much wider therapeutic armamentarium, form the rationale for the present far more aggressive attitude, aiming at restoring sinus rhythm in all patients in whom an even weak chance exists. The newer cla

The ESVEM trial. Electrophysiologic Study Versus Electrocardiographic Monitoring for selection of antiarrhythmic therapy of ventricular tachyarrhythmias. The ESVEM Investigators.

ESVEM (Electrophysiologic Study Versus Electrocardiographic Monitoring) is an ongoing multicenter trial supported by the National Heart, Lung, and Blood Institute that began enrollment of patients on October 1, 1985. We describe here the methodology of the trial and data regarding enrollment of patients in the trial. The purpose of the trial is to determine whether electrophysiologic study or electrocardiographic Holter monitoring more accurately predicts antiarrhythmic drug efficacy in patients with aborted sudden death or sustained ventricular tachyarrhythmias. Consenting patients with induc

Tachycardia-induced change of atrial refractory period in humans: rate dependency and effects of antiarrhythmic drugs.

BACKGROUND: Atrial fibrillation (AF) has been shown to shorten the atrial effective refractory period (ERP) and make the atrium more vulnerable to AF. This study investigated the effect of atrial rate and antiarrhythmic drugs on ERP shortening induced by tachycardia. METHODS AND RESULTS: Seventy adult patients without structural heart disease were included. For the first part of the study, right atrial ERP was measured with a drive cycle length of 500 ms before and after 10 minutes of rapid atrial pacing using five pacing cycle lengths (450, 400, 350, 300, and 250 ms) in 10 patients. [Therapy of postoperative ectopic junctional tachycardia by surface hypothermia].

Postoperative junctional ectopic tachycardia is associated with a poor prognosis in patients after open-heart surgery. This dysrhythmia has been shown to be resistant to medical treatment as well as to epicardial pacing and external cardioversion. Successful therapy with surface hypothermia in an infant with junctional ectopic tachycardia (ventricular rate 210-230 bpm) after repair of tetralogy of Fallot is presented. Hypothermia (rectal temperature 32-34 degrees C) was effective in decreasing the ventricular rate of the dysrhythmia (< 170 bpm). After spontaneous cessation of the tachycardia,

Beyond pooled analysis: sex-specific cardiovascular adverse event reporting signals with antiarrhythmic drugs in FAERS.

Sex differences in drug metabolism and cardiac electrophysiology may influence the cardiovascular safety profiles of antiarrhythmic drugs (AADs). However, pharmacovigilance studies often pool data, potentially obscuring sex-specific cardiovascular adverse event (CV-AE) reporting patterns. We aim to investigate sex-specific CV-AE reporting profiles associated with commonly used antiarrhythmic drugs using pharmacovigilance data. We analyzed 122,853 medication-role records representing 38,430 unique FAERS reports involving eight AADs. Disproportionality analyses defined index-drug exposure by the

Effectiveness and safety of Guipi Mixture for the treatment of premature ventricular contractions (heart-spleen deficiency syndrome): study protocol for a randomized, double-blind, placebo-controlled, multicentre trial.

INTRODUCTION: Premature ventricular contractions (PVCs) are a common arrhythmia. Long-term frequent PVCs may contribute to tachycardia-induced cardiomyopathy, exacerbate angina, or trigger more severe ventricular arrhythmias, increasing cardiovascular risk. Current Western guidelines recommend pharmacotherapy or catheter ablation, with conservative drug therapy (e.g., lidocaine, propafenone, metoprolol, amiodarone, verapamil) remaining the mainstay. Metoprolol tartrate, a first-line drug, is effective for PVCs but has single targets, limited efficacy, and potential long-term cardiovascular adv

Flecainide is well-tolerated and effective in patient with atrial fibrillation at 12 months: a retrospective study.

INTRODUCTION: Current atrial fibrillation (AF) guidelines recommend flecainide as a first-line rhythm control option in patients without structural heart disease. While there is proven efficacy in clinical trials and guideline support, it is hypothesized that flecainide may be underutilized due to negative outcomes in the CAST trial and that adverse effects are less common than previously perceived. METHODS: This retrospective chart review evaluated patients ⩾18 years initiated on flecainide for AF from August 2011 to October 2016 by a cardiology provider at the study site. Exclusion criteria

Pharmacogenetics of myotonic hNav1.4 sodium channel variants situated near the fast inactivation gate.

Sodium channel myotonia and paramyotonia congenita are caused by gain-of-function mutations in the skeletal muscle voltage-gated sodium channel hNav1.4. The first-line drug is the sodium channel blocker mexiletine; however, some patients show side effects or limited responses. We previously showed that two hNav1.4 mutations, p.G1306E and p.P1158L, reduce mexiletine potency in vitro, whereas another sodium channel blocker, flecainide, is less sensitive to mutation-induced gating defects. This observation was successfully translated to p.G1306E and p.P1158L carriers. Thus, the aim of this study

Electrophysiological mechanisms and catheter ablation of complex atrial arrhythmias from crista terminalis:

Paroxysmal atrial fibrillation (PAF) can be initiated by ectopic activation from the crista terminalis. The crista terminalis conduction gap is also a critical isthmus in atrial reentrant arrhythmias like upper and lower loop reentry. The aim of this study was to investigate the mechanism and results of catheter ablation for complex atrial arrhythmias originating from the crista terminalis using the noncontact mapping system (NCM). The study population consisted of six patients (5 men, 1 woman; 70 +/- 9 years) with drug refractory PAF and typical/atypical atrial flutter. NCM identified the ear
